If coughing produces purulent sputum, heat is accumulated in the Lung, forming Lung Abscess (Fei Yong); Jiegeng Tang (Platycodon Decoction) should be used. For external wind-pathogen invasion, the method should be to release the exterior and consolidate the Cou Li (skin and pores). Purging methods are not appropriate unless the pathogen has transmitted internally or there is excess heat in the Stomach. Pale complexion with short and choppy pulse indicates the root syndrome of the Lung, which is easy to treat. Red complexion with surging and rapid pulse indicates Fire insulting Metal, which is difficult to treat.
Clinical Cases:
A child presented with tidal fever, irritability, thirst, dry and hard stool, shortness of breath, cough, and redness on the right cheek. This indicated heat in the Lung and Large Intestine. One dose of Chaihu Yinzi (Bupleurum Drink) caused all symptoms to recede immediately. Later, convulsions, teeth grinding, and sudden dizziness occurred, indicating deficiency of Qi and Blood in the Liver and Spleen. Treatment with Sijunzi Tang (Four-Gentlemen Decoction) plus Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), and Goutenggou (Uncariae Ramulus cum Uncis) resulted in recovery.
A child presented with cough, nausea, nasal congestion, runny nose, and bluish-white color on the right cheek. This indicated deficiency of Spleen and Lung Qi with external pathogen invasion. Xingxing San (Xingxing Powder) was used first, curing the cough immediately. However, lack of appetite and cold fingers/toes indicated that although the external pathogen was gone, Yuan Qi (Original Qi) remained deficient. The Spleen Earth should be tonified to generate Lung Metal. Treatment with Liujunzi Tang (Six-Gentlemen Decoction) plus Shengma (Cimicifugae Rhizoma) resulted in recovery. Generally, when external pathogens invade causing phlegm-drool obstruction, releasing the exterior is appropriate. When the external pathogen is gone but wheezing/cough persists or worsens with shortness of breath, it indicates Lung Qi deficiency, representing deficiency of both form and disease. Tonifying Spleen Earth to generate Lung Metal with formulas like Liujunzi Tang is best. Directly tonifying Lung Qi may exacerbate the pathogen. The Lung is a delicate organ governing the Cou Li, with the Spleen as its mother. If the Cou Li are not secure, wind-pathogens invade externally, accumulate in the Lung, and cause various cough symptoms, it represents insufficiency of form and Qi with excess pathogenic Qi, which is hardest to regulate. Vomiting damaging Stomach Qi, or sweating/purging damaging Jin Ye (Body Fluids), can lead to Lung萎 (Fei Wei) or Lung Abscess (Fei Yong).
The son of Shi Wanyan from Wujiang, aged six, had a cold with cough. Overuse of diaphoretic agents led to rapid breathing, inability to eat, and blood in sputum. Jiegeng Tang (Platycodon Decoction) cured him. Later, due to incomplete recovery of Yuan Qi and failure of clear Yang to ascend, his stool resembled dysentery. Use of Wulin San (Five-Dampness Powder), Huanglian (Coptidis Rhizoma), and Zhishi (Aurantii Fructus) caused phlegm-wheezing, frequent blinking, and limb convulsions, leading to death from Slow Wind (Man Feng/Chronic Convulsion).
A child with wind-cold cough and fever took exterior-releasing agents, worsening into wheezing and sweating. I diagnosed Lung and Spleen Qi deficiency and proposed Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Wuweizi (Schisandrae Fructus). The family disagreed and self-administered Erchen Tang (Two-Aged Decoction), Sangbaipi (Morus Cortex), and Zhike (Aurantii Fructus), resulting in convulsions and phlegm surge. I continued with the previous prescription plus Goutenggou (Uncariae Ramulus cum Uncis), resulting in recovery.
A child with asthma had a mother whose symptoms were triggered by labor; the infant also coughed after breastfeeding. Treatment with Liujunzi Tang (Six-Gentlemen Decoction), Jiegeng (Platcodonis Radix), Sangbaipi (Morus Cortex), and Xingren (Armeniacae Semen) cured both mother and child.
