Xionghuang Shengbingzi (Realgar Holy Cake)
Crotonis Fructus (Babao, 巴豆) [100 seeds, oil membrane removed], Realgar (Banliang, 雄黄) [half liang], White Flour (Shiliang, 十两) [ten liang, stir-fried and sifted]
Take the above three ingredients. Excluding the white flour, grind the remaining ingredients into a fine powder together. Mix evenly with the white flour. Knead with fresh water to form cakes the size of a hand. Boil again in water until they float on the surface. Adjust hardness by kneading into cake shapes. Take five to seven cakes per dose, increasing gradually to ten or fifteen cakes. Chew thoroughly before swallowing. One cake induces one bowel movement; two cakes induce two bowel movements. Administer before meals with tea or wine as desired.
[Li Gao] Human life relies on the essence of the five grains, which transforms the five flavors to complete the physical form, thereby enabling the generation of the body. The Classics state: "Flavors nourish the form." If flavors are excessive, they can also damage the form. Currently, irregular diet leads to the inability of the intestines and stomach to bear it, and insufficient Qi to transform it, resulting in injury. The Classics say: "Strong Fire consumes Qi, Weak Fire generates Qi; Strong Fire scatters Qi, Weak Fire grows Qi." It also states: "If food and drink exceed double the normal amount, the intestines and stomach will be injured." Due to the loss of regulation according to the four seasons, disease arises, and the pulse manifests at the Qi Kou (radial artery). The Classics state: "A full pulse at the Qi Kou indicates injury from food; the heart and stomach feel full, and the mouth lacks taste." The mouth corresponds to the Qi Kou; the mouth represents Kun Earth. The mouth is the external indicator of the Spleen; thus, when the Stomach is injured, the Qi Kou pulse becomes tight and full. Injuries vary in degree and severity. For instance, if the Qi Kou pulse is one-fold stronger than normal, with six beats per respiration cycle, the injury is in the Jueyin (Liver/Gallbladder channel), representing a mild injury, treated with formulas like Zhizhu Wan (Immature Bitter Orange and Atractylodes Pill). If the Qi Kou pulse is two-fold stronger, with seven beats, the injury is in the Shaoyin (Heart/Kidney channel), representing a severe injury, treated with Xionghuang Shengbingzi, Muxiang Binglang Wan (Costus and Betel Nut Pill), or Zhike Wan (Citrus Aurantium Pill). If the Qi Kou pulse is three-fold stronger, with eight to nine beats, the injury is in the Taiyin (Spleen/Lung channel), characterized by chest filling and mental confusion, severe pain in the heart and stomach, treated with Beijji Wan (Emergency Rescue Pill), Shenbao Wan (Divine Treasure Pill), or Xiaoji Wan (Eliminate Accumulation Pill). Persistent nausea and urge to vomit, commonly known as "Food Dizziness Wind," occurs. The Classics state: "If there is a pulse in the upper part but none in the lower part, the person should vomit; if they do not vomit, death ensues." This is treated with Gouti San (Melon Pedicle Powder). If vomiting cannot be induced, there is no treatment. The Classics state: "For pathogens located high, use methods to cause upward expulsion." This refers to this principle. Using Beijji Wan to purge downward is also acceptable.
Binglang Wan treats excessive food intake causing abdominal distension and oppression.
Arecae Semen (Binglang, 槟榔) [2.5 qian], Citri Reticulatae Pericarpium (Chenpi, 陈皮), Pharbitidis Semen (Qianniu, 牵牛) [each 1 liang], Costi Radix (Muxiang, 木香) [2.5 qian], Immature Bitter Orange (Zhishi, 枳实) [stir-fried, 1 liang]
Grind into powder. Make pills using vinegar paste. Take 30-40 pills with Ginger Decoction.
Zhu Huang Wan treats excessive food intake causing abdominal distension and fullness, qi moving in the hypochondrium, and stabbing pain, with miraculous efficacy.
Realgar (ground, 1 liang), Crotonis Fructus (Babao, 巴豆) [5 qian, skin and heart removed, ground into paste]
Add 2 liang of white flour. Grind uniformly. Add drops of water to make pills the size of paulownia seeds. Boil 12 pills in hot rice gruel. Filter and place in cold rice gruel until submerged and cooled. When used, take one pill with the cold rice gruel used for soaking. Consume all 12 pills within 12 hours of the day. Stop when mild diarrhea occurs; it is not necessary to finish the entire dose.
[Zhang Zhongjing] Retained food in the Upper Jiao should be expelled via vomiting, suitable for Gouti San. Also states: "Those with Blood deficiency must not be vomited."
[Li Dongyuan] Gouti San is used for extreme fullness and excess, rebellious Qi rushing upward, presence of pulse in the upper part but absence in the lower part, and chest filling with mental confusion. If pulses at Cun, Guan, and Chi positions are all full, Beijji Wan should be used.
