Patient Wang Delin, male, aged 78, cadre. In January 1981, he presented with dysphagia (difficulty swallowing), progressive emaciation, and chest/back pain. Esophagoscopy and pathological examination diagnosed poorly differentiated squamous cell carcinoma of the upper esophagus, approximately 4 cm in length, with mucosal destruction; surgery was not indicated. Before radiotherapy, white blood cell (WBC) count was only 2×10^9/L, so treatment was temporarily deferred. At that time, the patient exhibited pale and puffy complexion, dizziness, shortness of breath, fatigue, vomiting of phlegm-saliva, and could only consume semi-liquid diet. Pulse was fine and weak, tongue coating thin and white. The syndrome was attributed to Qi deficiency and Yang depletion, with Phlegm-Dampness stagnation. The treatment principle was to tonify Qi and strengthen the Spleen, promote diuresis to resolve Dampness and dissipate nodules. Prescription: Biezhishen (Prepared Codonopsis Pilosula Root) 5g, Chao Baizhu (Stir-fried Atractylodis Macrocephalae Rhizoma) 12g, Jiang Banxia (Ginger-processed Pinelliae Tuber) 10g, Houpo (Magnoliae Officinalis Cortex) 10g, Yiyiren (Coicis Semen) 30g, Fuling (Poria) 12g, Daizheshi (Haematitum) 12g, Shougong (Gecko) 3 pieces, Xianhecao (Agrimoniae Herba) 30g. After taking 7 doses, WBC rose to 3.9×10^9/L, and symptoms improved. He then underwent a three-field cross radiotherapy plan with Cobalt-60 irradiation dose of 6500 C Gy over 36 fractions in 70 days. During this period, he continued taking the prescription without interruption. WBC remained stable around 4×10^9/L. Follow-up X-ray showed disappearance of original mucosal destruction, smooth tube wall, free dilation, and good passage of barium. He was instructed to replace Biezhishen with Renshen (Ginseng Radix et Rhizoma) 12g in the original prescription and continue taking it. In 1996, upon follow-up examination, his complexion was ruddy, swallowing was unobstructed. He had persisted in taking the prescription since the end of radiotherapy, consuming more than 10 kg of Ginseng in total, and survived well for 21 years.
Commentary: Carcinomas of the upper esophagus are mostly poorly differentiated with frequent lymph node metastasis; surgery is generally not adopted, and the 5-year survival rate after radiotherapy is about 10%. In this case, the initial prescription raised the WBC count, allowing successful completion of radiotherapy. Subsequently, treating both root and branch consolidated the curative effect, resulting in 21 years of survival, demonstrating the efficacy of Ginseng in supporting Zheng (Righteous Qi) and consolidating the foundation.
3. Ginseng combined with Lung-tonifying and Diuretic herbs to treat Lung Cancer
Patient, female, 68 years old, farmer. In February 1984, she developed cough, dyspnea, and chest tightness. Treatment for "bronchitis" for two months yielded no improvement. In April, X-ray of the whole lung and tomography revealed an 8 cm × 5 cm mass with uniform density and relatively clear margins in the lower segment of the right lung. Sputum cytology found squamous cell carcinoma. Diagnosis: Right Lung Cancer. On April 22, Cyclophosphamide 600 mg and Bleomycin 2 mg were administered via intravenous drip as part of a combination therapy once every other week, for 3 cycles. After injection, palpitations, nausea, vomiting, night sweats, insomnia, cough, and chest pain worsened. Difficulty urinating appeared; diuretics were ineffective, requiring catheterization. The catheter was removed several times but reinserted due to severe urinary retention. Unable to tolerate the toxic reactions of chemotherapy, she