[Jue Lun Chapter 24 (Part 1/2)]
Jue headache. The face appears swollen, and upon rising, the patient feels vexed and irritable. Treatment should involve selecting points from the Foot-Yangming and Foot-Taiyin meridians.
This chapter discusses diseases caused by the rebellion of meridian qi and the five zang organs, hence the title "Jue" (Rebellion). The Three Yin and Three Yang correspond to the Six Qi of Heaven; Wood, Fire, Earth, Metal, and Water correspond to the Five Elements of Earth. In Heaven, they manifest as phenomena; on Earth, they form shapes. The Five Elements of Earth generate the Five Zang organs, while the Six Qi of Heaven coordinate with the Six Meridians. Thus, the Five Zang organs are interconnected, and the transmission of pathogenic factors follows a specific sequence. Acupoint qi rotates and circulates continuously up and down. If this sequence is disrupted, Jue (rebellion) occurs, leading to disease. Furthermore, the Qi of colors (cyan, white, red, black, yellow) in Heaven distributes across the five directions, generating the Five Elements of Earth; the Five Elements of Earth then manifest upward as the Six Qi of Three Yin and Three Yang. This is the mutual generation of Heaven and Earth's Yin-Yang, Wu Yun (Five Movements), and Liu Qi (Six Qi). Humans also resonate with this pattern. Thus it is said: The East generates Wind, Wind generates Wood, Wood generates Sour flavor, Sour flavor nourishes the Liver; the South generates Heat, Heat generates Fire, Fire generates Bitter flavor, Bitter flavor nourishes the Heart. These are the form and Qi of the Five Zang organs, born from the Five Elements of Earth but rooted in the Six Qi of Heaven. The Twelve Meridians externally coordinate with the Six Qi and are rooted in the generation of the Zang-Fu organs. This is the integration of Zang-Fu and Meridian Qi. In the *Ling Shu* and *Su Wen*, mentions of Taiyang, Shaoyang, Yangming, Taiyin, Shaoyin, and Jueyin refer to the Six Qi, sometimes extending to the Six Meridians. Mentions of Liver, Heart, Spleen, Lung, and Kidney refer to the Zang-Fu and Meridians, sometimes involving the Six Qi. This is the Dao of Yin-Yang separation and union. The entry and exit of Yin-Yang, the coming and going of Cold and Heat, all originate from the Earth and move upward from the feet. Therefore, the wise person models the Head after Heaven to nourish it, the Feet after Earth to nourish them, and the Middle region after human affairs to nourish the Five Zang organs. If this nourishment is neglected, Qi will rebel causing headache, and Zang Qi will rebel causing heart pain. Yangming Qi ascends to the face; rebellious Qi rushes upward to the head, causing headache and facial swelling. When Yangming undergoes change, it causes palpitations, so upon rising, the patient feels vexed. This is Yangming Qi rebelling upward to the head, causing Jue headache. Therefore, one should select points from the Foot-Yangming meridian. Yangming derives its transformation from the interiorly-exteriorly paired Taiyin; thus, points from the Foot-Taiyin are also selected. This indicates that the rebellion is in the Qi level and has not yet affected the meridians.
Jue headache. Pain in the head's vessels, sadness, and frequent weeping. Observe the pulsating arteries in the head; if they appear excessively full, puncture to exhaust the blood, then regulate the Foot-Jueyin meridian.
This section discusses Jueyin Qi rebelling upward and entering the meridians, resulting in Jue headache. The Qi of the Three Yin and Three Yang all moves upward from below. Some rebellions remain at the Qi level without affecting the meridians, while others enter the meridians. Meridian Qi connects internally and externally, capable of separating and merging. Hence, the first section only discusses Qi rebellion; from here onward, it discusses Qi rebellion extending to the meridians. Since the rebellion is in the vessels, there is pain in the head's vessels. Jueyin governs "closing" (He); if closing fails, Qi severs, leading to sadness. Since the rebellion is in the Qi, the patient feels sad and weeps easily. Observe the head pain area where the pulse is excessively full; puncture to exhaust the blood, thereby draining the vessel rebellion. Then regulate the Foot-Jueyin to unblock the rebellious Qi.
Jue headache. Persistent, fixed pain; heavy head and pain. Drain the five rows of acupoints on the head, five points per row. First select Hand-Shaoyin points, then Foot-Shaoyin points.
