[Ben Cao] For women suffering from food stagnation during pregnancy, Muxiang Wan (Aucklandia Pill) is appropriate.
Muxiang (2 qian), Sanleng, Baifuling, Renshen (each 3 qian).
Grind the above into powder, make pills with wheat paste. Take 30 pills each time, swallowed with warm water.
For qi disharmony during pregnancy, leading to loss of taste in food, Baizhu San (Atractylodes Powder) is appropriate.
Baizhu, Zisu (each 1 liang), Baizhi, Renshen, Hezipi, Chuanxiong, Qingpi (each 0.5 liang), Gancao (1 qian).
Grind the above into powder. Take 2 qian each time, with one cup of water and three slices of ginger, decoct until 70% remains, take warm at any time. For pregnancy-related food stagnation, refer to the mild formulas for fluid retention mentioned earlier for reference.
The classics state: "If food intake doubles, the intestines and stomach are injured." It also states: "The generation of Yin originates from the five flavors. The five organs governing Yin are damaged by the five flavors." If pregnant women do not regulate their diet and indulge in cold, raw, or toxic foods, it leads to spleen and stomach diseases. Therefore, for pregnancy-related food stagnation, obtaining therapeutic efficacy is difficult; only these two formulas are stable and convenient.
[Zhang Zhongjing] There is Tan-Yin (Phlegm-Fluid), Xuan-Yin (Hanging Fluid), Yi-Yin (Overflowing Fluid), and Zhi-Yin (Branching Fluid). A person who was previously robust but is now emaciated, with water flowing between the intestines making a gurgling sound, is called Tan-Yin. Patients with Tan-Yin should be treated with warm herbs to harmonize. (It is also said: For those with phlegm-fluid below the heart, Linggui Gan Zhu Tang (Poria, Cinnamon Twig, Licorice, and Atractylodes Decoction) is the primary treatment.) When drinking water, if the fluid flows into the hypochondrium causing pain upon coughing and spitting, it is called Xuan-Yin. Patients with Xuan-Yin are treated with Shizao Tang (Ten Jujube Decoction). (Formula seen in Shanghan Lun.) For Liu-Yin (Retained Fluid), there is pain in the hypochondrium radiating to the Dazhui point (Qie Ren), worsening with coughing. If there is retained fluid in the chest, the patient suffers from shortness of breath and thirst, pain in all joints of the four limbs, and a deep pulse indicates retained fluid. When drinking water flows to the four limbs, sweating should occur but does not, resulting in body pain and heaviness, it is called Yi-Yin. Patients with Yi-Yin should be induced to sweat, treated primarily with Da Qing Long Tang (Major Green Dragon Decoction) or Xiao Qing Long Tang (Minor Green Dragon Decoction). (Both formulas are seen in Shanghan Lun.) Coughing with inability to lie flat, shortness of breath, and swelling appearance is called Zhi-Yin. For chronic cough lasting years, a weak pulse indicates curability; a solid, large, and rapid pulse indicates death. A rapid pulse implies severe dizziness due to pre-existing Zhi-Yin in the chest; treatment falls under the category of fluid disorders. Zhi-Yin also causes wheezing and inability to lie flat with shortness of breath; if the pulse is normal, it is manageable.
Water in the Heart causes hardness and palpitations below the heart, shortness of breath, aversion to water, and lack of desire to drink. Water in the Lung causes salivation, desire to drink water. Water in the Spleen causes Qi deficiency and heavy body. Water in the Liver causes fullness in the hypochondrium and pain upon sneezing. Water in the Kidney causes palpitations below the heart.
