When Fei Yong (Lung Abscess) has ruptured and wind pathogen invades, it is incurable. After expectorating pus from Fei Yong (Lung Abscess), if the pulse is short and choppy, spontaneous recovery occurs; if the pulse is floating and large, it is difficult to treat. If the complexion is white but turns red, this indicates Fire overcoming Metal, which is also incurable.
The *Jin Gui Fang Lun* states: "Heat in the Upper Jiao causes lung痿 (Fei Wei) due to coughing." How does one acquire Fei Wei (Lung Flaccidity)? It may arise from excessive sweating, vomiting, Xiao Ke (Wasting-Thirst disease) causing frequent urination, or constipation treated with harsh purgatives, leading to severe depletion of Jin Ye (Body Fluids). Thus, when the Cun Kou (Inch position) pulse is rapid, the person coughs, and there is instead turbid saliva and froth in the mouth, this is Fei Wei (Lung Flaccidity). If the mouth feels parched and dry, and coughing brings faint pain in the chest, while the pulse is slippery and rapid, this is Fei Yong (Lung Abscess) with expectoration of pus and blood. A rapid and deficient pulse indicates Fei Wei (Lung Flaccidity); a rapid and excess pulse indicates Fei Yong (Lung Abscess).
[Lou] This statement indicates that Fei Wei (Lung Flaccidity) belongs to Heat patterns. For example, chronic cough damaging the lungs, resulting in hoarseness and hemoptysis, belongs to Yin Deficiency with Fire Hyperactivity, which is correct. The original text treats Fei Wei (Lung Flaccidity) presenting with salivation without coughing, where the patient is not thirsty, inevitably experiences enuresis and frequent urination, because Upper Jiao deficiency fails to control the Lower Jiao; this is Cold in the Lungs, manifesting as dizziness and abundant saliva/sputum, treated with Zhi Gan Cao Gan Jiang Tang (Prepared Licorice and Dried Ginger Decoction), which belongs to Cold patterns. For Fei Wei (Lung Flaccidity) with salivation and a sensation of warmth in the heart, Zhi Gan Cao Tang (Prepared Licorice Decoction) is used, which tonifies Deficiency-Labor; this differs from tonifying Yin Deficiency with Fire Hyperactivity. All these require differentiated treatment. Therefore, Fei Wei (Lung Flaccidity) has both Cold and Heat variations.
[Jing] Labor injury damages Blood and Qi, making the Cou Li (interstices/pores) deficient, allowing Wind Pathogen to invade and accumulate internally in the Lungs. This presents with sweating, aversion to wind, coughing, shortness of breath, nasal congestion, neck stiffness, and chest/hypochondriac fullness. If prolonged and unresolved, it becomes Fei Wei (Lung Flaccidity).
[Bao] Fei Yong (Lung Abscess) is caused by consuming spicy, hot, roasted, or fried foods, or indulging in hot wine, leading to Dry-Heat injuring the Lungs. Treatment should be initiated early.
