[Commentary] Cough is one of the main symptoms of lung diseases. Whether it is internal injury cough or external contraction cough, it is caused by the disease of the Lung organ. External contraction cough starts in the Lung, while internal injury cough is mostly caused by other Zang-Fu organs getting sick first and then affecting the Lung. For the syndrome of cough, the first step is to differentiate between external contraction and internal injury, as well as the deficiency-excess nature of its manifestations. External contraction cough should be further distinguished into wind-cold, wind-heat, dampness-encumbering, summerheat-encumbering, and dryness-heat cough situations. Internal injury cough should be differentiated into phlegm-dampness, Liver fire invading the Lung, Spleen deficiency, and Kidney deficiency types.
Case 1 is latent summerheat cough. The patient took pills on his own, resulting in incomplete sweating, causing the Lung Qi to lose its dispersing and unblocking function. Treatment employed aromatic transformation methods; after taking the medicine, sweat was produced and the body felt light. Due to concurrent Qi stagnation, a subsequent formula with bitter descending and acrid unblocking properties was used, bringing the cough to a halt, promoting smooth bowel movements, and leading to recovery.
Case 2 belongs to internal injury cough. The patient's pulse was wiry and rapid, accompanied by cough, dry throat, and throbbing pain in both hypochondriac regions. This indicates insufficient Liver Yin and hyperactive Liver Fire (Wood overacting on Metal), which inevitably consumes Lung Yin. Yin deficiency with Yang floating upward easily damages the Yang collaterals, raising the concern of hemoptysis. Treatment adopted nourishing Yin and moistening the Lung, along with subduing the Liver and descending Fire. Once the Fire descended and the Lung Qi became clear and purifying, the cough naturally subsided.
Case 3: Pulmonary heat with excessive phlegm is the branch manifestation, while Spleen and Kidney Yang deficiency is the root cause. Although multiple treatments with formulas clearing heat and resolving phlegm-fire were used, the efficacy was mediocre. Switching to warming the Yang of the Spleen and Kidney, with slight use of upper-clearing agents as counter-assistance, resulted in the basic disappearance of the gray-black tongue coating, and other symptoms improved accordingly. Subsequently, methods of warming and nourishing the Spleen and Kidney, and cultivating Earth to generate Metal were used for consolidation. The key to the success of this case lay in the meticulous differentiation of root and branch.
Case 4: Cough accompanied by wheezing, belonging to Spleen and Kidney Yang deficiency. Since Kidney Yang fails to rise and transform Spleen Earth, the essence and grains of food transform into phlegm-fluids. These fluids occupy the Lung collaterals, obstructing Qi mechanism and causing cough and wheezing. Fluid pathogens overflowing upward cause facial edema and frequent expectoration of clear water; Spleen failure in transportation and Kidney failure in storage lead to loose stools; Yang Qi weakness causes aversion to wind and cold limbs. Treatment employed the method of benefiting Fire to generate Earth, yielding considerable efficacy. As stated above, the saying in *Neijing* "All five Zang and six Fu organs can cause cough, not just the Lung" is indeed profound truth.
Case 5: Diagnosed as pulmonary alveolar proteinosis, falling within the scope of rare and difficult diseases. According to TCM syndrome differentiation, sinking of the Great Qi in the chest is the root, while blood stasis obstructing collaterals and turbid phlegm failing to resolve are the branches. The so-called Great Qi refers to the Ancestral Qi (Zong Qi) mentioned in *Neijing*, discussed in detail by Zhang Xichun in *Medical Records of Chinese Medicine Aiding Western Medicine*. Zhang believed: "It is called Great Qi precisely because it supports the whole body, serves as the guideline for all Qi, encompasses the pivot of respiration outside the lungs... When this Qi is deficient, breathing becomes difficult, limbs feel sore and lazy, spirit becomes confused and exhausted, and mental power and thoughts are suddenly reduced." When Ancestral Qi is deficient, body fluids fail to distribute, and blood circulation becomes stagnant and sluggish, thus complicating with phlegm-turbidity and blood stasis syndromes. If treatment only attends to the branch of phlegm-turbidity and blood stasis without considering the root of Ancestral Qi sinking, it puts the cart before the horse. Therefore, based on TCM syndrome differentiation and legislation, Zhang's Shengxian Tang (Huangqi, Shengma, Chaihu, Jiegeng, Zhimu) was selected as the main formula, added with Danggui, Chuanxiong, and Danshen to promote blood vessels; Xuanfuhua and Tinglizi to descend Qi and purify the Lung; Sheng Yiren and Haifushi to strengthen the Spleen and resolve phlegm. Treating both root and branch with emphasis on the root, with clear syndrome differentiation and precise medication selection, for difficult diseases, not only improves symptoms but also significantly improves lung function, deserving full attention.

