Apoplexy (Zhong Feng) is distinguished into true and false, internal and external; a slight error in differentiation leads to a thousand-mile deviation. Why? The northwest has high terrain and cold climate, with fierce wind. When the person with deficient true Qi is suddenly struck by it, as stated in *Jin Gui Yao Lue* (Essential Prescriptions of the Golden Cabinet): "Evil enters the collaterals, causing insensitivity of the skin and muscles." When evil just enters the Wei (Defensive) level, Qi cannot circulate, hence skin insensitivity. When evil enters the Jing (Main) channels, heaviness and weakness occur. When evil penetrates the Ying (Nutritive) vessels, both internal bones and external muscles lose nourishment, resulting in severe heaviness and inability to bear weight. When evil enters the Fu (Yang) organs, consciousness is lost. Entering the Fu organs inevitably affects the Stomach; heat steams body fluids, condensing them into phlegm-saliva, causing Stomach Qi to stagnate and rebel, leading to unconsciousness. When evil enters the Zang (Yin) organs, speech becomes difficult, and saliva foams from the mouth. When pathogenic factors severely affect various Zang organs, they must enter the Heart and disturb the Shen (Spirit), causing tongue relaxation and difficulty speaking, opening the Lianquan point and flowing saliva. Apoplexy affecting the Zang organs often results in death; affecting the Fu organs leads to disability; affecting the collaterals and channels can be cured through regulation. The treatment method should first disperse wind evil. Jiayan (Zhang Jiebin) said: "For those with deficiency, if deficiency is not tonified, wind will not exit; for those with phlegm, if phlegm is not resolved, wind will not exit; for those with fire, if heat is cleared, wind will not exit; for those with Qi stagnation, if depression is opened, wind will not exit; for those with dampness, if dampness is guided out, wind will not exit." Therefore, for wind striking collaterals and channels, it is generally appropriate to diffuse and scatter. Medicines such as Renshen Baidu San (Ginseng Decoction to Overcome Toxins) and Jiuwei Qianghuo Tang (Nine-Ingredient Notopterygia Decoction) are used for modified treatment. Do not mistakenly use purgatives, allowing wind evil to enter the Zang and Fu organs due to deficiency,酿成 irreversible disaster. Next, treat the root by tonifying and nourishing Qi and Blood. For symptoms of Zang apoplexy, medical texts often suggest purgation; however, Zang apoplexy usually involves chronic deficiency and unstable Zhen Qi; purging would cause immediate death. Only in cases of Fu organ involvement with internal excess and constipation is purging occasionally applicable, merely to mildly relieve烦热 (irritating heat); how can purging be a fixed rule! Once the wind tendency subsides, wind-drying herbs must be prohibited. One should reside in a quiet room, avoid rich and fresh foods, and only use sweet-cold herbs, taken frequently while hot, so that internally wind is not attracted and externally there is no path for entry. Sweet-cold herbs extinguish wind, tonify deficiency, and can be taken long-term. Careful regulation for a thousand days allows one to enjoy longevity. This outlines the general principles of True Apoplexy; prescriptions are numerous, so no further elaboration is needed. Regarding Similar Apoplexy (Lei Zhong Feng), treatment methods are not yet perfect, so clarification is necessary. According to Zhongchun (Chen Shigong): In the southeast, there is no fierce wind, but much damp-heat. People's constitutions are often soft and fragile, prone to heat and phlegm. With depleted True Yin, internal heat is severe, boiling body fluids into phlegm, blocking airways, preventing smooth flow. Extreme heat generates wind, causing sudden collapse. Symptoms of Similar Apoplexy include unconsciousness, slurred speech, facial paralysis, or hemiplegia. Before onset, internal heat signs appear externally: dry mouth and bitter taste, constipation, scanty dark urine. This confirms Liu Hejian's view that this condition