Medical texts record that the needle insertion depth for Zhongwan (CV12, Middle Jiao) is 0.8 cun. However, the depth of subcutaneous tissue and peritoneum varies among individuals; the practitioner must carefully perceive this. When inserting the needle, the skin initially feels firm; advance the needle slowly. Once past the skin into the subcutaneous layer, it feels as if falling into empty space. Upon reaching the peritoneum, the patient will suddenly feel a sensation of resistance and slight movement. At this point, stop advancing the needle, retain it for ten breaths, then withdraw it slowly. For other acupoints, needling may be performed every other day, but for Zhongwan (CV12), needling should be performed once every seven to eight days. Although frequent eating is permitted after needling, one must avoid overeating, otherwise harm may result.
The accumulation in the Heart is called Fuliang (Overlapping Beam); in the Lung, Xiben (Panting Accumulation); in the Liver, Feiqi (Fatty Qi); in the Spleen, Piqi (Stagnant Qi); in the Kidney, Ben Tun (Running Piglet). "Ji" (Accumulation) primarily governs Zang organ diseases, while "Ju" (Gathering) primarily governs Fu organ diseases. "Ji" forms from food stagnation and knotting that does not dissipate; "Ju" arises from phlegm-dampness lurking above the diaphragm, mainly manifesting as dizziness and pain in the head and eyes, often with spitting of saliva, and sometimes causing mild fever.
For phlegm accumulation forming masses: Moxibustion on Feishu (BL13, Lung Shu) 100 moxa cones; Moxibustion on Qimen (LR14, Gate of Life) 3 moxa cones.
For Ben Tun Qi (characterized by lower abdominal pain): Moxibustion on Qihai (CV6, Sea of Qi) 100 moxa cones; Moxibustion on Qimen (LR14) 3 moxa cones; Moxibustion on Duyin (EX-HP1, Unique Yin) 5 moxa cones; Moxibustion on Zhangmen (LR13, Door of the Rib) 100 moxa cones; Moxibustion on Shenshu (BL23, Kidney Shu) according to age in years; Needling Taichong (LR3, Great Surge) 3 needles; Needling Taixi (KI3, Supreme Stream) 3 needles; Needling Sanyinjiao (SP6, Three Yin Intersection) 3 needles; Bloodletting at the nail root 3 needles each.
For lower abdominal accumulation, generalized bi-syndrome of the waist and spine, cough, and difficult defecation: Moxibustion on Shenshu (BL23) according to age in years; Moxibustion on Feishu (BL13) 7 needles; Moxibustion on Dachangshu (BL25, Large Intestine Shu) 7 needles; Moxibustion on Ganshu (BL18, Liver Shu) 7 needles; Needling Zhongquan (EX-UE9, Central Spring); Needling Duyin (EX-HP1); Needling Quchi (LI11, Pool at the Crook).
For abdominal accumulation with qi rebellion moving up and down: Moxibustion on Zhongji (CV3, Central Pole) 100 moxa cones; Moxibustion on Xuanshu (GV9, Suspended Pivot) 3 moxa cones (located below the spinous process of the 13th thoracic vertebra), taken when lying prone.
Another prescription: For treating painful qi, needle along the pain site and apply Gang Moxibustion using a three-edged needle. ○ Gang Moxibustion method applies to the abdomen.
Specifically treating stagnant masses: Moxibustion on Pigen (EX-B2, Mass Root) located 3.5 cun lateral to the lower border of the 12th thoracic vertebra, moxa heavily on the left side. ○ If masses exist on both sides, moxa both sides.
Another prescription: One acupoint above the head, insert needle 2.5 cun, moxa 14 cones; One acupoint in the center of the mass, insert needle 1-2 cun, moxa 21 cones; One acupoint at the tail of the mass, insert needle 3.5 cun, moxa 7 cones.
For nodules under the umbilicus resembling a basin: Moxibustion on Guanyuan (CV4, Gate of Origin) 30 moxa cones; Moxibustion on Jianshi (PC5, Interseas) 30 moxa cones; Needling Taichong (LR3) 3 needles; Needling Taixi (KI3) 3 needles; Needling Sanyinjiao (SP6) 3 needles; Moxibustion on Shenshu (BL23) according to age in years; Moxibustion on Duyin (EX-HP1) 5 moxa cones.
For Fuliang and Ben Tun accumulations: Moxibustion on Zhangmen (LR13); Moxibustion on Pishu (BL20, Spleen Shu); Moxibustion on Sanjiaoshu (BL22, Triple Energizer Shu); Moxibustion on Zhongwan (CV12); Needling Duyin (EX-HP1); Needling Taichong (LR3).
