Wuweizi (Fructus Schisandrae Chinensis) (1 qian), Huanglian (Rhizoma Coptidis Chinensis) (3 fen), Rougui (Cortex Cinnamomi Cassiae) (1 fen). Decoct in water and take orally. After one dose, bleeding from the tongue stops immediately; after four consecutive doses, the ulceration on the tongue heals.
This prescription does not directly treat the tongue but aims to facilitate the interaction between the Heart and Kidney (Heart-Kidney Interaction). When the Heart and Kidney interact, Heart Qi descends to the Kidney, making it unnecessary to seek specific relief for the tongue. If the tongue is not depleted of fluids by Heart Fire, it can nourish itself. This approach of not directly treating the tongue is superior to direct treatment; this method of stopping bleeding without forcibly arresting blood flow is more effective than mere hemostasis.
For this condition, Qingxin Mingming Dan (Clear Heart and Save Life Pill) is also highly effective.
Xuanshen (Scrophularia Root), Maidong (Ophiopogon Root) (each 1 liang), Gancao (Licorice Root) (1 qian), Changpu (Acorus Tatarinowii Rhizome) (3 fen), Fushen (Poria with Hostwood) (3 qian), Renshen (Ginseng Root) (3 qian), Sanqi Gen Mo (Powder of Notoginseng Root) (3 qian), Wuweizi (Schisandra Fruit) (3 grains).
Decoct in water and take with the decoction. One dose stops bleeding.
In cases where bleeding occurs from the gum margins, with blood flowing out like a single thread, this is due to the boiling of Deficient Fire in the Kidney. Teeth belong to the Kidney; if the Kidney is hot, the teeth are hot; if the Kidney is deficient, the teeth are weak. If the Kidney desires to bleed, the teeth will bleed. Although, if the teeth are firm, even if the Kidney desires to bleed, there is no gap to exploit, seemingly requiring treatment of the teeth. However, treating only the teeth is useless; one must still treat the Kidney, as the Kidney is the root and the teeth are the branch. The Fire in the Kidney is Dragon-Lei Fire (Vivid Fire), rushing straight to the throat. Blood should exit from the mouth, why enter the gum margins? This is because Kidney Fire travels along the Ren and Du meridians upwards towards the lips and teeth. With no other outlet, it exploits gaps in the gum margins. Dragon-Lei Fire is most urgent in nature, and the small gaps in the gum margins are insufficient to allow free discharge, causing agitation and emergence like a line. The formula used is Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill) added with Maidong, Wuweizi, and Gusuibu.
Shudi (Prepared Rehmannia Root) (1 liang), Shanyao (Dioscorea Rhizome) (4 qian), Shanzhuyu (Cornus Fruit) (4 qian), Danpi (Moutan Bark) (5 qian), Zexie (Alisma Rhizome) (3 qian), Fuling (Poria) (3 qian),
Maidong (Ophiopogon Root) (5 qian), Wuweizi (Schisandra Fruit) (1 qian), Gusuibu (Drynaria Rhizome) (1 qian). Decoct in water and take orally. Bleeding stops after one dose. After four consecutive doses, it never recurs.
Liuwei Dihuang Tang greatly tonifies the True Water (True Yin) in the Kidney. When Yin is sufficient, Fire naturally descends; when Fire descends, blood ceases to act recklessly. Furthermore, fearing that merely tonifying Kidney Water would make water difficult to generate, Maidong and Wuweizi are used to tonify Lung Metal, thereby supplementing from the source of transformation for the Kidney (Metal generates Water). Tonifying the Lung makes water generation easier. Adding Gusuibu penetrates the bone to fill leaks, making it impossible for blood not to stop.
For this condition, Hefeng Dan (Seam-Closing Pill) can also be used:
Houjiang (Asarum Root), Renshen (Ginseng Root), Bei Wuwei (North Schisandra Fruit), Sanqi Gen Mo (Notoginseng Root Powder) (each 1 qian), Gancao (Licorice Root) (3 fen). Grind all into fine powder, apply to teeth, swish and spit or swallow to stop bleeding.
