Women's irregular menstruation is caused by fatigue and injury to Qi and Blood, leading to a weak constitution susceptible to Wind-Cold. The pathogenic factor of Wind-Cold lodges in the interior of the uterus (Bao Gong), damaging the Chong Mai (Penetrating Vessel) and Ren Mai (Conception Vessel), as well as affecting the Hand-Taiyang Small Intestine Meridian and Hand-Shaoyin Heart Meridian. Both the Chong Mai and Ren Mai originate from the interior of the uterus and are regarded as the "Sea of Meridians." The Hand-Taiyang Small Intestine Meridian and the Hand-Shaoyin Heart Meridian are exterior-interior paired meridians; their functions include transforming substances upward into breast milk and downward into menstrual blood. Since menstrual blood is the surplus of the meridian system, when Cold and Heat are harmonious, the Qi of the Chong and Ren meridians becomes abundant, and the blood governed by the Hand-Taiyang and Hand-Shaoyin meridians flows freely and descends at regular intervals. However, if Cold and Warmth are imbalanced, the meridians become deficient, allowing Wind-Cold to invade. Pathogenic factors contend with the blood; under Cold conditions, blood coagulates, while under Warm conditions, blood dissipates. Consequently, menstrual volume fluctuates between excessive and scanty, constituting irregular menstruation.
Palpation of the Spleen pulse reveals a soft (Ru) quality upon deep pressure and a deficient (Xu) quality upon superficial pressure. The patient suffers from abdominal distension and fullness with irritability, heat in the Stomach, anorexia, indigestion, difficult defecation, bi-syndrome (obstruction) in the four limbs, and occasional numbness. These conditions arise from improper sexual activity, manifesting as amenorrhea or menorrhagia (excessive bleeding).
Furthermore, a choppy (Se) pulse in the Shaoyin position indicates blocked blood flow, known as "Ju Jing" (Resident Menstruation), where menstruation occurs once every three months. A faint (Wei) pulse indicates deficiency of both Qi and Blood; in young individuals, this is a pulse signifying blood loss. In nursing mothers or those suffering from diarrhea, such a pulse may be acceptable; otherwise, it also constitutes "Ju Jing," occurring once every three months. Regarding menstruation occurring twice a month: during the onset of menstruation, the pulse should normally appear regular. If it is instead faint, accompanied by dysmenorrhea/oligomenorrhea and absence of sweating, the menstruation will inevitably occur only once every three months.
The *Yangsheng Fang* states: Worry, anger, and weeping cause the stagnation of Yin and Yang Qi and disharmony, resulting in irregular menstrual volume (sometimes scanty, sometimes excessive), internal heat with severe thirst, poor complexion, emaciation of muscles, and heaviness of the body.
Irregular and obstructed menstruation is caused by fatigue injuring Qi and Blood, leading to a weak constitution susceptible to Wind-Cold. Wind-Cold lodges in the interior of the uterus, damaging the Chong Mai and Ren Mai, as well as the Hand-Taiyang and Hand-Shaoyin meridians. The Chong and Ren meridians are the Sea of Meridians, originating from the interior of the uterus. The Hand-Taiyang corresponds to the Small Intestine Meridian, and the Hand-Shaoyin to the Heart Meridian; these two are exterior-interior pairs, primarily governing the downward transformation into menstrual blood. When Wind-Cold lodges in the meridians and contends with Qi and Blood, blood becomes stagnant due to cold, causing menstruation to be obstructed and not flow smoothly.
Palpation of the pulse: If pathogenic Qi enters from the Cun Kou (Radial Artery) and moves upward, it is named "Jie Mai" (Loose Pulse). The pulse feels taut like a zither string. The patient suffers from lower abdominal pain, menstrual obstruction for months, and sores in the orifices. Additionally, the pulse at the Guan position on the left hand corresponds to the Foot-Jueyin Liver Meridian. A deep (Chen) pulse belongs to Yin; Yin deficiency governs obstructed menstruation accompanied by waist and abdominal pain. A slippery (Hua) pulse at the Chi position indicates substantial Qi and Blood, but if menses are absent and obstructed, it signifies pathology. If the pulse at the left Chi position appears and then cuts off abruptly, it indicates obstructed menstruation. Furthermore, if the Cun and Guan pulses are regular and normal, but the Chi pulse is absent, it indicates obstructed menstruation. The patient should suffer from lower abdominal pain radiating to the waist with colic, and Qi accumulation rising to pierce the chest and hypochondrium.
