Dou Cai treated a patient suffering from hemiplegia (paralysis of one side of the body). He first moxibusted the Guanyuan (CV-4) acupoint 500 moxa cones, administered Baxian Dan (Eight Immortals Pill) twice daily, and Hangu Dan (Bone-Replacing Pill) once every five days. The patient sweated at the affected site, and the next day his condition improved by four-tenths. After one month, he was completely cured. He then took half a jin of Yanshou Dan (Longevity Pill) and one jin of Baoyuan Dan (Origin-Sustaining Pill), and the disease did not recur for fifty years. Formulas such as those in *Qianjin Fang* do not moxibust Guanyuan or administer mineral pills, relying only on ordinary medicines. Although they may achieve a cure, it is difficult to maintain long-term health.
Another patient suffered from left-sided hemiplegia with deep, fine, and forceless pulses in all six positions. Dou Cai said, "This must be due to the consumption of harsh and purgative medicines, which damaged the True Qi, hence the deep and fine pulse." The patient replied, "Last month I took Zhuohu Dan (Tiger-Catching Pill) and vomited two sheng of saliva; since then, my symptoms have slightly lightened, but I am not fully recovered." Dou Cai sighed and said, "Stroke is originally caused by deficiency and damage of Yuan Qi. Now, taking emetics further damages the Yuan Qi. Although symptoms are reduced now, it will relapse in a few days, and there will be no way to save him." Indeed, within ten days, the condition severely worsened. Despite seeking treatment again and taking mineral pills, he could not be saved.
Once, while traveling through Hengzhou, I met the Magistrate of Liling returning from Hengyang, ill and retiring due to sickness. Asking about the cause, he said, "I had pork, but went deep into the mountains where no meat was available. My attendants ate Pangolin meat, so I tasted several pieces. I had a history of wind disease, which recurred then, leaving my left hand and foot paralyzed." I gave him some wind-dispelling medicine from my chest, and heard half a month later that he had suddenly recovered. Upon reaching Yongzhou, I read in *Tu Jing* that "Pangolins should not be killed on embankments; if their blood enters the soil, the embankment cannot be sealed again, as they can penetrate the earth's veins." It is strange that this magistrate fell ill from accidentally eating pangolin meat, yet his chronic illness was cured in ten days. Nowadays, people use it to unblock women's menstruation with great efficacy.
Wan Jia was poor, making a living by fortune-telling with characters. He suffered from limb disease, tying his arm with silk around his neck and walking with a cane in his left hand; medicines were ineffective. One day he met Lü Dongbin, who said, "Your current fortune is stagnant. Liver Fire is excessive, generating heat like thunder striking and wind swirling. Yin and Yang Qi are discordant, so Fire does not generate Earth, causing Earth to scorch; Earth does not generate Metal, causing Metal to melt; Metal fails to control Wood, so Wood counter-controls. Displeasure from descendants led to your fall. Blood and Qi stagnated in the meridians, resulting in this." He palpated Wan's waist and arm, asking, "Is it sore?" "No." Palpating down to the knee, he asked again, "Sore now?" "Yes." Lü said, "This is the location of the Huantiao (GB-30) acupoint. Since you feel soreness, you will discard this cane someday." Seeing the silk hanging from Wan's hand and him rubbing inside his clothes three times, Lü said, "Fortunately, you are thin; you can be cured. Your Five Viscera all have Fire. No need for medicine; only Wuyi Tea can resolve it. Tea from branches facing southeast is best. Brew it with spring water from a ravine, and the tea leaves will stand upright; well water makes them lie flat." After staying home for a few days, he suddenly found he could lift his hands and step without knowing how. (From *Xu Jinling Suoshi*. Xu Lingtai notes: Tea specifically clears and descends Lung Metal.)
Li Shizhen treated a patient with hemiplegia whose limbs could not move. He used Castor Oil mixed with Sheep Fat, Musk, and Pangolin Scales (Lin Qi Jia), decocted into a massage ointment, applied several times a day. After over a month, gradual recovery occurred. Concurrently taking formulas to dispel wind, resolve phlegm, and nourish blood, he was cured in three months. Another patient had swelling and pain in an arm area; applying crushed Castor seed paste cured him overnight. (From *Bencao Gangmu*. External treatments are excellent and should not be ignored.)