arises from improper care, violent Heart Fire, and deficient Kidney Water unable to control it. Zhu Danxi attributed it to Qi deficiency, Blood deficiency, and damp-phlegm. This is Internal Deficiency Dark Wind, truly Yin-Yang dual deficiency, predominantly Yin deficiency, distinct from external wind evil. Treatment should clear heat [Er Dong (Tian Dong, Mai Dong), Gan Ju, Bai Shao, Tong Bian], regulate Qi [Su Zi, Ju Hong, Pi Pa Ye, Yu Jin], resolve phlegm [Bei Mu, Hua Fen, Gua Lou, Zhu Li] to address the branch, then treat the root. For Yin deficiency, nourish Blood [Di Huang, Dong, Gou Qi, Bai Shao, Niu Xi, Zao Ren, Hu Ma, Shou Wu, Ren Ru, Bie Jia]; for Yang deficiency, tonify Qi [Ren Shen, Huang Qi, Bai Zhu, Fu Ling, Lu Rong]; for dual Qi-Blood deficiency, tonify both, recovering over time. Mistaking it for True Apoplexy and using wind herbs causes dryness to injure Yin and dispersion to injure Qi, worsening mild cases and killing severe ones. Jingyue (Zhang Jingyue) stated: True Apoplexy comes from outside, initially presenting with fever, aversion to cold, headache, and body pain, clearly observable. Similar Apoplexy arises internally, without external signs, appearing suddenly like wind, caused by alcohol, sex, fatigue, emotional stress, and diet injuring Zang Qi, leading to Yin deficiency. Treatment should cultivate True Yin to save the root. Yin deficiency has two types: Water deficiency within Yin, and Fire deficiency within Yin. For Water deficiency, strengthen the master of Water, using Liu Wei Di Huang Wan or Zuo Gui Wan; for Fire deficiency, benefit the source of Fire, using Di Huang Yin Zi or Ba Wei Wan. Understanding Water and Fire in treatment makes one a skilled physician.
Nose-Insufflation Method treats trismus (lockjaw).
Xi Xin (Asarum) and Zao Jiao (Gleditsia Sinensis Fruit) [mixed as powder]. Blown into nose to induce sneezing; no sneeze indicates Lung exhaustion and death. These two are acrid-dispersing herbs, penetrating the crown to open orifices and awaken spirit, contraindicated in Similar Apoplexy as they leak True Qi.
Mume Fructus Trismus Formula also treats lockjaw.
Rub Mume Fructus flesh on the jaw. [If the jaw feels sore and soft, it opens easily.]
Gai Rong Gao treats facial paralysis. If deviated to the left, apply blood to the right; if deviated to the right, apply blood to the left.
Eel Blood [taken from the tail; eels burrow, lack feet, move like snakes, sharing nature with snakes, thus entering collaterals and treating wind evil.] Bing Pian (Borneol) and She Xiang (Musk) [to promote Qi movement and penetrate orifices, small amounts each.]
These two formulas are usable for both True and Similar Apoplexy. Eel blood is red eel blood.
Guizhi Tang (Cinnamon Twig Decoction) [see Shang Han Lun]. Wind enters from outside, lodging in collaterals and channels, lingering without departing. This formula is the ancestor of all formulas for apoplexy, used for True Apoplexy because wind enters from outside and must be driven out.
Xiao Xuming Tang (Minor Continuation of Life Decoction)
Ma Huang (Ephedrae Herba) and Xing Ren (Armeniacae Semen Amarum) [suitable for Zhang Zhongjing's treatment of Taiyang syndrome cold damage with no sweat.] Gui Zhi (Cinnamomi Ramulus) and Bai Shao (Paeoniae Radix Alba) [suitable for Zhang Zhongjing's treatment of Taiyang syndrome wind strike with spontaneous sweating. For symptoms of headache, fever, aversion to cold, body ache, lower back pain, and stiffness, choose accordingly.] Ren Shen (Ginseng Radix et Rhizoma) and Gan Cao (Glycyrrhizae Radix et Rhizoma) [tonify Qi, suitable for Qi deficiency.] Chuan Xiong (Ligustici Chuanxiong Rhizoma) [with Bai Shao to nourish Blood, suitable for Blood deficiency.] Fang Feng (Saposhnikoviae Radix) [used because wind is the final disease.] Fang Ji (Akebiae Caulis) [used for damp-wind abdominal disease.] Huang Qin (Scutellariae Radix) [used for Yang excess heat disease. Each 1 qian.] Fu Zi (Aconiti Lateralis Radix Praeparata) [used for Yin excess cold disease. 0.5 qian. Diseases rarely come alone but mix together, so herbs cover multiple aspects.]