Vomiting is caused by fluid accumulation in the chest and hypochondrium. Zhang Zhongjing says: For vomiting with epigastric stuffiness, there is water in the chest, treat with Banxia Fuling Tang (Pinellia and Poria Decoction). It is also said: Vomiting patients usually feel thirsty; if they are not thirsty, it is due to Zhi-Yin below the heart, treat with Xiao Banxia Tang (Minor Pinellia Decoction). It is also said: Thirst followed by vomiting indicates water retention below the heart, treat with Xiao Banxia Jia Fuling Tang (Minor Pinellia Plus Poria Decoction). Luo Qianfu says: Phlegm-fluid disorders may present as vomiting or nausea, dizziness or palpitations, discomfort in half the body, or alternating chills and fever; all are treated with Er Chen Tang (Two-Cured Decoction). (Formula seen in Danxi's Phlegm section.) Dizziness and palpitations are signs of fluid. Zhang Zhongjing says: Sudden vomiting, epigastric stuffiness, dizziness, and palpitations indicate water between the diaphragm, treat with Banxia Jia Fuling Tang (Pinellia Plus Poria Decoction). (Formula seen in Vomiting section.) It is also said: In thin people, palpitations below the navel, salivation, and vertigo indicate water, treat with Wuling San (Five-Ingredient Powder with Poria). (Formula seen in Vomiting section.) It is also said: Chest fullness with dizziness is treated with Linggui Zhugan Tang (Poria, Cinnamon Twig, Atractylodes, and Licorice Decoction). (Formula seen in Shortness of Breath section.) It is also said: Zhi-Yin below the heart causing severe dizziness is treated with Zexie Tang (Alisma Decoction). (Formula seen in Dizziness section.)
[Zhang Zhongjing] Phlegm disease above the diaphragm presents with fullness, wheezing, coughing, and expectoration. Attacks involve chills, fever, back pain, and waist pain, spontaneous tearing, and severe body tremors, indicating Fu-Yin (Hidden Fluid). (Treatment seen in Tremor section.) A large pulse with retained fluid below the heart causes coldness in the back. Intermittent arm pain with a deep and fine pulse, not wind, indicates fluid in the Upper Jiao. A deficient pulse indicates slight labor and disharmony of Ying and Wei Qi, which will heal naturally over time. (Deficient pulse: use Fuling Wan; Non-deficient pulse: use Kongyan Dan.)
Chest fullness is due to fluid. Zhang Zhongjing says: Zhi-Yin with chest fullness is treated with Nongpo Dahuang Tang (Magnolia Bark and Rhubarb Decoction). (Formula seen in Fullness section.) It is also said: Phlegm-fluid below the heart causing chest and hypochondriac fullness and dizziness is treated with Linggui Zhugan Tang. (Formula seen in Shortness of Breath section.)
Thirst and dryness are generated by fluid in the intestines and stomach. Zhang Zhongjing says: Thirst with ineffective urination is treated with Wuling San. It is also said: Thirst followed by vomiting indicates water retention below the heart, treated with Xiao Banxia Jia Fuling Tang. It is also said: Abdominal fullness with dry mouth and tongue indicates water gas in the intestines, treated with Fangjiao Lihuang Wan. However, symptoms vary depending on where the water resides.
Spontaneous diarrhea involves fluid, so Danxi treats chronic dysentery by inducing vomiting to remove sticky phlegm for self-healing. Zhang Zhongjing says: If the pulse is hidden, the patient desires spontaneous diarrhea, feels relief after defecation, but the epigastrium remains hard and full, this is retained fluid attempting to exit, treated with Gansui Banxia Tang. (Formula seen in Diarrhea section.) It is also said: Ineffective urination, heavy painful limbs, and spontaneous diarrhea indicate water Qi, treat with Zhenwu Tang removing Shaoyao and adding Ganjiang. For prolonged Shanghan with unresolved exterior syndrome, vomiting, coughing, and spontaneous diarrhea indicate water Qi, treat with Xiao Qing Long Tang removing Mahuang and adding Yuanhua. Rough skin on the Chi position like dried fish scales is a sign of fluid retention. (Full text seen in Diagnosis section.) Bright complexion indicates retained fluid.
[Su Wen] A liver pulse that is wiry and scattered, with lustrous color, indicates Yi-Yin (Overflowing Fluid). Yi-Yin presents with sudden thirst and excessive drinking, invading muscles, skin, and outside the intestines and stomach. (Jingwei Lun) A very choppy liver pulse indicates Yi-Yin. (Full text seen in Treating Deficiency and Excess section.)
