Yi Lao said: Liange San removes Mirabilite and Rhubarb, adds Platycodon, acting as a boat/oar agent to float upwards, treating heat in the diaphragm and Six Channels. Since the Qi of Hand and Foot Shaoyang descends to the diaphragm, and the Qi of the Sanjiao travels with Ministerial Fire on the body surface, the diaphragm and Six Channels are at the highest level. This drug floats to the highest level, thus removing heat from the diaphragm and Six Channels invisibly.

Wuling San (Five-Ling Powder) (Zhongjing Fang): Treats cold damage with irritability, difficult urination with thirst, or cholera with incessant vomiting and diarrhea. Dongyuan said: Wuling San is the descending method for Taiyang syndrome.
Since Taiyang is high, sweat to disperse; low, guide to exhaust. Thirst indicates pathogen entering the root of Taiyang, should be descended to exit via Bladder. Kidney dryness, Bladder heat, difficult urination are treated by this. Not suitable if urination is free. However, in Taiyang disease with heat and thirst, even if urination is free, this descending method is suitable. Failure to take it leads to grain consumption, water loss, form destruction, turning to Yangming dry fire, Wei organ yellowing, due to failure to descend Taiyang into its root.
Alisma (Zexie, 1.5 liang) Atractodes Macrocephala (Baizhu, 1 liang) Red Poria (Chifuling) Polyporus (Zhuling, 1 liang each) Cinnamon Bark (Rougui, 5 qian)
Grind into fine powder. Take 3 qian per dose, mixed with warm water or clear rice drink. Or finely cut, add ginger and jujube, decoct. For severe heat, remove Cinnamon, add Scutellaria. If alternating chills/fever and spontaneous diarrhea occur on day 3-4 of cold damage, pathogen enters Taiyin while Shaoyang disease remains, combine this formula with Xiao Chaihu Tang, named Chai Ling Tang, add ginger and jujube, decoct, to separate and benefit Yin and Yang.
Dongyuan's "Ci Shi Nan Zhi" says: The classic states: Those with sweat must not take Ephedra; those without sweat must not take Cinnamon Twig. Mistaken use leads to countless changes. Thus, this law was established to avoid Three Yang taboos, a divine formula for resolution, Yi Lao's method, named Jiwei Qianghuo Tang.
Jiwei Qianghuo Tang (Nine-Ingredient Notopterygium Decoction) (Dongyuan)
Notopterygium (Qianghuo, treats Taiyang limb/joint pain, is the sovereign drug, but not leaderless; it is the leader of restoring order, penetrating all, removing joint pain) Saposhnikovia (Fangfeng, treats whole-body pain, like a subordinate soldier following orders) Atractodes Lancea (Cangzhu, has strong ascending qi, removes dampness, calms Taiyin, prevents pathogen transmission to Foot-Taiyin Spleen) Asarum (Xixin, treats Shaoyin Kidney headache) Ligusticum (Chuanxiong, treats Jueyin headache in brain) Angelica Dahurica (Baizhi, treats Yangming headache in forehead) Rehmannia (Shengdi, treats Shaoyin Heart heat inside) Scutellaria (Huangqin, treats Taiyin Lung heat in chest) Licorice (Gancao, relieves internal urgency, harmonizes drugs)
Although these nine ingredients form one formula, do not adhere rigidly. Rigidity without flexibility is like rigidity alone. Observe differences in meridians (front/back/left/right), adjust dosage based on severity, size, and quantity. Effects are miraculous. Finely cut, decoct in water. For urgent sweating, take hot with gruel. For mild sweating, take warm without soup. If pulse is floating and unresolved, start urgent then mild. If pulse is deep and unresolved, start mild then urgent. This drug treats miscellaneous diseases effectively besides cold damage. For stroke moving through meridians, add Aconite. For stroke with constipation, add Rhubarb. For stroke combining three pathogens into Bi syndrome, adjust tonifying/reducing based on Twelve Meridians, interior/exterior, cold/heat/warm/cool, four seasons, and six qi.
