Zhu Danxi stated: For carbuncles and sores occurring internally, including lung abscess (Fei Yong), liver abscess, kidney abscess, intestinal abscess, intrascrotal abscess, and deep bone abscess, only lung abscess requires first resolving the exterior syndrome. Now, we disperse it from the exterior.
The *Qianjin Fang* records: Coughing up pus and blood with a rapid and excess pulse indicates lung abscess. If there is coughing in the mouth followed by dull pain in the chest, and the pulse is slippery and rapid, this is lung abscess. The question arises: How do we know a patient with coughing and counterflow is suffering from lung abscess? Is death inevitable once pus and blood are expectorated? What is the pulse like? The answer is: The cun (inch) pulse is faint and rapid. Faintness indicates wind, rapidity indicates heat. A faint pulse implies sweating; a rapid pulse implies aversion to cold. Wind pathogen injures the Wei (Defensive) level, preventing exhalation from entering; Heat pathogen forces the Ying (Nutritive) level, preventing inhalation from exiting. Wind injures the skin and hair pores; Heat injures the blood vessels. When wind lodges in the Wei level, the person will cough, experiencing dry mouth, asthma, and fullness, dry throat without thirst, and frequent expectoration of turbid froth. There may be periodic chills. Wherever heat passes, blood becomes stagnant, accumulating and forming an abscess. The expectorated material resembles rice porridge. In the early stage, it can be saved; once pus has formed, it is often fatal. Another question: Chills and fever with a slippery and rapid cun pulse, while the patient's diet and daily activities remain normal, indicate a swelling. If the physician does not recognize this and treats it as a Cold Damage (Shanghan) disease without effect, how can one know pus exists? How can its location be distinguished? The Master said: If pus is in the chest, it is lung abscess, characterized by a rapid pulse and expectoration of pus and blood. If pus has not yet formed, the pulse is naturally rapid. If the tight quality disappears leaving only rapidity, it indicates that pus has formed.
Xue Ji commented: The Lung is the canopy of the five Zang organs, located in the chest, governing Qi, and manifesting on the skin and hair pores. Laborious strain damages Qi and Blood, making the interstices (Cou Li) loose, allowing external pathogens to invade and affect the Lung internally. Alternatively, excessive sexual activity depletes Kidney Water, causing deficient Fire to flare upward. Consumption of strong wine, roasted/fried foods, and pungent or rich flavors steams and insults the Lung. Excessive coughing and expectoration of phlegm, or excessive sweating and purging, severely damage Body Fluids. Symptoms include aversion to wind, coughing, nasal congestion, neck stiffness, chest and hypochondriac distension, difficulty breathing, dry throat with thirst, and in severe cases, slight swelling of the four limbs, and expectoration of pus and blood. If the sputum is foul and turbid, the pus is bloody and fetid, accompanied by dull chest pain, and the right cun pulse is rapid and excess, it is termed Lung Abscess (Fei Ju). If only frothy saliva is expectorated without pus, and the pulse is rapid but deficient, it is Lung Flaccidity (Fei Fei). For coughing with asthma and urgency, use Xiao Qing Long Tang. For coughing with chest distension, use Ting Li Da Zao Xie Fei Tang. For expectoration of pus and foul matter, use Jie Geng Tang. For coughing, asthma, shortness of breath, or scanty urination, supplement with Ren Shen Bu Fei Tang. For fatigue and poor appetite, supplement with Shen Shu Pi Wei Tang. For copious phlegm obstruction due to Kidney deficiency with water overflowing, use Liu Wei Di Huang Wan. For dry mouth and throat due to deficient Fire flaring upward, use Jia Jian Ba Wei Wan. These conditions all stem from Spleen Earth deficiency failing to generate Lung Metal, and Lung Metal failing to generate Kidney Water. Thus, in the early stage, it can be saved; once pus forms, death is common. If the pulse is faint, tight, and rapid, there is no pus. If the pulse is very tight and rapid, pus has formed. The *Nei Jing* states: "When blood is hot, flesh decays; when Ying and Wei do not circulate, pus must form." Generally, for Lung sores with prolonged expectoration of pus and blood resembling millet porridge, treatment is difficult. If expectoration of pus continues indefinitely, it is incurable. If vomiting of pus stops spontaneously, recovery occurs. A short and choppy pulse indicates self-healing. A complexion that should be white but appears red indicates Fire overcoming Metal, which is incurable. However, if one tonifies the Spleen and Lung and nourishes Kidney Water, there may be hope. The danger lies in solely attacking the sore, further weakening the Spleen and Stomach, leading to many errors.