After stopping bleeding, use Liuwei Wan (Six-Ingredient Pill) to prevent recurrence.
In cases where bleeding occurs from the umbilicus, the amount of blood is not excessive, mixed with fluid, and the patient is not overly distressed. However, the umbilicus connects to Qihai, Guanyuan, and Mingmen points. How can Qi be leaked? Although bleeding is not equivalent to Qi leakage, daily bleeding causes Qi to follow and leak. The treatment method should naturally seal the umbilical gate. However, clearing the source while merely closing the gate is futile. The reason for bleeding from the umbilicus is Fire in the Large and Small Intestines. When both Fires are vigorous, they contend within the intestines. Fire from the Small Intestine seeks to exit into the Large Intestine, while Fire from the Large Intestine seeks to ascend into the Small Intestine. Unable to accept each other, the Fire loses its foundation. Since neither top nor bottom can vent it, it attacks the gap at the umbilicus and exits. Fire exits from the umbilicus, and blood follows. Therefore, treating umbilical bleeding requires urgently calming the Fire in the Large and Small Intestines.
However, the reason for Fire moving in the Large and Small Intestines is Dryness and lack of Water in the Kidney Meridian failing to moisten them. Thus, treating Intestinal Fire still requires prioritizing Kidney treatment. The formula used is Liangzhi Tang (Two-Stopping Decoction):
Shudi (Prepared Rehmannia Root) (3 liang), Shanzhuyu (Cornus Fruit) (1 liang), Maidong (Ophiopogon Root) (1 liang), Bei Wuwei (North Schisandra Fruit) (5 qian), Baizhu (Atractylodes Macrocephala Rhizome) (5 qian). Decoct in water and take orally.
One dose stops bleeding and flow; four doses eradicate the root cause.
Shudi and Shanzhuyu tonify Kidney Water; Maidong and Wuweizi benefit Lung Qi. Using more Wuweizi not only generates Water but also utilizes its sour astringent property. Adding Baizhu benefits the waist and umbilicus. When the waist and umbilicus are unobstructed, Water and Fire circulate freely. Naturally, the Large and Small Intestines draw from Kidney Water, eliminating contention. When Water is sufficient, Fire subsides, and blood stops spontaneously.
For this condition, Zhangqi Tang (Umbilical Barrier Decoction) is also very effective.
Dahuang (Rhubarb Rhizome) (5 fen), Danggui (Angelica Sinensis Root), Shengdi (Rehmannia Root) (each 1 liang), Diyü (Sanguisorba Root) (3 qian). Decoct in water and take orally. One dose stops bleeding.
In cases of bleeding from the nine orifices, the patient's breath is faint, prefers lying down over seeing sunlight, suffers from dizziness and body fatigue. People think it is possessed by spirits, but do not realize this is Reckless Flow of Hot Blood dispersing to the nine orifices. Although the symptoms appear severe, they are actually milder than Wild Blood traveling through a single meridian. Guiding blood back to its vessels stops the bleeding. The human body is full of blood. Bleeding from the nine orifices spreads from near to far, not entirely originating from Zang-Fu organs. However, treatment must focus on Zang-Fu organs, not just collaterals, because Zang-Fu organs govern and control the collaterals. The formula used is Danggui Buxue Tang (Danggui Tonify Blood Decoction) with modifications.
Danggui (Angelica Sinensis Root) (5 qian), Huangqi (Astragalus Root) (1 liang), Jingjie (Schizonepeta Spike, stir-baked black) (3 qian), Renshen (Ginseng Root) (3 qian), Baizhu (Atractylodes Macrocephala Rhizome) (5 qian), Shengdi (Rehmannia Root) (5 qian).
Decoct in water and take orally. One dose stops bleeding; two doses prevent recurrence.
