Dou Cai treated a patient suffering from Leprosy (Lifeng, 疠风), whose beard and eyebrows had completely fallen out, with redness and swelling of the face and eyes, and ulcers covering the hands and feet. He prescribed moxibustion on Feishu (BL13) and Xinshu (BL15) acupoints, ten moxa cones each, along with one dose of Huangu San (Bone-Substituting Powder). The patient recovered completely in two months, with beard and hair regrowing.
(Mao Dazun notes: Dou Shi used Huangu Dan for stroke and Huangu San for Leprosy; this case also utilizes Huangu San.)
Another patient presented with generalized erythema and swelling accompanied by pain like needle pricks. Dou Cai stated, "Your illness is easy to treat." He ordered moxibustion on Xinshu (BL15) and Feishu (BL13) acupoints, one hundred cones each, followed by the administration of Huma San (Sesame Powder). The patient recovered after two doses. For slight motor impairment in the limbs, he repeated moxibustion on the previous acupoints for fifty cones and administered another two doses of Huma San, resulting in full recovery. (Mao Dazun notes: Huma San consists of Sesamum indicum seed [Semen Sesami Nigrum], Viola yedoensis [Zi Fuping], Mentha haplocalyx [Bohe], Arctium lappa seed [Niubangzi], and Glycyrrhiza uralensis [Gancao]. This formula by Dou Shi is particularly mild and safe.)
A third patient exhibited black swelling on the face, with a cloud-like mass arising below the left ear, purple and resembling a coiled snake. Pain within the muscles felt like knife stabs, while sensation in the hands and feet was lost. Upon inquiry, it was revealed that after drinking heavily and sleeping for three days during a social gathering, the patient woke to find his left arm swollen and blackened like a snake. Previous treatment with wind-dispelling herbs provided temporary relief, but the condition recurred. Dou Cai diagnosed, "This is not Wind pathogen, but Dampness lodging in the Back-Shu points of the Zang organs. The previous wind-dispelling herbs likely overcame dampness temporarily, but the root of the toxin remains." He prescribed moxibustion on Shenshu (BL23) acupoints, one hundred cones each, and one dose of Huangu Dan (Bone-Substituting Pill). The patient recovered, and his complexion returned to its former luster.
Ziheyang treated Sang Huimin for Facial Wind (Mianfeng), characterized by dark complexion, aversion to wind preventing going outdoors, incessant scratching, and loss of eyebrows, having been treated as Leprosy for three years. Ziheyang declared, "This is not Leprosy." Citing *Suwen · Feng Lun* (Plain Questions: Discussion on Wind): "The manifestations of Kidney Wind include profuse sweating, aversion to wind, back pain preventing upright standing, dark discoloration, and facial puffiness." He diagnosed the patient with Kidney Wind and advised first puncturing the face to release substantial amounts of blood, which should appear ink-black. After three punctures, the blood color would change. Needles were inserted from the forehead down and up to the vertex, releasing ink-black blood as predicted. All swollen areas were punctured, except the lateral canthus, as this region belongs to the Shaoyang channel, which is characterized by scant blood and abundant Qi. Puncturing was repeated every other day; the blood turned purple, then red after two days. Initial punctures caused itching, subsequent ones caused forehead pain, and the third caused unbearable pain due to the retreat of the pathogen. After twenty-plus days, light puncturing was performed once more to conclude the procedure. The face was washed with water after each session. The black discoloration faded in ten days, the face became slightly red in one month, and returned to a ruddy white hue in three months. However, without taking root-clearing and heat-purging herbs, the disease relapsed. In the East, no one else could cure this condition. (Mao Dazun notes: Today, even fewer can cure it. Yet, one must be aware of such syndromes.)
