In jaundice diseases, some require clearing. This occurs in young adults with robust qi, where damp-heat is constrained and stagnant, urine is scanty, pulse is full and rapid. It is suitable to promote urination, using formulas like Yinchen Wuling San (Artemisia Capillaris and Five-Ingredient Poria Pill). Some require sweating. This is due to damp-heat constrained in the exterior. There must be body ache and aversion to cold. It is suitable for Guizhi Dahuang Tang (Cinnamon Twig and Rhubarb Decoction). Some require warming. This is due to cold-fluid retention in the spleen and stomach failing to transform. It is suitable for Yinchen Fuzi Ganjiang Tang (Artemisia Capillaris, Aconite, and Ginger Decoction).
The Classic states: The center corresponds to yellow, connecting to the Spleen. Yellow is the color of the Spleen. The spirit of the human body values concealment and silent operation; when revealed externally, the interior is deficient. Jaundice arises because Spleen Earth is deficient, losing its function of transformation and transportation, causing damp-heat to linger and turbid qi to stagnate. Thus, it is said: Jaundice need not be divided into five types; all are damp-heat, fundamentally rooted in Spleen deficiency, with damp-heat as the branch. In cases of long-standing illness with body ache, stiffness in shoulders/back, obstruction of both bowels and urine, yellowing of the whole body, face, and nails, and urine resembling dirty water, this is true Jaundice. If not post-illness, but sudden injury to Stomach Qi, presenting with pale yellow mouth odor and swollen eye sockets, this is Stomach Heat. If pale yellow hinders appetite, without thirst but with frequent diarrhea, this is Spleen-Stomach Weakness. If steaming fever, deep yellow color, abdominal distension, craving earth, thirst, and yellow urine, this is Childhood Malnutrition Heat (Gan Re). If whole-body ache and sallow complexion like smoke-stained yellow, this is Dampness Disease. If no body ache but orange-yellow color, this is Jaundice Disease. If caused by the mother's long-term malaria or maternal jaundice leading to neonatal jaundice, it is called Fetal Jaundice (Taihuang). If tidal fever with red and obstructed urine and stool, it is Yang Jaundice, caused by Spleen Earth clashing with Heart Fire. If Spleen-Kidney deficiency-cold with deep, thin pulse, cold body, spontaneous sweating, diarrhea, and white urine, it is Yin Jaundice, caused by Yang deficiency failing to transform yin-cold accumulation. Furthermore, Spleen weakness leading to sallow atrophy requires treatment prioritizing regulating the Spleen, followed by draining water. Adults often develop dampness from deficiency, which turns to heat over time, primarily involving Spleen and Kidney. Children often develop dampness from internal constraint of excess heat, primarily involving Lung and Stomach. When damp-heat combines to form jaundice, one must distinguish Yin/Yang and Deficiency/Excess to treat.
Excess-Heat presents with a surging and rapid pulse; if the pulse is faint and choppy, it belongs to Deficiency. Zhang Zhongjing stated: Yellowing with free urination should be treated similarly to Consumption (Xu Lao), suitable for Xiaojianzhong Tang (Minor Construct the Middle Decoction). Hai Zang (Wang Haigu) stated: Internal contraction of Cold Damage, physical exhaustion, and irregular diet cause the Middle Jiao to become cold and generate jaundice. It is not acquired from external pathogens; Lizhong Tang and Da/Xiao Jianzhong Tangs suffice. Yinchen is unnecessary. Since Spleen and Stomach are injured by damp-heat, Qi and Blood gradually weaken over time. Tonification is required, such as Renshen (Radix Ginseng) and Baizhu (Rhizoma Atractylodis Macrocephalae) to strengthen the Spleen, Danggui (Radix Angelicae Sinensis) to harmonize Blood, and Qinjiao (Radix Gentianae Macrophyllae) to scatter Dampness. When Righteous Qi is strong, Pathogenic factors recede naturally.