sought Traditional Chinese Medicine (TCM) treatment. Four diagnostic methods revealed a haggard appearance, rapid breathing, more exhalation than inhalation, pale tongue body with purple stasis spots on the sides, and fine-rapid pulse. This indicates cancer masses obstructing the Lung, failure of descending function, abnormal distribution of Jin-Ye (Body Fluids), and dysfunction of water passages, preventing downward transmission to the Bladder, hence urinary obstruction. Treatment method: Raise Lung Qi using Du Shen Tang (Single Ginseng Decoction), preparing Biezhishen (Prepared Codonopsis Pilosula Root) 6g by stewing. Two hours after ingestion, urination became smooth, expelling urine along with the catheter. The next day's prescription: Biezhishen (Prepared Codonopsis Pilosula Root) 5g, Shengma (Cimicifugae Rhizoma) 9g, Shanhai Luo (Helwingia Japonica Root) 30g, Tiandong (Asparagi Radix) 10g, Dongchongxiacao (Cordyceps) 10g, Zhi Tinglizi (Honey-processed Descurainiae Semen) 30g, Banzhilian (Scutellaria Barbata Herb) 30g, Dilong (Pheretima) 30g, Cheqianzi (Plantaginis Semen) 15g. After 3 doses, urination was smooth, complexion regained luster, and breathing stabilized. Third visit prescription: Hongshenxu (Red Ginseng Stems) 12g, Sheng Huangqi (Astragali Radix) 30g, Baizhu (Atractylodis Macrocephalae Rhizoma) 15g, Dongchongxiacao (Cordyceps) 10g, Dilong (Pheretima) 30g, Fangji (Stephaniae Tetrandrae Radix) 12g, Tufuling (Smilax Glabrae Rhizoma) 30g, Banzhilian (Scutellaria Barbata Herb) 30g, Sheyescao (Hedyotis Diffusa Herb) 30g, Yiyiren (Coicis Semen) 30g, taken intermittently. In May 1989, upon follow-up, she reported being able to perform light housework. X-ray showed lesions similar to previous ones. She lived with the tumor for 4 years before being lost to follow-up.
Commentary: Cancer masses obstructing the Lung, combined with chemotherapy toxicity, weakened Lung Qi. When Clear Qi fails to ascend, Turbid Yin fails to descend, and Lung Qi loses its descending function, leading to more exhalation than inhalation and inability to regulate water passages. Treating the disease must seek the root. Using Biezhishen (Prepared Codonopsis Pilosula Root) to raise Lung Qi produced immediate results, restoring normal urination. Subsequently, continuing with Ginseng, Cordyceps, Fangji, and Dilong to tonify the Lung and promote unblocking allowed the patient to turn danger into safety and extend life by several years.
4. Reflections
Modern research considers Ginseng a tonic herb with bidirectional regulatory effects, capable of regulating and balancing diseases caused by excess or deficiency in the human body. It can significantly enhance immune function, improve endocrine function and cellular vitality, and inhibit tumor occurrence. Ginseng has the ability to restore bone marrow hematopoietic function and increase hemoglobin levels. As a general tonic, it enhances patients' tolerance to treatment, thereby increasing opportunities for various therapies and improving efficacy. It has obvious effects on common side effects of radiotherapy and chemotherapy, such as leukopenia, liver damage, nausea, emaciation, dry mouth, and sweating.
There are many varieties of Ginseng with different usage methods. For acute conditions, severe illness, and Yang deficiency, Biezhishen (Prepared Codonopsis Pilosula Root) or Yeshanshen (Wild Ginseng) are suitable. For general deficiency syndromes and long-term use, Hongshen (Red Ginseng) or Shenxu (Ginseng Stems) can be selected. If Yin deficiency with fire is present, or when used jointly with heat-clearing and detoxifying herbs, Xiyangshen (American Ginseng) is appropriate.