Danxi treated a young woman who had a fistula opening in her chest, discharging pus and blood corresponding to what she coughed up. She was cured using Ren Shen, Dang Gui, and herbs for clearing heat and expelling pus. Another opinion suggested this was caused by Lung Flaccidity. Xiang Yanzhang treated a man with chest oppression, coma, and unconsciousness. He administered a decoction of Xing Ren and Yi Yi Ren, restoring consciousness immediately. Subsequently, he used Sheng Ma, Jie Geng, and Huang Qi to resolve the pus, curing him after more than a month. Xiang diagnosed this based on a floating and slippery Yangming pulse and a deficient Yin pulse. Floating indicates Fire; slippery indicates blood accumulation. Initially, wind injured the Lung, causing stagnation to lodge in the Lung. The deficient Yin pulse indicated excessive dispersal. Since all Qi originates in the Lung, regulating Lung Qi facilitates respiration and removes stagnation, thus curing the disease. Wang Shishan treated a woman near thirty years old, thin and pale, who occasionally coughed and had irregular menstruation. After cooling herself in summer, she suffered severe coughing for over a month, unable to lie down. Her sputum sometimes contained blood or pus, and severe coughing induced vomiting. Treatment with Huang Qin, Huang Lian, and Er Chen Tang failed. Further administration of tonics like Ren Shen worsened the condition. Wang observed a floating and slippery left pulse and a slightly weak and slippery right pulse. Given her childhood wrist injury limiting hand extension, the right pulse was deemed unreliable. Relying on the left pulse, he suspected pregnancy complicated by lung abscess. He prescribed lung-clearing and draining formulas. After three doses, she could lie down, stopped coughing up phlegm and pus, though she experienced occasional morning sickness. Wang explained this was normal in pregnancy and prescribed Yi Yi Ren, Bai Zhu, Fu Ling, Mai Dong, Huang Qin, and E Jiao. The condition improved. A month later, pulses were slightly slower and floating, indicating reduced heat. Due to significant hemoptysis damaging the Stomach, he prescribed Si Jun Zi Tang with Chen Pi, Huang Qin, and Zhi Ke. At six months gestation, eating chicken triggered a relapse, which resolved upon cessation. At nine months, symptoms recurred with hoarseness, managed with child's urine. Wang predicted a good outcome postpartum unless recurrence occurred. She delivered at term, but developed spleen/stomach issues with diarrhea and died.
Xue Lizhai treated a scholar with lung abscess, clear nasal discharge, coughing up pus and blood, and chest distension, attributed to external wind pathogen. He first used Xiao Feng San with Luan Fa Hui (two doses) to clear the nose, then Si Jun Zi Tang with Xiong Gui, and finally Jie Geng Tang to cure him. Later, due to overwork, he coughed up pus, had dribbling urination, and a yellowish-white complexion, indicating Spleen Earth failing to generate Lung Metal, and Lung Metal failing to generate Kidney Water. He was cured with Bu Zhong Yi Qi Tang and Liu Wei Di Huang Wan. Another scholar, a heavy drinker, presented with coughing pus, neck stiffness, and dull skin, indicating Spleen-Lung Qi deficiency with external pathogen invasion. He was cured with Jie Geng Tang followed by Ren Shen Bu Fei Tang. A man with coughing pus, abdominal distension, and surging-rapid pulses at both cun and right guan positions (indicating Fire failing to generate Earth, and Earth failing to generate Metal) was cured with Jie Geng Tang supplemented by Bu Zhong Yi Qi Tang. Another patient with intermittent cough, thirst, spontaneous sweating, fever, and frequent stools self-medicated with lung-clearing and diuretic herbs, resulting in urinary retention, red face, cracked lips, and phlegm obstruction. Pulses at Spleen, Lung, and Kidney sites were floating-large and rapid upon pressure, indicating deficiency of the three Yin meridians failing to generate each other. He needed to nourish the source of transformation. Ignoring advice, he continued diuretics and developed lung abscess, cured by me using Jie Geng Tang and Liu Wei Di Huang Wan. A man with red face, pus expectoration, fever, thirst, restlessness, and chaotic surging-rapid pulse was treated with large doses of Jia Jian Ba Wei Wan plus Mai Men Dong. Heat and thirst ceased immediately, followed by sleep. Upon waking, he felt refreshed and hungry. Two more doses