Wu Xu, courtesy name Ruxin, from Qiantang, was well-read but poor in his youth, selling Wushe Wan (Black Snake Pills) to treat madness. When the Imperial Envoy for Gathering Folklore encountered a sufferer, he summoned Wu Xu for consultation, was amazed by his insights, and entrusted him with the treatment, which resulted in a cure. (*Hangzhou Prefecture Gazetteer*)
Mi Yun treated severe Leprosy using one large Black Snake (Dahuashe), killed and stored until decayed. It was soaked in two bowls of water for seven days, then skinned and deboned. One sheng of rough rice was added and soaked for one day, then dried. A white hen was starved for one day, fed the rice until feathers fell off, then slaughtered, cooked, and eaten with wine. Following this, the patient bathed in a basin of hot water for half a day, leading to spontaneous recovery. (*Compendium of Materia Medica*)
Wang Haizang reported: Mr. Wang suffered from severe Leprosy, with loss of eyebrows and beard and palm ulcers. After taking Ziwan Wan (Aster Pill) for half a month, he expelled two sheng of leprosy worms resembling horse tails, about one cun long. Zhou Zigong treated Wang Junhai's son for Leprosy, whom other physicians could not cure. Zhou prescribed seven medicated mouthwashes, causing the gums to bleed several sheng of foul blood. The patient subsequently became emaciated and skeletal. He was then nourished with delicious foods and recovered in several months. (*Collected Works of Jiuling Shanfang*)
Xue Lizhai treated a man who, in winter, experienced bitter taste in the mouth, tinnitus, and damp-itching of the scrotum. In spring, purple patches appeared on the face, slightly swollen and numb. By winter, the whole body turned purple, insensitive to pain or itch. In the following spring, the purple areas enlarged; by summer, they ulcerated. In the next spring, eyebrows fell out and fingers ulcerated. Xue diagnosed involvement of the Liver and Gallbladder channels. He prescribed bloodletting at the finger webs and wrists/ankles to release black blood, followed by two doses of Zaizao San (Reconstruction Powder) to purge toxic filth. Then, Xiao Chaihu Tang (Minor Bupleurum Decoction) combined with Siwu Tang (Four Substances Decoction) plus Angelica dahurica (Baizhi), Saposhnikovia divaricata (Fangfeng), Gastrodia elata (Tianma), and Tribulus terrestris (Jiaoci) was used to gradually improve the condition, followed by Huanji San (Skin-Replacing Powder). The body remained slightly red, indicating Blood deficiency with Empty Heat. Due to poverty, proper convalescence was impossible. Fever occurred in autumn; facial patches recurred in spring. Using the previous powder with Blood-nourishing herbs, and relying on the patient's youth and compliance, he recovered.
A man accustomed to rich food developed nasal destruction, eyebrow loss, finger detachment, and body ulceration, with heat and thirst worsening in the late afternoon. Siwu Tang (Four Substances Decoction) plus stir-fried black Anemarrhena asphodeloides (Zhimu), Phellodendron amurense (Huangbai), Schisandra chinensis (Wuweizi), Ophiopogon japonicus (Maidong), Angelica dahurica (Baizhi), Gastrodia elata (Tianma), and Tribulus terrestris (Jiaoci) was administered for over thirty doses, alleviating heat and thirst. In midsummer, he presented with fatigue, asthma, fever, yellow/scanty urine, and loose stools, indicating Qi deficiency with seasonal Heat excess. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) brought immediate relief. Huanji San (Skin-Replacing Powder) and Yiqi Tang (Qi-Augmenting Decoction) were taken concurrently for two months, followed by Shengji San (Flesh-Generating Powder) as tea, reducing ulcers. In mid-autumn, dizziness, anorexia, spontaneous sweating, heaviness, and loose stools indicated seasonal Dampness. Qingzao Tang (Clear Dryness Decoction) resolved this. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with minor additions of stir-fried black Zhimu, Huangbai, Jiaoci, and Tianma cured him in two months. Later, due to overwork, he developed deafness, heat, and thirst. Mistakenly taking wind-dispelling herbs worsened the condition, causing red rashes. Yiqi Congming Tang (Augment Qi and Brighten the Orifices Decoction) cured him in a month. (Note on Leprosy: Initially numb and insensible, followed by red spots, eventually ulcerating with edema and no pus. Skin death causes numbness; muscle death causes insensitivity to cutting; blood death causes weeping ulcers; tendon death causes joint detachment; bone death causes nasal collapse. Note on Lepra: Begins with itchy blisters becoming sores, oozing serous fluid, intense itching penetrating to bone, spreading over the body, preferring hot baths. Xu Lingtai states: Leprosy is generally incurable, though it may linger for years; however, cases are rare. If symptoms merely resemble wind-disease but are not true wind, doctors mistakenly use harsh wind-treating drugs, damaging Qi and Blood and transforming the condition. Such cases are common and must be recognized. He adds: Lepra resembles Leprosy but has shallower toxicity confined to muscles, making it treatable. Yu Jiayan states: Treating Leprosy focuses on clearing Ying and Wei. Sweating should be frequent; bloodletting should be frequent; both are efficient methods to clear Ying and Wei. Worms arise from Lung Heat, requiring simultaneous Lung clearing, but Ying and Wei must be cleared first because corrupted Ying and Wei Qi entering the Lung damages its descending function. Treatment prioritizes generating Blood and clearing Heat, with dispelling Wind and killing parasites as secondary. Laoyi Qu Feng Wan (Dispel Wind Pill) and Dongpo Sishen Dan (Master Dongpo's Four Divine Pills) are safest for long-term use. Qu Feng Wan: Astragalus membranaceus (Huangqi), Aurantium fructus (Zhike), Saposhnikovia divaricata (Fangfeng), Lycium barbarum (Gouqizi), Paeonia lactiflora (Baishao), Glycyrrhiza uralensis (Gancao), Lycium bark (Digupi), Rehmannia glutinosa raw (Shengdi), and Rehmannia glutinosa prepared (Shudi) in equal parts, honey pills. Sishen Dan: Notopterygium incisum (Qianghuo), Scrophularia ningpoensis (Xuanshen), Angelica sinensis (Danggui), and Rehmannia glutinosa raw (Shengdi) in equal parts, decocted or pill form.)