Xue Lizhai treated a woman prone to anger, with spontaneous sweating and early menstruation, diagnosing Liver Fire and Blood Heat. She disbelieved this and indiscriminately used cooling agents, causing delayed menstruation. Later, due to labor and anger, she developed lockjaw, groaning, paralysis, surging pulses, and red face/eyes. Using Bazhen Tang, Ophiopogon Radix (Maidong), Schisandra Fructus (Wuweizi), Gardenia Fruit (Shanzhi), and Moutan Cortex (Danpi), she gradually improved over several doses. Combined with Xiaoyao San and Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) for over thirty doses each, she fully recovered.
Another woman, after recovering from Phlegm-Jue (syncope due to phlegm) caused by anger, had a left arm unable to extend, numb fingers, facial deviation, attacking phlegm-Qi, hot bones in both legs, or bone ache. Taking Youyao Shunqi San (Aucklandia Powder to Smooth Qi) worsened symptoms, causing frequent fainting, internal heat, and afternoon tidal fever. This indicated Liver Meridian Blood Deficiency, internal heat generating wind; previous medicines consumed Liver Blood, intensifying deficient fire. First, Chaihu Zhizi San (Bupleurum and Gardenia Powder) was used to nourish Liver Qi and Blood. Days later, Bazhen Tang plus Gouteng Teng (Uncaria Ramulus cum Uncis) relieved symptoms. Then, Jiajian Bawei Wan (Modified Eight-Ingredient Pill) with slight amounts of wine-fried Phellodendron Bark (Huangbai) and Anemarrhena Rhizome (Zhimu - black variety) cleared symptoms rapidly. Finally, Bazhen Tang and Chaihu Zhizi San for half a year achieved full recovery. Relapses due to overwork were managed with Chaihu Zhizi San.
In the twelfth year of Zhizheng, Ye Zhonggang from Kuocang served as a prefectural clerk in Tiantai for three years. One day, his limbs became uncontrolled. Physicians diagnosed it as Dongfeng (Wind piercing the limbs), believing wind penetrated all four limbs. Treatment failed. He sought Shi Jingzhong. Shi palpated the pulse and said, "The pathogen has accumulated in the body for a long time. Take large doses, and it will be cured in ten days." People marveled at this. Shi explained, "Palpating the pulse, it was large but slow in arrival, indicating accumulated heat郁结 internally, unable to vent for a long time. Suddenly encountering wind triggered the disease. I treated based on the pulse, hence the cure. What is miraculous about it?" (From Baiyun Ji, *Yishuo Xubian*.)
Wang Kentang treated a patient with right-sided hemiplegia bedridden for three years, unresponsive to Qi-regulating and phlegm-resolving drugs. At night, consciousness would cloud, and survival seemed impossible. Taking Shiquan Dabu Tang (Perfect Ten Great Tonifying Decoction) brought clarity; within days, he could rise with a cane, soon walking without it. The Classic says: "Where evil gathers, Qi must be deficient." Treating the deficiency alone, ignoring the evil, causes the evil to depart naturally.
Such statements mislead later generations. This patient became deficient due to excessive Qi-regulating and phlegm-resolving drugs, so Shiquan Dabu Tang worked. If one assumes pure tonification cures evil regardless of its presence, it is a grave error.
Li Dongyuan treated Governor Guo Juji of Shaanxi with hemiplegia, unable to extend two toes. He inserted a long needle into Weizhong (BL-40) deeply to the bone without pain sensation, draining one to two sheng of dark, ink-like blood. He also applied Miao Ci (Mirror Needling). After six or seven sessions and three months of medication, the disease largely resolved. (From *Shixiao Fang*.)
