[Commentary by Zeng] Fetal epilepsy refers to a condition where, prior to birth, the fetus is startled within the womb, or the mother consumes excessive amounts of sour and salty foods, or is disturbed by the Seven Emotions (Joy, Anger, Worry, Pensiveness, Sorrow, Fear, Fright), thereby damaging the fetal Qi. It manifests in infants within one hundred days after birth. During an attack, symptoms include restlessness of the Heart, slightly yellowish complexion, rebellious Qi with phlegm surging, upward deviation of the eyes, opisthotonos (body bent backward), and inability to cry. Initially, Shen Su Yin (Ginseng and Perilla Decoction) is used for harmonization, followed by intermittent administration of Bu Jing Dan (Non-Startle Pill) or Hu Po Bao Long Wan (Amber Dragon-Hugging Pill). Mild cases can be cured, while severe cases are difficult to fully recover from. Shen Su Yin treats Fever; Bu Jing Dan treats Convulsions due to Startle; Hu Po Bao Long Wan treats Convulsions due to Startle.
When a newborn’s body resembles burns from boiling water or fire, this is caused by the wet nurse consuming excessive rich and greasy foods. The wet nurse should take Qing Wei San (Stomach-Clearing Powder) (see Formula in Miscellaneous Diseases, Teeth Section) and Xiao Yao San (Free and Easy Wanderer Powder) (see Formula in Gynecology, Deficiency-Labor Section) to clear heat from Qi and Blood; the infant may also drink a few drops of the decoction. If the infant lacks skin on the body and it is not red, this is due to insufficiency of the Mother’s Spleen Qi; rice flour can be applied externally. If the area is red and hot, this is due to excess Fire in the Mother’s Stomach; gypsum can be applied externally. The Classics state: "The Spleen governs muscles, and the Lungs govern skin and hair." Thus, we know the disease involves the Spleen and Lungs. For example, sores on the forehead result from the combination of Fire and Earth (Spleen/Stomach), forming damp-heat that flows downward and attacks Kidney Water; this is difficult to treat. Sores on the feet indicate Yin deficiency with hyperactive Fire; such infants will die before reaching five years of age. If, within one month of birth, the infant exhibits mouth puckering (mouth closing like a fish's mouth), clenched fists, and flaccid weakness of the waist, this is caused by pathogenic factors prevailing over righteous Qi in the Liver and Kidneys; death is inevitable within three days. If male infants have fingers curling inward and female infants have fingers curling outward, the condition is treatable; however, if the eyebrows are red, it is untreatable. Treatable cases are managed with Quan Xie San (Scorpion Powder) (see Formula in Mouth Lock Section) and Diao Teng San (Uncaria Decoction) (see Formula in Slow Convulsion Section), among others. If the condition arises from damage to Stomach Qi due to major illness, leading to weakness of all Zang organs, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) (see Formula in Emaciation Section) and Qian Shi Di Huang Wan (Master Qian's Rehmannia Pill) (see Formula in Kidney Section) should be used. If the face and lips are red, this indicates insufficiency of Kidney Water, with Yin deficiency in the Liver Channel causing internal generation of Wind-Heat; treatment should focus on nourishing Kidney Water to control hyperactive Yang. Flaccidity of the body results from insufficient innate Qi and unfirm muscles; treatment should follow the principles for "Five Flaccidities" (Wu Ruan). The Tai Feng (Fetal Wind) described in *Tai Ping Sheng Hui Fang* was originally confused with Tai Jing (Fetal Startle); therefore, it is not adopted here.