Reckless Flow of Hot Blood is treated by tonifying Qi rather than clearing Fire because the root is Qi Deficiency. Qi Deficiency fails to contain blood; blood, agitated by Fire, acts recklessly and drills into orifices. Now, tonifying Qi makes Qi vigorous, and vigorous Qi naturally contains blood. If using restraining methods, closing one orifice does not guarantee others remain closed. Using Buxue Tang, which moves Qi and cools blood, inherently contains fire-clearing properties, hence its miraculous efficacy.
For this condition, Yanqiao Dan (Orifice-Covering Pill) can be used:
Renshen (Ginseng Root), Danggui (Angelica Sinensis Root), Shengdi (Rehmannia Root), Xuanshen (Scrophularia Root) (each 1 liang), Chaohei Jingjie (Stir-baked Black Schizonepeta Spike) (3 qian), Gancao (Licorice Root) (1 qian). Decoct in water and take orally. One dose stops bleeding; two doses result in complete recovery.
In cases of hematochezia (blood in stool), bleeding may occur before or after defecation. People believe pre-defecation bleeding belongs to Large Intestine Fire, and post-defecation bleeding belongs to Small Intestine Fire. In reality, both are Large Intestine Fire. There is normally no blood in the intestines. Due to excess Fire in the Large Intestine drying intestinal fluids, the intestinal wall becomes thin and cracks. Blood seeps into these cracks. If the crack is upper, blood appears later; if lower, blood appears sooner. It is not Small Intestine bleeding. Small Intestine bleeding would cause immediate death, as the Small Intestine has no blood; if it did, the Heart would be injured, and survival would be impossible. Therefore, discussing hematochezia under the Small Intestine is acceptable for differentiation, but attributing post-defecation blood specifically to the Small Intestine is incorrect. Treatment of hematochezia should focus solely on the Large Intestine. However, the Large Intestine's bleeding is not its own fault. The Kidney governs both urination and defecation. Without Kidney Water assisting the Large Intestine, Fire becomes vigorous causing bleeding. The formula used is Sandi Tang (Three Earths Decoction):
Shudi (Prepared Rehmannia Root) (1 liang), Danggui (Angelica Sinensis Root) (1 liang), Shengdi (Rehmannia Root) (1 liang), Diyü (Sanguisorba Root) (3 qian), Mu'er Mo (Wood Ear Mushroom Powder) (5 qian). Decoct in water, mix with powder, and take orally. One dose
stops bleeding; two doses result in complete recovery.
Tonifying Essence and Blood simultaneously moistens the intestines. With no dryness, there is no leakage. Moreover, Diyü cools and Wood Ear seals, ensuring rapid efficacy.
For this condition, Qishu Di Tang (Shepherd's Purse and Rehmannia Decoction) is also effective.
Shudi (Prepared Rehmannia Root) (3 liang), Dili (Water Chestnut) (3 liang, crushed for juice, decocted together with Shudi). Two doses stop bleeding.
In cases of hematuria (blood in urine), symptoms include painful, obstructive sensation at the urethral meatus, unbearable like knife cuts or stings. People think it is Small Intestine blood, but this is incorrect. Small Intestine bleeding causes immediate death; how could one survive with such pain? This results from imprudence in alcohol and sexual activity, desire to release essence but failure to do so, compounded by fright. Essence intended to be released retracts due to fright. Once inside, the essence leaves its palace and cannot return to the Kidney. The Small Intestine, also frightened, cannot directly drain water. Water accumulates generating Fire. Extreme heat boils the retained essence, transforming it into blood exiting via urine. Essentially, this is Essence from the Kidney Meridian, not blood from the urine. Treatment should clear Small Intestine Fire. However, clearing Fire without promoting urination leads to water stagnation, preventing Fire from exiting, and thus Essence/Blood cannot escape externally. The formula used is Shuihuo Liangtong Dan (Water-Fire Mutual Unblocking Pill):
Cheqianzi (Plantago Seed) (3 qian), Fuling (Poria) (5 qian), Mutong (Akebia Stem) (1 qian), Zhizi (Gardenia Fruit) (3 qian), Huangbai (Phellodendron Bark) (1 qian), Danggui (Angelica Sinensis Root) (5 qian),
Baishao (White Peony Root) (1 liang), Lümi Cao (Herba Siegesbeckiae) (1 qian), Shengdi (Rehmannia Root) (1 liang). Decoct in water and take orally. One dose removes obstruction and pain; two doses stop hematuria; three doses achieve full recovery; four doses are unnecessary.
