Liver Wind (caused by external wind invading due to Qi heat, or internal wind generated due to Qi warmth) manifests as fixed gaze (when heat is excessive, both eyes are tight and do not rotate. If fixed gaze is accompanied by bluish discoloration, convulsions will definitely occur; severe teeth grinding also indicates convulsions). The child performs actions such as picking at clothes and randomly grasping objects because the Liver governs planning and deliberation. If wind is severe causing body stiffness and opisthotonos, use Xie Qing Wan (Green Dragon Decoction); if the Heart is affected by wind-fire combusting leading to spasms, use Dao Chi San (Guide Out Red Powder); if accompanied by thirst and wheezing, use Xie Bai San (Drain White Powder).
If it is a deficiency pattern, it manifests as teeth grinding, yawning, and palpitations.
When the Heart is not invaded by heat, symptoms may include eyelid twitching without spasms, or spasms with weak intensity. In all these cases, one should tonify the Kidney and treat the Liver using Shen Qi Wan (Kidney Qi Pill). If excess pathogenic factors from the Heart invade the Liver, manifesting as high fever and spasms, use Li Jing Wan (Relieve Convulsion Pill) or Liang Jing Wan (Cool Convulsion Pill); if Lung pathogenic qi invades the Liver (Thief Pathogen), with strong qi manifesting as yawning and slight spasms, the treatment method should be to tonify the Liver with Shen Qi Wan and drain the Lung with Xie Bai San; if Spleen mild pathogen invades the Liver, manifesting as excessive sleepiness, heavy body, and spasms, first use Xie Qing Wan to stop spasms, then regulate according to the specific syndrome; if Kidney deficiency pathogen invades the Liver, manifesting as aversion to cold, yawning, and spasms, use Qiang Huo Gao (Notopterygium Paste). The term "Cheng" (invade/overcome) is like riding a vehicle. The interactions among the Five Zang organs are unpredictable. For example, in Liver disease, one must first treat the Lung and tonify the Kidney, then examine the Liver's deficiency or excess for regulation; other Zang organs follow this principle.
Heart惊 (Convulsion) manifests as difficulty speaking and a preference for sleeping face-down. Heat rises irritably, the tongue is stiff making speech difficult despite crying, chest heat causes a desire to lie face-down for coolness. Use Dan Xie Xin Tang (Single Drain Heart Decoction), Dao Chi San, or Xiao Sheng Xi San (Minor Rhinoceros Horn Powder).
If it is a deficiency pattern, it manifests as lethargy, lying down, and increased palpitations. Use Wen Jing Wan (Warm Convulsion Pill). Lung pathogen invading Heart manifests as wheezing and high fever; use Xie Bai San. Liver wind-heat invading Heart uses Da Qiang Huo Tang administered with Xie Qing Wan. Spleen pathogen invading Heart manifests as body heat; use Xie Huang San. Kidney pathogen invading Heart manifests as fear and aversion to cold; use Zhu Sha An Shen Wan (Cinnabar Calming Spirit Pill).
Spleen Kua (Fatigue) manifests as body heat, thirst, and anorexia.
If it is an excess pattern, it manifests as drowsiness without exposed pupils, body heat, thirst with desire to drink water, constipation or diarrhea with yellow-red stool color; use Xie Huang San.
If it is a deficiency pattern, it manifests as vomiting, diarrhea, generation of wind, and phlegm. Deficiency patterns show vomit with fishy smell, white diarrhea, excessive sleep with exposed pupils, gradual generation of wind in four limbs, possibly with phlegm. Use Qian Shi Bai Zhu San (Qian’s Atractylodes Macrocephala Powder), Yi Gong San (Benefit Work Powder), or Li Zhong Wan (Regulate Middle Pill). Liver pathogen invading Spleen manifests as wind-diarrhea and vomiting; use Er Chen Tang plus Huang Qin. Heart pathogen invading Spleen manifests as high fever, heavy body, and diarrhea; use Qiang Huo Huang Qin Cang Zhu Gan Cao Tang as the main formula. Lung pathogen invading Spleen manifests ability to eat but no bowel movement, vomiting, and cough; administer Ting Ni Wan (Descurainia and Coptis Pill) with decoction of Bing Lang and Da Huang. Kidney pathogen invading Spleen manifests as aversion to cold and diarrhea; use Li Zhong Wan.