[Zhang Zhongjing] Bilateral wiry pulses indicate fluid. This often occurs after severe purgation leading to deficiency. Unilateral wiry pulse indicates fluid. Lung fluid is not wiry but causes distress with wheezing and shortness of breath. Wiry and rapid pulse indicates cold fluid, difficult to treat in winter and summer. Deep and wiry pulse indicates internal pain from Xuan-Yin. Floating, fine, and slippery pulse indicates injury by fluid.
[Sun Simiao] Director Liu of a silk shop suffered from Shanghan for six or seven days, unable to sleep day or night, rising immediately upon lying down and collapsing immediately upon standing, without rest. The attendants grew weary. Sun Simiao was summoned and said: "If this were Qi disorder, there would be panting. Since there is no such symptom, it is not Qi. Occasional cold sweats on the body, deep pulses at Cun, Guan, and Chi positions, beating strongly against the fingers upon heavy pressure, indicate phlegm accumulation and cold gathering in the chest." He prescribed Chenpi, Banxia, and Ganjiang (1 liang each) as a decoction. Ginger (0.5 liang) crushed, decocted in two bowls of water until 70% remained, strained, and divided into two doses. After some time, the patient slept for a day and night without waking. Upon waking, he expectorated a mass of phlegm the size of a chicken egg, and the illness was cured. All phlegm disorders manifest with cold sweats, confirming the diagnosis.
[Zhu Danxi] Wind-phlegm often presents with strange symptoms. Wind-phlegm diseases require wind-phlegm herbs such as Baifuzi, Tianma, Xionghuang, Niuhuang, Jiangcan, and Zaorao. Damp-phlegm often presents with fatigue and weakness. Damp-phlegm is treated with Cangzhul and Baizhu, and oil-fried Banxia.
Clear phlegm belongs to cold, treated with Er Chen Tang.
Banxia, Chenpi, Fuling, Gancao.
Take 4 qian each time, add seven slices of ginger and one Wumei, decoct, take warm or cool as desired. For severe vomiting, add Dingxiang.
Daotan Tang
Banxia (4 liang), Nanxing, Juhong, Zhike, Chifuling (each 0.5 liang), Gancao. Decoct with ginger water.
Drying Damp-Phlegm Formula
Nanxing (processed), Banxia (processed, each 0.5 liang), Haifen (2 liang), coated with Qingdai.
Hot-phlegm often accompanies wind, presenting mostly with external symptoms. Use Qingdai, Huanglian, and Tianguafen to clear upper burner hot-phlegm. When phlegm rises due to excessive fire, treat the fire first with Baizhu, Huangqin, and Shigao. For insufficient Middle Qi, add Renshen, Baizhu, and Huangqin to descend the heat. Food-stagnation phlegm uses Shenqu, Maiya, and Shanzha. To induce vomiting of food-stagnation phlegm, use fried Radish seed powder mixed with grain water to provoke vomiting. Food-stagnation phlegm requires purging; if Qi is deficient, send the medicine with Qi-tonifying herbs. Old-phlegm uses Haishi, Banxia, Gualou, Xiangfu, and Wubeizi, assisted by other herbs to treat stubborn phlegm. Stagnant-phlegm uses Jiangcan, Xingren, Gualou, Hezi, Beimu, and Wubeizi. For phlegm with Spleen deficiency, clear Middle Qi with Er Chen Tang plus Baizhu, combined with lifting herbs. Internal injury with phlegm requires Renshen, Baizhu, [missing character], mostly transported with ginger juice, or added Banxia; if very deficient, add Zhuli. Vertigo and acid regurgitation result from fire moving phlegm, treated with Er Chen Tang plus Zhizi, Huangqin, and Lianqiao. Belching and acid regurgitation indicate food stagnation with heat and rising fire, using Huangqin as sovereign, Nanxing, Banxia, and Juhong as ministers; add Qingdai if heat is severe. A sensation of a lump in the throat that cannot be spat out or swallowed is old-phlegm; severe cases require vomiting with Guiti San; if Qi is excess, use Jingli. (Zhang Zhongjing used Banxia, Houpo, Fuling, and Shengjiang with miraculous effect.) Phlegm nodules in the throat that are dry and immobile require salty herbs to soften hardness in phlegm-resolving formulas, such as Gualouren, Xingren, Haishi, Jiegeng, and Lianqiao, with minor amounts of Houpo, Xiaoshao, and Jiang, made into honey pills to be held in the mouth. Any lump in the upper, middle, or lower body is phlegm; ask about dietary preferences, then use medication after vomiting or purging. Phlegm moves up and down with Qi, reaching everywhere. Phlegm in the intestines and stomach can be cured by purging. Phlegm in the meridians must be expelled by vomiting, which has a dispersing intent. Phlegm above the diaphragm must be vomited; purging will not remove it. Sticky phlegm and floating pulses both require vomiting methods. (Vomiting method seen in Treatment Methods sections.) Phlegm in the hypochondrium cannot be reached except by Baijiezi.