II. Toad
The toad appears ugly, with gray-black skin covered in warts. It gets angry, inflates its belly, and makes a miserable "ga ga..." sound. It often inhabits ponds, under stone steps, and in grass piles, moving clumsily. People dislike it upon sight, and children may beat it with sticks. When someone attempts something beyond their capability or desirable yet unattainable, others mock them as having unrealistic fantasies, adding, "You toad wanting swan meat!" In reality, people's attitude towards toads is not just prejudice but cruelty. Recently, newspapers reported illegal trading markets where tons of toads were sold daily. In suburban Shanghai, there is a custom of eating toads as delicacies, which has spread to Guangdong and other regions. Restaurant menus list "Xun Lasi" (actually toad), prepared braised, stewed, or fried. Nowadays, toads, once ubiquitous, are hard to find; they might eventually receive protection like giant pandas. Actually, toads deserve attention and protection. Although ugly, the "ink-green" color on their tongues is the most beautiful in the world, more alluring than Burmese jade. Toads are beneficial insects, eliminating hundreds of pests daily, such as flies, mosquitoes, cabbage worms, and ants. They also have high research and medicinal value. Today's Reference News published American research stating that treating Hepatitis B with toad serotonin is several times more effective than Western interferon. Scholars, literati, and common people throughout history have praised it.
Historical Records
Toad was anciently referred to as "Moon Essence." *Taiping Yulan* Vol. 4 cites *Chunqiu Wei Yan Kong Tu*: "Toad is the essence of the moon." In the imperial examination era, "plucking the laurel in the Moon Palace" metaphorically represented achieving top honors like Zhuangyuan (first place). Friends often used blessings like "May you pluck the laurel in the Moon Palace early." Ten years of bitter study hope to pluck the laurel branch in the Moon Palace. Toad is a traditional Chinese medicine. According to medical literature, its nature is pungent, cool, and toxic. It has effects of breaking accumulations, removing stagnation, promoting water metabolism, resolving toxins, relieving pain, and strengthening the heart. Ancient people already used it clinically to treat malignant sores, scrofula, tumor swelling, edema, chronic osteomyelitis, and heart failure.
The famous traditional Chinese patent medicine Liushen Wan (Six God Pills) from Hu Qing Yu Tang is made primarily from Chan Su (Toad Venom). Compiled at the end of the Eastern Han Dynasty, *Shennong Ben Cao Jing* (*Divine Farmer's Materia Medica*), the earliest existing pharmacological monograph in China, recorded the functions of toads, which were further elaborated in subsequent dynasties like Tang, Song, Ming, and Qing.
Historical Records: *Shennong Ben Cao Jing*: "Taste pungent and cold. Treats pathogenic factors, breaks hard masses, blood abscesses, and genital wounds. Taking it prevents febrile diseases. Grows in ponds."
Tang Dynasty · Zhen Quan *Yaoxing Bencao*: "Kills intestinal parasites, treats rat fistulas and malignant sores. Burn to ash and apply to all insect-infested malignant sores and infant eczema."
Song Dynasty · *Rihuazi Zhujia Bencao*: "Treats childhood malnutrition, yellow face in children, and abdominal masses. Breaks hard masses. Burn to ash, mix with oil, and apply to malignant sores. Mix Chan Su (Toad Venom) with cow butter or Evodia seedling juice, rub on waist/kidney area and scrotum to treat cold waist and kidneys, boost Yang Qi, and treat toothache caused by worms."
Ming Dynasty · *Bencao Gangmu*: "Treats all five types of childhood malnutrition, eight types of dysentery, swelling/toxins, tetanus, rectal prolapse, back carbuncles, and all malignant swellings."
Ming Dynasty · *Bencao Huiyan*: "Herb for treating childhood malnutrition, reducing distension, and resolving carbuncle toxins. It can resolve all kinds of stagnant and obstructed diseases, such as accumulated toxins, masses, distension, and internal carbuncle abscesses, possessing the power to attack and extract toxins."
Qing Dynasty · *Bencao Qiuyuan*: "Toad tastes pungent and cold. It is effective for abdominal masses, dog bites, childhood malnutrition, epidemic macules, and back carbuncles. Because pungency disperses and coldness eliminates heat, external application shows miraculous effects, and internal consumption removes accumulation and clears heat. Chan Su (Toad Venom) tastes pungent, warm, and is toxic. It can extract all wind-fire pathogens. Pungent-warm treatments disperse and move, causing pathogenic factors to exit through sweat, preventing internal entry, thus clearing heat."