The formula promotes the mutual unblocking of Water and Fire, adding Liver-calming and Blood-tonifying herbs. Hemorrhagic diseases fear Liver Wood overcoming Spleen/Stomach, causing Spleen/Stomach Qi to fail to ascend and sink. Sinking Qi prevents blood from rising and dispersing. Calming the Liver soothes Liver Qi, which in turn soothes Spleen/Stomach Qi. With mutual unblocking of Water and Fire in the Small Intestine, stale essence is quickly expelled.
For this condition, Tongni Yin (Unblocking Urine Decoction) is also effective.
Huangbai (Phellodendron Bark), Cheqian (Plantago Seed) (each 3 qian), Fuling (Poria), Baizhu (Atractylodes Macrocephala Rhizome) (each 5 qian), Wangbuliuxing (Vaccaria Seed) (2 qian), Rougui (Cinnamon Bark) (3 fen), Huanglian (Coptis Rhizome) (1 qian).
Decoct in water and take orally. Two doses stop bleeding.
In cases of bleeding from skin and hair follicles, blood may emerge like a thread, ooze like a filament, or come from the head, body, or between the calves. These are deficiencies in the Lung and Kidney meridians, with Fire exploiting gaps to escape outward. For such conditions, tonifying Kidney Water is the only remedy.
However, tonifying the Kidney is slow; urgent Qi tonification is required. Vigorous Qi strengthens Lung Metal, securing the skin and hair. The formula used is Feishen Liangyi Tang (Lung-Kidney Dual-Benefiting Decoction):
Shudi (Prepared Rehmannia Root) (2 liang), Renshen (Ginseng Root) (1 liang), Maidong (Ophiopogon Root) (1 liang), Sanqi Gen Mo (Notoginseng Root Powder) (3 qian). Decoct in water and take orally. One dose stops bleeding. Subsequently, use Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill) added with Maidong and Wuweizi, taken for one month, to prevent recurrence.
Shudi strengthens Water, Maidong benefits Metal. Metal and Water support each other, extinguishing Lung-Kidney Fire. Blood returns to its vessels, preventing escape into skin and hair. Moreover, Sanqi root naturally stops bleeding.
For this condition, Gui Lianxue Tang (Angelica Astringent Blood Decoction) is also highly effective.
Huangqi (Astragalus Root), Xuanshen (Scrophularia Root) (each 1 liang), Danggui (Angelica Sinensis Root) (5 qian), Maidong (Ophiopogon Root) (1 liang), Bei Wuwei (North Schisandra Fruit) (1 qian), Suzi (Perilla Fruit) (2 qian), Sanqi Gen Mo (Notoginseng Root Powder) (3 qian).
Decoct in water, mix with powder, and take orally. One dose stops bleeding.
