[Wan Quan] The disease of smallpox (Dou Chuang) is caused by fetal toxins (Tai Du) from the parents, accumulated in the Mingmen (Gate of Life). The Mingmen refers to the Ministerial Fire (Xiang Huo) of the Kidneys, which is the root of life and transformation in the human body; therefore, the toxin resides here. If one encounters mild weather in winter, causing a sudden leakage of Yang Qi, the person will be affected by this pathogen, stirring up the Ministerial Fire. When spring and summer arrive, times of growth and development, the toxin erupts. It is contagious, similar to epidemic diseases, known as Tian Xing Yi Li (Heavenly Epidemic Pestilence). For those who have not yet manifested smallpox or measles, if they feel the warmth of winter, prevention should be undertaken immediately. It is appropriate to take detoxifying medicines, such as Chen Sha San (Cinnabar Powder), San Dou Tang (Three Beans Decoction), and Dai Tian Xuan Hua Wan (Pill for Heaven's Declaration and Transformation), all of which can be used. Administer them frequently to reduce the toxicity of the rash, ensuring that the eruption is easy and recovery is smooth, avoiding complications such as toxin sinking inward, stagnation, or lingering. While Chen Sha San, San Dou Tang, and Dai Tian Xuan Hua Wan are suitable for resolving the toxins of seasonal epidemics, if the cause is insufficient essence and blood from the parents, or if the child has pre-existing conditions, or if symptoms of other Zang-Fu organs appear during fever, treatment must address these concurrently. One cannot rely solely on detoxification while forgetting the root. If the Spleen and Stomach are inherently weak, nourishing the Spleen should be prioritized over detoxification: take two-thirds Nourish Spleen Pill (Yang Pi Wan) and one-third detoxifying medicine. If the cause is excessive indulgence of the parents leading to accumulated toxins in essence and blood, and the child has inherent fetal diseases, both toxins should be resolved simultaneously by alternating Su Yuan Jie Du Tang (Decoction for Tracing the Source and Detoxifying) and Dai Tian Xuan Hua Wan. Zhu Danxi said: When smallpox or measles are just beginning to emerge or have not yet emerged, if one sees others in the vicinity suffering from the disease, one should take this medicine preventively to reduce severity. Take three inches of Luffa fruit (Si Gua) near the stem, including skin and seeds, burn to ash retaining its nature, grind into powder, mix with rock sugar, and consume dry. Adding Cinnabar powder (Zhu Sha) is even better.
San Dou Tang (Three Beans Decoction)
Vigna angularis (Chi Xiao Dou), Glycine max (Hei Da Dou), Phaseolus radiatus (Lu Dou) (each 1 sheng, raw); Glycyrrhiza uralensis (Xiao Gan Cao) (3 liang, raw and cut).
Wash the three beans thoroughly, boil together with Licorice in 8 sheng of snow water (if unavailable, use running long-flowing water) until the beans are cooked. Remove the Licorice, dry the beans, then soak them in the decoction again and dry them repeatedly until the liquid is absorbed. Daily, take these beans and boil in water, consuming as desired.

If fever has already started, grind with Ginger tea, cover thickly to induce sweating. This greatly reduces severity or prevents eruption entirely, with miraculous effects.
Yuan Shi's Pre-Eruption Treatment Methods: To treat smallpox, one must identify the syndrome before eruption and prescribe accordingly. Severe cases become mild, mild cases heal completely. However, if the disease resolves spontaneously before eruption, the physician gains no merit, so many are unwilling to exert full effort. By the time eruption occurs, it is too late. Benevolent physicians must prioritize saving lives and protecting infants. I would rather bear no credit than let an infant lose their life. Below are eighteen syndromes and their treatments.
Before eruption, if the child contracts Wind pathogen, internal heat burns, headache, spontaneous sweating, and persistent cough occur, and smallpox follows unresolved Cold Damage, this is called "Monkey Jumping Lock" (Yuan Hou Tiao Suo) by smallpox specialists. After Cold Damage, when Yuan Qi is depleted, nourish Yin and tonify Blood, clear heat and disperse Wind. Ziyin Sanbao San (Nourishing Yin Three Treasures Powder) can be taken.

Cut finely. Decoct with Ginger and Jujube.
If diet is unregulated, Summer-heat and Dampness are not guarded against, damaging the abdomen, causing frequent diarrhea, loss of appetite, and emaciation, followed shortly by smallpox eruption, this is called "Guanyin Sweeping the Seat" (Guan Yin Fo Zuo). This differs from diarrhea preceding eruption where toxin is reduced and eruption is favorable. Here, prolonged diarrhea has just healed, indicating Spleen deficiency and weak Yuan Qi. Pure tonics may cause false distension; cold medicines are strictly contraindicated. Only Spleen regulation is suitable, such as Si Zhi Bai Zhu San (Four-Processed Atractylodes Powder).
Si Zhi Bai Zhu San
Atractylodes macrocephala (Bai Zhu) 8 liang, divided into four parts: one part stir-fried with Amomum villosum (Sha Ren), one with Glutinous Rice (Nuomi), one with Bran (Fu Pi), one with Wall Soil (Bi Tu). Pick clean, grind to powder. Adjust dose by age, take with milk or wine.
After Malaria, wasting of heat and cold thins muscles. If eruption occurs upon recovery or alongside malaria, it is named "Horse Galloping on Sword Path" (Ma Chi Jian Dao). Often caused by Damp-Heat. Caosu (Tsooco Fructus) and Changshan (Radix Dichroiae) are absolutely forbidden. Even Bupleurum (Chai Hu) is a depredatory herb. At age five or six, if the child has weak constitution, fever, dry thirst, and cough, and smallpox erupts shortly after measles, this is a Taiyin Splein Meridian syndrome, called "Single Reed Crossing Sea" (Yi Wei Hang Hai). Unlike the normal order (measles then smallpox is smooth; smallpox then measles is reverse), this case involves weakness, fever, prior cough/thirst, and recent measles recovery followed by smallpox. The condition is critical. Need to tonify Yin, clear Lung, and fortify Spleen. Huang Qi (Astragalus) is strictly contraindicated. Neituo Zhiqi Tang (Inner-Supporting Extraordinary Decoction) can be used.