This section discusses Shaoyin Qi rebelling upward and affecting the Taiyang meridian, causing Jue headache. "Zhenzhen" means firm and immovable. The five rows on the head refer to the five points of the Foot-Taiyang bladder meridian: Wuchu, Chengguang, Tongtian, Luoque, and Yuzhen. Shaoyin and Taiyang govern Water and Fire, Yin and Yang Qi, combining upper and lower biao-ben. Thus, one first drains Taiyang, then selects Hand-Shaoyin, and finally Foot-Shaoyin. Mr. Shen states: The Six Qi of Yin-Yang combine only with the Six Meridians, moving from Foot to Hand, hence selecting Hand first then Foot. Mr. Shang states: Above Shaoyin, Ministerial Fire dominates, hence selecting Hand first then Foot. Zhang Kaizhi says: Shen discusses Six Qi combining with Six Meridians regarding Hand/Foot hierarchy; Shang discusses Six Qi regarding Biao-Ben hierarchy. Both views should be understood.
Jue headache. Forgetfulness; pressing yields no tenderness. Select arterial pulses on the left and right sides of the head and face, then select Foot-Taiyin points.
This section discusses Taiyin Qi rebelling upward and affecting the head and face meridians, causing Jue headache. The classic states: Qi aggregating in the upper part causes confusion and forgetfulness. The Spleen houses Yi (Intellect). Taiyin Qi rebellion causes the spirit of the Spleen to become confused, leading to forgetfulness. The Head governs Heavenly Qi; the Spleen governs Earthly Qi. Pressing finds no pain because Earthly Qi rises to Heaven, entering the interior of the head. First select arterial pulses on the head and face to drain rebellious Qi, then select Foot-Taiyin to regulate. Mo Yuncong says: The arterial pulses on the left and right of the head belong to the Foot-Yangming stomach meridian.
Jue headache. Neck pain precedes, with corresponding pain in the waist and spine. First select Tianzhu (BL10), then select Foot-Taiyang points.
This section discusses Taiyang Qi rebelling upward to the head, causing Jue headache. The Six Qi of Yin-Yang all ascend along the meridians. The Taiyang meridian runs from the head and neck down to the waist and spine. In Taiyang Jue headache, neck pain precedes, with corresponding pain in the waist and spine. This is rebellion in Qi manifesting in the meridians. Thus, first select Tianzhu at the neck to drain rebellious Qi, then select Foot-Taiyang to regulate.
Jue headache. Severe headache; surging pulses around the ears with heat sensation. Drain the blood, then select Foot-Shaoyang points.
This section discusses Shaoyang Qi rebelling into the head and neck meridians, causing Jue headache. Above Shaoyang, Ministerial Fire dominates. Fire Qi rebels upward, causing severe headache and surging, hot pulses around the ears. First drain the blood, then regulate the Qi. The above sections discuss how Qi of the Three Yin and Three Yang rebels to cause headache, not due to external pathogens.
True headache. Extremely severe headache; pain throughout the brain; cold hands and feet reaching the joints. This is a fatal condition, untreatable.
True headache is not a rebellion of the Six Qi, but rather an invading pathogen attacking the brain, causing severe, whole-brain pain. The head is the meeting place of all Yang meridians; the brain is the sea of essence and water. Cold hands and feet reaching the joints indicate that True Qi is damaged by pathogens, making it fatal and untreatable.
Headache cannot be treated by selecting distant acupoints if caused by trauma or falling, with bad blood retained internally. If muscle injury persists and pain remains, needle the local painful area; do not select distant points.
This section discusses headache caused by head trauma from blows or falls. Do not select standard acupoints. Due to trauma, bad blood is retained internally. If muscle injury persists with pain, needle the local painful area; do not select distant acupoints. This implies that selecting lower points for headache addresses Qi rebellion from below ascending upward, as meridian Qi connects vertically. Traumatic pain is not a matter of meridian Qi dynamics.
Headache cannot be needled if caused by severe Bi syndrome (obstruction). Daily attacks may be slightly alleviated but not cured.
This section states that headache caused by severe Bi syndrome also cannot be treated by needling acupoints. Severe Bi involves Wind-Cold pathogens lodging in sinews and bones, causing severe pathology. Daily attacks should be treated by addressing sinews and bones to achieve slight improvement. If unresolved, it cannot be cured immediately. This indicates Wind-Cold pathogens have penetrated deeply into sinews and bones; thus, acupoints are not suitable, and immediate cure is impossible.
