During pregnancy, if one experiences a startled heart, timidity, restlessness, and discomfort, it is termed "Zi Fan" (Fetal Agitation). The primary treatment is Zhuye Antai Yin (Bamboo Leaf Fetus-Calming Decoction). Generally, this arises from deficiency-heat in the Heart and Lung meridians, causing mental disturbance and vexation. Alternatively, phlegm accumulation in the chest may lead to salivation, aversion to food, and vexation; in severe cases, it causes fetal movement insecurity. For deficiency-heat in the Heart and Lung, or phlegm accumulation in the middle jiao, Qianjin Zhuli Tang (Thousand-Gold Bamboo Sap Decoction) is indicated. If vomiting is severe, accompanied by fetal movement insecurity, vexation, dry mouth, insomnia, and excessive salivation, despite no external heat sensation but feeling five-center heat (heat in the palms, soles, and chest), or feeling heat at night but not during the day, Jiawei Zhuye Tang (Modified Bamboo Leaf Decoction) is suitable. In cases of Qi deficiency, double the dosage of Renshen (Ginseng Radix et Rhizoma); for Qi stagnation, Zisu Yin (Perilla Decoction); for Phlegm stagnation, Erchen Tang (Two-Aged Decoction) with Baizhu (Atractylodis Macrocephalae Rhizoma), Huangqin (Scutellariae Radix), and Zhike (Aurantii Fructus); for hypochondriac fullness and alternating chills and fever, Xiaochaihu Tang (Minor Bupleurum Decoction); for Spleen and Stomach weakness, Liujunzi Tang (Six-Gentlemen Decoction) with Zisu (Perillae Folium) and Shanzhi (Gardeniae Fructus). Regarding irritability and dry mouth during pregnancy, it occurs because the Qi of the Foot-Taiyin Spleen meridian connects to the mouth, and the Qi of the Hand-Shaoyin Heart meridian connects to the tongue. Visceral Qi deficiency allows Heat to invade the Heart and Spleen, drying up body fluids, resulting in vexation and dry mouth. This is largely similar to Zi Fan. Jiajian Shenmai Tang (Modified Ginseng and Ophiopogon Decoction) is appropriate, though Zhimu Wan (Anemarrhenae Pill) has also proven effective. If Liver Meridian Fire is active, Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) is used. If Kidney Meridian Fire is active, Dihuang Wan (Rehmanniae Pill) is indicated, modified according to symptoms. For pregnant women with dry mouth and inability to lie down, Yiwai Huanglian San (Single Coptis Powder) or Zunsheng Jiawei Antai Yin (Respect-Life Modified Fetus-Calming Decoction) may be used. Medical texts record cases of sudden thirst in women, cured only by drinking Wuwei Zhi (Five-Flavor Juice), which is due to blood condensing to form the fetus and harmonious pulse conditions, not a disease. However, Fire in the Lung and Kidney meridians causes vexation when entering the Lung and restlessness when entering the Kidney. Since the fetus is rooted in the Kidney, Kidney Water nourishes the fetal origin; if insufficient to control Kidney Fire, the Fire scorches Lung Metal, leading to irritability. This indicates a deficiency of Metal and exhaustion of Water. The treatment should focus on nourishing the source of transformation, clearing Metal to protect the Lung, and strengthening Water to nourish the Kidney.

Zhimu Wan (also known as Yimu Wan): Treats fetal insecurity caused by medication during pregnancy, presenting with symptoms similar to deficiency-vexation and insomnia, referred to as Zi Fan by Chao Yuanfang. Physicians unaware of this may treat it as deficiency-vexation, thereby damaging the fetal Qi.

Usage: Grind into fine powder, form pills with Jiarou (Zizyphi Fructus pulp) the size of a bullet. Take one pill per dose, dissolved in Renshen Tang (Ginseng Decoction).
Yiwai Huanglian San: Treats maternal dry mouth and inability to lie down.
