(one by one decocted and taken daily, within less than ten doses, the foot sores healed; as the pathogen ascended to the head, taking the same prescription again cured the head sores, and the spirit became even more vigorous.)
Again, he treated a student of the commandery scribe named He, who suffered from rotting and ulceration of the mouth, tongue, and throat without pain, extreme stifling oppression in the chest, diaphragm, and abdomen, sleeplessness throughout the night, and inability to eat or drink. Palpation of his pulse revealed the Right Cun and Guan positions as wiry, surging, and striking the fingers, while the Left Cun and Guan were deep, minute, and on the verge of collapse; both Chi positions lacked root upon heavy pressure. Inquiring about the onset of the illness, it was found that he had previously overworked his mind, experienced excessive anger, and labored strenuously despite being ill, leading to internal injury and fever. The physician mistakenly used dispersing methods. When red spots appeared, they were diagnosed as measles (Ma Zhen). Further use of resolving and clearing methods caused severe fatigue, swelling and hardness inside the cheeks, which was suspected to be measles toxin. More cooling and detoxifying agents were administered, resulting in unbearable chest and diaphragmatic distension, followed by excruciating pain. I then realized this was due to medical error. Generally, fever from labor injury originates from deficiency of Middle Qi. Mistaken dispersion causes Ying Qi to flow counter-currently, forming spots. Subsequent mistaken use of clearing and resolving methods causes Yin Fire to float upward to the teeth and cheeks, causing swelling. Still misdiagnosed as residual measles toxin, further intake of cold and clearing agents further depleted the Spleen and Stomach, damaged Yuan Yang, and allowed Yin shadow fire to lodge in the throat, causing bloody ulceration without pain. Like an object losing the illumination of heaven and sun, it easily rots; hence it is called "Yin Lan" (Yin Ulceration), distinct from the swelling and pain caused by Yang Fire impact.
I initially prescribed Shu Di Huang (Prepared Rehmannia Root) 1 liang 2 qian, Chao Bai Zhu (Stir-fried Atractylodes Macrocephala Rhizome) 4 qian, Niu Xi (Achyranthes Bidentata Root) 3 qian, Chao Mai Dong (Stir-fried Ophiopogon Japonicus Tuber) 2 qian, Wu Wei Zi (Schisandra Chinensis Fruit) 8 fen, and Zhi Fu Zi (Prepared Aconitum Carleilii Lateral Root) 1 qian 5 fen. After two consecutive doses, chest distension gradually decreased, sleep stabilized, and the six pulses became slightly harmonious. The next day, I prescribed Ren Shen (Ginseng Radix) 3 qian, Zao Ren (Ziziphus Jujuba Seed) 2 qian, Shu Di Huang 4 qian, Dang Gui (Angelica Sinensis Root) 1 qian 5 fen, Niu Xi 2 qian, Chao Mai Dong 2 qian, Wu Wei Zi 6 fen, and Rou Gui (Cinnamon Bark) peeled 8 fen, decocted with fresh ginger and jujube, administered twice daily. By the next day, the six pulses showed spirit, the patient's mental state was clear, appetite returned, and the throat ulcers began to feel slight pain. This indicated the return of Yang Harmony. Just as frozen earth and stagnant water revive upon receiving Yang warmth, the return of sensation (pain) indicates recovery.
I prepared Tong Qing (Malachite) 3 qian, Ren Zhong Bai (Human Tooth Ash) 2 qian, Xi Niu Huang (Indian Buffalo Calculus) 1 fen, Da Bing Pian (Borneol) 2 fen, and She Xiang (Musk) 1 fen, ground into an extremely fine powder. Applied sparingly into the lesion, old coagulated phlegm and pus flowed out continuously. Repeated application led to healing within days. After several days of regulation, spirit and appetite improved daily. Later, I formulated Ba Wei Wan (Eight-Ingredient Pill) with added Niu Xi and Wu Wei Zi into pills, taken in the morning with light salt water (5 qian per dose). The previous prescription for nourishing Heart and Spleen Qi and Blood was decocted into a thick paste, taken in the evening. Within ten days, full recovery was achieved. Observing this case, all conditions affecting external muscles and skin, or internal throat and intestines, stem from Yin-Yang imbalance, manifesting as the formless transformation of Qi and Blood. Is there truly any tangible external pathogenic toxin that can be cleared by cold and purgative methods? Even if Fire is excessive, it is due to Water insufficiency; tonifying Water transforms Yang.
The *Classic* states: "Wherever evil gathers, Zheng Qi must be deficient." Moreover, people today have particularly weak constitutions. Having fallen ill due to deficiency, how can they endure prolonged disease? Especially infants, whose tissues are tender and Qi-Blood incomplete. Recently, many children have suffered from malignant warm rashes. Those susceptible to evil already have deficient Zheng Qi. Once high fever occurs, the Yin aspect becomes dry and parched. If physicians recklessly administer wind-dispersing drugs, Ying Yin is further injured. Overuse of toxic-resolving drugs damages Middle Qi further. Or, due to fever, food intake is cut off, further depleting Yuan Qi. Initially, Stomach Qi is not yet exhausted, and hunger persists. Prolonged deficiency leads to loss of appetite. Thus, tangible diseases remain uncured while intangible Yuan Qi collapses first. Some cases involve fierce drug potency causing blood boiling and Ying Qi reversing to the surface, forming macules and papules. However, some have inherent Yang deficiency preventing ascent, so the head and face do not rise (rash fails to erupt); others have insufficient Middle Qi, so the rash appears and disappears quickly; some have prolonged heat injuring Yin, leading to chronic low-grade fever; some have Middle Qi exhaustion failing to transport, causing anorexia; some have Yang without Yin containment, leading to solitary Yang hyperactivity above (high fever, panting, thirst) and deficiency-cold below (diarrhea, cold feet). Zhang Jingyue often uses Quan Zhen Yi Qi Tang (Complete True One Qi Decoction). For those with dryness and parching, this provides moistening—nourishing Water is equivalent to inducing sweating. Moreover, with herbs that anchor Middle Qi, even if external evil exists, it cannot lurk internally. Once Middle Qi strengthens, pathogenic factors dissipate, food intake resumes, Jing Qi generates naturally, True Fire returns downward, Yin shadows resolve externally, Ying Yin moistens internally, and all meridians irrigate the exterior. Naturally, skin becomes lustrous, fever subsides, body cools, interior harmonizes, appetite returns, and healing occurs. Conditions manifesting from Qi and Blood require substances有情 (sentient/nourishing) to Qi and Blood to enter effectively. Such herbs possess the function of nurturing righteousness and enduring disease, which is essential for dispelling illness and preserving life. How can one use drugs hostile to Qi and Blood, aiding Wind and Fire, constantly opposing Zheng Qi, only to exacerbate various ailments and exhaust spirit? As the *Classic* says: "Causing evil to prevail and losing Zheng Qi cuts short human life," this is precisely what is meant.