Dongyuan said: The classic states: Dual infection with cold is fatal and untreatable. Day 1: Taiyang and Shaoyin both diseased, headache, fever, aversion to cold, dry mouth, irritability, fullness, thirst. Taiyang is Fu, enters from back, known to all; Shaoyin is Zang, enters from nose breath, unknown to all. Nose connects to Heaven, so formless cold qi enters from nose. Kidney is Water, water flows to dampness, so Kidney receives it. Classic says: Injury by dampness affects lower part first. Like attracts like. Also: Heavenly pathogenic qi harms Five Zang organs. Thus, internal/external dual infection means Zang-Fu both diseased. Wanting to release exterior involves interior; wanting to purge interior involves exterior. Since exterior/interior cannot be treated simultaneously, death ensues. However, constitution varies (deficient/excess), pathogen depth varies. Deep infection in deficient constitution is fatal; shallow infection in excess constitution may be treatable. Some treated fail to recover, none recover without treatment. I have used this formula, some survive, 2-3 out of 10. Thus, I established this formula for benevolent gentlemen, named Jieliang Lianggan Shenfang Da Qianghuo Tang.
Jieliang Lianggan Shenfang Da Qianghuo Tang (Yi Lao Fang)
Saposhnikovia (Fangfeng) Notopterygium (Qianghuo) Angelica Pubescens (Duhuo) Tetrapanax (Fangji) Scutellaria (Huangqin) Coptis (Huanglian) Atractodes Lancea (Cangzhu) Atractodes Macrocephala (Baizhu) Licorice (Gancao, roasted) Asarum (Xixin, 3 fen each) Anemarrhena (Zhimu) Ligusticum (Chuanxiong) Rehmannia (Dihuang, 1 qian each)
Finely cut the herbs as one dose, add two bowls of water, decoct until one and a half bowls remain, strain the dregs and drink hot. If not resolved, take 3-4 more doses, stop when cured. If residual symptoms exist, follow Zhongjing's methods according to syndrome.
Liushen Tongjie San (Six-Spirit Unblocking Release Decoction) (Zihehe): Treats summer cold damage affecting Taiyang and Yangming channels, anhidrosis, head/neck pain, stiff waist/spine, eye pain, dry nose, inability to sleep. Replaces Mahuang Ge Gen Tang for releasing exterior.
Atractodes Lancea (Cangzhu, 3 qian) Gypsum (Shigao) Talc (Huashi) Scutellaria (Huangqin, 1 qian 5 fen each) Ephedra (Mahuang, 0.75 fen) Licorice (Gancao, 5 fen)
Finely cut the herbs as one dose, add two bowls of water, decoct until 0.7 parts remain and take. In spring, add Saposhnikovia (1 qian).
Liu Hejian Shouzhen's Key Points from "Zhi Shang Zhi Ge" on Treating Cold Damage:
Cold damage within first three days is in the exterior, method requires sweating. Shuangjie San can be taken continuously for several doses to cure.
Shuangjie San (Below are Hejian formulas, i.e., the next two.)
Fangfeng Tongsheng San (See Stroke Section.)
Yiyuan San Also known as Liuyi San, also known as Tianshui San. (See Dysentery Section.)
Take these two medicines together, expect sweating and resolution. If not resolved, disease has transmitted.
Cold damage after three days is in the interior, method requires purging. If purged too early, exterior pathogen enters interior via deficiency, forming Jie Xiong, empty stagnation, mental restlessness, macules/rashes, yellowing. Mild cases become critical, critical cases die. Use mild medicines to disperse exterior and harmonize interior. Depending on sweating/no sweating, persistent fever, use Xiao Chaihu, Liange, Tianshui combined.
If disease is half-exterior half-interior, method requires harmonizing. Xiao Chaihu and Liange govern.
If interior has slight heat, slight purging is needed. Dachaihu combined with Jiedu Tang governs. If heat persists, Dachaihu combined with Sanyi Chengqi purges.
Sanyi Chengqi Tang
Rhubarb (Dahuang) Mirabilite (Mangxiao) Magnolia Bark (Nongpu) Immature Bitter Orange (Zhishi, 5 fen each) Licorice (Gancao, 1 qian)
Finely cut the herbs as one dose, add one large bowl of water and three slices of ginger, decoct until 0.7 parts remain, take hot.
For chest/diaphragm fullness, panting, vomiting, or tight/surging Yang pulse, use Genti Discharge Powder (Gua Di San) to induce vomiting.
After sweating, vomiting, and purging methods, if no other symptoms, regulate with Liange San.
If major heat is gone but slight heat remains, take Yiyuan San to prevent recurrence. These are the main principles of cold damage treatment.
For wind injury with spontaneous sweating and floating/slow pulse, use Shuangjie San minus Ephedra to resolve.
For half-exterior half-interior disease, Baihu Tang harmonizes.
