[Ping An: This section from the beginning of the chapter to "then select the Foot Shaoyang and Yangming" is found in *Ling Shu* (Spiritual Pivot), Volume 5, Chapter 24, "Jue Bing" (Jue Headache), and also in *Jia Yi Jing* (Classified Classic of Acupuncture and Moxibustion), Volume 9, Part 1. From "Jue pain clamping the spine" to the end, it is found in *Ling Shu*, Volume 5, Chapter 26, "Za Bing" (Miscellaneous Diseases). Also from "Jue pain clamping the spine" to "in the blood collaterals", it is found in *Jia Yi Jing*, Volume 7, Middle Part 1. Also from "Jue chest fullness and facial swelling" to the end, it is found in *Jia Yi Jing*, Volume 7, Part 3.]
**Jue Tou Tong** (Headache caused by the counterflow of Jue Qi), with a swollen appearance of the face and vexation in the heart, should be treated by selecting points on the Foot Yangming Stomach Meridian and Foot Taiyang Bladder Meridian. (The original text likely contained questions and answers, but they were lost due to long-term transmission and copying errors. The Hand Yangming Large Intestine Meridian, Hand Taiyang Small Intestine Meridian, Foot Yangming Stomach Meridian, and Foot Taiyang Bladder Meridian all traverse the head and face. The Hand Taiyang Small Intestine Meridian connects to the Heart and belongs to the Small Intestine. When the Qi of these four meridians becomes rebellious, it causes headache, facial swelling resembling edema, and heart vexation; therefore, the Shu-Stream points of these four meridians should be selected to treat the primary disease. Ping An: In *Ling Shu*, "Taiyang" is written as "Taiyin". Furthermore, according to the commentary, "Foot Yangming Taiyang" should read "Hand and Foot Yangming Taiyang" based on this note.)
**Jue Tou Tong**, with pain in the head's vessels, sadness, and propensity to weep, where the pulsating vessels in the head appear excessively full, should be treated by pricking to release blood, followed by regulating the Foot Jueyin Liver Meridian. (The Foot Jueyin Liver Meridian belongs to the Liver and connects to the Gallbladder, ascends to connect with the eye system, emerges above the forehead, and meets the Kidney Meridian at the vertex. Thus, rebellious Qi causes headache, pain in the head's vessels, sadness, and propensity to weep. The Jueyin meridian governs sadness and weeping. "Observing the head's movement" refers to observing the trembling or pulsating of the head at that time. "Excessively full vessels" indicates that the collateral vessels are hyperemic; one should first prick to release blood, then select the Shu-Stream point of the Jueyin meridian to treat the primary disease. Ping An: In *Jia Yi Jing*, "head vessel pain" lacks the character "head"; "propensity to weep" is written as "joyful weeping". The note suggests that "Xian Mai" might be a typo for "Du Mai" [Governing Vessel].)
**Jue Tou Tong**, characterized by a heavy and severely painful head, requires draining the five rows of acupoints on the head, five points per row, first selecting the Hand Shaoyin Heart Meridian, then the Foot Shaoyin Kidney Meridian. ("Zhen" is pronounced like "zhu geng fan". "Zhen zhen" describes the severe nature of the headache. The Hand Shaoyin Heart Meridian originates in the heart, connects from the heart system to the eye system; the Foot Shaoyin Kidney Meridian penetrates the spine, belongs to the Kidney, ascends through the Liver into the Lung, emerges from the Lung to connect to the Heart. Thus, rebellious Heart Qi rushes upward to the head, causing severe headache. Since the head is the residence of the Heart Spirit, one should first select the Shu-Stream point of the Heart Meridian, then the Shu-Stream point of the Kidney Meridian to treat the primary disease. Ping An: "Zhen zhen" in *Jia Yi Jing* is written as "Yuan yuan", without the characters "head heavy" below. The note states: "*Ling Shu* writes 'Zhen zhen'." Another note mentions that in "penetrates Liver enters Lung", Yuan edition mistakenly wrote "Mai" [vessel] instead of "Fei" [Lung].)
