Nasal congestion with loss of smell is commonly called "Lung Cold," but treatment with acrid-warm exterior-releasing herbs often fails. This is actually due to latent fire pathogen in the Lung Meridian, causing congestion upon exposure to cold. The treatment principle focuses on clearing the Lung and descending fire, assisted by agents that unblock qi. If there is no history of nasal congestion, but it occurs suddenly due to wind-cold invasion, resulting in stuffiness, heavy voice, or clear rhinorrhea, treat it as Wind-Cold. However, in persons with Qi deficiency, weak Qi fails to ascend, leading to nasal stagnation. As stated, "obstruction of the nine orifices" arises from the intestines and stomach; frequent use of *Buzhong Yiqi Tang* (Tonify the Middle and Augment the Qi Decoction) will naturally restore patency.
The Lung opens into the nose. The Stomach Meridian of Foot-Yangming also runs alongside the nose up to the brain, which is the mansion of the Primordial Spirit. The nose is the aperture of the Gate of Life. When the Middle Qi is insufficient and Clear Yang fails to ascend, the head tilts and the nine orifices become dysfunctional. The Classics state: "Heavenly Qi connects with the Lung." If the intestines and stomach lack phlegm-fire and accumulated heat, what ascends normally is pure Qi. Thus, the Qi and Blood of the twelve meridians and 365 collaterals all ascend to the face and flow into the orifices. The Pectoral Qi emerges from the nose to perceive odors. It is said that Yang Qi and Pectoral Qi are both the generating Qi arising from the Stomach. If dietary irregularities or overwork damage the Spleen and Stomach, the generating Qi becomes weak, and transporting Qi cannot ascend. Consequently, pathogenic factors block the orifices, causing nasal obstruction and loss of smell. The treatment should nourish the Stomach and strengthen Ying Qi; when Yang Qi and Pectoral Qi ascend, patency is restored. However, the *Nan Jing* states: "The Heart governs the five odors, and the Lung governs all Qi." The nose is the aperture of the Lung. Why does it perceive odors? Structurally, it belongs to the Lung; functionally, it belongs to the Heart. If Defensive Qi fails to hold, and Cold Pathogen lodges in the head and face, affecting the nose, it loses its function, resulting in anosmia. The Classics say: "Five Qi enter the nose and are stored in the Heart and Lung. If the Heart and Lung have disease, the nose becomes obstructed." Clear vision and hearing bring coolness; distinguishing odors brings warmth. The treatment should first disperse Cold Pathogen, then tonify Stomach Qi, allowing Heart and Lung Qi to communicate, thereby restoring nasal patency and smell perception.
Continuous flow of turbid nasal discharge is called Bi Yuan (Sinusitis/Nasal Fistula). This results from Wind-Heat scorching the brain, causing fluids to seep downward; the discharge may be yellow, white, or bloody like pus. This is a symptom of Kidney Deficiency. The Classics state: "Brain fluid seeps out as mucus." Also: "Gallbladder transfers Heat to the Brain." The *Yuan Bing Shi* states: "Like fire smelting metal, extreme Heat transforms back into Water." Investigating the root, it is invariably Kidney Yin Deficiency failing to grasp Qi and return it to its source. Thus, Fire acts without restraint, pressing upward on Lung Metal. Consequently, the Qi of body fluids cannot descend but instead accumulates in the orifices, turning turbid into mucus, causing body fluids to flow in reverse. This further depletes the Liver and Kidney, creating a state of ascending only without descending, Yang without Yin. Yin Deficiency leads to disease; Yin exhaustion leads to death. One must strictly avoid anger to nourish Yang, abstain from desires to nourish Yin, stop eating fried/roasted foods, and avoid alcohol and wheat to prevent generating Heat. Then, relying on a skilled physician, use Kidney-Nourishing and Lung-Clearing herbs as Sovereign, Qi-Unblocking and Regulating herbs as Minister, and Yin-Nourishing and Blood-Tonifying herbs as Assistant. This extinguishes Fire, clears Metal, allows the Descending Function of the Kidney to operate, promotes smooth Qi flow, resolves stagnation, clears the orifices, balances Yin and Yang opening/closing, ensures mutual support, and enables organs to perform their storage functions. Naturally, the root is cultivated, and medication eliminates the disease. If one is arrogant, careless, or mistakenly uses cooling herbs, even Cang Gong or Bian Que cannot prolong life.
The nose is the gateway for respiration. Heat steaming externally causes Lung-Wind Red Nose (Rhynophyma), not solely due to alcohol consumption. Heat reaching internally causes nasal polyps and sores. Polyps arise from accumulated Heat in the Upper Jiao and stagnation.
A pale nose indicates blood loss. A red nose indicates Blood Heat. The face is Yang within Yang, and the nose sits in the center of the face. All blood in the body transported to the face and nose is extremely pure and refined. If blood is lost and cannot transport, the color becomes pale and dull. If Blood Heat boils, the color becomes distinctly red. The Lung is a high-positioned, fragile organ that fears Cold and dislikes Heat. Therefore, in those who drink heavily, alcohol fumes cause the nasal tip to redden; with more heat, it turns redder. Hot blood congeals when cooled, becoming stagnant and dark, resulting in a purple-black color. Treatment methods: For blood loss, warm and tonify; for Blood Heat, clear and drain; for Cold Congestion, resolve stasis and generate new tissue, using *Si Wu Tang* (Four Substances Decoction) plus Wine-Fried Huang Qin (Scutellariae Radix) and Wine-Soaked Hong Hua (Carthami Flos); for Qi weakness, add Wine-Soaked Huang Qi (Astragali Radix) to promote movement. The treatment for alcoholic rhinophyma follows the same principles. Long-standing cases with thick, foul-smelling nasal discharge are named Nao Lou (Brain Leakage). For Qi Deficiency, use *Buzhong Yiqi Tang* (Tonify the Middle and Augment the Qi Decoction). For Yin Deficiency, use *Mai Wei Di Huang Tang* (Lycium and Schisandra Rehmannia Pill).
