心咳汤用北沙参，牛蒡甘桔石杏仁，茯神远志麦冬夏，小麦煎医心咳珍。（按∶五脏六
腑皆令人咳，究不离乎肺经。此心咳一证，虽属心火上逆，以余验之，其始多由外感。夫外
感温邪，必先肺卫而及心营，观经文“咳则心痛”句，义可推矣。逮至咳呛不已，震动心胞
之火，势必上逆，而为咽肿喉痹。前人谓生脉散加茯神、远志，能治心咳，遵用不甚见效，
余因参入开泄肺经之药，重用小麦煎汤代水，治之乃验。盖小麦甘平，为心之谷，缓心宁气，
大有殊功，即从浓朴麻黄汤意化出，故附于此，以就正于当世。）

射干 细辛 紫菀 款冬花（各三两） 半夏 五味子（各半升） 麻黄 生姜（各四

射干麻黄汤细辛，紫菀款冬味夏朋，姜枣同煎疗肺冷，咳而上气水鸡声。酸收苦泄兼辛
散，肺气宣通痰自平。（此治形寒饮冷伤肺之要方也。喉中水鸡声者，痰气出入而 咯也。
由肺中冷，阳气不能宣其液，郁于肺而生声，乃复用《本经》主治咳逆上气之品，大泄阴液，
宣通肺气。射干、紫菀，以苦泄之也，麻、辛、款、夏、生姜，以辛泻之也，五味子酸以收
其正气，大枣甘以缓其下行，则射干、细辛、五味之性，从麻黄外达肺经，内通肺脏，泄肺
之苦，遂肺之欲，补肺之正，温肺之阳，俾气道平而肺得阳和之致，自无 咯之声矣。）
（由旭高新创制的）治疗心咳，咳嗽时伴随心痛，咽喉中有异物感如同梗塞状，严重时会出现咽喉肿痛、喉痹。

心咳汤使用北沙参，配合牛蒡子、甘草、桔梗、石膏、杏仁，加上茯神、远志、麦冬、半夏，用小麦煎汤来医治珍贵的心咳症。（按语：五脏六腑的病变都会导致咳嗽，但究其根本，离不开肺经。这种心咳证候，虽然属于心火上逆，根据我的临床经验，起初多由外感引起。外感的温邪，必定先侵犯肺卫，进而波及心营，从经文“咳嗽时心痛”这一句，道理可以推知。等到咳嗽呛逆不止，震动心包之火，势必向上逆行，从而出现咽喉肿痛和喉痹。前人曾说生脉散加茯神、远志能治疗心咳，遵照使用却不太见效，我因此加入开泄肺经的药物，重用小麦煎汤代替水，治疗才有效。因为小麦味甘性平，是心的谷物，能缓和心神、宁定气机，有特殊的功效，这是从厚朴麻黄汤的意思变化而来，所以附在这里，以请教当代名医。）

