[General Principle]
"Deficiency" refers to the emptiness and insufficiency of Qi and Blood. "Damage" refers to the impairment of Zang-Fu organs. (From *Shengmo*) Generally, the soaring and reckless movement of Fire in the Five Zang organs caused by the Seven Emotions (Joy, Anger, Worry, Pensiveness, Sorrow, Fear, Fright), as well as excessive sexual desire in men and women, are all causes of deficiency and damage. (From *Zhengchuan*)
[Internal Causes]
Due to the deficiency and damage of the Three Yin Meridians of the Foot (Liver, Spleen, Kidney), during the peak of summer when Metal energy is weak and Kidney Water is dried up, one fails to conserve and nourish oneself. When winter arrives and Yang Qi should be hidden, frequent intimacy with women occurs, or essence and blood are not yet full but early depletion happens, or Qi and Blood are still growing but mental labor causes loss, all these can lead to deficiency and damage. (From *Lizhai*)
[External Manifestations]
External symptoms include reduced appetite, physical fatigue, tidal fever, spontaneous sweating, body heat, cough, and pain in the lower back and hypochondrium. Men will experience spermatorrhea and nocturnal emission (night sweats); women will inevitably suffer from leukorrhea, metrorrhagia, or amenorrhea. The *Suwen* states: "Diseases of the Yangming Meridian originate from the Heart and Spleen; there are unspoken hidden grievances, and women will have no menstruation." This is what is meant. (From *Huibu*)
[External Causes - New Damage]
Contracting cold damages Yang Qi, progressing from top to bottom: First damage to the Lung results in flaccid skin and falling hair; second damage to the Heart results in non-nourished blood vessels; third damage to the Stomach results in food failing to transform into muscle and flesh. Contracting heat damages Yin fluids, progressing from bottom to top: First damage to the Kidney results in bone atrophy and inability to stand; second damage to the Liver results in lax tendons that cannot contract; third damage to the Spleen results in indigestion. (From *Nanjing*)
[Internal Causes - Chronic Deficiency]
Accumulated deficiency becomes damage; accumulated damage becomes exhaustion. If it persists for years without cure, it is called chronic deficiency. There are distinctions among Five Exhaustions, Six Extremes, and Seven Injuries. The Five Exhaustions correspond to the Five Zang organs: overuse of mental effort exhausts the Heart; striving in planning and thinking exhausts the Liver; excessive worrying about unexpected matters exhausts the Spleen; preoccupation with worry exhausts the Lungs; rigid adherence to moral principles exhausts the Kidneys. The Six Extremes correspond to the Six Fu organs: Extreme Blood results in sallow complexion, swollen throat, hair loss, and forgetfulness; Extreme Tendons result in spasms and convulsions, dark nails, and nail pain; Extreme Flesh results in emaciation, fatigue, and lethargy; Extreme Qi results in panting, coughing, shortness of breath, dry skin, and scorched hair; Extreme Bones results in dirty complexion, floating teeth, sore lower back, and spinal pain; Extreme Essence results in dim vision, tinnitus, seminal emission, turbid urine, and penile weakness. The Seven Injuries trace the origins of exhaustion: prolonged viewing damages Blood; prolonged lying down damages Qi; prolonged sitting damages Flesh; prolonged standing damages Bones; prolonged walking damages Tendons; sexual excess and mental labor damage the Heart and Kidneys. (From *Rumen*)
[Differentiating Causes]
Any deficiency of Yin and Yang is due to the failure of Water and Fire to interact harmoniously. (From *Rumen*) Internal deficiency of True Yin leads to flaming deficiency Fire, injuring Lung Metal, which fails to generate Kidney Water; Kidney Water dries up and fails to control Heart Fire; once Heart Fire flares, Kidney Fire follows, leading to dreams of emission and seminal loss. Once Lung Qi is deficient, the interstices become loose, leading to night sweats and spontaneous sweating. Fire agitates Blood, and Blood rises with Fire, leading to coughing and hemoptysis. (From *Zhangzhang*)
[Differentiating Symptoms]
Symptoms of deficiency and damage, investigated by name to find the substance, are merely differences between Qi deficiency, Blood deficiency, and Yin deficiency. Insufficiency of the Spleen and Lungs is Qi deficiency; insufficiency of the Heart and Liver is Blood deficiency; insufficiency of Kidney Water is Yin deficiency. (From *Zhangzhang*) Therefore, damage to the Heart and Lungs with exhausted complexion and profuse sweating indicates Yang deficiency; damage to the Kidneys and Liver with emaciated form and profuse sweating indicates Yin deficiency. (From *Jiyao*)
[Pulse Diagnosis]
Deficient pulses are often wiry. A wiry, large, and forceless pulse indicates Blood deficiency; a wiry, fine, and forceless pulse indicates Qi deficiency. Deep and fine indicates severe Qi deficiency; deep and choppy indicates severe Blood deficiency. Fine pulse at the Cun position and large pulse at the Chi position indicates Blood deficiency with Fire; floating, rapid, and hollow indicates Blood loss and isolated Qi. (From *Huibu*)
[Therapeutic Methods]
If Yang is excessive and Yin is insufficient, Tonify Yin; if Yin is excessive and Yang is insufficient, Tonify Yang. (From *Hezi*) For damage to the Lung, benefit its Qi; for damage to the Heart, tonify its Ying and Blood; for damage to the Spleen, regulate diet and adapt to cold and warmth; for damage to the Liver, soothe its interior; for damage to the Kidney, benefit its Essence. (From *Nanjing*)
