Postpartum, due to anger causing qi rebellion and stagnation, there is fullness and oppression in the chest and diaphragm, and severe pain from blood stasis masses. It is appropriate to use Shenghua Tang (Life-Birthing Decoction) with 3 fen (approx. 1g) of ground Muxiang (Aucklandiae Radix) taken orally with water. This will soothe the qi, dissipate anger, and allow the mass to resolve spontaneously. If one exclusively uses aromatic and qi-moving herbs such as Xiangfu (Cyperi Rhizoma), Sharen (Amomi Fructus), Zhishi (Aurantii Fructus Immaturus), or Wuyao (Linderae Radix) to scatter qi and dissolve masses, it will instead deplete primordial qi and exacerbate the fullness and oppression. If food is consumed immediately after anger, leading to dietary stagnation due to deficient stomach qi, one must examine what specific food was ingested. If damaged by meat, add Sharen (Amomi Fructus) and Shanzha (Crataegi Fructus). If damaged by rice or grains, add Shenqu (Massa Medicata Fermentata) and Maiya (Hordei Fructus Germinatus). If pain is caused by cold foods with stagnation in the hypochondrium and abdomen, add Rougui (Cinnamomi Cortex) and Wuyu (Evodiae Fructus) to dispel cold and alleviate pain. Do not take Binglang Wan (Areca Seed Pill) for moving qi and digesting food.
Jianpi Huaqi Yin (Spleen-Strengthening and Qi-Transforming Decoction)
Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Danggui (Angelicae Sinensis Radix) (each 2 qian); Chuanxiong (Ligustici Chuanxiong Rhizoma) (1 qian); Jiangtan (Zingiberis Rhizoma Carbonisatum), Zhigancao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle) (each 4 fen).
Decoct in water. If the pain from the mass persists, continue using Shenghua Tang (Life-Birthing Decoction) with 3 fen of ground Muxiang (Aucklandiae Radix) taken orally. Adding 8 fen of Rouguixin (Cinnamomi Cortex Interior) to the formula is even better, as Cinnamon can pacify the Liver and benefit qi flow.
Postpartum physical fatigue injures the Spleen qi; therefore, rich and heavy flavors should not be consumed excessively. If ingested, although the Stomach may accept a small amount, the Spleen's transportation function becomes stagnant, resulting in epigastric fullness, belching, and acid regurgitation. Treatment should involve warming and tonifying qi and blood, strengthening the Spleen and assisting the Stomach, supporting the righteous qi while simultaneously promoting digestion. One must also identify the specific offending food and assist with digestive aids. Thus, when Spleen qi recovers and transportation resumes normally, and stagnant matter moves, the Stomach can resume its function of receiving grain. Excessive use of digestive agents will inevitably lead to an inability to eat, making the condition difficult to treat. In such emergencies, Changsheng Huoming Yin (Long-Life and Life-Saving Decoction) should be used to rescue the patient.
Jiawei Shenghua Tang (Modified Life-Birthing Decoction)
Chuanxiong (Ligustici Chuanxiong Rhizoma) (1 qian), Danggui (Angelicae Sinensis Radix) (2 qian), Shanyao (Dioscoreae Rhizoma) (1 qian), Paojiang (Zingiberis Rhizoma Praeparatum), Zhigancao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle) (each 5 fen).
If undigested food appears in stool, add one roasted Roudoukou (Myristicae Semen). For pain from masses, add Taoren (Persicae Semen). Once pain ceases, add Baizhu (Atractylodis Macrocephalae Rhizoma). For deficiency, add Renshen (Ginseng Radix et Rhizoma). The treatment method for the specific offending food is as described above. If fever is present, cold-natured medicines are contraindicated. If epigastric fullness is present, qi-depleting medicines are contraindicated.
Changsheng Huoming Yin (Long-Life and Life-Saving Decoction)
Renshen (Ginseng Radix et Rhizoma) (2 qian).
Decoct in two bowls of water until reduced to eight-tenths. Add Guojia mo (powdered scorched rice crust) and take orally. If poverty prevents the use of Ginseng, use only powdered scorched rice crust mixed with rice paste. Alternatively, decoct Baizhu (Atractylodis Macrocephalae Rhizoma) in water and take orally. This may help stimulate appetite.
