For infantile acute convulsion (Jing Kong), moxa the Qian Ding (GV 20) acupoint once, three moxa cones. The method for locating this point is one cun anterior to Bai Hui (GV 20). If the condition does not improve, moxa the area between the two inner canthi, as well as Ren Zhong (GV 26) located below the nose. The moxa cone should be the size of a wheat grain. For infantile chronic convulsion (Man Jing), moxa Chi Ze (LU 5) acupoint, three moxa cones per side. Locate it in the middle of the transverse crease of the elbow, approximately at the arterial pulse, with the moxa cone the size of a wheat grain. If convulsions occur during sleep, moxa the distal ends of the first and second toes of the foot, about the width of a leek leaf away from the nail, one moxa cone each. If opisthotonos (Jiao Gong Fan Zhang) and body stiffness occur, moxa three fen above the hairline on the nose bridge, three moxa cones; then moxa the interspace below Da Zhui (GV 14), three moxa cones. If startled during sleep with eyes unable to close, moxa three fen in the middle of the posterior transverse crease when the elbow is flexed, one moxa cone each.
[Qian Yi] Severe teeth grinding indicates the onset of convulsion. Eyes staring straight ahead with cyanotic complexion and body arching backward indicate impending convulsion. Yawning with a cyanotic complexion indicates wind-stroke (Jing Feng). Yawning with a sallow complexion indicates spleen-deficiency fright. Red eyes accompanied by cyanosis indicate spasms.
In convulsive epilepsy, when boys have spasms, their eyes look to the left without sound, and to the right with sound. When girls have spasms, their eyes look to the right without sound, and to the left with sound. This is due to the principle of mutual overcoming [in Five Elements]. There are also specific manifestations during attacks. (Mr. Jiegu stated: Boys belong to Wood, so looking left towards the Wood position produces no sound; looking right towards the Metal position causes Metal and Wood to clash, producing sound. Girls belong to Metal, so looking right towards the Metal position produces no sound; looking left towards the Wood position also causes clashing and sound.) The son of Li Sicheng was three years old when he had spasms lasting from Mao hour to Si hour, with eyes looking to the right and crying loudly. Qian Yi observed and said this is a contrary pattern. Boys are Yang; originally, looking left without sound is the favorable pattern, while looking right with sound is unfavorable. The reason is that the left corresponds to Liver Wood and the right to Lung Metal. In an unfavorable pattern, the two organs battle, Metal and Wood clash, resulting in sound. Treatment should involve draining the excess and tonifying the deficiency. Suppose a girl has spasms and looks to the left; this indicates Lung Qi invading Liver Wood, which cannot withstand it, hence the crying. One should drain the Lung, then treat the Heart, and subsequently treat the Liver. If the disease occurs in Autumn (similarly in the evening hours when Lung Metal is prosperous), one should strongly drain the Lung. If the disease occurs in Spring (similarly in the morning hours), when Liver Wood is prosperous yet still fails to overcome Lung, it indicates Lung is strong and Liver Fire is weak; one should tonify Liver and Kidney, and strongly drain the Lung. If a boy has spasms and looks to the right, this indicates Liver Wood overcoming Lung Metal, causing crying; one should drain Liver and Heart. If the disease occurs in Spring and Summer (similarly at noon when Liver and Heart are prosperous), one should strongly drain the Liver. If the disease occurs in Autumn and Winter (similarly in the afternoon hours when Lung Metal is prosperous) yet still fails to overcome Liver, it indicates Liver is strong and Lung is extremely deficient; one should tonify the Lung and strongly drain the Liver. The reason for retroversion of the eyes is that the Liver governs the eyes. All spasms result from Wind-Heat battling internally. Wind belongs to the Liver, thus manifesting externally in the eyes. Since this case presents a male patient with female symptoms, it is slightly easier to treat than the female pattern. First, drain the Lung using Xie Fei Tang (Lung-Draining Decoction). If there is no restlessness after two days, the disease recedes. Then use Di Huang Wan (Rehmannia Pill) to tonify the Kidney. After three doses, use Xie Qing Wan (Green-Draining Pill) twice each to drain Heart and Liver. The patient recovered in five days. Furthermore, why not drain the Lung if it is deficient? The answer is: suppose a boy looks to the right, indicating Wood overcoming Metal, with Liver excess overcoming Lung. If one only drains the Liver, and especially if it is Spring or Summer when Metal Qi is extremely deficient, one must tonify the Lung and absolutely avoid draining it.
[Tang Shi] Generally, for spasms, boys looking left and girls looking right is the favorable pattern, easy to treat; boys looking right and girls looking left is the unfavorable pattern, difficult to treat. *Mai Jue Qi Meng* states: A rapid and urgent pulse in children indicates deficiency-fright. *Zhi Zhi* states: Floating, rapid, full, and tight pulses indicate acute convulsion. Deep, slow, scattered, and slack pulses indicate chronic convulsion. Cyanotic and purplish tiger-mouth vein patterns indicate wind-stroke. Red indicates mild wind-heat. Crimson indicates severe wind-heat. Purple indicates fright-heat. Green indicates fright-with-stagnation. Half green and half purple indicates both fright-with-stagnation and wind-heat, primarily acute convulsion. Green with pale purple, extending and retracting, indicates chronic convulsion. Intermixed faint purple, green, or black threads appearing but not fully emerging indicate chronic Pi-wind. Curving inward is favorable; curving outward is unfavorable.