Feng Chuzhan treated Yu Mou with hemiplegia, swollen right arm, numbness/pain, difficulty walking. Others attributed it to phlegm, fire, or wind, using cooling, draining, and dispersing agents. Suddenly, he became comatose with phlegm-wheezing and profuse sweating; all six pulses were deep and minute. This was chronic Middle Qi deficiency. Without harsh tonification, but instead heavy draining, only heavy doses could reverse it. He used Ren Shen (Ginseng Root) 6 liang, fried Atractylodes Macrocephala Rhizome (Baizhu) 4 liang, raw Aconite Car root (Fuzi) 1 piece (peeled, fried in ginger juice), decocted in water to one bowl and灌ed. Sweat stopped, pulses rose, phlegm-wheezing ceased, consciousness cleared. After one month of regulation and tonification, he recovered.
Wu Fuxian treated Wang Jiheng with left-sided hemiplegia. Some treated it as gout with dispersing methods; others as phlegm with Gun Tan Wan (Rolling Phlegm Pill). After several purgations, spirit was exhausted. Left pulses were deep, fine, and weak. This was not damp-phlegm or dead blood, but Blood Deficiency. The left belongs to Blood, but without Qi to govern it, it does not flow. The method should be "leading Yin from Yang." Morning: Sijunzi Tang (Four Gentlemen Decoction) plus Astragalus Membranaceus Root (Huangqi), Cinnamon Twig (Guizhi), Polygonum Multiflorum Root (Shouwu), processed Aconite (Zhi Fu). Afternoon: Siwu Tang (Four Substances Decoction) plus Gentian Macrophylla Root (Qinjiao), Drynaria Rhizome (Xuduan), roasted Ginger (Pao Jiang), and a little Xinjiang (New Crimson Silk). Silk implies continuity; crimson implies entering the Heart to transform into red blood. This is especially suitable for left hemiplegia. After forty doses, hands moved. Doubling the dose, feet walked normally. (Recently, practitioners in Wu men specialize in this method, which has stirred my humble thoughts.)
Han Yifeng treated Kong Xueshi Shangxian with hemiplegia, difficult walking, slurred speech, hoarse voice, intermittent breath. He needled Huantiao (GB-30), Fengshi (GB-31), and Zusanli (ST-36) twenty-one times each. The patient immediately walked independently without support, tendons relaxed, blood circulated, pain vanished, though speech remained unchanged. The next day, he needled Tiantu (CV-22) and Danzhong (CV-17) fourteen times. Speech became clear and coherent.
Grand Minister Helun initially suffered from numbness in the left hand, inability to flex/extend fingers, a precursor to hemiplegia. Han treated with seven needles, restoring finger movement. Two months later, leg disease required a cane, leading to resignation requests. Han needled Huantiao, Fengshi, and Zusanli several times, quickly alleviating the disease, allowing him to resume duties.
Yu Jiayan treated Ji Hengweng, nearly seventy, with two-year hemiplegia. Attacks involved left-side stiffness tilting right, syncope, and urinary incontinence. Initially, Ginseng and Aconite were appropriate, but confusion over "left belongs to blood, avoid tonifying Qi" nearly ruined him. Shi Lizewen from Yunjian used Ginseng and Aconite to stabilize. However, general warming tonification did not exhaust the pathology. Pulses were soft and slippery, occasionally rapid/tense, indicating mixed Phlegm and Wind (Phlegm primary, Wind secondary), and mixed Heat and Cold (Heat primary, Cold secondary). Hands cold as ice meant Phlegm agitation caused Jue; vomiting Phlegm restored sleep/appetite. Winter tolerance of cold showed external cold was false, internal heat was true. Heat steamed dampness into Phlegm, blocking orifices, disrupting Wei Qi, inviting external wind. Sexual excess depleted Essence/Qi, attracting wind. Thus, mixed pathology arose. To treat, one must vigorously regulate Right/Spleen-Stomach Qi to transport Left/Heat-Phlegm-Deficient-Wind. Pure warming tonification is insufficient. Mixed diseases require mixed medicines adapted to seasons. In winter cold, internal heat is bound, forcing it into sinews/bones. Ginger/Aconite temporarily expel external cold, allowing internal heat to vent. If seasonal heat merges with internal heat, adding Ginger/Aconite causes three heats to flare, burning sinews/flesh.