Ancients mixed treatments for apoplexy without distinguishing collaterals/channels or deficiency/excess/cold-heat. Without symptom-based modification, it worsens the condition. Note: Baidu San contains Ren Shen and Fu Ling to tonify deficiency; Qianghuo Tang contains Huang Qin and Di Huang to clear heat. Collectively treating True Apoplexy with modifications is stable.
Shun Qi Kai Tan Yin (Regulate Qi and Open Phlegm Drink) [self-made] treats Similar Apoplexy with excessive heat-phlegm.
Su Zi (Perillae Fructus), Ju Hong (Citri Reticulatae Pericarpium Rubrum), Pi Pa Ye (Loquat Leaf), Yu Jin (Curcumae Radix) [regulate Qi]; Bei Mu (Fritillariae Bulbus), Hua Fen (Trichosanthis Radix), Gua Lou Shuang (Trichosanthis Fructus Pulvis), Fu Ling (Poria), Zhu Li (Bambusa Textilis Sap) [resolve phlegm]. If phlegm obstructs the Pericardium causing coma, add Xi Jiao (Rhinocerotis Cornu) and Ling Yang Jiao (Capra Hircus Cornu), mixed with Niu Huang (Bovaris Calculus) and Tian Zhu Huang (Bambusa Textilis Tabaosha).
This formula first regulates Qi and resolves phlegm to treat the branch. Treating phlegm requires regulating Qi first; when Qi flows, phlegm descends. However, clearing the Lung is fundamental. The Lung prefers clarity and肃降 (descending); heat boils fluids into thick phlegm, blocking airways and impairing ascending/descending. Thus, for Lung heat, Ren Shen is contraindicated as it may aid fire and injure the Lung, increasing heavy voice and phlegm, potentially drying fluids until no water drops to unblock the urinary pathway. Caution is essential.
Jie Ming Dan (Connect Life Pill) treats Similar Apoplexy with aphasia [due to phlegm-fire obstructing upper orifices], flaccid paralysis of limbs [Tan means flat/relaxed, sinews loose and unable to lift; Huan means scattered, Qi and Blood dispersed and unusable. Both caused by phlegm-fire], limb pain, difficulty moving [also due to phlegm-fire stagnating collaterals], and reduced appetite [diffuse heat-phlegm, stagnant Stomach Qi]. Also treats Qi-Blood deficiency with rising phlegm-fire and consumptive diseases.
Ren Ru (Human Milk) [milk transforms from Blood, rapidly tonifies Yin. 2 cups.] Li Zhi (Pyrus Sinkiangensis Juice) [descends fire, resolves phlegm. 1 cup.] Steamed in water, warm at dawn. [To prevent accumulation and diarrhea if taken with food.] Add Zhu Li if phlegm is excessive [slippery, flows smoothly, enters orifices, expels phlegm. 1 cup.] This formula tonifies Yin, clears fire, resolves phlegm, using human milk to supplement humans, unparalleled in efficacy.
Jia Wei Di Huang Tang (Modified Rehmannia Decoction) treats violent Heart Fire and deficient Kidney Water unable to control it, or extreme emotions (joy, anger, worry, grief, fear) causing heat stagnation, leading to mental confusion, sudden collapse, aphasia [True Yin loss, fluid depletion, tongue collaterals unnourished, free movement impossible], facial paralysis, limb spasms [Water fails to nourish sinews, sinews tense and relaxed. Folk 'wind' is an illusion of wind-yin terminal disease; wind arises from fire], dry stool [Kidney governs five fluids and two bowels; deficiency fails to moisten], urgent urination with dribbling [Fire nature is rapid].
Liu Wei Di Huang Tang (Six-Ingredient Rehmannia Pill) [tonifies Kidney, strengthens Fire.] Add Mai Dong (Ophiopogonis Radix) and Xi Jiao (Rhinocerotis Cornu) [clears Heart, resolves heat.] Ling Yang Jiao (Caprae Hircus Cornu) and Gou Teng (Uncariae Ramulus cum Uncis) [calms Liver Wood, relaxes sinews.]
