(Gastric fire and phlegm-dampness.)
Use Arisaema Cum Bile (Nanxing, 天南星), Pinellia Rhizoma Preparatum (Banxia, 半夏), Gypsum Fibrosum (Ruan Shigao, 软石膏), and Cyperi Rhizoma (Shacaogen, 莎草根), prepared as a decoction or pill.
Dai Yun stated: Damp-heat resides at the gastric opening. When food enters the stomach, it is constrained by damp-heat, preventing proper digestion and transmission, thus causing acidity. It is like grain and meat in a container becoming sour when exposed to damp-heat.
One must use Evodiae Fructus (Zhuyu, 吴茱萸) to follow its nature and restrain it, employing the method of counter-assistance: combining Evodiae Fructus (Zhuyu, 吴茱萸) with Coptidis Rhizoma (Huanglian, 黄连).
(It is merely phlegm stirred by fire.)
Dai Yun stated: This is what is commonly referred to as "xin cao" (heart palpitations with discomfort).
Gardeniae Fructus (Zhizi, 栀子) should be stir-fried with ginger juice; Coptidis Rhizoma (Huanglian, 黄连) must not be omitted. Gardeniae Fructus (Zhizi, 栀子) and Scutellariae Radix (Huangqin, 黄芩) serve as the sovereign herbs.
Arisaema Cum Bile (Nanxing, 天南星), Pinellia Rhizoma Preparatum (Banxia, 半夏), and Citri Reticulatae Pericarpium (Chenpi, 橘皮); if heat increases, add Indigo Naturalis (Qingdai, 青黛).
For obese individuals suffering from heart palpitations with discomfort, use Er Chen Tang (Two-Cured Decoction) plus Chuanxiong Rhizoma (Fuxiong, 抚芎), combined with Atractylodis Macrocephalae Rhizoma (Cangzhu, 苍术), Atractylodis Rhizoma (Baizhu, 白术), and stir-fried Gardeniae Fructus (Zhizi, 栀子).
There is no need to categorize into five types; all are due to damp-heat, similar to the fermentation of yeast.
Dai Yun stated: In the five jaundices, the entire skin and eyes appear yellow as if dyed with Gardeniae Fructus water. For jaundice caused by food stagnation, one must assess deficiency or excess and purge the food stagnation. For other types, promoting urination is prioritized; once urine flows freely and turns white, the yellow discoloration naturally recedes.
Mild cases are treated with Xiao Wen Zhong Wan (Minor Warming the Middle Pill); severe cases with Da Wen Zhong Wan (Major Warming the Middle Pill).
If heat predominates, add Coptidis Rhizoma (Huanglian, 黄连). If dampness predominates, use Yinchen Decoction (Yinchen) and Wu Ling San (Five-Ingredient Powder with Poria), adding herbs for food stagnation.
First, use Atractylodis Rhizoma (Baizhu, 白术) and Paeoniae Radix Alba (Baishao, 白芍药), stir-fried and ground into powder. After taking this, administer medicine for treating Xiao Ke (Wasting-Thirst).
The treatment of Xiao Ke (Wasting-Thirst) focuses on nourishing Lung Yin, descending fire, and generating blood. It is treated according to the Upper, Middle, and Lower Jiao.
Take Coptidis Rhizoma powder (Huanglian mo, 黄连末), Trichosanthis Radix powder (Tianhuafen, 天花粉), human milk, fresh lotus root juice, and Rehmanniae Radix Fresh Juice (Sheng Dihuang Zhi, 生地黄汁).
Boil the two juices into an extract, mix in the aforementioned powders, and further process with ginger juice and honey into a paste. Slowly hold the paste on the tongue and swallow with a small amount of plain boiled water.
If appetite is good, add Gypsum Fibrosum (Ruan Shigao, 软石膏). Trichosanthes Root (Gualougen, 栝蒌根) is a miraculous herb for treating Xiao Ke (Wasting-Thirst).
Dai Yun stated: Edema refers to the condition where the whole body's skin is swollen, shiny, and blister-like. Treatment should focus on strengthening the Spleen, leaching out dampness, promoting urination, and improving dietary intake. Patients with substantial Yuan Qi may undergo purgation.
This occurs because Spleen deficiency fails to control water, leading to water flooding and erratic movement. One should tonify with Ginseng Radix et Rhizoma (Renshen, 人参) and Atractylodis Rhizoma (Baizhu, 白术). Once Spleen Qi is solidified, it will naturally transport and transform, ascend and descend, and regulate the pivot mechanism, allowing water to move naturally. This differs from the forced diuresis of Wu Ling San (Five-Ingredient Powder with Poria). The appropriate approach is to tonify the Middle, move dampness, and promote urination; purgation must strictly be avoided.
The primary formula is Er Chen Tang (Two-Cured Decoction) augmented with Atractylodis Rhizoma (Baizhu, 白术) and Ginseng Radix et Rhizoma (Renshen, 人参), assisted by Atractylodis Rhizoma (Cangzhu, 苍术), stir-fried Gardeniae Fructus (Zhizi, 栀子), Scutellariae Radix (Huangqin, 黄芩), and Ophiopogonis Radix (Maidong, 麦门冬) to subdue Liver Wood. If abdominal distension occurs, add Magnoliae Officinalis Cortex (Houpo, 浓朴). If Qi fails to circulate, add Aucklandiae Radix (Muxiang, 木香) and Akebiae Caulis (Mutong, 木通).
If Qi sinks, lift it with Cimicifugae Rhizoma (Shengma, 升麻) and Bupleuri Radix (Chaihu, 柴胡). Adjust according to symptoms, but always prioritize tonifying the Middle.
Postpartum conditions require primarily tonifying Qi and Blood, with minor additions of Atractylodis Rhizoma (Cangzhu, 苍术) and Poria (Fuling, 茯苓) to allow water to descend naturally. Use a large dose of Atractylodis Rhizoma (Baizhu, 白术) to tonify the Spleen. For fullness and obstruction, use Pinellia Rhizoma Preparatum (Banxia, 半夏), Citri Reticulatae Pericarpium (Chenpi, 陈皮), and Cyperi Rhizoma (Xiangfu, 香附) to monitor and control.
If heat is present, clear the Lungs using Ophiopogonis Radix (Maidong, 麦门冬) and Scutellariae Radix (Huangqin, 黄芩).
Another prescription: Use Gardeniae Fructus (Zhizi, 栀子) with the peel removed, take the seed, stir-fry, crush, and administer with rice water. If there is heat in the epigastrium and the pathology is located in the upper jiao, use the fruit with the peel.
