At the Great Cold solar term, the Jueyin Wind-Wood qi begins to be dominated by Water qi. At the Spring Equinox, Shaoyin Minister-Fire qi occupies the second position. At the Grain in Ear solar term, Shaoyang Minister-Fire qi is in the third position. At the Great Heat solar term, Taiyin Damp-Earth qi is in the fourth position. At the Autumn Equinox, the fifth position of Yangming Dryness-Metal qi manifests as dryness. At the Light Snow solar term, the sixth position of Taiyang Cold-Water qi manifests as deficiency-cold. If physicians cannot understand these principles, reading five carts of books will be a waste of time.
The first qi is dominated by Jueyin Wind-Wood before the beginning of the year. The second qi is Shaoyin Minister-Fire, and the third qi is Shaoyang Minister-Fire, closely connected. The fourth qi is Taiyin Damp-Earth, with a nature often damp. The fifth qi is Yangming Dryness-Metal, with a nature of dryness. The sixth qi is Taiyang Cold-Water, with a nature of cold.
Entering the fourth qi each year involves the change of guest qi. There are ten or more types of guest qi rotating at the upper level, while the lower level corresponds to the host qi. Host qi and guest qi interact; their victory or defeat determines the weakness or strength of the disease. The first qi is Jueyin Wind-Wood, the second qi is Shaoyin Minister-Fire, the third qi is Shaoyang Minister-Fire, the fourth qi is Taiyin Damp-Earth, the fifth qi is Yangming Dryness-Metal, and the sixth qi is Taiyang Cold-Water.
In Zi and Wu years, Shaoyin Minister-Fire governs the Heaven (Si Tian). In Mao and You years, Yangming Dryness-Metal governs the Heaven. In Chen and Xu years, Taiyang Cold-Water governs the Heaven. In Chou and Wei years, Taiyin Damp-Earth governs the Heaven. In Yin and Shen years, Shaoyang Minister-Fire governs the Heaven. In Si and Hai years, Jueyin Wind-Wood governs the Heaven.
For example, in a Jia-Chen year, the Heavenly Stem Jia transforms into Earth qi. If the Kidney Meridian is diseased, in the ninth month, Jia-Xu month, Earth qi is also dominant. If one further encounters Jia-Ji days or Chou-Wei hours, death may occur. Other years follow this pattern for calculation.
Pulse diagnosis is divided into three positions, where Yin and Yang influence each other. Ying (Nutritive) and Wei (Defensive) Qi and Blood exist within the human body. Respiration enters and exits, Qi moves up and down within the body, nourishing breath spreads throughout, and Body Fluids flow smoothly. Movements change according to seasons, presenting corresponding images. In Spring, the pulse is Xian (Wiry); in Autumn, it is Fu (Floating); in Winter, it is Chen (Deep); in Summer, it is Hong (Surging). Observing complexion and pulse, sizes differ. Within one Shichen (two-hour period), pulse changes are unpredictable. Differences between Cun, Guan, and Chi positions occur; pulses may be short or long, with upper and lower parts disordered, appearing present or absent. Diseases alter facial expressions, advancing or retreating irregularly. One becomes confused and loses discipline. It is hoped that you will state them fully so that you may clearly understand the origin of the Dao you asked about. Pulse has three positions: Cun, Chi, and Guan. Ying and Wei Qi circulate without violating standards. Kidney pulse is Chen (Deep), Heart pulse is Hong (Surging), Lung pulse is Fu (Floating), Liver pulse is Xian (Wiry). Follow these constants, losing not a hair's breadth. Entry and exit, rising and descending, rotate like water clock measurements. Under two ke (48 minutes), one cycle completes. Returning to the Cun Kou (Radial Artery), deficiency and excess become visible. Changes multiply, Yin and Yang interfere. Wind causes Xu (Empty) and Fu (Floating) pulses; Cold causes Lao Jian (Tight/Firm) pulses. Sinking and submersion indicate water accumulation; Zhi Yin (Fluid Retention) causes Ji Xian (Rapid and Wiry) pulses. Dong (Moving) pulse indicates pain; Shuo (Rapid) pulse indicates heat and vexation. If there are discrepancies, know the cause of change; different pulses in the three positions indicate different disease origins. Excess is strange, deficiency is also notable. Evil Qi does not enter empty spaces; if it enters, it must encounter resistance. Examine exterior and interior, distinguish the San Jiao (Triple Burner). Know its combinations, carefully diagnose, estimate Zang-Fu organs, relying solely on Divine Spirit (Shen). For the sake of descendants, do not transmit to unworthy persons.
Left Cun corresponds to Heart and Small Intestine; Left Guan corresponds to Liver and Gallbladder; Left Chi corresponds to Kidney and Bladder. Right Cun corresponds to Lung and Large Intestine; Right Guan corresponds to Spleen and Stomach; Right Chi corresponds to Mingmen (Gate of Life) and San Jiao (Triple Burner).