The Classic states: Red-yellow urine, quietude, and desire to lie down, with yellow eyes, indicate Jaundice. Feeling hungry after eating indicates Stomach Jaundice. The Classic mentions red urine and hunger after eating as signs of Heat. Quietude and lethargy are signs of Dampness. Yellow is the color of Central Wu-Ji, mostly belonging to Taiyin Spleen Earth. When the Spleen cannot overcome Dampness and is accompanied by Fire-Heat, stagnation produces yellow. Like fermenting flour, when Dampness and Heat clash, yellow forms. However, Dampness and Heat differ: Dampness-type jaundice is dark and dull; Heat-type jaundice is bright and lustrous. Sweat staining clothes, yellow as cypress juice, is named Huanghan (Yellow Sweat). Zhang Zhongjing said sweat bathing causes water to enter pores, combining Dampness and Heat. Injury from food is called Grain Jaundice (Gu Dan), often due to depression causing Grain Qi to stagnate and fail to circulate. Spleen is invaded by Liver Wood; food entering fails to nourish Stomach Yang and instead disturbs Spleen Yin. Stomach accumulates damp-heat, steaming it into corrupt turbid qi, damaging the Bladder downward. Thus, dizziness upon eating, urinary obstruction, and whole-body yellowing occur. After various blood-loss conditions, the face is often yellow because ruddy color comes from Blood nourishment. Loss of Blood reveals yellow, like leaves turning yellow in autumn/winter. Suitable for Yangrong Tang (Nourish Blood Decoction) and Shiquan Dabu Tang (Ten Complete Great Tonifying Decoction). Post-malaria yellowing is due to Spleen Yin deficiency revealing Earth color; regulating Spleen is primary. Whole-body yellowing excluding eyes/ears is different. Alcohol injury is named Wine Jaundice (Jiu Dan); alcohol is the peak of damp-heat. Long accumulation rises upward causing vomiting, nausea, and restlessness, with difficult urination and inability to eat. Female Labor Jaundice (Nv Lao Dan) presents with slight sweating, black forehead, hot palms/soles, evening fever, aversion to cold, urgent bladder but free urination. Some attribute it to intercourse followed by water exposure. Ministerial Fire flares up from Water, merging with Monarch Fire of Heart, smoke-color penetrating the forehead. Hot palms/soles are common in internal injury. At dusk, Yangming governs, closing and gathering damp-heat downward. Bladder becomes urgent. Lower abdomen full, free urination, black loose stool indicate Bladder Blood Stasis. Abdominal distension resembling water is actually Blood Stasis. If excessive thirst and abdominal distension, prognosis is poor. Treat with heat-clearing, diuretic herbs plus blood-invigorating stasis-resolving agents like Hupo (Fossilized Amber), Danpi (Cortex Moutan), Hongqu (Red Yeast Rice), Honghua (Flos Carthami), Tao Ren (Semen Persicae), Yanhu (Rhizoma Corydalis), Puhuang (Pollen Typhae), Wulingzhi (Faeces Trogopterori), Jiuzhi (Jui Juice).
Kidney deficiency failing to move water leads to floating dampness. Long stagnation generates heat; damp-heat steams upward, yellowing eyes and face. Treat with Jingui Shenqi Wan (Golden Cabinet Kidney Qi Pill) modified with Maidong (Rhizoma Ophiopogonis) as a decoction. Pulse at both Chi positions will be slow, weak, and powerless; leg soreness confirms this. Free white urine and deficiency-cold pulse indicate that only major warming tonification works. When Yang Qi spreads, Yin Jaundice retreats. Misusing cooling diuretics leads to water depletion, black face, and death.
Jaundice is an acute disease; Zhang Zhongjing says it heals in 18 days. Food/Labor/Gan Jaundice, commonly called "Yellow Puffiness," is chronic. Some persist long. Ancients used Zhen Sha (Needle Sand), Cu Fan (Vinegar Alum) to subdue Liver, and Zhu Mi Zao Rou (Atractylodes, Rice, Jujube Flesh) to assist Spleen. Suitable for robust individuals or laborers. For delicate constitutions, tonification serves as attack. Qinjiao (Radix Gentianae Macrophyllae) alone is excellent for all jaundices, driving damp-heat from Yangming channel outward. Its effect is achieved, nature is mild, not harsh. Do not recklessly use cooling drugs to further injure Spleen/Stomach. Obstructed urination and no sweating indicate Excess. Free urination and sweating indicate Deficiency. White unchanged urine indicates No Heat. Yellow-red urine indicates Damp-Heat. Young and robust with large pulse heals easily. Small pulse, free urination, no thirst indicates survival. Surging pulse, diarrhea, and thirst indicate death. No pulse near Cun Kou (wrist crease), cold nose/mouth, black discoloration indicates death. Thirsty jaundice is hard to treat; non-thirsty is easy. Jaundice toxin entering abdomen with wheezing/fullness indicates death. Elderly with Qi deficiency and faint pulse are hard to cure. Long-standing jaundice turning into Black Jaundice is fatal. For robust individuals, urgently crush Tudigua Gen (Radix Trichosanthis Kirilowii) for 6 liang juice, take at once. Yellow water exits via urine, and healing occurs.