射干麻黄汤包含细辛，搭配紫菀、款冬花、五味子、半夏等药友，生姜和大枣同煎治疗肺寒，针对咳嗽且气息上逆、喉咙发出水鸡叫声的症状。酸收、苦泄兼用辛散，肺气宣通则痰自平息。（这是治疗因受寒饮冷损伤肺脏的要方。所谓喉咙中水鸡声，是指痰气出入时发出的咯咯声。由于肺中寒冷，阳气不能宣发津液，郁结在肺中产生声音，于是重新使用《神农本草经》中主治咳逆上气的药物，大力宣泄阴液，宣通肺气。射干、紫菀，是用苦味来泄降；麻黄、细辛、款冬花、半夏、生姜，是用辛味来泻散；五味子酸味以收敛正气，大枣甘味以缓和下行趋势，那么射干、细辛、五味子的药性，借助麻黄向外通达肺经，向内通利肺脏，宣泄肺部的苦滞，顺应肺的喜好，补益肺的正气，温煦肺的阳气，使得气道平和而肺获得阳和的状态，自然就没有咯咯的声音了。）
(Originally formulated by Xu Gao) Treats Heart Cough (Xin Ke), characterized by cough accompanied by heart pain, a sensation of obstruction in the throat as if something were stuck, and in severe cases, swelling and soreness of the throat with Bi syndrome (obstruction).
Bei Sha Shen (Radix Glehniae, 3 qian) Shi Gao (Gypsum Fibrosum, 3 qian, ground together with Bo He Tou/Menthae Haplocalycis Caulis et Folium head), Niu Bang Zi (Fructus Arctii, 1.5 qian), Xing Ren (Semen Armeniacae Amarum, 3 qian, skin and tip removed), Jie Geng (Radix Platycodonis, 5 fen), Gan Cao (Radix Glycyrrhizae, 5 fen), Mai Dong (Radix Ophiopogonis, 3 qian, core removed), Ban Xia (Rhizoma Pinelliae, 1 qian), Fu Shen (Poria with Hostwood, 3 qian), Yuan Zhi (Radix Polygalae, 5 fen), Xiao Mai (Frumentum Tritici, 5 qian). Use three zhan of water, first decoct the Xiao Mai until the volume reduces by one zhan, then add the remaining herbs, decoct to one zhan, and take orally. (If there is excessive phlegm, add Chuan Bei Mu; if there is throat swelling and pain, remove Ban Xia; if there is excessive sweating, add Wu Wei Zi.)
Heart Cough Decoction uses Radix Glehniae, combined with Fructus Arctii, Radix Glycyrrhizae, Radix Platycodonis, Gypsum Fibrosum, and Semen Armeniacae Amarum, along with Poria with Hostwood, Radix Polygalae, Radix Ophiopogonis, and Rhizoma Pinelliae, decocted with Frumentum Tritici water to treat the precious condition of Heart Cough. (Note: Diseases of the five zang and six fu organs can all cause cough, but ultimately it does not depart from the Lung Meridian. This syndrome of Heart Cough, although belonging to the counterflow of Heart Fire, based on my clinical experience, initially often arises from external contraction. External warm pathogens must first invade the Lung Wei (defensive level) and then affect the Ying level of the Heart. The meaning can be inferred from the text "cough accompanied by heart pain." When coughing and wheezing persist, stirring up the fire of the Pericardium, it inevitably counterflows upward, resulting in throat swelling, soreness, and Bi syndrome. Predecessors stated that Sheng Mai San (Generate the Pulse Powder) added with Poria with Hostwood and Radix Polygalae can treat Heart Cough; following this was not very effective. I therefore added herbs to open and drain the Lung Meridian, heavily using Frumentum Tritici decocted as water substitute, and the treatment proved effective. Because Frumentum Tritici is sweet and neutral, being the grain of the Heart, it can soothe the Heart and calm the Qi, possessing special efficacy. This formula is derived from the intent of Hou Po Ma Huang Tang (Magnolia Bark and Ephedra Decoction), so it is attached here to seek correction from contemporary physicians.)
Treats cough with upward rushing of Qi, where the sound in the throat resembles that of a water fowl.
She Gan (Radix Belamcandae), Xi Xin (Herba Asari), Zi Wan (Radix Asteris), Kuan Dong Hua (Flos Farfarae) (each 3 liang), Ban Xia (Rhizoma Pinelliae), Wu Wei Zi (Fructus Schisandrae) (each half sheng), Ma Huang (Herba Ephedrae), Sheng Jiang (Rhizoma Zingiberis Recens) (each 4 liang), Da Zao (Fructus Jujubae).
She Gan Ma Huang Tang (Belamcanda and Ephedra Decoction) contains Herba Asari, paired with Flos Farfarae, Radix Asteris, Fructus Schisandrae, and Rhizoma Pinellae as allies; decocted with Rhizoma Zingiberis Recens and Fructus Jujubae to treat Lung Cold, addressing cough with upward rushing of Qi and the water fowl sound in the throat. Sour astringency, bitter drainage, combined with acrid dispersion, allows Lung Qi to ventilate freely, causing phlegm to settle naturally. (This is an essential formula for treating Lung injury caused by cold form and cold drinks. The "water fowl sound" in the throat refers to the rattling sound produced by phlegm and Qi moving in and out. Due to cold in the Lung, Yang Qi cannot ventilate fluids, leading to stagnation in the Lung and producing sound. Therefore, herbs indicated in the *Shen Nong Ben Cao Jing* for treating counterflow cough are used to vigorously drain Yin fluids and ventilate Lung Qi. She Gan and Radix Asteris use bitterness to drain; Ma Huang, Herba Asari, Flos Farfarae, Rhizoma Pinelliae, and Rhizoma Zingiberis Recens use acridity to disperse; Fructus Schisandrae uses sourness to consolidate healthy Qi; Fructus Jujubae uses sweetness to moderate downward movement. Thus, the properties of She Gan, Herba Asari, and Fructus Schisandrae, aided by Ma Huang, reach outward to the Lung Meridian and inward to the Lung Organ, draining Lung distress, accommodating Lung desires, supplementing Lung healthy Qi, and warming Lung Yang. This ensures the airway becomes peaceful and the Lung attains a state of Yang harmony, naturally eliminating the rattling sound.)
