When pathogenic factors invade the Lung, skin pain occurs, accompanied by aversion to cold and fever, panting with upward rushing of Qi, sweating, and cough that draws pain in the shoulders and back. Treatment should select the Shu points on the anterior chest (Yingzhong Waiyu) and the sides of the third thoracic vertebra on the back. Press the area quickly with the hand; if the patient feels comfortable and relieved, then needle. Also select the Quepen point (CV15) to vent the pathogenic factors.
The Yellow Emperor asked: Why does disease of the Lung cause cough? Qibo replied: All five Zang organs and six Fu organs can cause cough when diseased, not only the Lung. The skin and hair are the exterior correspondence of the Lung. When pathogenic factors first invade the skin and hair, they enter the Lung through this correspondence. If cold food and drink enter the Stomach, the cold Qi travels up along the Lung meridian to the Lung, causing Cold in the Lung. Thus, external Cold and internal Cold combine, lodging in the Lung, resulting in Lung Cough. Each of the five Zang organs becomes diseased when it is its ruling season; if it is not its season, pathogenic factors from other organs may transmit to it. Humans correspond with Heaven and Earth, so each Zang organ contracts disease when exposed to Cold during its respective season. Mild exposure results in cough, while severe exposure leads to diarrhea or pain. In Autumn, dampness injures the body, and the Lung is the first to receive the pathogen. In Spring, the Liver receives it first; in Summer, the Heart; during the late summer (Zhiyin), the Spleen; and in Winter, the Kidney. The manifestation of Lung Cough includes cough with wheezing and audible sounds, and in severe cases, expectoration of blood. The manifestation of Heart Cough includes cough with heart pain, a sensation of obstruction in the throat (described as "yi" in *Suwen*), feeling like something is stuck, and in severe cases, sore throat and Bi syndrome of the throat. The manifestation of Liver Cough includes cough with pain in the hypochondrium, so severe that turning is impossible, leading to fullness in the hypochondrium upon turning. The manifestation of Spleen Cough includes cough with pain in the right hypochondrium, dull pain radiating to the shoulder and back, and in severe cases, expectoration of saliva/phlegm and inability to move, which exacerbates the cough. The manifestation of Kidney Cough includes cough with pulling pain between the waist and back, and in severe cases, expectoration of saliva/phlegm. If cough from the five Zang organs persists for a long time, the pathogen transfers to the six Fu organs. If Spleen Cough persists, the Stomach is affected; Stomach Cough manifests as cough with vomiting, and in severe cases, expulsion of roundworms. If Liver Cough persists, the Gallbladder is affected; Gallbladder Cough manifests as cough with vomiting of bile. If Lung Cough persists, the Large Intestine is affected; Large Intestine Cough manifests as cough with involuntary defecation. If Heart Cough persists, the Small Intestine is affected; Small Intestine Cough manifests as cough with flatus, where gas escapes with the cough. If Kidney Cough persists, the Bladder is affected; Bladder Cough manifests as cough with urinary incontinence. If cough persists for a long time, the Sanjiao (Triple Burner) is affected; Sanjiao Cough manifests as cough with abdominal fullness and loss of appetite. These conditions all involve accumulation in the Stomach and involvement of the Lung, causing excessive phlegm and drool, facial edema, and rebellious Qi. To treat Zang diseases, select their Shu points; to treat Fu diseases, select their He-sea points; for edema, select their Jing-river points. Injury by dampness in Autumn generates cough in Winter.
A question was posed: The *Jiu Juan* (Nine Scrolls, i.e., *Ling Shu*) mentions the "Zhen Ai" (Shaking Dust) needling method, which involves needling Yang meridians to eliminate Yang-pathologies. I wish to hear the details. The answer is: When Yang Qi rebels severely, filling the chest, causing mental agitation, shoulder breathing, and upward rushing of Great Qi, leading to noisy panting, restlessness in sitting and lying, and throat obstruction with dyspnea, select Tianrong (SI17). For cough with rebellious Qi and severe chest pain, select Lianquan (CV23). When needling Tianrong, the depth should not exceed one cun ("li" is likely an error). When needling Lianquan, stop when the blood color changes (from dark to bright red).
For cough with rebellious Qi, Bahu (BL42) and Qishe (ST11) are indicated. For cough with rebellious Qi, Yixi (BL68) is indicated.
