Treated an elderly patient who had overindulged in nourishing foods, damaging the Spleen with diet, resulting in frequent diarrhea; this is also a case of Pi Xie (Spleen Diarrhea).
Baizhu (Atractylodis Rhizoma, 2 liang, stir-fried) Baishaoyao (Paeoniae Radix Alba, 1 liang, stir-fried after mixing with wine) Shenqu (Massa Medicata Fermentata, 1.5 liang, stir-fried) Shanzha (Crataegi Fructus, 1.5 liang, stir-fried) Banxia (Pinelliae Rhizoma Preparatum, 1 liang, washed) Huangqin (Scutellariae Radix, 5 qian, stir-fried)
Grind the above into powder, wrap in lotus leaves, and steam-cook over rice to make pills.
Treated an elderly patient aged seventy, with a pale complexion, wiry and rapid pulse, but specifically a deep and slippery pulse at the Stomach position. Due to drinking white wine, he developed dysentery, passing blood-tinged watery pus, followed by abdominal pain, difficult urination, tenesmus. Ren Shen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Rhizoma) were used as the sovereign drugs; Gancao (Licorice Radix et Rhizoma), Huashi (Talcum), Binglang (Arecae Semen), Muxiang (Aucklandiae Radix), and Cangzhu (Atractylodis Rhizoma) were used as ministers. He was given at least twenty-five pills of Baohe Wan (Preserve Harmony Pill). On the second day, all previous symptoms subsided, except for the difficult urination, which was treated with Yi Yuan San (Benefit Origin Powder).
Dai Yun stated: Huoluan (Cholera-like disease) involves vomiting, characterized by both sound and material expulsion. When there is only sound without material expulsion accompanied by restlessness and chaos, it is called Gan Huoluan (Dry Cholera).
For persistent muscle cramps, men should pull their genitals with their hands, and women should pull near the sides of their breasts; this is a wonderful method from Qianjin Fang (Essential Prescriptions Worth a Thousand Gold).
Internal accumulation combined with external pathogenic invasion causes Yang to fail to ascend and Yin to fail to descend, resulting in disorder and separation. Do not drink rice soup under any circumstances, as it will cause immediate death. The pulse is often hidden (Fu), indicating a critical condition nearing extinction.
For existing vomiting and diarrhea, emesis can still be induced to raise the Qi.
General principle: Shengjiang Lizhong Tang (Fresh Ginger Decoction to Regulate the Middle) is best. Some cases are suitable for emesis, others for purgation. For emesis, Erchen Tang (Two-Cured Decoction) modified can be used; or decocting Catalpa wood can also induce vomiting.
This disease is extremely difficult to treat, with death occurring suddenly due to the obstruction of ascending and descending mechanisms.
This condition results from internal injury by food stagnation and external constraint by pathogenic factors. Using emesis implies a dual action of dispersing and resolving.
Emesis to raise the Qi is an excellent method. People commonly use salt water for this purpose.
Some use warm herbs to disperse the pathogen; this method avoids cold herbs for dispersion.
Erchen Tang (Two-Cured Decoction) plus Jiehe San (Harmonizing Powder): Chuanxiong (Ligusticum Chuanxiong Rhizome), Fangfeng (Saposhnikoviae Radix), Cangzhu (Atractylodis Rhizoma), Baizhi (Angelicae Dahuricae Radix).
When there is both sound and material expelled, it is called Ou Tu (Vomiting); when there is only sound without material, it is called Yue (Hiccup/Inverted Stomach Qi).
Cases include phlegm obstructing the middle jiao preventing food descent, rebellious Qi, cold qi stagnating at the stomach gate, or phlegm and heat in the Stomach. However, vomiting caused by fire and phlegm in the Stomach is very common.
Zhu Fengyi used Banxia (Pinelliae Rhizoma Preparatum), Shengjiang (Zingiberis Rhizoma Recens), and JuPi (Citri Reticulatae Pericarpium) as main ingredients.
Sun Taoren mistakenly identified Yue (Hiccup) as Ke Ni (Coughing against the diaphragm).
Liu Hejian stated that Ou (Vomiting) is caused by Fire Qi flaming upward; this is merely one aspect.
If there is heat in the Stomach and phlegm in the Diaphragm, use Erchen Tang (Two-Cured Decoction) added with Chao Zhizi (Gardeniae Fructus, stir-fried), Huanglian (Coptidis Rhizoma), and Shengjiang (Zingiberis Rhizoma Recens).
For chronic vomiting, it indicates Stomach deficiency failing to receive grain. Use Shengjiang (Zingiberis Rhizoma Recens), Ren Shen (Ginseng Radix et Rhizoma), Huangqi (Astragali Radix), Baizhu (Atractylodis Rhizoma), and Xiangfu (Cyperi Rhizoma).
