1. Regarding the East, Jia-Yi Wood: The patient's complexion is greenish, indicating a deficiency pattern accompanied by a sour sensation. The Kidney is the mother of the Liver (Mother-Child relationship in Five Elements). When the Liver suffers from deficiency, the main symptoms are abdominal pain and cold tears flowing from the eyes. It is appropriate to apply the Pan-Yao (plate-shaking) technique along the specified pathways, performing the needle manipulation 21 times (3 x 7), which treats abdominal pain. When the Zang organs are diseased, using the "Xiang" method (producing sound via lifting and scraping) constitutes a contrary treatment. A compliant treatment involves needling Ganshu (BL18), primarily using lifting and scraping techniques to treat conditions such as cold tears. This is known as the Shun-Xing-Pan (compliant plate rotation) technique.
The Yao (shaking) method is contrary and non-compliant. No elaborate explanation is needed.
2. Regarding the South, Bing-Ding Fire: Its color is red, indicating a deficiency pattern with bitter taste. The corresponding Zang organ is the Heart; the Heart governs Blood, and the nature of Fire is to flare upwards. Stagnation of blood flow injures Heart Qi and Lung Qi, causing coughing; this results in the formation of purple blisters in the Lungs. If these purple blisters rupture, it manifests as coughing up blood, with a yellow or reddish face. Expectoration of phlegm indicates that Lung Qi fails to harmonize with Blood. If only phlegm-blood is expectorated accompanied by lower back pain, it signifies Kidney deficiency, leading to death. Death may occur within two years or up to twelve years. Hemoptysis leads to death in Spring and Summer, which is incurable. Additionally, for those with Heart injury causing five-Zang pain: expectorating blood in Winter leads to death on a day in Summer; expectorating in Spring leads to death on a day in Autumn; expectorating in Summer leads to death on a day in Winter. It is suitable to apply the principles of Yao-Yao and Pan-Pan without further annotation.
3. Regarding the West, Geng-Xin Metal: During manipulation, the color is white. The corresponding Zang organ is the Lung, which governs Qi and generates Blood. Heart blood flows to the Lungs, forming purple blisters. If these blisters rupture, continuous hemoptysis occurs. One should moxa the Shenshu (BL23) acupoint twice and Zusanli (ST36) acupoint twice. Moxibustion is forbidden inside the abdomen; instead, the principle of Pan-Yao should be used. If there are many Pan-Pan methods, one must not rotate arbitrarily to avoid chaotic Qi in the five Zang organs. For any disease, use light extraction and light rotation, ensuring the needle produces sound through lifting and extending.
4. Regarding the North, Ren-Gui Water: Its color is black. The corresponding Zang organ is the Kidney, governing bones and joints, associated with the salty flavor. The Lung is the mother of the Kidney. Regardless of gender, moxibustion should be performed first. Inside the abdomen, the Pan-Yao technique should be used, nourishing Kidney Water as the foundation.
5. Regarding the Center, Wu-Ji Earth: Its color corresponds to the Dan Xue points on the abdomen. Central Earth is the root. The corresponding Zang organ is the Spleen, governing internal functions. [Text refers to specific timing/methods involving 'Wang Ri Yuan' and 'Pan San', initially using methods shown in diagrams.] Spleen Earth is prosperous for eighteen days at the end of each season. Encountering Kun Earth, Earth can transform and generate the eleven Zang organs, receiving influence from the East of the Stomach, thereby enabling transformation and generation. Rotating within the abdomen can resolve cold and hot cold air. Distributing this to various Zang organs transforms them into the functions of the eleven Zang organs. For cold pain, rotate left 49 times (7 x 7); for heat pain, stop. Rotate right 49 times with lifting and shaking. For cold qi pain, lift first, then rotate left. Whenever the "Xiang" method is used inside the abdomen: seven lifts, three tonifications, five shakes. For cold patterns, use lifting; for heat patterns, use right-side sounding. This addresses cold qi pain; lifting stops the pain, and treating below Neiting (ST44) on the left yields immediate effect.