A man presented with redness, pain, heat, thirst, dripping pus, vexation, hot palms, blurred vision, slurred speech, and palpitations. This was a Heart channel syndrome. Anshen Wan (Calming Spirit Pill) combined with Bazhen Tang (Eight Treasures Decoction) with minor additions of Akebia trifoliata (Mutong), stir-fried black Coptis chinensis (Huanglian), and Polygala tenuifolia (Yuanzhi) gradually restored Qi. Bian stimulation was applied to expel external pathogens. Constipation and thirst persisted, resolved by Chaihu Yin (Bupleurum Drink) and Bazhen Tang (Eight Treasures Decoction). Huanji San (Skin-Replacing Powder) completed the cure.
A man showed red veins on the abdomen, purple blisters on the face, fever, thirst worsening at Yin-Mao hours (5-9 AM), wiry-rapid pulse, leg cramps, and scanty/difficult urination. This was Liver Fire. Chaihu Yin (Bupleurum Drink) reduced fever and regulated bowels. Xiao Chaihu Tang (Minor Bupleurum Decoction) combined with Siwu Tang (Four Substances Decoction) plus Gentiana scabra (Longdancao) and stir-fried Gardenia jasminoides (Zhizi) for over thirty doses, followed by Bazhen Tang (Eight Treasures Decoction) with Chaihu and Zhizi to regulate Qi and Blood, then Huanji San (Skin-Replacing Powder) to remove internal toxins, stabilized him. Over a year later, due to labor and improper diet, he developed chills, fever, headache, and body rash. Self-medicating with Zuixian San (Intoxicating Immortal Powder) led to death.
A man had purple nodules on the face, dripping pus, nocturnal convulsions, generalized numbness, and gradually appearing red patches. Labor induced numbness; anger induced itching. Liver pulse was surging-large. Bloodletting at arms, legs, and wrists was performed. Qingwei Tang (Clear Stomach Decoction) plus Rhubarb (Dahuang) and Jiaoci was given for four doses, followed by Xiaqing Wan (Drain Green Pill) orally, slightly softening the Liver pulse. Shengma Tang (Cimicifuga Decoction) aided the pill, mildly improving symptoms. One jin of Huangu San (Bone-Substituting Powder) from *Baodian* was used, followed by Xiao Chaihu Tang (Minor Bupleurum Decoction) combined with Siwu Tang (Four Substances Decoction) plus Atractylodes lancea (Cangzhu), Gastrodia elata (Tianma), and Jiaoci for over a hundred doses, plus Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia), curing him in six months. Later, due to labor, generalized numbness/itching and faint-slow pulse indicated dual Qi and Blood deficiency failing to nourish the interstices. Shiquan Dabu Tang (Ten Complete Tonifying Decoction) with Wuweizi and Maidong restored health over a year.
A man had red face, purple blisters, lower body itching/pain, afternoon fever, and dry/black stools. This was Excess Fire with Blood deficiency. Zaizao San (Reconstruction Powder) and Siwu Tang (Four Substances Decoction) plus Stephania tetrandra (Fangji) and Gentiana scabra (Longdancao), along with bloodletting at toe webs to release toxic blood, regulated bowels. Adding Atractylodes macrocephala (Baizhu), Angelica dahurica (Baizhi), Poria cocos (Fuling), Notopterygium incisum (Qianghuo), and Angelica pubescens (Duhuo) turned stool yellow. Removing Longdancao and Fangji from Siwu Tang (Four Substances Decoction), using less Duhuo, adding Scrophularia ningpoensis (Xuanshen) and Dioscorea hypoglauca (Bixie) for over fifty doses reduced ulcers. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Tianma and Maidong replenished Qi and Blood. In mid-autumn rain, generalized soreness was resolved by Qingzao Tang (Clear Dryness Decoction). Huanji San (Skin-Replacing Powder), Huma San (Sesame Powder), and Bazhen Tang (Eight Treasures Decoction) cured him.