reduced symptoms significantly. Maintenance with Ren Shen, Mai Men Dong, and Wu Wei Zi tea brought peace after a month. Red face requires tonifying Lung and Kidney; pale face requires tonifying Spleen and Lung. A woman with chronic blood deficiency, fever, and cough, worsened by phlegm-fire clearing herbs, presented with pus/blood expectoration, red face, and very rapid pulse, indicating formed pus with Qi/Blood deficiency. I used Ba Zhen Tang to tonify Yuan Qi and Jie Geng Tang for the lung condition, achieving gradual recovery. A woman with wind-cold invasion, treated with exterior-releasing herbs, developed coughing, asthma, poor appetite, chest discomfort, and constipation. Right cun/guan pulses were floating-rapid. I advised against diuretics, warning that Spleen deficiency failing to generate Lung Metal would worsen with fluid depletion, leading to lung abscess. Ignoring this, she developed severe deficiency and expectorated pus. I treated mornings with Yi Qi Tang and evenings with Jie Geng Tang, stopping the pus. Then mornings with the former and evenings with Shi Quan Da Bu Tang for over fifty doses, leading to recovery due to her compliance. A woman with cough, phlegm, chest pain, floating-slippery right cun/guan pulses, and neck tension was diagnosed with Spleen-Stomach heat generating phlegm, not an abscess. Cured with Er Chen Tang plus Shan Zhi, Bai Zhu, and Jie Geng. A woman with blood deficiency and internal heat coughing in winter had floating-surging chi pulses. I warned of potential lung disease. In spring, she coughed with surging-rapid cun pulses, indicating Heart Fire insulting Lung Metal, forming lung abscess with pus already present. Summer posed a risk. I used water-nourishing and spleen-tonifying herbs for slight improvement. Neglecting care, she died in mid-summer.

Wei Jing (3 sheng, cut), Yi Yi Ren (0.5 sheng), Dong Gua Ren (0.5 sheng), Tao Ren (50 kernels, peeled).
Boil 1 dou water with Wei Jing until 5 sheng remains. Strain, add kernels, boil until 2 sheng remains. Divide into five warm doses. Expectoration of porridge-like material occurs. (*Qian Jin Fang*: Lung abscess expects pus/blood. Wei Jing is coarse reed/grass from marshes.)
Jia Wei Xiao Feng San: Treats expectoration of pus/blood resembling lung abscess, bad breath, unresponsive to other formulas. Use Xiao Feng San mixed with male hair ash, taken with clear rice drink. Removes root cause in two doses. Also treats hematemesis. (Formula: Jing Jie, Chuan Xiong, Qiang Huo, Ren Shen, Fu Ling, Jiang Can, Fang Feng, Huo Xiang, Chan Tui 2 qian each; Hou Po, Chen Pi 0.5 liang each, powdered.)

Uses Jie Geng, Bei Mu, Chuan Gui, Gua Lou Ren, Zhi Ke, Yi Yi Ren, Sang Bai Pi, Fang Feng (1 liang each); Sheng Gan Cao, Xing Ren, Zheng Bai He (0.5 liang each), Huang Qi (1.5 liang). Prepare and chop. Take 4 qian, decoct with 1.5 bowls water and 5 slices ginger until 80% remains. Strain. Add Da Huang for constipation, Mu Tong for scanty urine.
Zhong Jing Jie Geng Bai San: Treats cough with chest fullness, chills, rapid pulse, dry throat/no thirst, foul turbid sputum, and long-term expectoration of pus like porridge, indicating lung abscess.
Jie Geng, Bei Mu (3 fen each), Ba Dou (peeled, fried, ground to paste, 1 fen).
Make powder. Strong adults take half qian-bi with water; weak patients reduce dose. Lesions above diaphragm cause expectoration of pus/blood; below cause diarrhea. If diarrhea persists, drink one bowl cold water to stop. (This essential formula was adopted by Danxi for subtle reasons.)
*Liu Lei Yuan Rong* suggests Tou Zhi (expectorant) Sou Feng Tang for lung abscess.
Mu Dan San (*Benshi Fang*): Treats lung abscess with foul-smelling pus/blood expectoration and pain in chest/breast.
Mu Dan Pi, Chi Shao Yao, Di Yu, Ku Geng, Yi Yi Ren, Chuan Sheng Ma, Huang Qin, Sheng Gan Cao (1.5 qian each).
One dose, boil 2 zhong water to 1 zhong. Take away from meals. Alternative: Remove Huang Qin, add ginger, name Sheng Ma Tang.

One dose, boil 2 zhong water to 0.8. Sip slowly over ten+ doses, do not take quickly.
[Xue Ji] Comment: This formula clears fire and detoxifies. Effective for fever, irritability, thirst, and surging-large pulse. For rapid pulse, foul sputum, or pus/clots, use Jie Geng Tang. Generally, lung abscess with surging-large lung pulse or continuous pus expectoration is difficult to treat. Self-stopping pus with short/choppy pulse indicates healing. Red face (Fire overcoming Metal) is incurable.