Ren Ru (Human Milk) [tonifies Yin, benefits Blood.] Li Zhi (Pyrus Sinkiangensis Juice) and Zhu Li (Bambusa Textilis Sap) [descends fire, resolves phlegm.] Ren Shen (Ginseng Radix et Rhizoma) [add as appropriate.] Sang Zhi (Mori Ramulus) [3-4 liang stir-fried.] Decoct to cook medicine. [Promotes joint mobility, treats limb spasm, suitable for hemiplegia.] This formula primarily tonifies Kidney and strengthens Water, assisted by clearing Heart, calming Liver, descending fire, and resolving phlegm. Since Metal is Mother of Water, for Water deficiency, add Sheng Mai San (Generate Pulse Powder) to nourish the source of transformation.
Bu Qi Yang Xue Tang (Tonify Qi and Nourish Blood Decoction) [self-made]
Treats Similar Apoplexy with hemiplegia, known as Pian Ku (Half-Withered) in classics. [Classics: Pian Ku, one side useless and painful, speech unchanged, mind undisturbed, disease in fen-coupling. Also: Empty evil occupies half body; deep invasion lodges in Ying-Wei; when weak, True Qi departs, evil remains alone, causing Pian Ku. Shallow evil causes unilateral pulse pain. Though described as external contraction, pain indicates milder condition.] Men affected on left [Right is adverse, Left is favorable]. Women affected on right [Left is adverse, Right is favorable]. Non-aphasic with tongue turning recoverable, recovery in 30 days. [Though meridian adverse, not severe in Zang.] Favorable cases with aphasia recover in 3 years. [If man right/woman left, symptoms favorable but voice aphonic, exterior light interior heavy.] Under 20 years old, death in 3 years. [In prime Qi-Blood years, Pian Ku indicates insufficient constitution and premature decline.]
Ren Shen (Ginseng Radix et Rhizoma), Huang Qi (Astragali Radix) [tonify Qi.] Di Huang (Rehmanniae Radix), Dang Gui (Angelicae Sinensis Radix), Bai Shao (Paeoniae Radix Alba), Shou Wu (Polygoni Multiflori Radix), Hu Ma (Sesami Fructus), Gan Ju (Chrysanthemi Flos) [extinguish wind, clear heat.] Tian Dong (Asparagi Radix), Mai Dong (Ophiopogonis Radix) [nourish Yin, clear heat.] Qin Jiao (Gentianae Macrophyllae Radix), Niu Xi (Achyranthis Bidentatae Radix), Xu Duan (Dipsaci Radix) [regulate blood vessels.] Hu Gu (Pantheris Os) [strengthen sinews and bones.] Fu Ling (Poria), Ju Hong (Citri Reticulatae Pericarpium Rubrum) [with Li Zhi and Zhu Li, descend fire, resolve phlegm.] Add Ren Ru, Li Zhi, Zhu Li, decoct with Sang Zhi Soup.
This formula primarily tonifies Qi, nourishes Yin and Blood, assisted by regulating vessels, strengthening sinews/bones, clearing phlegm-heat. If blood stasis exists, add stasis-resolving herbs.
Note: Pian Ku stems from Qi-Blood disharmony, like trees where one side lacks sap, branches wither, but roots remain intact. Hence, above treatment applies. Zhu Danxi claimed left is dead blood, right is damp-phlegm, which is too rigid and ineffective. Classics say: Left and Right are paths of Yin and Yang. Qi deficiency causes stagnation, vessels cannot move; left/right can be dead blood. Qi closure prevents fluid flow; left/right can be damp-phlegm. Some divide left as Blood deficiency, right as Qi deficiency. Zhao stated: Men's left belongs to Water, women's right belongs to Water. Hemiplegia men often have left, women right, due to Water failing to nourish. Treatment should tonify Yin and strengthen Water. I suggest following this method to infer pathology and treat appropriately, not dogmatically. Zhao also noted: Some healthy people have normal upper body but weak/numb lower body, with dysuria or incontinence, indicating Foot Three Yin deficiency. Must know this.
Ji Ling Gao (Collection of Spirits Paste) Tonifies Qi and Blood, nourishes Yin and strengthens Water, prolongs life.
Ren Shen (Ginseng Radix et Rhizoma) [tonifies Qi, Great Qi circulates everywhere, benefiting all organs.] Er Dong (Tian Dong, Mai Dong) and Er Di (Sheng Di Huang, Shu Di Huang) [Sheng Di generates essence, Tian Dong guides to generation site; Shu Di tonifies essence, Mai Dong guides to tonification site.] Niu Xi (Achyranthis Bidentatae Radix) [strengthens sinews and bones.] Gou Qi (Lycii Fructus) [fills essence, nourishes Ying. Each 0.5 jin.]