**Jue Tou Tong**, with forgetfulness and inability to locate the pain upon palpation, requires selecting the arterial pulses on the left and right sides of the head and face, followed by selecting the Foot Taiyin Spleen Meridian. (The Foot Taiyin Spleen Meridian combines with the Foot Yangming Stomach Meridian. The Foot Yangming Stomach Meridian traverses the left and right sides of the head and face, with arterial pulses located at the Keyuzhuren [ST7] and Daying [ST5] points, which are areas where Spleen Qi reaches. The spirit of the Spleen is "Yi" [Intellect/Intent]; its meridian is the Foot Taiyin. Therefore, when Taiyin Qi becomes rebellious, there is frequent forgetfulness, the pain resides in the spiritual level, and it is difficult to locate by pressing. One can select the arterial pulses of the Foot Yangming Meridian on the left and right sides of the head and face, then select the Shu-Stream point of the Foot Taiyin Meridian to treat the primary disease. Ping An: In *Jia Yi Jing*, this paragraph appears before "Jue Tou Tong Zhen Zhen", and "Taiyin" is written as "Taiyang", with the note saying: "Also written as Yin".)
**Jue Tou Tong**, with severe headache and surging, hot pulses around the ears, requires draining the blood, followed by selecting the Foot Shaoyang Gallbladder Meridian. (The Foot Shaoyang Gallbladder Meridian originates at the outer canthus, ascends to the corner of the head, descends behind the ear. Its branch enters the ear from behind the ear and exits in front of the ear. Thus, when Foot Shaoyang Qi becomes rebellious, severe headache occurs, and the pulses around the ears surge, indicating heat. One can prick to release hot blood, then select the Foot Shaoyang Meridian to treat the primary disease. Ping An: In *Jia Yi Jing*, "severe headache" is written as "pain severe", "surging pulse with heat" as "bone pulse heat", "drain out" as "first drain", and "Foot Shaoyang" as "Foot Taiyang Shaoyin".)
**Jue Tou Tong**, where pain in the neck and waist/spine correspond to each other, requires selecting Tianzhu [BL10], followed by selecting the Foot Taiyang Bladder Meridian. (The Foot Taiyang Bladder Meridian originates at the inner canthus, ascends to the forehead, crosses at the vertex, enters to connect with the brain, re-emerges down the neck along the spine to the middle of the waist, enters to traverse the paraspinal muscles, connects to the Kidney, and belongs to the Bladder. Thus, when Foot Taiyang Qi becomes rebellious, headache occurs, pain starts in the neck, and corresponds to the waist and spine. First, select the Tianzhu point of the Foot Taiyang Meridian, then select the lower Shu-Stream point of the Foot Taiyang Meridian to treat the primary disease. Ping An: In *Ling Shu* and *Jia Yi Jing*, this paragraph appears before "Jue Tou Tong Severe Headache". "Neck" is written as "First Neck Pain" three characters. "Correspond" has the character "First" below it.)
**True Headache**, with severe headache, pain throughout the brain, and cold hands and feet extending to the joints, is a fatal condition that cannot be treated. (Since the headache and brain pain are severe and Qi is rebellious, the hands and feet become cold up to the joints; if extreme, death ensues.)
If headache cannot be treated by selecting Shu points, it may be due to trauma or falling, with blood stasis retained internally, or internal injury. If the pain persists, immediate local pricking is permissible, but distant Shu points should not be selected. (Selecting Shu points is difficult to cure, hence it is said "not permissible". If there is retained blood from trauma, local treatment is possible; one can prick immediately, but should not select distant Shu points. Ping An: "Shu" in *Ling Shu* is written as "Shu" [acupoint], and in *Jia Yi Jing* as "Yu". Above "blood", *Ling Shu* and *Jia Yi Jing* have the character "E" [aversion/foul]. "Internal injury" in *Ling Shu* is written as "Mei Shang" [flesh injury]. "Ke Ji Ci" [can immediately prick]: in *Ling Shu*, "Ji" is written as "Ze"; in *Jia Yi Jing*, the character "Zhi" [particle] follows "Ci".)
