[Pathogenesis] After the fever of a febrile disease subsides, the Stomach Qi remains deficient, and residual pathogenic factors have not yet been cleared. At this stage, one should first consume light rice water in small amounts, followed by thick rice water, and then thin porridge. These must be given in small quantities and frequently. One must absolutely avoid overeating at will. If grain intake is too rapid, the transportation and transformation functions cannot keep up, and residual pathogens may take advantage of food stagnation to recur. This is termed "food relapse" (Shi Fu). Generally, robust individuals require two months of recuperation, while weak individuals require one hundred days, before the disease ceases to recur.
[Symptoms] Manifestations include fever, headache, mental restlessness, irritability, and anorexia. In mild cases, there is slight irritability towards dusk, caused by eating too early or consuming too much food. Due to Stomach deficiency and inability to digest food, recovery can be achieved by reducing dietary intake. In severe cases, symptoms include fever, difficulty in defecation, and delirium.
[Pulse Diagnosis] In mild cases, the pulse is choppy and slow. In severe cases, there is irritability with thirst, delirium, constipation, and the Guan position pulse is excess and forceful.
[Treatment] If pathogenic factors and food stagnation steam upwards, causing fever and headache, this indicates food injury complicated by external pathogens. The appropriate formula is Zhishi Zhizi Chi Tang (Immature Bitter Orange, Gardenia, and Fermented Soybean Decoction), added with raw Hawthorn Fruit (Shanzha), Malt (Maiya), Forsythia (Lianqiao), and Radish Seed (Laifuzi), to cool and disperse. If there is no fire manifestation, with a moist tongue and no thirst, Tiaozhong Tang (Regulate the Middle Decoction) or Xiangsha Zhizhu Tang (Aucklandia and Amomum, Atractylodes and Immature Bitter Orange Decoction) can be used. If there is fever, dryness with thirst, delirium, constipation, and an excess Guan pulse, use Zhishi Zhizi Chi Tang (composed of Zhishi, Zhizi, Douchi, Shigao, and Mus Shi/Rat Feces) plus Dahuang to purge downwards. If the heat does not resolve but bowel movements are normal, use Shenfu Sanbai Tang (Ginseng and Aconite Three Whites Decoction) with modifications. For epigastric fullness and stuffiness, add Zhishi, Huanglian, and Jiegeng. For phlegm and vomiting, add Banxia and Zhuru. For undigested rice, add Shenqu and Maiya. For undigested meat, add raw Shanzha and Caoguo.
[Bingzhang's Commentary] Drinking alcohol after the recovery from a febrile disease causes recurrent fever. Alcohol is pungent and extremely hot. If alcohol is consumed while the pre-existing heat of Shaoyang syndrome has not yet resolved, it intensifies the heat and exacerbates the condition. Symptoms inevitably include irritability, dry retching, dry mouth, and anorexia. Urgently treat with Chuanlian (Coptis Rhizome), Gehua (Kadsura Flower), Lianqiao, raw Zhizi, Zhishi, Wumei, and Yinhua to resolve the toxicity. Lin Lan used Xiaochaihu Tang (Minor Bupleurum Decoction) added with Gegen, Huanglian, and Wumei. For a洪大 (Hongda/Great) pulse, use Renshen Baizhu Tang (Ginseng and White Tiger Decoction) added with Gegen and Huanglian. Alternatively, Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction) added with Jijuzi is also effective. The *Qianjin Fang* states: After a major illness, consuming pork, sheep blood, fatty fish, and greasy foods will inevitably cause severe diarrhea that is difficult to treat. Consuming cakes, pastries, cereals, millet, maltose, sweets, delicacies, dates, chestnuts, and other fruits, as well as hard-to-digest solid foods, will cause heat accumulation due to Stomach deficiency and inability to digest. Failure to purge leads to death; purging leads to danger. All are difficult to treat. Eating all kinds of meat and noodles after recovery causes the disease to recur. Drinking alcohol and eating garlic and leeks causes the disease to recur. Eating raw fish causes incessant diarrhea. Eating raw vegetables and melons causes permanent deterioration of complexion. Eating raw dates and mutton causes steaming heat in the chest and diaphragm. Eating dog or mutton causes bone-steaming consumption. Drinking cold water immediately after sweating resolves the exterior, damages the Pericardium, causes deficiency, and even tonification cannot restore health. The *Jingui Yaolue* states: When seasonal diseases have just recovered, eating raw vegetables causes swelling of the hands and feet. These are all dietary prohibitions after recovery.