[Commentary] Wheezing syndrome refers to the syndrome characterized primarily by rapid breathing, even opening mouth and lifting shoulders. Clinically divided into deficiency and excess types. Excess wheezing is mostly due to external pathogens congesting the Lung, causing loss of dispersing and descending of Qi; deficiency wheezing is mostly due to dual deficiency of Lung and Kidney, Lung failing to exhale, Kidney failing to grasp Qi, caused by internal deficiency of essence and Qi. Excess syndromes often accompany phlegm and fluids, so wheezing, cough, and asthma syndromes are often discussed together.
Case 1 is wind-heat wheezing and cough, mostly caused by contracting wind-heat transforming into phlegm. In acute cases, treat the branch first; the method uses clearing and descending, which is the correct treatment. When the illness has gone 80-90% but phlegm remains, it is due to heat in Liver and Stomach and still deficient Lung Qi. Treatment clears Liver and Stomach and tonifies Lung Qi, which is the method of treating the root.
Case 2 is asthma caused by external cold and internal fluid retention. Thus, modified Fuzi Tang is used as the main formula, plus Heixi Dan to assist in warming the Kidney and grasping Qi, and Danjia Pian to resolve phlegm and expel fluids. This not only dispels pathogens to stabilize healthy Qi but emphasizes supporting Yang to dispel fluid pathogens, embodying the meaning of "when the sun shines brightly, the dark clouds disperse."
Case 3: Wheezing disease occurred for three years, syndrome belongs to fire reversing upwards, Qi unable to descend, Lung Qi blocked by phlegm-heat, thus causing wheezing. Although the herbs are not extraordinary, the combination is appropriate, achieving quick results.
Case 4: Suffers from chronic asthma, this episode has lasted for days, current symptoms show signs of impending collapse. Urgently restore Yang and grasp Qi. *Wenzhai Medical Cases* states: "The Lung is the master of Qi, the Kidney is the root of Qi. When Kidney is deficient, Qi does not return to its origin; when Lung is damaged, Qi has no attachment, lone Yang floats and causes wheezing..." Thus, Korean Ginseng, Agarwood, and Black Tin Pill are used to warm Kidney Yang and dispel yin cold, so wheezing naturally calms. Subsequently, tonifying Qi and resolving phlegm is used, modifying Xingren Decoction from *Waitai Miyao* for regulation, so the image of weak Qi and excessive phlegm can naturally recover.
(VI) Phlegm-Fluids
Case 1 Yu, Female. Violent cold binds externally, phlegm-fluids gather internally, obstructing the Lung, loss of descending function, severe wheezing and cough, thus unable to lie flat day or night, chills and fear of cold, poor appetite and nausea, white greasy tongue coating, floating, wiry, and slippery pulse. Prescribe modified Xiaolongdan Tang (Minor Dragon Decoction). Disperse external pathogens, warm and transform phlegm-fluids. Mi Zhi Mahuang (Honey-roasted Ephedra) 1.2g, Chuan Guizhi (Cinnamon Twig) 2.4g, Yunling (Poria) 9g, Jiang Banxia (Ginger-prepared Pinellia) 6g, Wuweizi (Schisandra) 1.2g, Dan Ganjiang (Light Dried Ginger) 1.2g, Zhi Suzi (Perilla Fruit) 6g, Guang Xingren (Apricot Kernel) 9g, Shu Fupian (Prepared Aconite) 3g, Eguanshi (Goose Tube Stone, calcined) 3g, Xiao Hou Zijin Dan (Purple Gold Pill for Asthma) 2 pills (swallowed separately), taken continuously for two days. Second visit: After taking two doses of Xiaolongdan Tang, wheezing and cough greatly reduced during the day, but remain at night, poor appetite and nausea. Thin greasy coating, wiry and slippery pulse. Night is the time of Yin abundance, fluid pathogens secretly occupy Yang positions, obstructing Qi mechanism, losing the command of Lung and Stomach descent. Again warm-transform fluid pathogens, purify-descend Lung Qi. Chuan Guizhi 2.4g, Yunling 9g, Jiang Banxia 6g, Juhong (Tangerine Peel) 3g, Wuweizi 1.2g, Dan Ganjiang 1.2g, Shui Zhi Yuanzhi (Water-processed Polygala) 1.5g, Guang Xingren 9g, Zhi Suzi 15g, Xuanfuhua (Inula Flower, wrapped) 15g, Shu Fupian 3g, Eguanshi 3g (calcined). Third visit: Wheezing and cough, even at night, have alleviated, nausea stopped. Only the root of phlegm-fluids has existed for a long time, difficult to dissolve rapidly. Spleen is the source of generating phlegm, Lung is the container storing phlegm. Now plan to regulate Spleen and purify Lung, warm-transform phlegm-fluids. Remove Xuanfuhua, Yuanzhi, Wuwei from original formula, add Sheng Baizhu (Atractylodes Macrocephala Rhizoma) 15g, Chao Buguzhi (Psoraleae Fructus Stir-fried) 15g. (*Ding Ganren Medical Cases*)