Half-head cold pain. First select Hand-Shaoyang and Hand-Yangming points, then select Foot-Shaoyang and Foot-Yangming points.
This section discusses hemi-cephalgia caused by Cold pathogens lodging in meridians. Cold injures Ying (Nutrient) Blood, causing cold pain. The meridians of the three Yang limbs ascend to the head; left ones connect to the left, right ones to the right. Injury on the left causes left-sided pain, and vice versa, unlike Qi rebellion which affects the head generally. Hand and Foot Shaoyang and Yangming meridians connect to both sides of the head. Selecting Hand first then Foot is because Hand meridian Qi ascends to the head and intersects with Foot meridians. Taiyang is not selected because it occupies the central position.
Note: In the *Ling Shu* and *Su Wen*, discussions of Six Qi are followed by one meridian syndrome; discussions of Six Meridians are followed by one Qi syndrome. This is the sages' compassion, hoping students will comprehend. Shen Liangchen says: Among thousands of disasters, none exceed the Three Causes. Jue headache is internal Qi rebellion; True headache is invasive pathogens attacking the brain; Severe Bi is Wind-Cold reversing outside the vessels; Half-head pain is Cold pathogens lodging inside the vessels. These are external causes. Trauma is neither internal nor external. Analyzing these, all causes are exhausted. If one carefully nurtures oneself, harmonizing internal Blood and Qi, ordering Yin-Yang, ensuring external Yuan Zhen (Original Qi) flows freely, securing cou li (pores), preventing invasive pathogens, preserving the body and tempering nature, avoiding trauma, one can preserve natural lifespan without premature death.
Jue heart pain. Pain radiating to the back, spasms, feeling as if touched from behind, stooping posture. This is Kidney heart pain. First select Jinggu (BL64) and Kunlun (BL60). If ineffective after needling, select Ran Gu (KI3).
This section discusses rebellion of the Five Zang meridian Qi causing Jue heart pain. The True Qi of the Zang connects to the Heart, which houses the Qi of Blood vessels. Thus, rebellion of the other four Zang Qi ascends via meridians to the Heart. The Back is Yang; the Heart is Yang within Yang (Taiyang). Hence, pain radiates to the back, correlating Heart and Back. Extreme tension in Heart vessels causes spasms, feeling like being touched from behind. The Kidneys attach to the spine; Kidney Qi ascends from the back to the Heart. Due to heart pain, the patient stoops and cannot straighten up. This is Kidney Qi rebelling downward to the Heart causing pain. First select Jinggu and Kunlun of the Bladder meridian to drain the Yin Zang's rebellious Qi via the Yang Fu. If ineffective, select Ran Gu of the Kidney meridian. This is Zang Qi rebellion transmitting via meridians, unrelated to Six Qi; hence, names are Kunlun and Ran Gu, not Taiyang and Shaoyin.
Jue heart pain. Abdominal distension, chest fullness, especially severe heart pain. This is Stomach heart pain. Select Dadu (SP2) and Taibai (SP3).
Stomach Qi rebels upward, causing abdominal distension and chest fullness. Stomach Qi connects upward to the Heart, causing severe heart pain. The Spleen and Stomach are connected by membranes, serving as the transport hub for the Stomach. Thus, select Dadu and Taibai of the Spleen meridian to vent Stomach rebellious Qi. Mr. Shang Yugong says: The previous section drained Zang via Fu; this section vents Fu via Zang. Both utilize the subtle interaction of Yin-Yang and meridian Qi. The Blood and Qi of the Five Zang are generated by the Stomach Fu; hence, discussions of the Five Zang often include the Stomach.
Jue heart pain. Pain like needle pricks in the heart, extremely severe. This is Spleen heart pain. Select Ran Gu (KI3) and Taixi (KI3).
Spleen meridians pass through the diaphragm to the Heart, causing pain like needle pricks. Ran Gu should be Lougu, and Taixi should be Tianxi. Ancient texts often contain transcription errors.
Jue heart pain. Complexion cyanotic like a corpse, unable to sigh deeply all day. This is Liver heart pain. Select Xingjian (LR2) and Taichong (LR3).
The Liver governs color and belongs to Spring Wood Qi. Liver Qi rebellion causes a cyanotic complexion resembling death. Liver disease causes Gallbladder Qi to rebel, leading to inability to sigh deeply all day. This is Liver Qi rebelling against the Heart, causing Liver heart pain. Select Xingjian and Taichong of the same meridian to soothe rebellious Qi.