In cases of persistent spitting of blood, where only one mouthful is spat and not much, people think the disease is mild due to the small amount. They do not realize it is actually severe. This blood originates from the Spleen, not the Stomach. The Spleen and Stomach are exterior-interior paired. Blood invading the Stomach damages Central Earth, depleting the Pre-Heaven foundation. How much more so when invading the Post-Heaven Spleen Yin? The Stomach receives, the Spleen transforms. If Spleen Qi is damaged, it cannot transform fluids for the Stomach. Even if waste is processed, it can only process coarse matter, not refined essence, to transport among the Five Viscera and Six Bowels. Thus, all Zang-Fu become exhausted. Spleen-related spitting of blood is worse than Stomach vomiting of blood. However, the reason for Spleen spitting blood lies in Stomach Earth Deficiency. Not only Stomach Earth Deficiency, but especially Kidney Water Decline. The Stomach is the Gatekeeper for the Kidney. Kidney Decline means the Stomach cannot manage opening and closing for the Kidney. Spleen blood desires to spit upward, but without Stomach constraint, it freely escapes above the throat. Thus, Spleen spitting blood, though boiling Spleen Fire, is actually assisted by Kidney and Stomach Fires. Treatment to calm Spleen Fire must tonify Spleen Earth, and further tonify Kidney Water to stop Stomach Fire. The formula used is Zi Pi Yin (Nourish Spleen Decoction):
Renshen (Ginseng Root) (3 fen), Fuling (Poria) (2 qian), Xuanshen (Scrophularia Root), Danpi (Moutan Bark), Qianshi (Euryale Seed), Maogen (Imperata Rhizome), Shanyao (Dioscorea Rhizome) (each 3 qian), Shudi (Prepared Rehmannia Root) (1 liang),
Shashen (Glehnia Root) (5 qian), Gancao (Licorice Root) (5 fen). Decoct in water and take orally. One dose stops spitting blood; two doses achieve full recovery.
This prescription lightly treats the Spleen but heavily tonifies the Kidney, truly seeking the root. If one only drains excess Spleen Fire, one inevitably damages deficient Stomach Earth. Damaged Stomach Qi further injures Spleen Qi. Only then tonifying the Kidney fails to generate Kidney Water, unable to control vigorous Spleen Fire. Regardless of difficulty stopping spitting blood, I fear the Stomach Gate will not close, leading to massive vomiting of blood. This is the subtlety of Zi Pi Yin.
For this condition, Tonggui Tang (Return Together Decoction) is also highly effective.
Baizhu (Atractylodes Macrocephala Rhizome), Xuanshen (Scrophularia Root) (each 1 liang), Shudi (Prepared Rehmannia Root) (2 liang), Bei Wuwei (North Schisandra Fruit) (1 qian), Jingjie (Schizonepeta Spike, stir-baked black) (3 qian), Beimu (Fritillaria Bulb) (5 fen).
Decoct in water and take orally. One dose stops bleeding.
In cases of bleeding from both eyes, even shooting out directly, women experience amenorrhea, men suffer dry mouth and lips. People think it is reckless Liver Blood flow, who knew it was stirring of Fire in the Kidney? The Fire in the Kidney is Ministerial Fire (Xiang Huo). If Sovereign Fire (Jun Huo) is tranquil, Ministerial Fire dares not ascend. Only when Sovereign Fire declines, and the Heart slightly moves with desires, does Ministerial Fire seize the Sovereign's command to役使 (serve/control) the nine orifices. The nine orifices respect the Sovereign's order, dare not disobey, and obey. The Heart's connection reaches the eyes; the Liver's orifice opens at the eyes. Fire in the Liver is also Ministerial Fire, identical in kind to Ministerial Fire in the Kidney's Mingmen and Pericardium. Similar energies assist each other in boiling, like villains forming cliques, inseparable. They go straight to the Heart-Liver orifice connections, blood flows upward instead of downward. Treatment seems to require tonifying weak Sovereign Heart Fire to control Kidney Fire movement. However, since Heart Fire is already deficient, tonifying the Heart makes it hard to flourish. One must tonify the Kidney to generate Heart Fire. Then Heart Fire remains unmoved, and Kidney Fire becomes quiet. The formula used is Zhuxin Dan (Assist Heart Pill):
Maidong (Ophiopogon Root) (1 liang), Yuanzhi (Polygala Root) (2 qian), Fushen (Poria with Hostwood) (3 qian), Shudi (Prepared Rehmannia Root) (1 liang), Shanzhuyu (Cornus Fruit) (5 qian), Xuanshen (Scrophularia Root) (5 qian),
Danpi (Moutan Bark) (3 qian), Qianshi (Euryale Seed) (3 qian), Lianzixin (Lotus Plumule) (1 qian), Danggui (Angelica Sinensis Root) (3 qian), Chaihu (Bupleurum Root) (3 fen). Decoct in water and take orally. One dose stops bleeding; two doses prevent recurrence.