Phlegm in the subcutaneous tissue and membranes is guided by Jiangzhi and Zhuli. Phlegm in the four limbs cannot be opened except by Zhuli. Er Chen Tang effectively treats whole-body phlegm; add guiding herbs downwards for lower body phlegm and upwards for upper body phlegm. Runxia Wan is excellent for descending phlegm. Qingmengshi Wan focuses on Fenghua Xiao. Xiaowei Dan damages the stomach; use for food-stagnation phlegm, but do not take excessively. Cangzhu is extremely effective for phlegm-fluid forming nests. Haifen treats hot-phlegm, descends damp-phlegm, softens knotted phlegm, and eliminates stubborn phlegm; most effective; can be used in pills, not in decoctions. Zhishi drains phlegm, capable of breaking through walls. Zhuli treats phlegm between the diaphragm, manic depression, forgetfulness, and wind-phlegm; it also nourishes blood. Jingli treats slightly heavier phlegm. Using these two liquids is fast and steady. For phlegm knotted in subcutaneous tissues and meridians, assist with Jiangzhi. Jiuzhi treats blood stasis and fluid in the Middle Jiao; take chilled natural juice 3-4 cups, feeling chest irritability before healing. Excess Qi hot-phlegm is difficult to vomit out, forms clumps, or is accompanied by Qi stagnation, making it hard to treat. Phlegm in the Middle Jiao nourishes Stomach Qi; sudden depletion leads to deficiency. Overusing draining herbs for phlegm causes Spleen Qi deficiency, leading to increased phlegm production.
[Zhu Danxi] Discussions on phlegm diseases are rarely detailed. Only "San Yin" mentions it briefly, without modification methods. The sources of phlegm are diverse: generated by phlegm itself, by heat, by Qi, by wind, by fright, by accumulated fluid, by overeating, by summerheat, by cold injuries, by Spleen deficiency, or by alcohol consumption. Their manifestations include: Fright-phlegm causes Pericardium pain and epilepsy; Hot-phlegm causes irritability, headache, rotting eyes, dry constipation, palpitations, anxiety, and惊眩; Wind-phlegm causes paralysis, severe vertigo, and dark wind confusion; Fluid-phlegm causes hypochondriac pain, immobility of limbs, and daily vomiting; Food-phlegm causes malaria-dysentery and foul breath; Summerheat-phlegm causes dizziness, jaundice, and headache; Cold-phlegm causes bone bi, limb immobility, and stabbing Qi pain; Wine-phlegm causes indigestion of alcohol and next-day vomiting upon drinking again; Spleen-deficiency phlegm causes poor appetite, reverse vomiting; Qi-phlegm causes wandering stabbing pain. Women suffer most from fright-phlegm, often due to intercourse during menstruation when Qi is deficient, forming masses. There is always a mass in the abdomen, appearing like pregnancy, causing unbearable pain upon movement. Immobility of arms or widespread joint pain, restlessness, indicates phlegm entering bones. Chronic Hong Kong foot (gout) unresponsive to gout medicine is a phlegm disease. Post-Shanghan vomiting preventing medication intake is due to saliva-phlegm in the diaphragm from early purging. Yellowing in Shanghan is also a phlegm disease caused by heat binding. Constipation is also phlegm knotting.
