Zhang Dairen said: During the Taihe era, I witnessed a monk at Guangji Hospital suffering from cholera. A practitioner decocted Fuzi (Aconite Lateral Root) and Ganjiang (Dried Ginger) together, allowed it to cool, and administered it. After ingestion, the patient vomited blood and died (Such a death indicates a heat syndrome caused by summerheat. Huixi stated: Summerheat syndromes contraindicate the use of Shengjiang (Fresh Ginger); even when used with Huanglian (Coptis Rhizome) and Huangqin (Scutellaria Root), it causes significant harm. How much more destructive is its combination with Fuzi?). Shortly after, Li Yanzhi of Heliu Town suddenly experienced vomiting and diarrhea in the middle of the night. He self-administered Lizhong Wan (Regulate the Middle Pill). (Huixi noted: This formula treats cold-type cholera; it is applicable in less than one percent of cases. Misuse leads to immediate disaster). When the physician arrived, he diagnosed food stagnation and prescribed three to five pills of Badou (Croton Seed) medicine to induce purgation. There was no effect, and the patient died by dawn (Even if there were food stagnation, why use the harsh purgative Badou?).
Later, Li Zhongan from Pingliang, accompanied by a servant and a tenant farmer, traveled to Yancheng. They stayed overnight at the home of Shao Fu. That night, the servant fled. Upon discovering this, Li Zhongan mounted his horse and pursued him with the tenant farmer toward Linying. It was July, the weather was extremely hot, and the scorching wind felt like arrows, with dust filling the sky. They returned by the Chen hour (7-9 AM). In less than three shichen (6 hours), they had traveled over 120 li round trip without catching the fugitive, and they lodged again in the study of the Shao residence. Suddenly, moaning sounds were heard at night, crying for help, but the source was unknown. Carrying a torch to search, they found Li Zhongan's tenant farmer. The patient suffered from upper vomiting and lower diarrhea, eyes fixed upward, severe pain in the chest and hypochondrium preventing movement, jaw dislocation due to yawning, and cold limbs. This was a syndrome combining Wind, Dampness, and Summerheat (The large wind at night brought cold qi; riding fast added physical exhaustion). Having heard my advice previously, he took Liuyi San (Six-to-One Powder), mixed it with fresh well water and chopped Shengjiang (Fresh Ginger), and ingested half a sheng at once. The patient vomited again, so another half sheng was prepared and administered slowly. After a long while, the symptoms subsided (In vomiting syndromes, medicines are often rejected; slow administration is the correct method). By dawn, after several more doses, the patient could rise (Shengjiang is not decocted but chopped into fresh well water to mix with Liuyi San, utilizing its mild acridity to assist the sweet-cool formula in resolving wind-summerheat and clearing damp-heat, without aiding fire. This serves as a model for medication). After three days of recuperation, he recovered (First clear external pathogens, then regulate physical injury; this is a model for treatment).
Luo Qianfu treated a Mongolian man who developed cholera after consuming alcohol, meat, and Tongru (a dairy product). From morning until noon, he was mentally confused, with floating and rapid pulses (Summerheat pathogen remains), which were empty upon pressure (Righteous Qi is deficient), as the damaged food had already been expelled. Luo immediately mixed Guiling Baizhu San (Cinnamon Twig, Poria, and Atractylodes Macrocephala Powder) with fresh well water and administered it slowly (Excellent). Then, he prepared Dijiangshui (Earth Slurry Water), clarified it, took one cup of the clear liquid, and administered it again (Superb). Vomiting and diarrhea ceased. The next day, he had slight irritability and thirst, so Qianshi Baizhu San (Qian's Atractylodes Macrocephala Powder) was given intermittently until recovery (Given the pulse and symptoms, and the expulsion of damaged food, it is known that Middle Qi is injured and Summerheat remains unresolved. Thus, tonifying righteousness and clearing pathogens is the approach. For deficient individuals contracting this from summerheat, this case serves as a model. Formulas such as Lizhong Tang and Sini Tang treat Yin-cold diseases, not Summerheat. Confusing these boundaries shows one is unfit to be called a physician).