This divine formula tonifies Qi-Blood and essence-marrows, suitable for deficient persons. Er Chen Tang treats obese persons with excessive phlegm-saliva due to dampness. [Phlegm clear/thin, mouth not thirsty.]
Ban Xia (Pinelliae Rhizoma) [dries dampness.] Fu Ling (Poria) [leaches dampness; when damp leaves, phlegm has no source.] Ju Hong (Citri Reticulatae Pericarpium Rubrum) [regulates Qi; when Qi flows, phlegm does not stagnate.]
Gan Cao (Glycyrrhizae Radix et Rhizoma) [tonifies Spleen; when Spleen prospers, Earth controls dampness.] Can combine with Xing Xiang San [Nan Xing, Mu Xiang. Nan Xing dispels wind-phlegm; processed with ox gall nine times reduces dryness, borrowable.] Add Zhu Li and Jiang Zhi. Original formula plus Dan Xing and Zhi Ke is named Dao Tan Tang.
This is a damp-drying, Qi-regulating, phlegm-resolving formula. Use only for damp-phlegm, do not misuse.
Du Shen Tang (Single Ginseng Decoction) Treats various deficiencies and Qi weakness, critical conditions. Also treats sudden dizziness and collapse. [Yin deficiency leaves solitary Yang unattached, flying away.] No phlegm sound in throat [fault not in phlegm]. No pathogenic heat in body [illness not due to infection]. This is sudden Yin deficiency and Yang collapse; urgently administer large dose.
Ren Shen (Ginseng Radix et Rhizoma) [1-3 liang.] For Yin deficiency, add Shu Di Huang. [Ren Shen with Shu Di enters Three Yin directly. 1-2 liang.] For irritability and faint pulse, add Tong Bian. [Attributed to deficiency with fire.] For excessive phlegm, add Zhu Li. [Phlegm follows chaotic Qi.] Add Huang Qi, name Shen Qi Tang. [For Qi deficiency with spontaneous sweating.] Add Bai Zhu, name Shen Zhu Tang. [For Spleen-Stomach weakness.] Add Fu Zi [1-3 qian], name Shen Fu Tang. For True Yang deficiency, panting, shortness of breath, dizziness [Yang about to escape upward], sweating and cold limbs [Yang about to escape outward], can temporarily use, never blindly.
This formula secures Yuan Qi and tonifies Qi. Single use or large dose for focused power. Formidable Yin Blood cannot generate quickly; formless Yang Qi must be urgently secured. Suitable for Yang/Qi deficiency; also temporarily for Blood/Yin deficiency to preserve remaining life for gradual regeneration. Added formulas applied according to suitability.
Liu Jun Zi Tang (Six Gentlemen Decoction) Treats obese persons with Qi deficiency, sudden stupor [Obese prone to Similar Apoplexy, Qi abundant externally but gathered internally], unconsciousness [Qi fails to control phlegm, phlegm confuses Heart orifices], excessive phlegm-saliva [Obese prone to damp-phlegm], stiff tongue base, unclear speech, facial paralysis, limb immobility [All due to phlegm].
Ren Shen (Ginseng Radix et Rhizoma) [1 qian-1 liang.] Bai Zhu (Atractylodis Macrocephalae Rhizoma), Fu Ling (Poria) [Each 1-3 qian.] Gan Cao (Glycyrrhizae Radix et Rhizoma) [Tonifies Middle Qi to resolve phlegm. 0.5-0.7 qian. Omit for drinkers.] Ju Hong (Citri Reticulatae Pericarpium Rubrum) [Promotes Qi to move phlegm.] Ban Xia (Pinelliae Rhizoma) [Dries dampness to control phlegm. Each 1-3 qian.] Add Zhu Li [Opens collaterals, guides phlegm. 1 cup.] Jiang Zhi [Opens phlegm. Several spoons.] Can combine with Xing Xiang San. Chronic drinkers with this condition add Gan Zhe Zhi [1 cup. To resolve alcohol toxin.] If thirsty, remove Jiang Zhi and Ban Xia, add Mai Dong and Shi Gao [Clears Lung-Stomach heat. This formula serves as a model for treating deficient wind.] Decoct with Sang Zhi Soup.