Jue heart pain. Pain relieves when lying still or sitting quietly, worsens with movement, complexion unchanged. This is Lung heart pain. Select Yuji (LU10) and Taiyuan (LU9).
The Lung governs Qi throughout the body. Lying still keeps Qi rebellious internally, not utilizing the body. Movement stirs internal rebellious Qi, causing pain. It may briefly relieve but worsens with activity. The Heart matches the vessels; its luster is in the complexion. The Lung is the canopy of the Heart. Rebellion from above downward prevents Heart Qi from ascending to the face, leaving the complexion unchanged. Select Yuji and Taiyuan of the Lung meridian to drain rebellious Qi.
True heart pain. Hands and feet turn cyanotic to the joints, severe heart pain. Death if onset in morning, death by evening; death if onset in evening, death by next morning.
Heart pain from the four Zang rebellion approaches the Heart via meridians. The Heart is the Monarch, housing Shen (Spirit); thus, it does not accept pathogens. If True Zang is injured causing True heart pain, death occurs within less than a day. The Heart corresponds to Solar Fire, circling the earth once a day. Heart Qi injury leads to death within a day. Cold and Heat are Qi of Heaven; Cyan and Red are Colors of the Five Elements. Thus, True headache presents with cold to joints; True heart pain presents with cyanosis to joints.
Heart pain cannot be needled if there is excessive accumulation internally; do not select acupoints.
This section states that heart pain caused by Qi issues cannot be treated by acupoints. Excessive accumulation refers to overly strong rebellious Qi of the Five Zang aggregating in the middle, causing heart pain, not ascending via meridians. This section discusses Five Zang meridian rebellion, concluding with a Qi syndrome to prove distinct meridian Qi characteristics. Hence, it only states "do not select acupoints" without specifying treatment methods.
Roundworms or abdominal masses in the intestines cannot be treated with small needles. If heart and intestinal pain occur, with cramping pain, migratory swelling, intermittent pain, abdominal heat, thirst, and salivation, these are roundworms. Press firmly to immobilize the mass, insert a large needle, hold for a long time until the worm stops moving, then remove the needle.
Abdominal pain with a raised middle shape. (Pronunciation note provided in original).
This section discusses how roundworms and masses can also cause heart pain. Roundworms form masses in the intestines. Roundworms causing heart pain occur because Fu Qi connects upward to the Heart. Masses accumulating outside the intestines cause heart pain because the Heart, governing Shen, resides centrally and high up, excluding pathogens. Small needles are not used because this does not involve meridians or skin.
"Aonao" refers to inner restlessness. Swelling and aggregation result from worms clogging the chest and abdomen; ascending causes pain, descending brings relief, hence intermittent pain. Roundworms arise from Damp-Heat, causing abdominal heat. Thirst arises as worms seek water. Worm movement opens Lianquan, causing salivation. These symptoms indicate roundworms. Press firmly, insert large needle, hold until worms die, then remove. Abdominal fullness with inner restlessness and pain indicates a mass type rising from the middle. Shen Liangchen says: Like the previous section on trauma and the next on cerumen, these do not involve meridian Qi.
Deafness and hearing loss: select acupoints in the ear. Tinnitus: select arterial pulse in front of the ear. Ear pain cannot be needled if there is pus or dry cerumen causing deafness. For deafness, select the point between the nail and flesh at the tip of the fourth finger of the hand. First select Hand meridian, then Foot meridian. For tinnitus, select the point on the nail of the middle finger of the hand. Left side treats right, right side treats left. First select Hand meridian, then Foot meridian.