Melon Pedicle (Gouti, 瓜蒂) [stir-fried], Adzuki Bean (Chixiaodou, 赤小豆) [boiled, equal parts]
Grind into fine powder. Take 2 qian per dose, mixed with warm sour plum water. Induce vomiting as the limit. Zhang Zhongjing used Fermented Soybean (Xiangchi, 香豉) [7 he] boiled to extract juice, mixed with one bi of powder for administration. If the Chi pulses at both wrists are not absent, this medicine should not be taken immediately, as it may damage Yuan Qi and prevent Stomach Qi from recovering. If there is only chest obstruction and mental confusion without blockage, induce vomiting mechanically. If mechanical induction fails, then use this formula to induce vomiting. Some ask: "When food fills the chest causing痞 (blockage) and confusion, the Cun pulses should be active, yet the Chi pulses are invisible. What is the rationale?" The answer is: Food retention in the chest indicates Liver Wood stagnation that needs to be reached, hence the method of inducing vomiting. Food is an object; objects belong to Kun Earth, which is the designation of the Foot Taiyin Spleen. The chest is the domain of the Lung, Hand Taiyin Metal. Metal governs killing and pruning. With Kun Earth and Metal both residing in the upper part and flourishing in Heaven, Metal overcomes Wood. Thus, the generating Qi of Liver Wood is suppressed underground. Is this not Wood stagnation? Expelling the Yin Earth objects from the Upper Jiao allows Wood Qi to flow freely, resolving the stagnation. Food blocking the upper part causes pulse absence in the lower part. Without understanding the Dao of Heaven and Earth, one cannot comprehend this profound truth. Water and Fire are manifestations of Yin and Yang, and alternative names for Heaven and Earth. Therefore, solitary Yang does not generate, and solitary Yin does not grow. The function of Heaven lies beneath the earth, allowing all things to grow; the function of Earth lies above in heaven, allowing all things to store. This is Heaven and Earth interacting so that all things flow smoothly; this is the way Heaven and Earth root each other. Thus, the root of Yang Fire is in the earth, and the source of Yin Water is in the heaven; hence it is said, "Water flows from a high source." The five Zang organs manage有形 (form/substance) objects. Objects are Yin. Yin is Water. The right-side pulses (Cun, Guan, Chi on the left wrist in some contexts, or generally referring to specific pulse diagnoses) govern this, manifesting predominantly at the Qi Kou. Food blocking the upper part cuts off the source of the five Zang organs. When the source is cut, water does not flow down, leading to the absence of Chi pulses. This is the logic. Why doubt it? However, one must observe whether the ingested food was cold or hot and adjust accordingly. If one part cold food and two parts hot food were consumed, use two parts cold-natured medicine and one part hot-natured medicine, adjusting according to circumstances. The Classics state: "First consider the annual Qi, do not violate the Harmony of Heaven." This is what is meant.
[Wang Haogu] Qin Sheng liked to consume Sansheng Tea and cold foods, leading to accumulation and entrenched Cold. The pulse was neither floating nor sinking; pressing up and down, inside and out, it was extremely forceful, hard and unyielding, protruding through the skin upon touch. Its rhythm could not be counted by beats, and its shape could not be described by magnitude. This is the Drumming Pulse of Yin Syndrome. The fire circulating throughout the body gathered in the chest, forced by Cold Qi, resulting in strong pulsation. Prescribe Zhenwu Tang (True Warrior Decoction), Sini Tang (Reverse the Extremity Decoction), Lizhong Tang (Regulate the Middle Decoction), etc., assisted by Paeoniae Radix (Shaoyao, 芍药) and Fennel (Huixiang, 茴香) to prevent the medicine from soaring upward. Take 100 pills daily and 80 at night. Recovery occurred with sweating at night.
[Li Gao] Since there are methods for purging the Three Yin channels, there must also be methods for tonifying the Three Yin channels. Hence I stated: Internal injury to the Three Yin channels can be treated with Warm agents. If internal injury is caused by drinking cold liquids, although it damages the Stomach, one must determine which channel the color and pulse indicate. If the complexion is green-black, pulse varies in depth, wiry and weak, the injury is in Jueyin. If the complexion is red, pulse varies in depth, fine and faint, the injury is in Shaoyin. If the complexion is yellow and clear, pulse varies in depth, slow and delayed, the injury is in Taiyin. If injured in the Jueyin Liver channel, treat with Danggui Sini Tang (Angelica Decoction for Nourishing the Extremities) added with Wuzhuyu Shengjiang Tang (Evodia and Ginger Decoction). (Formula see Shanghan Treatise.) If injured in the Shaoyin Kidney channel, administer Tongmai Sini Tang (Unblock the Extremities Reverse the Extremity Decoction). (Formula see Shanghan Treatise.) If injured in the Taiyin Spleen channel, treat with Lizhong Wan/Tang (Regulate the Middle Pill/Decoction). For soft stools, use Decoction; for constipation, use Pill. (Formula see Shanghan Treatise.)
[Wang Haogu] Tang Chen had a craving for cold food, eventually developing Yin Syndrome. The pulse was slow and choppy, stopping every 7-8 beats, later slowing to 3 beats. I urgently administered Warm agents several times, but no relief after 4-5 days. Continued with three more doses. On the next day, the pulse returned, and recovery followed profuse sweating.
[Luo Tianyi] Yin Qi: when still, the Spirit is stored; when restless, it dissipates. If food and drink exceed double the normal amount, the intestines and stomach are injured. This means one should not greedily consume large quantities of food, but value moderation, thereby preserving the Harmonious Qi and nurturing life. If one seeks satiety to excess, becoming stuffed and unable to digest, one merely accumulates hidden damage, inviting illness. Eating too much consumes Qi in multiple ways: food may rise and vomit, draining True Yuan; undigested drinks may transform into Phlegm, consuming Shen Water through spitting; frequent loose stools drain the transformation of Grain Qi; slippery urination depletes the infiltration of the Source. Eventually, clear cold Essence leaks downward, and sweat pours outward. All stem from excessive food injury and overly rich flavors. If one moderates satisfying meals and reduces palatable tastes, one will not reach excessive fullness. Then, every meal will cause no harm, and every item will be beneficial. Jin Ye (Body Fluids) will accumulate internally, Jing Hua (Essence and Brilliance) will harmonize and condense, Evil Toxins cannot invade, and diseases will have no origin.