Left/Right are paths of Yin/Yang. Liver/Gallbladder reside left, but Qi moves right; Spleen/Stomach reside right, but Qi moves left. Liver Wood governs Wind; Spleen Dampness forms Phlegm. Wind/Phlegm attack不分左右. But the elder depleted Essence/Blood; after age 64 (8x8), Tian Gui ended, Left deficit appeared first. Right-side dietary Phlegm and skin-invading Wind accumulated in empty organs, erupting suddenly. Wind pulses are tense/rapid; Phlegm pulses are soft/slippery. Disease shortens large sinews (limb contraction) and tongue sinews (speech impediment). Small sinews relax, pulling left to right. Treating unilateral wind requires "leading Yin from Yang, Yang from Yin, Left from Right, Right from Left." Use Ginseng/Atractylodes as Sovereign/Minister, Aconite/Dried Ginger as Assistant/Envoy in winter. Use Ginseng/Atractylodes with Antelope Horn, Bupleurum, Anemarrhena, Gypsum in Spring/Summer/Autumn to avoid heat. Needle extremities to drain Ying-Blood and unblock Qi, preventing Phlegm/Wind becoming Leprosy. Disciple asked: "Does hemiplegia stay fixed after distinguishing left/right?" Answer: "Wind/Phlegm follow individual constitution, having no fixed form. Post-disease, it is not irreversible. Return follows departure in heaven, human affairs, and pathology. Wind (Qi of 4 seasons/8 directions via nose) is Heaven Qi. Phlegm (taste of grains via mouth, Spleen-Damp knot) is Earth Qi. Though distant, they combine, like Internal Injury and External Contraction. If Wind dominates, treat Wind first; if Phlegm dominates, treat Phlegm first; if equal, treat both. This is fixed law. *Neijing* states: 'Wind enters via skin/hair, then muscle, then sinews, then marrow.' Good physicians treat skin/muscle first to prevent deepening. Skin=Lung; Muscle=Stomach; Sinews=Liver; Marrow=Kidney. From outside to inside, depth increases. Also: 'Dampness enters via feet,' ascending,不分左右. But internal Wind/Damp attracts external counterparts. Internal Wind sufferers tremble before weather changes; Internal Damp sufferers feel heavy before humidity rises. Treat according to category. External pathogens expelled outward; Ascending pathogens expelled downward. For Wind, use Da/Xiao Xuming Tang. Contains Guizhi, Fuzi, Poria, Atractylodes, Ephedra. Complex, but Guizhi/Fuzi drive interior evil; Poria/Atractylodes drive middle evil; heavy Ephedra/Saposhnikovia drive exterior, aiming to expel internal evil outward. For chronic deficiency/deep wind, use mild sweating or mild purgation methods, balanced for 10-15 days, then gentle dispersion. Ancient rules exist. Ignoring them renders treatment useless. For Phlegm, rules are obscure in texts. Upper: Gua Di San, Zhichi Tang. Left: Longhui Wan. Right: Gun Tan Wan. Deficient: Zhuli Datang Wan. Cold-fixed: Sanjian Tang. No mystery. I clarify: Stomach is Sea of Water/Grain, master of Viscera. Excess diet creates Phlegm, transported by Spleen Transport into collaterals, like tides. Spleen moves = tide goes; Spleen stops = tide returns. To treat fixed Phlegm, use pungent-hot to mildly move cold-stagnation, then stop, fearing Spleen overheats and Phlegm wanders harmfully. Phlegm flows Stomach->Collaterals; Collateral Phlegm must return to Stomach to exit via mouth/intestines. This happens when Spleen rests. Thus, between meals, rest Spleen to allow Phlegm return to Stomach for upward/downward movement via Stomach Qi, not Spleen Qi dispersion. Light phlegm patients spit upon waking; severe comatose patients vomit upon waking. Is this because