An Shangshe had facial swelling, skin resembling tinea, later turning into purple nodules weeping fluid upon scratching. This was a Spleen-Lung syndrome. Qingwei Tang (Clear Stomach Decoction) cleared Stomach heat and released exterior toxins. Siwu Tang (Four Substances Decoction) plus Zhizi, Scutellaria baicalensis (Huangqin), Chaihu, Jiaoci, and Licorice segment (Gancao jie) nourished Yin Blood and dispelled Wind-Heat. Bloodletting at arms, legs, wrists, finger webs, and lesions unblocked collaterals. Bazhen Tang (Eight Treasures Decoction) plus Baizhi, Jiaoci, Acanthopanax senticosus (Wujiapi), and Scorpio (Quanxie), along with Ersheng San (Two Sage Powder), was taken for over a month to nourish Yin Blood and treat ulcer toxins. Buqi Xierong Tang (Augment Qi and Drain Ying Decoction) provided slight improvement, followed by Huanji San (Skin-Replacing Powder) for complete cure. Later, due to fatigue, red halos appeared, worse in late afternoon. Siwu Tang (Four Substances Decoction) plus Moutan cortex (Danpi), Chaihu, and Zhizi, combined with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) for over a year prevented recurrence despite labor.
A man had generalized tinea-like lesions, itchy sores, purple numbness, pain upon pinching, frequent/scanty urination. This was a Spleen-Stomach syndrome with pathogen mostly in the exterior. Qingwei San (Clear Stomach Powder) and bloodletting at lesions, arms, legs, and wrists to release black blood refreshed the mind. Aversion to cold, fatigue, and dry mouth indicated pathogen retreat and True Qi deficiency. Large doses of Ginseng, Astragalus, Ligusticum chuanxiong (Chuanxiong), Angelica sinensis (Danggui), Tribulus terrestris (Jili), and Platycodon grandiflorus (Jiegeng) rapidly restored Qi. Bazhen Tang (Eight Treasures Decoction) plus Huangqi, Baizhi, Jili, Tianma, and soft Chaihu, along with Ersheng San, healed ulcers. Huanji San (Skin-Releasing Powder) and Bazhen Tang (Eight Treasures Decoction) for six months cured him. Later, symptoms recurred. Mistakenly using attacking/toxin-expelling herbs led to death from Cold damage with concurrent Taiyang and Yangming involvement.
A man had generalized nodules, itchy when scratched, painful when pinched, constipation, and thirst. Pathogen was internal. Zaizao San (Reconstruction Powder) for two doses caused mild purging three times. Taoren Chengqi Tang (Peach Kernel and Cassia Decoction) plus Danggui for four doses, plus bloodletting to release black blood, increased pain/itch slightly. Fatigue required Qi-tonifying agents to restore original Qi. Ersheng San (Two Sage Powder) quickly healed ulcers. Major tonification cured him in a year. Later, copious phlegm, stiff tongue, and speech impediment were cured by Erchen Tang (Two Aged Decoction) plus Cangzhu, Zhimu, Huangbai, and Ze xie for several doses. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) ensured stability.
A man of ascetic habits lost all eyebrows; pathology involved Liver and Gallbladder channels. Bloodletting at arms, legs, wrists, and lesions released black blood. Bazhen Tang (Eight Treasures Decoction) with Wuweizi, Humazi, Polygonum multiflorum (Shouwu), and Salvia miltiorrhiza (Weilingxian) was taken fasting; Huanji San (Skin-Replacing Powder) after meals. Absence of complicated symptoms and strict adherence to taboos led to recovery in under six months.
A scholar had itchy/painful or numb/swollen soles. Two years later, body itching, nodules, fever, tinnitus worsening in late afternoon indicated Kidney deficiency. Bloodletting at arms, legs, wrists, and finger webs removed stasis. Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) base with Wuweizi and Chaihu for over fifty doses tonified Kidney. Huanji San (Skin-Replacing Powder) and Qu Feng Wan (Dispel Wind Pill), one jin each, treated ulcers. Recovery came through nourishment and abstinence.
An Shangshe suffered generalized lesions with deficient form and pathology. He was told to tonify Original Qi before treating ulcers. Disbelieving, he strictly took Snake Wine to attack internal toxins and applied Realgar (Pishuang) to erode external toxins, causing sudden vomiting of clear fluid and piercing body pain. Some thought toxins were emerging externally, unaware that Spleen governs muscles; this was Spleen injury from toxic drugs. Taking detoxifying agents caused fatal vomiting and diarrhea.
A man with Leprosy underwent herbal fumigation/washing, resulting in continuous sweating, asthma/cough/anorexia, abdominal borborygmus, cold feet, and limb convulsions. This was attributed to Heat injuring Original Qi, loose interstices, and sweating leading to Yang collapse. He died that day.