He also treated Director Gong, nearly eighty years old. In June, he suffered from cholera with vomiting and diarrhea, daily coma, and unconsciousness (Summerheat disturbing internally). His pulse was flooding and forceful, beating seven or eight times per breath (Fire energy surging). His head was hot as fire (Pathogenic heat rising, not Yang floating), while his feet were cold as ice (Lung Qi failing to descend, not Lower deficiency). He had hemiplegia (Stomach Qi severely disrupted, unable to control bones and joints) and locked jaws (Heat entering the Yangming collaterals, not wind-stroke). Luo used Ganlu San (Sweet Dew Powder) to purge heat, tonify Qi, and calm the Spirit, adding Fuling (Poria) to differentiate Yin and Yang. Mixed with ice water and灌 (instilled), the patient gradually regained consciousness, and all symptoms resolved. Later, he was advised to speak sparingly (Crucial for deficiency) and regulate diet (Applicable to all illnesses, even for the healthy). After three days, Renshen (Ginseng) and Baizhu (Atractylodes Macrocephala) were used to regulate Middle Qi. Ten days later, he fully recovered.
Wang Shishan treated a man in his thirties, thin and weak, who suddenly developed vomiting and diarrhea, unable to keep down fluids for seven days, believing he would die (Without using drying-hot drugs, death was not inevitable). His pulse beat eight times per breath and was rapid. Wang said: "In summer, this pulse indicates deep penetration of Summerheat." (Director Gong, at eighty, had this pulse. This man, without fluids for seven days, also had this pulse; only an expert would distinguish the prognosis). Half a cup was administered, followed by another cup after a long interval (Slow administration is effective). Feeling slightly better, after three doses, Shigao (Gypsum) and Zhimu (Anemarrhena Rhizome) were reduced, while Renshen (Ginseng) was gradually increased to four or five qian (Manipulation is skillful). Huangbai (Phellodendron Bark), Chuanpi (Tangerine Peel), and Maidong (Ophiopogon Tuber) were added according to accompanying conditions (Formulation is skillful). He recovered after one month (Treatment for severe Summerheat damaging Yuan Qi; this case is a model).
A servant from Beijing, indulging in alcohol and irregular diet, suffered from cholera with vomiting, diarrhea, and muscle cramps, nearly dying from irritability and thirst. It was June or July, with continuous rain day and night. After returning home, he became wealthy, had descendants around his knees, and lived to ninety. If wealthy people suffer this, they are often misled by warming-tonic drugs). The *Qianjin Fang* states: "Mild cases heal with water" (Truly, truly; did the ancients deceive us?). This coincidentally matches ancient formulas. I witnessed this event personally. Later, on the road and in Liuhe County, I saw a person recover well after drinking fresh well water (For any summerheat-heat illness where thirst favors cold drinks, slowly drinking fresh well water or winter snow water promotes recovery, not just in cholera. Why do moderns, despite knowing the heat pathology, strictly forbid cold drinks?).
One person suffered from cholera, wanting to vomit but unable, wanting to defecate but unable, with heart and abdominal colic. The pulse was deep, hidden, and almost imperceptible (Pain pulses are often like this). This is Dry Cholera. Immediately, salt water was used to induce vomiting of retained food and phlegm, about one bowl. Defecation ensued (Opening the upper orifice allows the lower orifice to open. Once vomiting or defecation occurs, treatment can proceed). He was cured with Liuhe Tang (Harmonize the Six Decoction).
Sun Wen Yuan treated the son of the Cheng family, who first got drunk, then engaged in sexual intercourse (His usual indulgence and love for coolness are evident). The next morning, his limbs were cold, epigastric pain was extreme, and the pulse was only four beats per breath. Someone treated it as constrained fire, administering cold-cooling herbs, which worsened the pain. Three days prior, un-digested watermelon was vomited (Severe cold injury). His father thought it was Yin-type Cold Damage (Moderns, hearing of sexual activity during illness, call it Yin-type Cold Damage even without cold food injury, and kill patients with Ding (Clove), Fuzi (Aconite), Jiang (Ginger), and Gui (Cinnamon). Tragic!). He wanted to use Fuzi Lizhong Tang but hesitated (This father was open-minded, hence the son's survival). Sun examined him: face dark green, screaming in pain with a weak voice, restless sitting and lying. This was Cholera combined with Ascaris Jue (Cold Limb Syndrome due to parasites). First, stop pain and calm parasites, then treat cholera to avoid death; delay means error. Quickly, Cu Chao Wulingzhi (Vitaminized Trogopterus Dung fried in vinegar) 3 qian, Cangzhu (Atractylodes Lancea Rhizome) 1.5 qian, Wumei (Mume Fruit) 3 pieces, Chuanjiao (Sichuan Pepper) and Paoyang (Fried Ginger) and Guixin (Cinnamon Bark) each 0.5 fen, decocted in water and drunk. Pain reduced by more than half (Indulgence in raw/cold food, alcohol injury, and mistaken cold herbs necessitate warming the stomach and calming parasites). In the afternoon, Dafupi (Areca Peel), Huoxiang (Patchouli), Banxia (Pinellia Tuber), Chenpi (Tangerine Peel), Shanzha (Hawthorn Fruit), Fuling (Poria), and Wulingzhi were used. Two prescriptions led to full recovery (Regulating Middle, resolving stagnation, and harmonizing Spleen/Stomach cured it. Omitting discussion on the sexual aspect is indeed praiseworthy).
