(The pulse presents as obstructed.) When the Yin pulse (Radial artery at the cun position) beats forcefully and is distinctly different from the Yang pulse (Cun position), it is called pregnancy. "Yin beating" refers to the chi position pulse being slippery and forceful against the fingers. "Yang differentiation" means the chi pulse seems distinct from the yang pulse at the cun position, without interpenetration. This indicates pregnancy. Because there is a fetus inside, the chi pulse becomes slippery like pearls. Wu stated: The following sections discuss pulses.)
If both Yin and Yang are deficient, leading to changpi (dysentery with bloody stool) and diarrhea, death ensues. Here, "Yin and Yang" refer to the chi and cun positions respectively. "Changpi" refers to dysentery and diarrhea. Ying (Defensive) and Wei (Nutritive) Qi, along with Blood and Qi, are all generated from water and grain. The Stomach is the organ of reception, and the Intestines are the officials of transmission. If both Yin and Yang pulses are deficient, and the root of their generation is lost, there is no hope for vitality. This explains that Yin and Yang Qi are generated from water and grain in the stomach and intestines.)
When Yang acts upon Yin, sweat is produced. Sweat belongs to Yin fluids but requires the dispersing action of Yang Qi to fill the body and moisten hair. If a rapid and numerous Yang pulse appears at the chi position (Yin region), this is termed sweat. It should be understood that sweat is Yang acting upon Yin fluids, and the pulse reflects a Yang pulse acting upon the Yin region.)
When Yin is deficient and Yang surges violently, it is called Bing (metrorrhagia or uterine bleeding). Yin deficiency with Yang excess forces blood to move recklessly.)
If the three Yin pulses (Taiyin, Shaoyin, Jueyin) all beat forcefully, death occurs at midnight on the twentieth day. "Bo" sounds like "Bo". "Three Yin" refers to the Taiyin pulse. "All beating" means the Spleen and Lung pulses both beat forcefully against the fingers without any harmonious Yang Qi. Two is the beginning of even numbers; Ten is the end of Yin numbers. Midnight is when Yin is exhausted and one Yang begins to arise. Taiyin is the Zhiyin (Extreme Yin) organ. With Extreme Yin Qi and absolutely no generating Yang, death occurs at the number of extreme Yin. Dong Weiyuan said: "Yin knot" and "Yang knot" discuss the knotting of Yin and Yang Qi. "Gang with Gang" refers to Fu meridions transmitting to Fu organs and Zang meridians transmitting to Zang organs. "Yin bo Yang bo" refers to the disharmony of Yin and Yang in the twelve meridians.)
If the two Yin pulses (Shaoyin) all beat forcefully, death occurs at dusk on the thirteenth day. "Two Yin" refers to the Shaoyin pulse. "All beating" means the Heart and Kidney pulses both beat forcefully. Shaoyin governs Water and Fire, Yin and Yang Qi. Tian Yi generates Water, Di Cheng transforms it; Di Er generates Fire, Tian Cheng transforms it. Thirteen days is the completion of the Cheng (completion) numbers. Dusk is the end of the day. Because these are Yin organs of Water and Fire, death occurs at the Cheng numbers and ends at the end of the day.)
If the one Yin pulse (Jueyin) all beats forcefully, death occurs after ten days. "One Yin" refers to the Jueyin pulse. "All beating" means the Liver and Pericardium pulses both beat forcefully. Ten days is the end of Yin numbers. Jueyin is the end of Yin. With Yin-ending Qi, death occurs at the end of Yin numbers.)
If the three Yang pulses (Taiyang) all beat forcefully and are distended/pulsating, death occurs after three days. "Three Yang" refers to the Taiyang pulse. "Gu" means movement/distension. "All beating and distending" means the Hand-Taiyang and Foot-Taiyang meridians both beat forcefully and distend. This is Yang at its extreme with absolutely no Yin harmony. Taiyang and Shaoyin are exterior-interior pairs, jointly governing Water and Fire. Tian Yi generates Water, Di Er generates Fire. Because these are Yang Fu organs of Water and Fire, death occurs at the Sheng (generation) numbers of Heaven and Earth. Generally, Heaven is Yang, Earth is Yin; Heaven governs generation, Earth governs completion. Thus, Taiyang dies at generation numbers, and Shaoyin dies at completion numbers.)
If the three Yin and three Yang pulses all beat forcefully, accompanied by chest fullness, abdominal skin becoming taut and shiny, and inability to pass urine or feces, death occurs after five days. Three Yin and Three Yang correspond to the Five Elements. When Yin and Yang Qi all beat forcefully without harmony, death occurs after completing the cycle of Five Elements. "Chest fullness" is Yang beating upwards. "Inability to perform hidden functions" (urinary/fecal retention) is Yin beating downwards. The abdomen is in the middle of the body, where Yin and Yang intersect. "Abdominal skin taut" means Yang fully disperses upwards and Yin fully disperses downwards, without the harmonious manifestation of Yin and Yang in the middle. This discusses pathological conditions of upper and lower Yin and Yang; the following text discusses cold-heat Yin-Yang pathology.)
If the two Yang pulses (Yangming) all beat forcefully, the disease is Wen (warm disease). Death is incurable and occurs within three days. "Two Yang" refers to the Yangming pulse. "All beating" means the Hand-Yangming and Foot-Yangming pulses both beat forcefully. "Wen disease" refers to cold-heat alternating warm diseases. Human Yin and Yang Qi are nourished by water and grain from Yangming. If the two Yang pulses all beat forcefully, the source of Yin and Yang generation is cut off. Without Yang, Yin causes cold disease; without Yin, Yang causes heat disease. Simultaneous exhaustion of Yin and Yang is an incurable death syndrome. Nine is the end of Yang numbers; Ten is the limit of Yin. Not exceeding ten days implies death at the intersection of Yin and Yang exhaustion. The previous section stated that Three Yang are at the head and Three Yin are at the hand, indicating unity. If Yin and Yang Qi cannot penetrate each other and instead beat against themselves, death results thus. This explains that the Yang Qi of the stomach cavity nourishes the Yin and Yang of the five Zang organs. If the two Yang pulses all beat, Yin and Yang are simultaneously exhausted, leading to death thus. Therefore, the last two sections specifically highlight the clinical manifestations.)