A child with food stagnation presented with fever, convulsions, vomiting, wheezing, and cough, indicating Spleen and Lung Qi deficiency with heat. Treatment with Liujunzi Tang (Six-Gentlemen Decoction), stir-fried Huanglian (Coptidis Rhizoma), and Zhizi (Gardeniae Fructus) resulted in recovery.
A child’s cough was caused by the wet nurse consuming rich, greasy, and roasted foods. Qingwei San (Clear the Stomach Powder) cured him. Later, the mother developed cough and hypochondriac pain due to anger, and her son exhibited similar symptoms. The mother took Xiaochaihu Tang (Minor Bupleurum Decoction), and the son recovered accordingly.
The son of Shi Wanzhou from Wujiang had wind-cold cough. Use of exterior-releasing and phlegm-resolving drugs worsened phlegm and abdominal distension, with pale complexion. I diagnosed Spleen and Lung Qi deficiency. One dose of Liujunzi Tang (Six-Gentlemen Decoction) and Jiegeng (Platcodonis Radix) cured him. Three days later, cough and clear nasal discharge returned due to new wind-cold exposure. The same formula plus Sangbaipi (Morus Cortex) and Xingren (Armeniacae Semen) cured him.
A child with fever and cough had redness on the right cheek, indicating Heat in Lung Metal. Xiebai San (Poria and White Peony Powder) cured him. The next day, re-exposure to wind caused recurrence with hoarse voice and runny nose. Shenqi Yin (Ginseng and Perilla Drink) added with Xingren (Armeniacae Semen) and Sangbaipi (Morus Cortex) cured him. Slight redness on the right cheek and forehead indicated Heart Fire insulting Lung. Renshen Pingwei San (Ginseng Stomach-Calming Powder) one dose resulted in recovery.
A child with cough, fever, red right cheek, thirst, irritability, fatigue, poor appetite, and abdominal distension indicated Wind injuring Lung and Diet injuring Spleen. First, Liujunzi Tang (Six-Gentlemen Decoction), Jiegeng (Platcodonis Radix), Xingren (Armeniacae Semen), and Chaihu (Bupleuri Radix) improved symptoms. Removing Xingren and Chaihu and taking another dose brought stability.
A child with fever, red right cheek, cough, and profuse sputum: I diagnosed Wind accumulating in the Lung causing phlegm. Erchen Tang (Two-Aged Decoction) plus Sangbaipi (Morus Cortex), Xingren (Armeniacae Semen), and Jiegeng (Platcodonis Radix) showed improvement. Self-medication with exterior-releasing and fire-clearing agents failed to resolve wind-phlegm and worsened fever. I diagnosed dual deficiency of Spleen and Lung. Wuwei Yigong San (Five-Ingredient Yi Gong Powder) added with Jiegeng (Platcodonis Radix) for four doses gradually improved the condition, followed by Liujunzi Tang (Six-Gentlemen Decoction) for full recovery.
A three-year-old child with surging phlegm, wheezing, chest fullness, loose stools, sleeping with eyes open, and cold fingers/toes indicated dual deficiency of form and disease. Liujunzi Tang (Six-Gentlemen Decoction) and Jiegeng (Platcodonis Radix) for one dose alleviated symptoms; four doses nearly cured him. Re-exposure to wind-cold caused recurrence. Adding Zisu (Perillae Folium), Xingren (Armeniacae Semen), and Sangbaipi (Morus Cortex) to the previous formula brought stability.
A child with wind-cold cough and profuse sputum, treated with mixed phlegm-resolving drugs, developed worsening chills and fever, with alternating blue and red complexion, indicating struggle between Wind and Heat. Niuhuang Qingxin Wan (Calves’ Bead Clearing the Heart Pill) one dose, followed by Liujunzi Tang (Six-Gentlemen Decoction) and Jiegeng (Platcodonis Radix) two doses, resulted in recovery.