Headache that cannot be needled indicates a major Bi syndrome. If the patient dreads daily attacks, symptoms may be slightly alleviated but not cured. (There are cases of headache that cannot be needled; this is a major Bi syndrome in the head, dreaded for its daily recurrence. "Fa" means attack/exacerbation. Needling can alleviate symptoms slightly but cannot eradicate them, because cold-damp pathogenic factors have entered the brain forming a major Bi. Ping An: In *Jia Yi Jing*, the character "Feng" [wind] follows "E". "Chu" [eradicate] in *Ling Shu* and *Jia Yi Jing* is written as "Yi" [finish].)
Half-head cold pain requires first selecting the Hand Shaoyang Sanjiao Meridian and Hand Yangming Large Intestine Meridian, followed by selecting the Foot Shaoyang Gallbladder Meridian and Foot Yangming Stomach Meridian. (The Hand and Foot Shaoyang and Yangming meridians are located on the left and right sides of the head and face. Thus, the Hand meridians run closer to the head, while the Foot meridians run further away. Therefore, for pain on the left half of the head, one can needle the Hand Shaoyang and Yangming meridians on the left side, then needle the Foot Shaoyang and Yangming meridians on the right side. The same applies to the right side. Ping An: *Jia Yi Jing* lacks the character "Ban" [half].)
Jue Qi clamping the spine and pain extending to the neck, with a heavy sensation in the head, blurred vision, and stiffness of the waist and spine, requires selecting the blood collaterals in the Foot Taiyang Bladder Meridian. (The head, eyes, neck, and waist/spine are the pathways of the Foot Taiyang Bladder Meridian; thus, disease arises within these pathways. Ping An: In *Ling Shu*, "Neck" is written as "Ding" [vertex], and "Kt Kt" as "[Character]". In *Jia Yi Jing*, "To Neck Head" is written as "Master Head Neck", "Chen Chen" as "Ji Ji", and "Kt Kt" as "[Character]".)
Jue Qi causing chest fullness and facial swelling, twitching lips, sudden difficulty in speech, and severe inability to speak, requires selecting the Foot Yangming Stomach Meridian. (These symptoms are all caused by the pathway of the Foot Yangming Stomach Meridian; therefore, the Shu-Stream points of the Foot Yangming Meridian should be selected to treat the primary disease. Ping An: "Chun Si Si Ran" [twitching lips] in *Ling Shu* is written as "Chun Luo Luo", and in *Jia Yi Jing* as "Jian Zhong Re" [heat in shoulder center].)
Jue Qi moving to the throat causing inability to speak, with cold hands and feet, and difficult defecation and urination, requires selecting the Foot Shaoyin Kidney Meridian. (Cold hands and feet occur because the Hand Shaoyin Heart Meridian and Foot Shaoyin Kidney Meridian are connected; thus, the hands and feet are cold. The Shu-Stream point of the Foot Shaoyin Meridian should be selected to treat the primary disease. Ping An: In *Jia Yi Jing*, "unable to speak" is written as "no speak", and the character "Wei" [slight] precedes "Qing". According to the note, "Qing" is defined as cold, so it should be written as "Qing" or similar, appearing as "[Character]" in the lower volume of this book.)
Jue leading to abdominal distension with much cold qi, rumbling in the abdomen, and difficult urination and defecation, requires selecting the Foot Taiyin Spleen Meridian. (Abdominal distension with much cold, and difficult urination and defecation, are all caused by the Foot Taiyin Spleen Meridian; therefore, it should be selected. Ping An: "Xiang Xiang Ran" in *Jia Yi Jing* is written as two characters "Peng Peng". "Xing Xing" in *Ling Shu* is written as "[Character]", and in *Jia Yi Jing* as "[Character]", with the note: "Pronounced Zui, *Jiu Xu* writes 'Xing Xing'." In the note "Taiyin Mai", the character "Mai" [meridian] is omitted in the Yuan edition.)