Jiang Huangnan treated his paternal uncle in July with cholera: vomiting, diarrhea, cramps, cold feet, excessive sweating, and retraction of the scrotum. One physician treated it as Cold Damage (Pulse hidden and faint). The Chi position was extremely faint, thirst for cold water (Cold feet and scrotal retraction suggest Jueyin; thirst suggests Shaoyin seeking water to save itself. How to distinguish? Direct invasion of Yin meridians lacks cramps and sweating; if Shaoyin has head sweat, death ensues). He prescribed Wuling San (Five-Ingredient Powder with Poria). (This treats Cold Damage cholera with exterior signs. Jiang failed to observe, sticking to "more heat desires water," causing error. Guiling Ganlu Yin would have been appropriate). Feeling slightly settled, by noon still thirsty (Scrotal retraction is Summerheat entering Jueyin, causing thirst for cold; not Cold Damage. Giving Cold Damage meds won't stop thirst). He added Maidong (Ophiopogon Tuber), Wuweizi (Schisandra Berry), and Huashi (Talc) to Wuling San (Only then realizing Summerheat; adding only Maidong and Huashi is insufficient, and Wuweizi's sour-warm nature is particularly unsuitable). Then, Huanglian (Coptis Rhizome) and Xiangru (Elsholtzia Herb) decoction was taken cold (The previous prescription stubbornly followed vulgar sayings, wrongly using Wuweizi. Who knows what happened after? Suddenly adding this agent is ridiculous). Next morning, pulse emerged slightly but lacked root, body emaciated (Continuous warm-dry drugs plus ascending-dispersing Xiangru caused this). Hiccups, cold hands and feet, immediate vomiting upon eating (Guizhi, Baizhu, Wuweizi, Xiangru etc. had effects), slight fecal incontinence. Moxibustion on Dantian for 8-9 cones relaxed the scrotum slightly and warmed hands/feet (Hidden heat begins to circulate with fire moxa). Subsequently, Lizhong Tang was given 2-3 doses (At a loss. If Jiang... [burnt-head guest], administered and cured (In this case, Jiang initially knew it was heat but stuck to antiquity, causing repeated errors. When hiccups, vomiting, and emaciation occurred, he didn't know to combine clearing and tonifying, using moxa and Lizhong instead, nearly leading to collapse. Fortunately, Fuzi was not used, so rescue was possible, though critical. Readers should take note).
Jiang Shaowei treated a woman in mid-June with cholera vomiting, diarrhea, and cramps. One physician prescribed Huoxiang Zhengqi San (Regulate Qi and Rectify Viscera Powder with Patchouli) (This treats cholera from catching chill, drinking cold, and cold-dampness). Irritability and red face increased, tearing clothes and lying on the ground (Huoxiang Zhengqi San is a warm-dispersing agent. It cannot be mistakenly used for Summerheat cholera. Those who mistakenly use Fuzi and Rougui often see immediate coma, cold limbs, and cyanosis upon swallowing, dying. Physicians still claim extreme Yin excess. Such great heat drugs cannot save them. Meeting this syndrome again and using this method, they remain ignorant until old age, and the冤 (wronged) dead have no one to appeal to. This is why I describe these treatments for cholera cramps to warn the world). Jiang diagnosed: pulse empty and weak, body heat with desire to drink. This is Summerheat injury. Suitable for pungent-sweet-great-cold agents to purge fire-heat. Wuling San plus Huashi (Talc) and Shigao (Gypsum) was used. After vomiting/diarrhea stopped, Guiling Ganlu Yin was given for recovery (Summerheat disease pulses are often empty, fine, choppy, weak, string-taut, knotted, or delayed due to heat injuring Qi. Even hidden or absent at Chi positions occur due to pathogen stagnating in collaterals and mutual rejection above and below. Do not mistake for deficiency-cold. Also, pulses may be flooding, slippery, and rapid abnormally due to fire fanning. Thus, cholera has no fixed pulse diagnosis; careful examination is crucial).
Chen Sansan treated a woman. In summer, after eating, she drank water and slept. She woke with extreme heart and abdominal pain, cold limbs extending past elbows and knees, wanting to vomit/defecate but unable, heart pain near death, six pulses hidden. Ordered Huoxiang Zhengqi San decoction to induce vomiting. One vomiting reduced pain by half; second vomiting brought peace (This is Dry Cholera from food retention, drinking cold, sleeping in draft. Using a matching agent to induce vomiting fits the situation; no need to stick solely to salt water).