Compatibility Application: Both Toad and Chan Su (Toad Venom), whether taken internally or applied externally, have effects of resolving toxins, reducing swelling, and relieving pain. Clinically, they are often combined with blood-invigorating, swelling-reducing, and pain-relieving herbs. For example, *Waike Dacheng*'s Yuhong San consists of Chan Su, Sheng Yao, Frankincense, and Myrrh, used for ulcers where necrotic flesh has not detached or non-healing ulcers forming sinuses. Gangrenous stomatitis (Zouma Yagan) is common in children, with acute onset and severe condition. *Yangshi Jiacang Fang*'s Bixiao San uses Chan Su, Renzhongbai, and Musk ground together for external application to treat all types of stomatitis, gingival erosion, bad breath, and bleeding. *Zhengshi Xiao'er Fang* uses toad, Coptis, Indigo Naturalis, and Musk ground together for external application to treat gangrenous stomatitis eroding the mouth and nose. *Houke Xinfa*'s Liushen Wan consists of Chan Su, Realgar, Pearl, Moschus, and Cinnabar ground into fine powder, formed into pills with rice paste, coated with Carbonis Herbæ Artemisiae Argyi. It treats epidemic toxins, sore throat with scarlet fever-like rash, throat wind/abscess, and bilateral/unilateral tonsillitis making swallowing difficult. It also treats carbuncles, back abscesses, appendicitis, abdominal abscesses, mastitis, breast cancer, and all unnamed swellings. Additionally, it treats pediatric phlegm-heat convulsions and critical pulmonary phlegm wheezing. For cholera-like vomiting/diarrhea with abdominal pain, summer damp-cold turbid qi, or ingesting unclean food causing abdominal pain, vomiting, diarrhea, cold limbs, hidden pulse, or even coma, internal Chan Su has effects of dispelling filth, relieving pain, opening orifices, and awakening the spirit. Clinically, it is often used with aromatic herbs like Musk, Clove, Atractylodes, and Amber, e.g., *Jiangnang Cuoyao* Chan Su. *Puji Fang*'s Chansu Tuoli Wan treats all malignant sores, consisting of Chan Su, Pangolin Scales, and Carbonis Herbæ Artemisiae Argyi. *Yixue Zhengchuan*'s Chansu Gao (Chan Su, White Civet Cat Excrement, Halloysitum Rubrum, etc.) ground fine for external application treats scrofula. Furthermore, combining Chan Su and Musk is extremely effective for toothache (*Shenghui Fang*). *Waike Dacheng*'s Chansu San uses Chan Su, Pinellia, Datura, and Pepper for surgical anesthesia. *Sanyin Ji Yi Bingzheng Fanglun*'s Chan Chu Gao treats deep bone abscesses that do not heal, with corrupted pus or bone protruding from the wound. One large toad, a piece of loose hair, and one jin of pork fat are decocted together, strained, and condensed into a plaster for application until healed...
Folk Application: In folk medicine, toads are highly favored. In Hangzhou, it has been a long-standing custom during the Dragon Boat Festival to eat toads and drink realgar wine to prevent summer heat toxins. To treat childhood malnutrition with abdominal distension, yellow/thin emaciation, head sores resembling wheat ears, one processed large toad taken after Start of Autumn causes accumulated filth to pass down, changing the appearance miraculously after one month. For umbilical sores in infants oozing fluid and not healing, burning toad and applying ash three times daily is very effective.
Treating Tetanus: Chop toad into mud, add one liang of Sichuan peppercorn, stir-fry with wine until cooked, then add two and a half liang of wine. Take warm. Profuse sweating follows shortly, showing miraculous effects. Alternatively, use two qian of Chan Su dissolved in water to make a paste, stir-fried dried scorpion and Tianma (Gastrodiae Rhizoma) each half liang, grind into powder, mix and pill to mung bean size. Take one to two pills with Doulin Wine.
Treating Rabies Bite: Recurs every seven days: Raw toad meat is excellent, or it can be burned to ash and eaten.
Fracture Setting: Grind fresh large toad into mud, wrap with split bamboo, and the bone will heal spontaneously.
Extracting Carbuncle Toxin: Make Chan Su into pills the size of wheat grains using white flour and Minium. Pierce the carbuncle, insert the pill, and cover with Water-Clear Plaster.
