The principle of creation and generation relies on dampness and heat. Just as rotting wood produces insects and decaying grass turns into fireflies, some parasites cannot form their own shape on plants and must parasitize them; this is actually due to the interaction and transformation of damp-heat qi. Parasites in the human abdomen arise similarly. This occurs because of unregulated consumption of rich and sweet foods, excessive intake of raw and cold items, long-term stagnation transforming into heat, and the brewing effect of damp-heat, which transforms into parasites or accumulations. At this stage, they have not yet caused harm. Over time, the zang-fu organs become weak, or stomach cold/heat develops, or upon re-consuming rich and fatty foods, the parasites become active. When active, they move up and down, attacking the heart and abdomen. Patients cry out, arch their backs, wave their hands, feel mental restlessness, salivate excessively, vomit clear fluid, and experience fluctuating severity of symptoms. Green veins appear on the abdomen, nausea resembles epilepsy, but eyes do not deviate, limbs do not convulse, complexion is abnormal (blue or black). This describes worm pain. Additionally, ascariasis pain arises from eating too early or food that is too coarse. When active, they attack the heart and abdomen, pain comes and goes, they move suddenly, patients crave sweets and dislike other foods, vomit clear fluid, cry loudly, and lips/mouth turn purple-black. In all abdominal pains, the pulse must be deep, weak, and wiry; if it is instead surging, it indicates ascariasis. Moreover, all worms originate in the Liver; thus, in worm pain, the Liver pulse is significantly enlarged. In severe cases, worms cluster together, causing extreme pain leading to syncope, resembling slow convulsions, except for the unique purple lips and mouth. Treatment principles: for excess and severe cases, use purgation; for deficiency and mild cases, use calming methods; if medication cannot be taken, use Zanthoxyli Pericarpium (Chuanjiao, 川椒) to subdue them; sometimes medicine is mixed with meat broth, as the worm head faces upward at this time, allowing expulsion. If the worm penetrates the heart, it is incurable.
Danxi stated: Worms are born of damp-heat; when zang-fu are deficient, they erode. The *Qianjin Fang* states: Labor generates heat, heat generates worms. Heart worms are called Ascaris (Hui, 蛔); Spleen worms are called Tapeworms (Cunbai, 寸白); Kidney worms resemble cut silk threads; Liver worms resemble rotten apricots; Lung worms resemble silkworms. All can be fatal, but Lung worms are most urgent. They reside within lung lobes, eroding lung tissues, causing consumption disease (Zhai Ji, 瘵疾), hemoptysis, hoarseness. Medicines cannot reach them easily, making treatment difficult. Worm production results from irregular diet, hunger and overeating, preference for raw/fatty meats, excessive cold/raw foods, addiction to fermented products, love for rich/sweet foods, leading to zang-fu weakness and internal damp-heat invasion, generating parasite accumulations. When taking vermifuges, the first half of the lunar month is best, as worm heads face upward then, whereas in the second half, they face downward. The three main worms are Long worms (Ascaris), Red worms, and Threadworms (Sao, 蛲). There are nine types of worms in total, with threadworms being most common. Patients with tapeworms contract it from eating beef and drinking white wine; if connected for a chi length, it is fatal. Taking medicine to expel them is ideal only when fully expelled. Other worms include Fu worms, Rou worms, Lung worms, Stomach worms, Ruo worms, and Hui worms. However, threadworms mainly affect children, though adults can get them, causing clear saliva vomiting, heart pain, irritability, intermittent pain. Generally, expelling other worms via bowel movement benefits humans. If present in the stomach, Huachong Wan should be used to remove them quickly. If there is internal heat and empty intestines/stomach, worms move seeking food. Ulcers on the upper lip are called "Huo" (confusion), where worms eat viscera; ulcers on the lower lip are called "Hu" (fox), where worms eat the anus. Those contracting this disease rarely survive. In abdominal pain, the pulse should be deep and wiry; if surging, it indicates ascariasis, because heat generates worms, hence the surging pulse. Generally, if one has a strong craving for a specific food, there are likely worms inside. Adding that preferred food to anti-worm medications ensures immediate efficacy. If middle Qi is deficient and worms are restless, tonifying and regulating the Spleen and Stomach will calm them. All worms are subdued by pepper, calmed by bitterness, and quieted by sourness. The "Echoing Worm": An ancient case involved a patient whose voice echoed every time he spoke. Reading the *Bencao* to the character "Lan" (Isatis Root), he fell silent. Isatis root juice was administered, and shortly after, he vomited a piece of flesh, over two cun long, resembling a human form, and the echoing sound ceased. Another patient did not respond to "Leiwang" (Polyporus Umbellatus) and was cured by taking Leiwang.