This prescription treats the Heart, Liver, and Kidney meridians simultaneously. Tonifying the Kidney to generate the Liver is equivalent to tonifying the Kidney to generate the Heart. Some doubt that with stirring Kidney Fire, heavy Kidney tonification is inappropriate. They do not know that stirring Kidney Fire is due to decline of Kidney Water. Water declines, so Fire stirs; if Water is vigorous, does Fire not quiet?
Moreover, Heart Fire must receive assistance from Kidney Water to flourish. If Heart Fire flourishes, Kidney Fire levels itself naturally. This is not random application.
For this condition, Gao Tang (Paste Decoction) is also highly effective.
Shudi (Prepared Rehmannia Root), Baishao (White Peony Root) (each 1 liang), Shanzhuyu (Cornus Fruit) (5 qian), Chaihu (Bupleurum Root) (5 fen), Jingjie (Schizonepeta Spike, stir-baked black) (3 qian), Bei Wuwei (North Schisandra Fruit) (10 grains),
Zhuli (Bamboo Sap) (1 he). Decoct together in water and take orally. Two doses cure.
In cases of persistent bleeding from the tongue, observing closely reveals small pores spurting blood. This is Heart Fire ascending to overcome Lung Metal. Epistaxis (nosebleed) affects the airway; tongue bleeding affects only minor collaterals. However, blood from the mouth invades the Stomach but not the Heart; blood from the tongue invades the Heart but not the Stomach. Invading the Stomach involves a Fu organ; invading the Heart involves a Zang organ. How can one lightly treat an illness of minor collaterals?
Treatment internally replenishes fluids in the Heart and externally fills the pores of the tongue orifice. Then Heart Fire calms, and tongue blood stops easily. The formula used is Buye Dan (Replenish Fluids Pill):
Renshen (Ginseng Root) (3 qian), Shengdi (Rehmannia Root) (3 qian), Maidong (Ophiopogon Root) (5 qian), Danshen (Salvia Root) (2 qian), Bei Wuweizi (North Schisandra Fruit) (10 grains), Shanyao (Dioscorea Rhizome) (3 qian),
Danggui (Angelica Sinensis Root) (5 qian), Huanglian (Coptis Rhizome) (1 qian), Xuanshen (Scrophularia Root) (5 qian), Beimu (Fritillaria Bulb) (1 qian). Decoct in water and take orally. Externally, use equal parts of Chao Huaihua (Stir-fried Sophora Flower) and Sanqi Gen (Notoginseng Root), grind into powder, and sprinkle on the area for healing.
Huaihua and Sanqi naturally stop bleeding, seemingly not needing reliance on Buye Dan. However, treating the branch without addressing the root internally may lead to bleeding resuming immediately after stopping.
For this condition, Baizi Anxin Tang (Seed-of-Tailow-Pod Calm-Heart Decoction) is also highly effective.
Renshen (Ginseng Root), Fushen (Poria with Hostwood), Baiziren (Seed of Tailow-Pod) (each 3 qian), Yuanzhi (Polygala Root) (1 qian), Changpu (Acorus Tatarinowii Rhizome) (3 fen), Danggui (Angelica Sinensis Root), Shengdi (Rehmannia Root) (each 5 qian), Wuweizi (Schisandra Fruit) (10 grains),
Beimu (Fritillaria Bulb), Huanglian (Coptis Rhizome) (each 5 fen). Decoct in water and take orally.
Two doses stop bleeding.
