Danxi stated: The Wuji San (Five Accumulations Powder) from Ju Fang treats postpartum pain caused by residual blood. It uses Cangzhu (Rhizoma Atractylodis Lanceae) as the sovereign herb, Mahuang (Herba Ephedrae) as the minister herb, Houpo (Cortex Magnoliae Officinalis) and Zhishi (Fructus Aurantii Immaturus) as the assistant herbs. Although it contains blood-nourishing herbs like Dang Gui and Shaoyao, their quantity is only one-third of that of Cangzhu. One fails to consider what pathogenic cold exists in postpartum women? At this stage, Qi and Blood are not yet replenished, making sweating therapy seemingly inappropriate. Even if one argues that the drug properties are mild and can promote renewal, how can one use the fierce nature of Mahuang combined with the dispersing actions of Cangzhu, Zhishi, and Houpo? This leads to deficiency upon deficiency, bringing disaster swiftly.
Appendix [Itching in the Intestines]
To treat unbearable itching in the intestines after childbirth: Place a small bag containing needles and thread under the sleeping mat, without letting others know. Another prescription: Place arrow feathers and [missing character] under the sleeping mat, without letting the puerpera know.
[Da Lun] Postpartum distending and Qi-stagnation pain in both hypochondriac regions is due to retained fluid previously existing in the Bladder. Because lochia is not completely discharged after childbirth, fluid accumulates, causing obstruction where Qi and fluid clash and accumulate in the Bladder. Thus, distension and fullness occur in the hypochondriac region. The agitation between Qi and fluid causes pain.
[Xue Ji] For the aforementioned syndrome, if it is Blood stasis in the Liver channel, use Xiansu San (Corydalis Powder). If it is Qi stagnation in the Liver channel, use Sijun Zi Tang (Four Gentlemen Decoction) added with Qingpi (Pericarpium Citri Reticulatae Viride) and Chaihu (Radix Bupleuri). If it is Blood deficiency in the Liver channel, use Siwu Tang (Four Substances Decoction) added with Renshen (Radix Ginseng), Baizhu (Rhizoma Atractylodis Macrocephalae), and Chaihu (Radix Bupleuri). If both Qi and Blood are deficient, use Bazhen Tang (Eight Treasures Decoction) added with Chaihu (Radix Bupleuri). If Kidney Water is insufficient to nourish Liver Wood, use Liuwei Wan (Six-Ingredient Pill with Rehmannia). If Lung Metal is excessive and suppresses Liver Wood, use Xiebai San (Drain White Powder). Refer to the preceding discussions for primary treatments. A puerpera suffered from distending and painful hypochondriac regions due to anger, with severe hematemesis, fever, aversion to cold, and chest/abdominal distending pain. I considered this a case of dual deficiency of Qi and Blood. Using Bazhen Tang (Eight Treasures Decoction) added with Chaihu (Radix Bupleuri), Mudanpi (Cortex Moutan), and Paojiang (Rhizoma Zingiberis Preparatum), the bleeding stopped immediately. Subsequently, using Shiquan Dabu Tang (Complete Great Tonifying Decoction), the chills and fever gradually subsided. In this syndrome, unless warm and acrid herbs like Jiang (Zingiberis Rhizoma) and Gui (Cinnamomi Cortex) are used to assist the Spleen and Lung in promoting the action of the medicines, not only would efficacy be lost, but the distension would be exacerbated.