This holy formula tonifies Qi, strengthens Spleen, dries dampness, resolves phlegm. Unsuitable for Lung dry-phlegm. Note Jiayan said: Obese people most have heat-phlegm, so facial paralysis and limb numbness, regardless of side, treated as phlegm, truly correct. But obese people often have deficient wind, suitable for sweet-cold herbs [Ren Shen, Wei Rui, Hu Ma, Gan Ju, Sheng Di. Mai Dong, Gua Lou, Shi Gao, Li Zhi, Zhu Li etc.], avoid drying agents, examine carefully at bedside.
Note: Alcoholics often have Similar Apoplexy. Alcohol is damp-heat, breeds phlegm; more phlegm moves internal fire easier. In youth, strong Blood and Qi, no harm. After middle age, meridians and bones soaked by waste flavors, grain intake decreases, steaming Stomach and rotting Intestines. Though complexion may be ruddy, interior is ruined. Any internal injury from labor or six exuberances/seven emotions triggers sudden stroke. Like withered poplar flowering, suddenly broken by strong wind. Thus, stroke from alcohol-induced deficiency and sexual exhaustion-induced deficiency are both critical. Zhao Wanlin said: Alcoholics with Similar Apoplexy should greatly tonify Stomach Qi, adding much Ren Shen to rescue.
Si Jun Zi Tang (Four Gentlemen Decoction) Treats sallow complexion [Observation reveals Qi deficiency], faint speech [Hearing reveals Qi deficiency], limb weakness [Questioning reveals Qi deficiency], weak pulse [Palpation reveals Qi deficiency].
Ren Shen (Ginseng Radix et Rhizoma) [Tonifies primal Qi of Five Zang.] Bai Zhu (Atractylodis Macrocephalae Rhizoma) [Tonifies mother Qi of Five Zang.] Fu Ling (Poria) [Brings clear Qi of Five Zang.] Gan Cao (Glycyrrhizae Radix et Rhizoma) [Regulates discordant Qi of Five Zang.] Add Ju Hong, name Yi Gong San.
This essential Qi-tonifying formula must be modified according to symptoms.
Gui Pi Tang (Restore the Spleen Decoction) Treats Similar Apoplexy with concurrent deficiency of Heart, Spleen, and Liver meridians, or palpitations and fright, easy awakening during sleep [Heart Blood deficiency], reduced appetite, difficult digestion [Stomach Qi weak, Spleen Yin deficient], irritability and anger [Liver Blood scarce, Liver Fire旺盛].
Ren Shen (Ginseng Radix et Rhizoma), Huang Qi (Astragali Radix), Bai Zhu (Atractylodis Macrocephalae Rhizoma), Gan Cao (Glycyrrhizae Radix et Rhizoma) [Tonify Spleen.] Fu Shen (Poria with Hostwood), Yuan Zhi (Polygalae Radix), Zao Ren (Ziziphi Spinosae Semen), Yuan Rou (Longan Arillus) [Calm Spirit.] Dang Gui (Angelicae Sinensis Radix) [Nourish Blood.] Mu Xiang (Aucklandiae Radix) [Move Qi vigorously.] For Liver Fire, add Dan Pi (Moutan Cortex) and Hei Shan Zhi (Gardeniae Fructus) [Drain Fire.]
This formula tonifies Qi, nourishes Blood, calms Spirit, harmonizes Yin-Yang. Apply flexibly with modifications for miraculous effect.
Jia Wei Xiao Yao San (Modified Free and Easy Wanderer Powder) Treats Similar Apoplexy with polyphagia [Wood wind excessive, Earth Spleen suppressed, seeking help from food.] Also treats Liver meridian wind-heat, Blood dryness with sinew cramps, limb immobility, internal heat, evening tidal fever, alternating chills and fever.
Chai Hu (Bupleuri Radix) [Liver Wood Qi stagnation suppresses Spleen Earth; use Chai Hu acrid to disperse binding, ascending to lift suppression.] Shao Yao (Paeoniae Radix) [Acid and astringent, controls Liver Wood excess.] Dang Gui (Angelicae Sinensis Radix) [Liver injury causes Blood disease, used to nourish Blood.] Fu Shen (Poria with Hostwood) [When Water full, Fire dry; take to calm
