Qian Zhongyang stated: The Heart governs fright. In excess patterns, symptoms include crying and weeping, fever, thirst for water, and convulsions; in deficiency patterns, symptoms include lethargy, sleepiness, palpitations, and restlessness. He further stated: If there is heat, the breath may feel warm during sleep, eyes may roll upward, teeth may be clenched, and the infant may prefer sleeping face down seeking coolness; these are all signs of Heart Heat, treated primarily with Daochi San (Coptis Decoction). If the infant sleeps face down, it indicates Heart Qi Excess, where Qi fails to circulate up and down, treated primarily with Xiexin San (Gallbladder-Draining Decoction). Heart diseases manifesting in winter indicate Fire overcoming Water (Kidney Water deficiency); treatment should tonify the Kidney and treat the Heart. Mild cases will resolve spontaneously. Symptoms such as eyes rolling downward and muteness indicate Kidney Deficiency and timidity.
Furthermore, Zhang Jiegu stated: The Heart governs heat. If Lung pathogenic factors invade the Heart, it is a minor evil; if Liver pathogenic factors invade the Heart, it is a deficient evil; if Spleen pathogenic factors invade the Heart, it is an excessive evil; if Kidney pathogenic factors invade the Heart, it is a destructive evil. Whenever the Heart becomes diseased, one must first regulate the Liver and Kidneys. When Liver Qi flows freely, Heart Qi harmonizes; when Liver Qi stagnates, Heart Qi becomes deficient. This principle dictates that treating Heart disease requires addressing the Liver first to clear the source. When the Five Zang organs are diseased, the pathology inevitably transmits to the organ it controls. Pathogenic factors from the Kidney inevitably transmit to the Heart; therefore, one must first treat the Kidney to expel the pathogen. Only when the pulses of both the Liver and Kidney are harmonious should one then examine the Heart for excess or deficiency before treating. I suggest that infants sleeping on their backs do so because of excess heat in the chest and heart, preferring to lie back towards empty space (seeking coolness). Infants sleeping face down do so because of deficient heat in the chest and heart, preferring to press against solid surfaces (seeking support). For excess patterns, regulate the Heart and Liver; for deficiency patterns, tonify the Spleen and Lungs. Distinguishing between these two exhaustively describes the condition. Regarding symptoms like clenching teeth, which often overlap, one should not be rigid. If using formulas like Xiexin San or Daochi San, and although pathogenic factors are expelled the illness recurs, one should regulate and tonify Yuan Qi (Primal Qi); if symptoms worsen, urgently warm and tonify Yuan Qi. If Heart Qi manifests in winter, or symptoms worsen during Hai and Zi hours (9 PM - 3 AM), or if eyes roll downward and muteness occurs, this indicates Kidney Water deficiency and Heart Fire excess, treated with Dihuang Wan (Rehmannia Pill). For nursing infants, the mother should take the medicine.
Qian's Xiexin San: Treats Heart Meridian Excess Heat.
Huanglian (Coptidis Rhizoma)
Grind into powder. Take 0.5 fen each time, dissolve in warm water before bedtime.
My commentary: The aforementioned symptoms, if accompanied by crying, fever, thirst for drinking, strong convulsions, and sleeping on the back, belong to Heart Meridian Excess Heat, suitable for Xiexin San or Daochi San. If accompanied by fever, desire for hot drinks, weak convulsions, fright with eye rolling and tooth grinding, and sleeping face down, it belongs to Heart Meridian Deficient Heat, treated with Buxin San. If accompanied by wheezing, coughing, red face, high fever, and thirst, it indicates Lung invading Heart, treated with Xiebai San. If accompanied by head shaking, blinking, body heat, and convulsions, it indicates Liver invading Heart, treated with Chaihu Qinggan San. If accompanied by closed eyes, drowsiness, diarrhea, and body heat, it indicates Spleen invading Heart, treated with Xiehuang San. If accompanied by darting gaze, palpitations, tooth grinding, and hot feet, it indicates Kidney invading Heart, treated with Anshen Wan. If the condition is caused by the wet nurse, use the aforementioned medicines to treat the mother.
Daochi San: Treats Small Intestine Excess Heat, characterized by scanty and dark urine.
Shengdihuang (Rehmanniae Radix Recens), Mutong (Akebiae Caulis), Gancao (Glycyrrhizae Radix et Rhizoma) in equal parts.
Grind into powder. Take 1 qian each time, decoct with Danzhuyeh (Lophatheri Herba) in water.
My commentary: Xiexin San and Daochi San are formulas that drain excess fire from the Heart and Small Intestine. Since the Heart is the Mother of the Spleen, and the Spleen is the Child of the Heart, once the Heart is diseased, the Spleen Earth becomes increasingly deficient. Users must exercise caution!
Qian's Shengxi San: Treats Heart Meridian Deficient Heat.
Digupi (Bark of Ground Tree Bark), Chishaoyao (Radix Paeoniae Rubra), Chaihu (Radix Bupleuri), Gange (Radix Puerariae Lobatae) each 1 liang; Gancao (Radix Glycyrrhizae) 5 qian; Xijiao (Cornu Rhinocerotis) 2 qian, shaved.
Grind into powder. Take 1-2 qian each time, decoct in water. (Added)
My commentary: The previous formula claims to treat Heart Meridian Deficient Heat, but its ingredients mostly belong to formulas that drain the Heart and Liver meridians. The term "Deficient Heat" here is likely a textual error. If the Heart Meridian itself is diseased causing blood deficiency heat, use Mizhi Anshen Wan. If Spleen deficiency steals Qi from the Heart causing heat, use Mizhi Bupi Tang. If Liver Wood fails to generate Heart Fire causing deficient heat, use Dihuang Wan.
Mizhi Anshen Wan: Treats Heart Blood deficiency causing palpitations during sleep, or onset due to fright.
Renshen (Ginseng Radix et Rhizoma), Banxia (Pinelliae Rhizoma Praeparatum cum Zingibere et Alumine, soaked in boiling water), Suanzaoren (Ziziphi Spinosae Semen, stir-fried), Fushen (Poria with Hostwood) each 1 qian; Danggui (Angelicae Sinensis Radix, washed with wine), Juhong (Exocarpium Citri Reticulatae Chishao (Radix Paeoniae Rubra, stir-fried) each 7 fen; Wuweizi (Schisandrae Fructus) 5 grains, crushed; Gancao (Glycyrrhizae Radix et Rhizoma, honey-fried) 3 fen.
Grind into powder, make pills with ginger juice paste to the size of Euryale Seed. Take one pill each time, swallow with Ginger Decoction.