fasting rests Spleen, giving Phlegm an exit? Those on harsh tonics crave food at night. People celebrate appetite, failing to cherish Spleen rhythm (move day, rest night). Will you ponder this proverb?" (Xu Lingtai notes: Qi Hequan says: Left is Yang/Southeast, Right is Yin/Northwest. Heaven surplus Yang, incomplete Northwest; Head corresponds to Heaven, Left senses clearer than Right. Evil attacks Right more. Earth surplus Yin, incomplete Southeast; Lower body corresponds to Earth, Right limbs freer than Left. Evil attacks Left more. Evil enters via deficiency. Upper Right heavy = Blood deficiency. Lower Left heavy = Qi deficiency. For hemiplegia with normal head/face, weigh Left-Yang/Right-Yin and Earth-Right-Stronger-than-Left. Left disease = Yang deficiency (Smooth). Right disease = Yin-Blood deficiency (Rebellious). Left-Yang/Right-Yin is Heavenly Law. Do not confuse Liver-Left-Blood/Lung-Right-Qi with Left-Blood-Disease/Right-Qi-Disease, confusing Yin-Yang paths. Men follow Heaven (Yang strong); upper disease shouldn't be Left. Lower body follows Earth (Southeast Yang insufficient); Left disease not rebellious. Women follow Earth (Yin strong); lower disease shouldn't be Right. Upper body follows Heaven (Northwest Yin insufficient); Right disease not rebellious.)
Zhu Danxi treated a obese man with worry/thought Qi stagnation, right-hand paralysis, facial deviation. Prescribed Buzhong Yiqi Tang. Added Pinellia, Bamboo Sap, Ginger Juice for Phlegm.
Cheng Yunlai stated: "An old doctor in my village had right-side paralysis bedridden for two years. Met him on road, he said: 'My illness nearly killed me. Initial Qi-regulating/phlegm-resolving drugs failed. Evening drowsiness, near death. Family brewed Shiquan Dabu Tang; clarity returned. Daily use allowed rising with cane after months, then walking unaided. *Classic*: 'Where evil gathers, Qi is deficient.' I treated deficiency, ignored evil, evil departed. Hence survival.' I said: 'True? Had he continued Qi-regulating/dispersing drugs, he would be dead. Yet he stuck to fixed formulas, lacking flexibility/adaptation, hence slow result. I would have done better.'" (Cheng Yunlai, *Yiyan Zhiyan*.)
Based on Cheng, only large-dose Ginseng is needed; add Bamboo Sap/Ginger Juice for Phlegm. Using Siwu, Erchen, Danxing, Tianma is grossly wrong.
Yu Dongfu said: "For hemiplegia, ancients said Left=Blood Def, Right=Qi Def. Yu's theory clarified this. Seasonal formula changes and extremity needling are clever. Recalling *Neijing·Shengqi Tongtian Lun*: 'Wind is start of hundred diseases... even great wind/toxin cannot harm if flesh/pores are closed.' Also: 'Wind/Rain/Cold/Heat cannot injure without deficiency.' And: 'Deficient evil wind meets deficient body, then lodges.' Clearly, deficiency precedes wind strike. *Lingshu* adds: 'True Qi departs, Evil remains alone, causing hemiplegia.' Hard to treat, summarized in two phrases. *Tongping Xuushi Lun*: 'Treat Xiao-Dan, sudden falls, hemiplegia, flaccidity/Jue, Qi-fullness/reversal. Obese/noble people suffer from rich food/grease.' This implies Phlegm-Damp-Heat, Yin/Yang deficiency, not just wind strike. Obese/noble avoid wind; rich food generates internal wind. Liu, Li, Zhu families realized this. Treat root. Falls/Hemiplegia root in deficiency. Reading Liu Zongnong's *Yuji Wei Yi* and Wang Yutai's *Linglan Yaolan*, I confirm: Northern regions have True Stroke; Jiangnan has Similar Stroke. Miao Zhongchun argued True Yin deficiency causes heat, boiling fluids..."