Treating Throat Bi and Tonsillitis: Mix Toad Eyebrow Venom with powdered Grass Aconite tip and Pig Tooth Soap Pod in equal parts, form into pills the size of mung beans. Grind one pill and apply to the affected area, showing miraculous effects.
Treating Early Stage Carbuncles and Unnamed Swellings: Heat Chan Su, Guang Jiao (Gelatinum Corii Asini), and Rice Vinegar together. Rub continuously on the affected area until healed.
Usage Precautions: Contraindicated in pregnancy. Chan Su should not enter the eyes when applied externally.
Modern Research
Toad has been recorded in medicine since the Eastern Han Dynasty, with effects of clearing heat, resolving toxins, invigorating blood, removing stasis, softening hardness, and dissipating nodules. Modern pharmacological research finds it has anti-cancer, leukocyte-raising, cardiotonic, pressor, and respiratory-stimulating effects.
Chemical Composition: Toad parotoid glands and skin glands contain various chemical components, including: Bufadienolides (Bufalin, Hellebrigenin, Resibufogenin, Bufogenin, Hellebrigenin, Hydroxybufagin, Lipobufagin...); Cardenolide bufotoxins (Samogenin-3-hexahydroazelaate, Samogenin-3-hexahydroazelaic acid arginine ester, Samogenin-3-heptahydroazelaic acid arginine ester, Samogenin-3-sulfate...); Indole alkaloids (Serotonin, Bufotenin, Bufotenine, Bufotenidine, Dehydrobufotenin...); Sterols (Cholesterol, 7β-Hydroxycholesterol, Ergosterol, etc.); Others include polysaccharides, organic acids, amino acids, peptides, and adrenaline.
Pharmacological Effects: Toad preparations improve hemorheology and visceral vascular function. They have cardiotonic effects. Regarding myocardial ischemia (in vitro experiments show: Chan Su prolongs fibrinogen coagulation time, its anticoagulant action is similar to urokinase, activates lysosomes after fibrinolysis, and increases coronary artery blood flow. For ischemic heart disorders like myocardial infarction caused by coronary stenosis due to thrombosis, it increases myocardial nutritive blood flow, improves microcirculation, and increases myocardial oxygen supply.), it has obvious arterial pressure-raising effects and good efficacy for endotoxemia-induced shock (mainly related to its cardiotonic, pressor, and platelet aggregation-inhibiting effects). It can raise white blood cells and increase serum IgG levels. It stimulates respiration. It has anti-tumor, anti-inflammatory, and anesthetic effects. It excites intestinal smooth muscle, increasing contraction amplitude and frequency. It also has antitussive effects. It inhibits Pseudomonas aeruginosa, Streptococcus, Staphylococcus, and Proteus.
Compatible Preparations: Chan Chu Zhitong Gao (Toad Analgesic Plaster) consists of Chan Su, Strychnos Nux-vomica, Arisaema, Aconitum, Realgar, and Angelica Dahurica. Functions: Reduce swelling, relieve pain, dissipate nodules. Suitable for pain relief and dissipation of various masses, and pain caused by muscle strain, bone spurs, rheumatoid arthritis, etc. Chan Chu Ding (Toad Suppository/Cake) consists of Chan Su, Musk, Borneol, Centipede, Derris, and Rhubarb. Functions: Clear heat, resolve toxins, reduce swelling, relieve pain. Used for carbuncles, back abscesses, unnamed swellings, early-stage malignant sores, hard painful swellings, and red/swollen/itchy/painful areas after venomous insect bites. Chansu Pian (Chan Su Tablets) consists of Chan Su and Dandelion. Functions: Anti-tuberculosis, anti-inflammatory. Mainly treats tuberculosis and infections. Jinpu Jiaonang (Jinpu Capsules) consists of Chan Su, Realgar, Coelia Gigantea, Sophora Flavescens, and Amomum. Functions: Clear heat, resolve toxins, reduce swelling, relieve pain, tonify Qi, transform phlegm.