The Spleen governs the four extremities (limbs). All sores and swellings of the hands and feet belong to the Back, which is associated with the Spleen and Stomach; all painful or itching sores belong to Heart Fire. Excess cold leads to tendon spasms and bone pain; excess heat leads to flaccid tendons and bone consumption. Treatment focuses on regulating the Six Yin and Six Yang meridians. Disease on the left is treated on the right; disease on the right is treated on the left; disease in the interior is treated via the exterior; disease in the exterior is treated via the interior; disease above is treated below; disease in the abdomen is treated via the back. This is called drawing Yin from Yang and drawing Yang from Yin.
For arm tendon spasms, soreness, and pain, leading to disability, inability to eat/drink, or loss of consciousness: The practitioner presses firmly on the tender spot where the tendon knot is located with the thumb of the left hand to immobilize it, then pierces the tendon knot directly with the needle. If the needle tip touches the injured tendon, the patient will feel unbearable soreness and pain; this point is known as Tianying (Ashi Point/Responsive Point). Needle along the pain, and the effect is miraculous; if not effective, needle again. ○ There is no method superior to this for needling meridian acupoints. If the needle injures the tendon, recovery is immediate; if not, the condition temporarily relieves, and needling the original acupoint brings rest.
For dislocation and chronic soreness/pain of finger joints: Flex the injured finger to limit skin and bone retraction, then deeply insert a round-sharp needle into the wrinkle lines, withdrawing without suction. Injuries to other joints are treated similarly.
For elbow joint soreness and pain: Have the patient flex the elbow to locate Quchi (LI11) near the transverse crease. Insert the needle deeply through the skin below the elbow, avoiding the tendons. Recovery occurs naturally within ten days; the effect is miraculous.
For shoulder pain lasting months, with stiffness like glue preventing elevation: Locate the hollow area between the lower shoulder and upper armpit. Insert the needle until the tip nearly protrudes from the skin, similar to the elbow treatment. Avoid injuring bones; also needle tendon knots. The effect is miraculous.
For bruises and falls: Needle according to the affected meridian and select Tianying (Ashi Points). Apply moxibustion afterwards to resolve blood stasis.
For muscle spasms in both arms and chest: Moxibustion on Daling (PC7, Great Empress) 7 cones; Moxibustion on Danzhong (CV17, Center of Chest), Jueque (CV14, Extreme Abyss), and Chize (LU5, Pool at the Crook). Also treats tendon spasms in hands and feet.
For thin and weak arms: Moxibustion on Quchi (LI11); Moxibustion on Lieque (LU7, Broken Sequence); Moxibustion on Chize (LU5); Moxibustion on Zhigou (TE6, Intercalated Valley); Moxibustion on Zhongzhu (TE3, Central Well).
For soreness and heaviness in elbows and wrists: Moxibustion on Neiguan (PC6, Inner Pass); Moxibustion on Waiguan (TE5, Outer Pass); Moxibustion on Jiegu (GB39, Bone Gap); Moxibustion on Shenmen (HT7, Spirit Gate); Moxibustion on Hegu (LI4, Joining Valley); Moxibustion on Zhongwan (CV12). If tendon spasms occur, needle Tianying (Ashi Points) for immediate effect.
For restless movements of the arms: Moxibustion on Quze (PC3, Marsh Curve) 7 cones; Needling Taichong (LR3); Moxibustion on Ganshu (BL18); Moxibustion on Shenmen (HT7).
For hot palms: Moxibustion on Neiguan (PC6); Moxibustion on Lieque (LU7); Moxibustion on Quchi (LI11); Moxibustion on Tongli (HT5, Connecting River); Moxibustion on Shenmen (HT7); Moxibustion on Houxi (SI3, Rear Stream).
For pain and itching on the medial aspect of the arm: Moxibustion on Quchi (LI11); Moxibustion on Feishu (BL13); Moxibustion on Pishu (BL20); Moxibustion on Shenmen (HT7); Needling Zhongwan (CV12).
For inability to flex and extend fingers: Needling below Quchi (LI11) towards Sanli (LI10); Needling Waiguan (TE5); Needling Zhigou (TE6); Needling Hegu (LI4); Needling Zhongwan (CV12); Needling Jiegu (GB39); Needling Zhongzhu (TE3); Cross hands and moxa one cone at the radial end of the transverse crease of the first phalanx of the thumb. Effect is miraculous.
For axillary swelling: Needling Xingjian (LR2, Moving Between); Needling Shenmen (HT7); Needling Taiyuan (LU9, Great Yuan); Needling Jiegu (GB39); Moxibustion on Danshu (BL19, Gallbladder Shu); Needling Wangu (SI4, Bone Peak).
For weakness in the left hand and foot: Moxibustion on Shenque (CV8, Divine Gate) 100 moxa cones. If no improvement, add 500 moxa cones.
For intense heat in both hands as if in fire: Moxibustion on Yongquan (KI1, Gushing Spring) 5 moxa cones. Effect is immediate.