This section discusses Qi rebellion moving from meridian to Qi, from Foot to Hand, upward from below. For deafness/tinnitus due to rebellion in upper meridians, select collateral vessels near the ear. For Qi rebellion causing deafness/tinnitus, select Jing-Well points of hands and feet. First Hand, then Foot. Because Six Qi combine with Six Meridians, and vigorous/rebellious Qi is in the Hand meridians, Yin-Yang rebellion causing deafness/tinnitus moves from Foot to Hand, Hand to Head. If pus or cerumen causes deafness/pain, it is unrelated to meridian Qi; thus, do not needle ear collaterals or finger Jing-Well points. Note: The fourth finger tip is Guanchong (TE1, Hand-Shaoyang); the middle finger tip is Zhongchong (PC9, Hand-Jueyin). The Foot meridian selected is Dazhun (LR1, Foot-Jueyin). The Three Yin meridians of Hand and Foot do not ascend to the head, nor do they cross laterally. This is due to Qi rebellion causing deafness/tinnitus. The ear is the orifice of the Kidney. Jueyin governs Spring; Shaoyang is initial Life Qi; both arise from Kidney Water. Rebellion of generated Qi blocks the mother organ's external orifice. Thus, selecting finger Jing-Well points accesses the source of meridian Qi. The first section discussed Jue headache (Qi rebellion affecting meridians); the second discussed Jue heart pain (Zang rebellion affecting meridians), showing Zang-Fu-Meridian connection. This section discusses rebellion in meridians (select collaterals) and Qi (select Jing-Well points), illustrating the separation and union of meridian Qi. Yin-Yang entry/exit, Cold/Heat cycles originate from Earth, moving upward. Hence, select Yang before Yin (Qi moves upward); select Hand before Foot (Qi moves from Foot to Hand). Shen Liangchen says: This section distinguishes the vertical flow of meridian Qi and the differentiation of Zang-Fu Yin-Yang.
Inability to lift the thigh: select while lying on side. Point is in the pivot joint (Huantiao GB30). Use a Yuanli needle; Da needle is contraindicated. For urgent bloody diarrhea, select Ququan (LR8).
This continues the discussion on Qi rebellion in the lower body, treating from below. Yin-Yang Qi originates below but circulates endlessly up and down. Rebellion can be upward or downward; treat according to direction. Inability to lift the thigh is Shaoyang Qi rebelling below. Side-lying selection refers to lateral recumbency. "He Shu Zhong" refers to Huantiao in the hip pivot. Deep insertion is required. Use Yuanli needle; Da needle is contraindicated because rebellion is in Qi, not the meridian itself. Superficial needling in the cou li suffices to disperse Qi; deep meridian needling is unnecessary. Urgent bloody diarrhea indicates Qi rebellion entering the meridian. Jueyin Liver governs Blood; Jueyin Qi rebellion in the meridian requires selecting Ququan of the same meridian to stop bleeding. Qi is Yang, Blood is Yin; Upper is Yang, Lower is Yin. Qi rebelling upward from below causes Qi-closure deafness/tinnitus; Qi rebelling downward from above causes urgent bloody diarrhea.
Wind-Bi with Yinluo (limb weakness/pain), incurable. Feet feel like walking on ice; sometimes feel like entering hot soup. Thighs and calves suffer Yinluo; vexation, headache, occasional vomiting and stifling, dizziness with sweating. Long-term: blurred vision, alternating sadness/fear, shortness of breath, depression. Death within three years.
This section discusses Jue Qi attacking upper and lower regions. Wind-Bi Yinluo is obstructive Wind pathogen moving up and down, characterized by Wind's mobility and variability. Yin-Yang splits into Three Yin and Three Yang, corresponding to Hand/Foot meridians, but fundamentally unites as Yin-Yang. Water and Fire are manifestations of Yin-Yang; Heart and Kidneys are their formative Zang organs. Wind pathogen moving up and down makes the condition incurable. Cooling damages Heart Fire; heating damages Kidney Yin. Unincurable. Yinluo below causes feet feeling like ice (Cold/Water Qi). Yinluo above causes feeling like hot soup (Heat/Fire Qi). Thigh/calf Yinluo affects lower limbs. Vexation/headache affects upper head. Vomiting/stifling affects middle jiao. All meridians converge at the eyes; dizziness indicates invasion of Blood level. Loose cou li causes sweating (Qi level invasion). Water's essence is Zhi (Will); Fire's essence is Shen (Spirit). Both gather at the eyes; long-term leads to blurred vision. Joy is Heart Will; Fear is Kidney Will. Sadness combined with joy/fear indicates Heart-Kidney Spirit damage. Kidney is root of Qi; shortness of breath indicates Kidney Qi injury. Depression indicates Heart Qi injury. Sun corresponds to Fire; Moon to Water. A celestial cycle is 365.25 degrees; a year is 365+ days. Sun and Moon reunite after one cycle. Death within three years signifies exhaustion of Water-Fire Yin-Yang cycles. This chapter discusses Jue rebellion diseases, including...
