Heart pain has traditionally been classified into nine types, while epigastric pain is located precisely at the heart's gate. Pain may arise from wind, heat, palpitations, cold, freezing, drinking, food retention, parasites, zhū disease (a chronic consumptive disease caused by evil spirits or parasites), and migratory pain (pain that comes and goes). Relief upon warming indicates a cold pattern; radiating pain to the front and back suggests long-standing qi stagnation. Blood stasis pain is accompanied by rebellious qi and audible gurgling sounds; phlegm obstruction pain presents with a slippery pulse and expectoration of turbid phlegm. Nausea and aversion to food result from dietary injury; frequent hunger is induced by stomach fire. Vomiting yellow fluid indicates roundworms (characterized by intermittent pain, with appetite returning when pain subsides); stifling pain relieved by vomiting indicates thick, stagnant phlegm. In the early stage due to cold, use warming and dispersing methods: dried ginger, pinellia rhizome, cyperi rhizome, amomum fruit, green tangerine peel, broad-leafed tangerine peel, and white cardamom seed. If the condition persists longer or arises from emotional depression transforming into fire, use fermented soybean mass, aurantium fruit, poria sclerotium, gardenia fruit, talcum, ligusticum chuanxiong rhizome, and paeonia lactiflora root. The principle is "blockage causes pain"; stagnation forms itself, but if unblocked, there is no harm.
Startled Heart Pain (palpitations and pain in the epigastrium after being startled by thunder or cannon fire; this is due to hyperactive liver yang failing to ascend and diffuse; treat by nourishing blood and pacifying the liver): Xiaoyao San (Free and Easy Wanderer Powder) minus bupleurum root, plus uncaria rhynchophylla and moutan bark.
Accumulated Fatigue Injury Heart Pain (fatigue injury leading to blood bi syndrome, severe pain causing fainting; treat by unblocking collaterals and harmonizing nutritive qi): fresh deer antler, cassia twig, pinellia rhizome, angelica sinensis rootlet, persica seed, administered with ginger juice.
Spleen Cold Jue Pain (saliva spitting, cold limbs; pathology in the collaterals; treat with pungent-aromatic opening methods): highland ginger, turmeric rhizome, black cardamom, raw atractylodes macrocephala rhizome, clove bud, and zanthoxylum stem.
Heart Labor Injury Pain (pain relieved by firm pressure; aggressive purging is difficult to apply; treat with the excellent method of pungent-sweet transforming yang): ginseng root, sichuan pepper pericarp, white honey, cinnamon twig, roasted licorice root.
Mr. Chen says: For severe cold heart pain, use fried dried ginger and cassia bark; for severe heat, use stir-fried coptis chinensis rhizome and bamboo shavings. For blood stasis, use persica seed paste, cyperi rhizome, tuckahoe, and angelica sinensis rootlet; for phlegm-fluid retention, use prepared arisaema cum bile and trichosanthes fruit; for parasitic jue, use chili pepper seeds, dried plum, and quinoa. If it is true heart pain, with cyanosis of all ten fingernails, death occurs in the evening if onset is in the morning, or danger in the evening if onset is in the morning; this is incurable.
Abdominal pain often uses paeonia lactiflora root and licorice root; one must be familiar with the earth-transforming formulas from the Jia Yi Jing. Atractylodes macrocephala rhizome, zanthoxylum schinifolium fruit, atractylodes lancea rhizome, poria sclerotium, cyperi rhizome, aurantium fruit, angelica dahurica root are suitable for reference alongside heart pain treatments. Deficiency patterns show pain relief upon manual pressure and a soft abdomen; inability to tolerate touch indicates excess patterns. Dull, continuous cold pain with a cold lower abdomen indicates cold; intermittent pain indicates heat. Fixed pain indicates blood stasis accumulation; wandering pain indicates qi blockage. Phlegm-dampness presents with a slippery pulse and constipation; anger-induced pain involves liver injury and connects to both hypochondriac regions. Blood deficiency causes lingering pain and tendon spasms; qi deficiency causes shortness of breath. Pain relief after diarrhea indicates food retention; drying dampness and guiding out stagnation with decoction is the priority. For summerheat-induced vomiting and diarrhea, use elsholtzia herb; for dampness injury, use wood tongue and atractylodes macrocephala rhizome.
Upper and Middle Jiao Qi Obstruction Abdominal Pain (vomiting, rapid and choppy pulse): pinellia rhizome, white cardamom seed, gardenia fruit, fermented soybean mass, broad-leafed tangerine peel, platycodon root.
Yang Qi Non-circulation Abdominal Pain (accompanied by cold lower back pain, worsening significantly): cinnamon twig, cyperi rhizome, fennel fruit, mugwort wool, green tangerine peel, white poria sclerotium.
Depression Injuring Spleen Yang Causing Pain: pinellia rhizome, cyperi rhizome, fresh ginger, perilla stem, zanthoxylum schinifolium fruit, melia toosendan fruit, black cardamom.
Filth-Turbidity Blocking Qi Abdominal Pain (use aromatic dispelling filth method): agastache rugosa, radish seed, zanthoxylum schinifolium fruit, pinellia rhizome, broad-leafed tangerine peel, white apricot seed.
Yin Turbidity Internal Obstruction, Fu Yang Blockage (use unblocking yang and draining turbidity method): raw processed atractylodes macrocephala rhizome, aconite lateral root, poria sclerotium, fennel fruit, processed zanthoxylum schinifolium fruit, mild evodia fruit, highland ginger, pinellia rhizome, raw alpinia oxyphylla fruit, ginger juice.
Liver Qi Stagnation Abdominal Pain: Xiaoyao San (Free and Easy Wanderer Powder) minus atractylodes macrocephala rhizome, plus curcuma aromatica rhizome and cyperi rhizome.
Long-standing Depression with Blood Stasis, Gui Water Irregularity, Formless Pain: cassia bark, cyperi rhizome, evodia fruit, costus root, angelica sinensis root, ligusticum chuanxiong rhizome, tuckahoe, paeonia lactiflora root.
Depression and Anger with Fluid-Qi Entering Collaterals: prepared arisaema cum bile, oyster shell, cinnamon twig, melia toosendan fruit, mandarin orange seed, east-growing pear tree root bark.
Summerheat Injuring Middle Qi Causing Pain: ginseng root, broad-leafed tangerine peel, alpinia oxyphylla fruit, rice sprout, paeonia lactiflora root, poria sclerotium.
Depression Injuring Liver and Spleen, Collateral Blood Stasis Coagulation (treat by disseminating and reaching nutritive collaterals): persica seed, old garlic bulb, angelica sinensis rootlet, cassia twig, pangolin scales, asafoetida pill, angelica sinensis root, paeonia lactiflora root, licorice root, processed rhubarb root, aurantium fruit, cinnamon twig.
Labor Injury Middle Yang, Abdominal Pain with Edema, Worsening Pain After Eating: angelica sinensis root, alpinia oxyphylla fruit, baked ginger, jujube flesh, paeonia lactiflora root, broad-leafed tangerine peel, roasted licorice root.
Mr. Chen says: For deficiency-cold in the Ying level with umbilical pain, occurring in winter and healing in spring, add cassia bark and poria sclerotium.