Lung Dryness (Wheezing/Cough) manifests as dry nose and hand-picking at eyebrows, eyes, and face.
If it is an excess pattern, it manifests as wheezing with vigorous qi, or thirst; use Xie Bai San to moisten. Hand-picking at eyebrows, eyes, nose, and face is treated with Gan Jie Tang (Licorice and Platycodon Decoction). If Lung is only affected by Cold Damage, there is no chest fullness; if Lung heat is compounded by wind-cold, manifesting as chest fullness, shortness of breath, wheezing, and cough, use Xie Bai San or Da Qing Gao (Great Green Paste).
If it is a deficiency pattern (white lips), it manifests as shortness of breath and panting without aversion.
Qi choking, long exhalations, and shortness of breath: first take Yi Huang San, then E Jiao San. Heart pathogen invading Lung manifests as heat, wheezing, and cough; first take Shen Qi Wan, then Dao Chi San and E Jiao San. Liver pathogen invading Lung manifests as aversion to wind, dizziness, wheezing, and cough; use Ren Shen Qiang Huo San. Kidney pathogen invading Lung manifests as aversion to cold, clear profuse sputum cough; use Bai Bu Wan. Spleen pathogen invading Lung manifests as heavy body, phlegm-cough, and diarrhea; use Si Jun Zi Tang plus Huo Xiang, Gan Ge, Mu Xiang, and Gan Cao.
Kidney Cold manifests as aversion to light and fontanelle opening.
Kidney insufficiency is seen mainly in smallpox: Kidney excess manifests as black depression. Pediatric Kidney deficiency, due to insufficient prenatal qi, results in deficient spirit, prominent whites of the eyes, photophobia, open fontanelles, and pale complexion; these are difficult to nurture or may die young. Those who become deficient due to illness can be tonified.
Downward dragging, hot feet, fire wanting to flare up.
"Downward dragging" refers to heavy bones wanting to sink and curl up. Hot feet disliking clothing cover indicate Heart fire descending to the Kidney region; use Shen Qi Wan or Zheng Qi Tang. Heart pathogen invading Kidney manifests as internal heat without aversion to cold; use Gui Zhi Tang plus Huang Qin made into pills. Lung pathogen invading Kidney manifests as wheezing, cough, dry skin, and aversion to cold; use Bai Bu Wan. Liver pathogen invading Kidney manifests as tension, qi spasms, and cold body; use Li Zhong Wan. Spleen pathogen invading Kidney manifests as heavy body, diarrhea, and cold body; use Li Zhong Wan. Whenever the primary Zang organ is weak, it is subject to destruction by controlling elements; one must tonify the primary Zang's righteous qi. For instance, in Lung disease with cough, tonify Kidney in Spring, rescue Lung in Summer, drain Lung in Autumn, and tonify Heart in Winter. Draining the primary Zang is also called Cold Draining. Generally, when each Zang reaches its own position, its qi is prosperous and should not be further tonified; when reaching the initial controlling position, it should not be further drained. Furthermore, in severe Lung disease, seeing Liver deficiency signs makes it easy to treat, while seeing Liver heat signs makes it difficult to treat. This is because prolonged Lung disease leads to deficiency-cold, while strong Liver excess overcomes it. The Classic states: "Being controlled but unable to control is called True Strength." The method should be to first tonify Spleen and Lung, then drain Liver; if Lung is victorious, tonify Liver and drain Lung. However, in chronic cough leading to emaciation, Xie Bai San should not be used; Shen Qi Wan is suitable. Additionally, Liver disease appearing in Autumn indicates Liver overcoming Lung; tonify Lung and drain Liver. Mild cases recover, severe cases with lips white as dead bone die. Lung disease appearing in Spring indicates Lung overcoming Liver. Heart disease appearing in Winter indicates Heart overcoming Kidney. Kidney disease appearing in Summer indicates Kidney overcoming Heart. Spleen disease appearing in relation to the other four Zang organs: smooth progression is easy to treat, reverse progression is difficult. The pathology of the Five Zang organs does not depart from the principles of Five Elements generation and restriction. Thus, diseases of Zang-Fu deficiency/excess and mutual overcoming exist. Laypeople often fail to examine this principle, rashly diagnosing as external contraction or internal injury, leading to death from incorrect medication. This is why the contributions of Qian Yi and Jiegu (Zhang Yuansu) are great!
