Postpartum hemorrhage with massive bleeding requires examination of the blood color (red or purple) and assessment of the patient's physical constitution (deficiency or excess). If the volume is large, the color is dark purple, and clots are present, this indicates stasis (blood stasis) that should be expelled. If there is merely residual lochia causing pain, it does not constitute metrorrhagia; however, if bright red blood flows profusely, it results from fright injuring the Heart (Zang-fu organ), causing failure to govern the vessels; anger injuring the Liver (Zang-fu organ), causing failure to store blood; or overexertion injuring the Spleen (Zang-fu organ), causing failure to control blood within the meridians. This condition should be treated as metrorrhagia. Initially, administer Shenghua Tang (Life-Birth Decoction) for several doses; this promotes blood circulation while simultaneously tonifying, allowing the blood to settle and flourish. If signs of collapse appear, such as pallor, sweating, or shortness of breath, increase the dosage of Renshen (Ginseng Root) in Shenghua Tang to boost Qi (vital energy); thus, Yang generates Yin, and blood flourishes. This cannot be stopped by astringents like Zonghui (Cinnabar Ash). If metrorrhagia occurs half a month after delivery, Shengju Dabu Tang (Great Tonification and Lifting Decoction) is appropriate. (The formula is recorded in Volume VIII.)
Shenghua Zhibeng Tang: Treats postpartum metrorrhagia.
Chuanxiong Rhizome (1 qian) Danggui Radix (2 qian, tail removed) Dried Ginger (4 fen) Peaches Seed (10 grains) Jingjie (5 fen, stir-fried) Prepared Licorice Root (5 fen)
Use Jujube Fruit as a guide, decoct in water.
Avoid ginger, pepper, hot-natured foods, and raw/cold foods.
For profuse bright red bleeding, add Jingjie Sui (Schizonepeta Spike, 1 qian, stir-fried) and Baizhi (Angelica Dahurica Root, 5 fen). For clots without pain accompanied by signs of collapse, add Renshen (Ginseng Root, 3 qian). For excessive sweating and shortness of breath, add Renshen (Ginseng Root, 3–4 qian). For no sweating, no collapse, and no shortness of breath, simply administer more Shenghua Tang (Life-Birth Decoction) until the blood settles. Do not claim that Chuanxiong and Danggui promote blood circulation and exacerbate hemorrhage; this is erroneous.
Zirong Yiqi Zhibeng Tang

For excessive sweating, add Ma Huang Gen (Ephedra Root, 1 qian) and Xiaomai (Triticum Vulgare Grain, a pinch). For constipation, add Congrong (Cistanche Stem, 1 qian). For heat symptoms, add Muxiang (Aucklandia Root, 1 fen, ground juice). For phlegm, add Beimu (Fritillaria Bulb, 6 fen), Zhuli (Bamboo Sap), and Jiangzhi (Ginger Juice). For cold cough, add Xingren (Apricot Seed, 1 qian), Jiegeng (Platycodon Root, 1 qian), and Zhimu (Anemarrhena Rhizome, 1 qian). For palpitations due to fright, add Zaoren (Sour Jujube Seed, 1 qian) and Baiziren (Biota Seed, 1 qian). For food stagnation from rice/noodles, add Shenqu (Medicated Leaven, 1 qian) and Maiya (Sprouted Hordeum Grain, 1 qian). For food stagnation from meat, add Shanzha (Hawthorn Fruit, 1 qian) and Sharen (Amomum Fruit, 8 fen). For internal heat due to deficiency, avoid Huangqin (Scutellaria Root), Huanglian (Coptis Rhizome), and Huangbai (Phellodendron Bark). Constipation contraindicates Dahuang (Rhubarb Root). For stagnation due to diet or anger, do not use solely draining/depleting herbs without tonifying agents. For elderly or deficient patients suffering from metrorrhagia, Shengju Dabu Tang (Great Tonification and Lifting Decoction) is appropriate. (Details in Volume VIII.)