Miu Zhongchun treated a maid in Gao Cunzhi's household with cholera. Sandaren (Amomum Fruit) 1 liang, stir-fried and ground, with a pinch of salt, mixed with boiling water, taken cold. One dose cured (This treats mild cases of summer coolness indulgence and Spleen/Stomach disharmony. Usable in winter for cold-induced cases, but taken warm. I have verified this myself). For cold-damaged food, add Wuzhuyu (Evodia Fruit).
Zhang Shiwan said: A young man, newly married, suddenly had abdominal pain, numbness, and dizziness (*YiTong* calls it Fan Sha, i.e., Dry Cholera due to heat). Someone ordered half a cup of fiery wine (Suspecting Yin syndrome, not knowing young newlyweds often have Fire syndromes. Why? Regardless of wealth, winter/summer bedding is new, and marital rites cause exertion generating fire). Abdominal pain worsened, followed by cranial distension and red spots on body (Heat toxin becomes apparent with wine; without urgent clearing/resolving, bleeding from seven orifices will occur). Given Lugenzhi (Reed Rhizome Juice) (Resolves wine toxin and clears heat). Vomiting relieved pain. Then epistaxis, dry mouth, slight red spots on chest/abdomen (Extreme heat in Blood level). Drank child urine for slight relief (Excellent for clearing Ying level). Strongly decocted Congchi Tang (Scallion and Soybean Decoction) (Holy medicine for dispersing foul qi and turbid toxins), still adding child urine, administered continuously. Great vomiting and sweating occurred, and he recovered.
Ye Tianshi treated a person post-cholera with severe Middle Qi deficiency, internal Liver Wind stirring, emptiness in heart, body pain, and limb edema. Yigong San (Extraordinary Efficacy Powder) plus Mugua (Chaenomelos Fruit), Jiang (Ginger), and Zao (Jujube) was used. Note: This is a Middle-boosting and Wood-restraining agent, excellent for post-cholera recuperation. If body pain/limb edema are seen and mistaken for exterior syndrome, using dispersing agents will stir internal wind further, severely injure Spleen Earth, and often lead to danger. Cholera is indeed a Middle Jiao Earth disease, but Earth diseases often result from Wood insulting Earth. Thus, even treating cold cholera, one must first observe Jueyin dynamics. If Liver Yin is chronically deficient with hidden internal wind, Jiang (Ginger) and Fuzi (Aconite) must be used with extreme caution. Even if necessary, they must be managed properly to avoid overdose. Without restraint, although not instantly fatal like heat cholera, symptoms will worsen and complications arise. Observe Zhang Zhongjing's use of Baitouweng Tang for Jueyin diarrhea; the meaning is clear. Jueyin, though ending two Yins, follows the logic of new moon/full moon. End of Yin is beginning of Yang. Its root is Yin, branch is Heat, body is Wood, function is Fire, thus uniquely called the "Gan Zang" (Rigid Organ). Ranked as General, the name reveals its treatment. Some treat Liver Qi only with rigid-dry herbs; acute cases become spasms/fainting, chronic cases become Guan Ge (Obstruction/Regurgitation). People know disease deepens but don't know drug-induced damage. Let this be a warning for physicians and patients alike.
Huai Baoqi treated a man who indulged in plum water, suffering endless vomiting/diarrhea, cold limbs, pale/dull face, voicelessness, hidden pulse, and complete urinary retention. Medicine was vomited immediately. Physician gave up. I reasoned Plum is sour and astringent, causing urinary retention. Musk defeats fruit. Musk is aromatic and opens orifices. So, half Musk was inserted into the umbilicus, half into nostrils. Patient touched nose asking "What is this?" Soon, urine flowed heavily 2-3 times. Suddenly awake from drunken dream-like state. Next day, asked for porridge and gradually recovered (No external factor, so defeating the fruit with musk to open orifices worked. Ingenious thinking is unmatched).