Case 2 Ji X, Female, approx. 30 years old. Cough and wheezing, leaning to breathe unable to lie down, aversion to cold and fever, headache and body pain, chest tightness and discomfort, heart pain penetrating to back. Pulse deep and slippery, tongue coating white greasy. This is wind-cold and phlegm-fluid, internal and external fighting and knotting, Lung Qi unable to descend forming Lung Distension. Use Xiaolongdan Tang to drive wind-cold, combined with Gualou Xiebai Tang to eliminate phlegm-fluids. Mahuang 1.2g, Guizhi 1.2g, Dan Ganjiang 1.5g, Bei Xixin (Asarum) 1.2g, Sheng Baishao 4.5g, Wuweizi 1.5g, Gancao 1.5g, Gualou Ren (Trichosanthes Seed) 9g (pounded), Gan Xiebai (Allium Macrostemon Bulb) 9g (washed with rice wine, pounded), Jiang Banxia 9g. Next visit: After taking, obtained sweat, cold/fever and wheezing both flattened, only body pain and cough remain. Change formula to Guizhi Tang (Cinnamon Twig Decoction) to harmonize Ying and Wei, add Ganjiang and Wuweizi each 1.5g, Xixin 0.9g to treat cough. One dose effective, two doses better... did not continue visits, gradually recovered. (*Reprinted National Famous Doctor Medical Cases Compilation*, Yuan Guisheng Medical Cases)
Case 3 Yu, Male, 60 years old, October. Spleen Yang deficiency leads to accumulation of dampness into phlegm, Kidney Yang exhaustion leads to accumulation of water into fluids. Fluid-phlegm flooding upward, cough and reverse Qi, phlegm tastes salty, chills and fear of cold, pulse slippery and powerless, tongue coating thin greasy. Although the body is deficient, heavy tonification is difficult to administer, fearing that phlegm-fluids are dominant. Paojiang (Roasted Ginger) 3g mixed and pounded, Chao Wuweizi 2.1g, Xixin 2.4g, Jiang Xia (Ginger Pinellia) 6g, Fuling (Poria) 12g, Zhi Juhong 4.5g, Jinfeigeng (Boilweed Stem, wrapped) 9g, Duan Daizheshi (Calcined Hematite) 15g, Duan Lingshishi (Calcined Magnetite) 15g, Chao Duzhong (Eucommia Bark) 12g, Chenxi Mo (Agarwood Powder) 1.8g (taken separately), Zhi Ziwan 9g, Red Dates 3 pieces. (*Ye Xichun Medical Cases*)
Case 4 Deng X, Female, 48 years old, Married. Admission date: June 15, 1963. Admitted due to edema and shortness of breath for half a year, aggravated for one week. After catching a cold in January 1961, began coughing and shortness of breath, lower limbs gradually swollen, epigastric fullness, coughing up white phlegm, oliguria, past history of eight-year chronic cough. Pulse wiry, fine, and rapid, tongue coating white, lip color purple. Western diagnosis: Chronic cor pulmonale. Syndrome differentiation: Heart and Kidney Yang deficiency, internal retention of water-fluids, phlegm-dampness obstruction, Lung Qi congestion. Treatment method: Clear and diffuse Lung Metal, descend Qi and resolve phlegm, warm Yang and promote diuresis. Prescription: Modified Yuebi Tang combined with Zhenwu Tang: Sheng Shigao (Gypsum Fibrosum) 12g, Mahuang 3g, Gancao 9g, Yunling 12g, Baizhu 9g, Hang Shao (White Peony Root) 9g, Fuzi 6g, Shengjiang 9g, Dazao (Jujube, split) 5 pieces, Cheqianzi (Plantago Seed) 15g, Baimaogen (Imperatae Rhizoma) 30g, Xingren 9g. After taking the above medicine for 3 doses, urine volume increased, reaching 1500-1900 ml daily, lower limb edema significantly reduced. After taking 5 doses, edema不明显, liver enlarged retracted, cough alleviated. Add Houpo (Magnolia Bark) 6g, Chenpi 6g to the above formula, wheezing also reduced, only chest tightness remains. Thus remove Baimaogen, Cheqianzi, Houpo from above formula, add Suzi 9g, take another 5 doses, symptoms alleviated, still coughing unresolved, due to Lung Qi not diffusing. Thus change to broad-chest regulating Qi and clearing Lung method, formula using modified Houpo Mahuang Tang: Houpo 6g, Mahuang 3g, Banxia 9g, Xingren 9g, Gancao 9g, Shashen (Glehnia Root) 18g, Xiaomai (Triticum Vulgare) 30g, Fuling 9g, Xixin 3g, Wuweizi 6g, Shengjiang 4.5g. After taking the above formula, symptoms greatly reduced... condition stabilized. (*Zhao Xiwu Medical Experience*)
Case 5 Wang xx, Male, 45 years old. External contraction of wind-cold, stirring up phlegm-fluids, Lung Qi not diffusing, cough and chest pain, wheezing unable to lie flat.
