The left side (Heart, Small Intestine, Liver, Gallbladder, Kidney) and the right side (Lung, Large Intestine, Spleen, Stomach, Mingmen Gate) correspond to internal organs; if injuries affect the whole body involving these areas, death occurs rapidly. For injuries occurring within seven days, blood-invigorating treatment should be administered. If more than fourteen days have passed and there is blood stasis in the chest or pain in the Large Intestine, urgent blood-invigorating medication is required. One must observe whether the fingernails are black? And whether the toenails are black? Blackness indicates a ominous prognosis. A dark complexion suggests Kidney injury. A rapid Liver pulse indicates blood stasis in the chest and abdomen, accompanied by hematemesis. Death results from panting and shortness of breath with rattling phlegm; fixed or deviated gaze; retracted lips (inability to close mouth); severe neck sprain (Shi Zhen); foul breath. Redness of the ears and nose indicates death. Bone fractures with a bluish-green hue indicate death; severance of the Qi throat (trachea/larynx) indicates death; a distended/high chest (severe respiratory distress) indicates death; grasping at empty air (unconscious motor activity) indicates death; unceasing hemorrhage indicates death; blood that is initially red but turns black indicates death; pain located away from the site of injury indicates death; putrefaction of muscles indicates death.
Fracture of the cranial bone indicates death; injury to the sides of the frontal temples indicates death; severance of the Tianzhu bone (upper cervical spine) indicates death; rupture of the scalp behind the ear indicates death; injury to both Taiyang acupoints (temples) indicates death; injury to the Mingmen point indicates death; comminuted fracture of the vertex skull indicates death; injury inside the eyebrows indicates death; fragmentation of the Huxin bone (near xiphoid process) indicates death; injury to a pulsating vessel in the arm indicates death. Injuries to the chest, back, flanks, scrotum, vulva, anus, inner thigh bones, crushing of the Haidi point on the left thigh of an elderly person, rupture of the Large Intestine with passage of black stool, injury to the Small Intestine resulting in clotted urine, and fragmentation of the Heart, Lungs, Spleen, Liver, Kidneys, and reproductive organs ("eggs") all constitute severe trauma. Any severe injury presenting with closed eyes and mouth, clenched jaw, inability to take fluids, and excruciating pain is fatal.
A six-pulse presentation that is deep, fine, deficient, and small indicates death; one that is floating, large, rapid, and excess indicates death. Excessive blood loss leading to a faint, intermittent, or absent pulse indicates death.
For severe injuries where the mouth and eyes are closed and speech is absent, immediately insufflate powder of Gleditsia sinensis (Tian Wang Jiao) into the nostrils; sneezing will allow the mouth to open. If no sneeze occurs, dip a rush stem in well-water-adhered Tian Wang Jiao powder and apply to the nasal cavity; if sneezing ensues and phlegm is expectorated, the case is treatable; otherwise, it is incurable. Subsequently, crush the white stalks of Chinese chives to extract juice, warm it with fresh male child's urine (Tong Bian), and administer orally. If swallowed, it is treatable; if not, it is untreatable. If vomited up along with blood stasis afterward, assess the severity. First, administer a decoction of Amomum villosum (Sha Ren), mixed with Jili San. Next, prescribe Qingxin Heqi Tang, and externally apply Sunshang Gao (Trauma Plaster). For excessively severe injuries presenting with headache, coma, and absence of hematemesis, urgently administer Chinese chive juice mixed with aged wine. For bone fractures and tendon ruptures, protect the area with sealing medicine. For urinary retention, use Hupo San to promote urination. For abdominal pain due to internal blood stasis, use Dahuang San to purge downward.