Tong Lu treated Mrs. Chen in midsummer with cholera vomiting/diarrhea, abdominal colic, cold limbs, profuse cold sweat, cramps, eyes rolling up, irritability, extreme thirst, desire for cold drinks, immediate vomiting upon drinking, six pulses hidden. Though called cholera, it is actually Zang Jue (Visceral Cold Limb). Classic says: "Large Qi enters Zang, abdominal pain and downward flow, can cause death, not life. Urgently save Yang, or Kidney Yang will perish." Sini Tang (Reverse the Five Decoction): Jiang (Ginger) and Fuzi (Aconite) each 3 qian, Zhi Gancao (Honey-fried Licorice) and Wuzhuyu (Evodia) each 1 qian, Mugua (Chaenomelos) 4 qian. Decocted and taken cold. Three doses day and night. Limbs harmonized, crisis receded. Thirst remained, but now preferred boiling water, revealing cold signs. Added fluids-generating ingredients to the formula. Two doses brought peace. Note: Case analysis and medication show outstanding insight. The truth lies in desiring cold drinks but vomiting immediately, and preferring boiling water after hot medicine. If cold drinks are tolerated, it is Internal True Heat/External False Cold. However, heat syndromes can also have chest rejection; even if desiring cold drinks, vomiting occurs unless sipped slowly. Also, Phlegm-Damp abundance can cause thirst preferring hot drinks. All must not be mistaken for cold. Must examine tongue coating color and urine presence/absence to reveal truth. The case omitted details, lacking thoroughness. "Large Qi entering Zang" is not a common epidemic, but occurs during epidemics. Physicians must prescribe based on disease, not stick to formulas killing people. This is benevolent art.
Mr. Ni suffered cholera vomiting/diarrhea. Initially no thirst, limbs not cold, pulse not deep/thin (Easy to treat). One physician used Dashun San for two doses, leading to this state. Seeing cold limbs, added Fuzi, worsening pulse/symptoms (I have seen much). Tong said: "This disease, mistreated twice, life is in danger. If truly cold, heat meds should cure it. Now reverse cold, pulse deep/thin/hiding, it has brewed Deep Heat/Deep Cold Limb, similar to Heat pathogen entering Jueyin (Accurate differentiation)?" Used Zhuye Shigao Tang (Bamboo Leaf and Gypsum Decoction), replacing Renshen with Xiyangshen (American Ginseng), adding Huanglian (Coptis) and Huashi (Talc). Two doses brought peace. Simultaneously, Mr. Lu suffered similarly. Physician used Yang-returning agents day and night (Does he really want quick death? Paper talk, blind readers often do this, not just crude workers. I've seen much). Cold limbs and irritability increased daily. Patient wanted cold water, denied (Detestable/Sigh). Patient even picked up phlegm from ground to quench thirst (Tragic/Pitiful). Delayed ten days and died (If ten days survived, when wanting cold water, if strategy changed, could live). Alas! Even if truly Yin-cold, with Yang returning and irritability/thirst like this, the nature of heat drugs is stagnant and masterless. Harmonizing with cool meds would cure immediately. Ignorance brings heavy sighs (Wonderful principle of harmonizing with cool meds, unheard of before. Xie Cheng).
Miss Zhang, summer cholera. Physician used Jiang (Ginger), Fuzi (Aconite), Huoxiang (Patchouli), Houpo (Magnolia Bark), Wuzhuyu (Evodia), Lian (Coptis). Vomiting stopped, but abdominal pain continued, becoming five-colored dysentery. On eighth day, Tong diagnosed: pulse fine/rapid, strong in deep position, cloudy eyes, crimson tongue/red lips, chest oppression, thirst drinking eagerly. This is Summer-Turbidity toxin disturbing Middle Palace causing cholera. Bitter-hot can open depression and stop vomiting, but ultimately aids pathogen, transforming into five-colored toxic dysentery (Summer toxin not very heavy, combined with dampness, so only five-colored dysentery. Without dampness and heavy summer toxin, early rescue would fail). Given Hezi's Guiling Ganlu Yin plus Huanglian (Coptis), Yinhua (Honeysuckle Flower), and Heidou (Black Soybean). Two doses cleared cloudiness, symptoms subsided sequentially, Stomach revived. Afraid of medicine, refused more. I said residual pathogen remains, harming organs, turning into other diseases. Gave Sandou Tang (Three Bean Decoction) plus Gancao (Licorice) frequently until cured.
Tang Zhiqing said: Wu from Changzhou, usually robust, suddenly cried out in pain while eating, fell, claiming chest/diaphragm cut by knife. Physicians couldn't treat. After three days, languishing waiting for death. An old man asked about condition, ordered grinding old ink juice to drink. Pain stopped immediately. Illness vanished. Asked what syndrome? Said: Remember neighbor with similar illness in youth, old doctor cured with this method, said accidental ingestion of "Sky Silk Poison". Thought ink harmless, tried it, didn't expect it to fit (Rare syndrome, if exists, people think Dry Cholera. Collected to broaden knowledge).
Someone from Gushi ate Tangyuan (glutinous rice balls) on New Year's Day, went out to celebrate, suddenly abdomen burned like fire, unbearable pain, fainted and fell, moved