These symptoms vary and are hard to distinguish or diagnose by pulse. Guard this secret carefully.
Anyone with phlegm has gray-black eyelids and under-eye areas, visible without pulse diagnosis. Black eyes with red cheeks indicate hot-phlegm. Large yellow face also indicates hot-phlegm. External symptoms include chest fullness, heat, dry mouth craving cold, constipation, and red hot urine. Long-term leads to wind, vertigo, tinnitus, and眼花, mostly deficiency syndromes; treating with hot herbs long-term leads to large solid pulses, high fever, and stroke, requiring urgent purging. Black eyes with groaning while walking and difficulty moving indicate bone-entering phlegm, with widespread joint pain. Black eyes with earthy complexion and flaccid paralysis indicate wind-damp phlegm. Black eyes with shortness of breath indicate fright-wind phlegm. Large pulses at left and right Guan positions indicate phlegm in the diaphragm, suitable for vomiting. If afraid of vomiting, gently purge. Daily line-like coldness rising on the back requires vomiting and purging.
For diseases unresponsive to hundreds of medicines, if the Guan pulse is hidden and large, it is phlegm. Use Miaoying Wan modifications. (Formula seen in Wandering Pain, i.e., Kongyan Dan.) For Hong Kong foot, add Songzhi (1 qian), Binglang and Mugua (1 liang each), Juanbai (0.5 liang). Decoct in salt water for half a day, then plain water for half a day, mix with previous herbs to make pills. Take 20 pills, increase to 40-50, expel evil matter for immediate effect. For fright-phlegm, add Zhusha (2 qian) and Quanxie. Take 8-9 pills, 3-5 doses to eliminate. For wine-phlegm, add Xionghuang and Quanxie (2 qian each), take 10 pills. For wandering lumbar pain, add 3 walnuts burnt to ash, take 30 pills, increase to 50, with warm wine. For Hong Kong foot wandering, add Wuzaoyu to the previous formula. For fright-Qi masses, add Shanjia, Chao Biejia (3 qian each), Yanshusu and Pengze (4 qian each). Take 50 pills, increase to 70, stop when purged. For Kidney Bi pain, add Mubiezi (1 liang) and Rougui (0.5 liang). For hot-phlegm, add Penxiao equally, take 3 pills. For cold-phlegm, add Huajiao, Dingxiang, Quanxie, and Rougui equally, take 25 pills.
[Dongxu Zi] Phlegm diseases cause hemiplegic headache, thunder-headache, and Taiyang headache. Vertigo like riding in boats, mental confusion. Mouth and eye twitching. Itching of eyebrows and ears. Swelling and hardness of jaw, cheeks, and limbs. Painful yet not painful. Whole body dry itching, scratching produces hidden rashes, hot skin, brocade-like spots. Teeth and cheeks itch and hurt unpredictably, floating teeth, varying pain and itch. Belching and acid regurgitation, smelling scorched odor. Fishy smell in throat,心烦, nasal congestion, difficulty swallowing, inability to spit out or swallow phlegm. Phlegm comes out with sneezing or movement, black like ink, broken cotton, peach gum, or clam meat. Heart feels like ice and iron, blocked and stifling, frequent belching and sneezing, resembling diaphragmatic Qi. Dreams of execution, swords, surrounded by walls, finding a way out but losing direction. Dreams of burning ground, smoke, burnt bones, no escape. Anger and weeping upon waking without trigger. Seeing two moons or golden lights in the sky. Sore knees, waist and kidney pain, difficult breathing. Pain in limbs like stabbing, intermittent. Arm numbness and pain like wind-damp. Restless sleeping like thorns or insect bites. Limb immobility, heavy hands and feet. Eyes like ginger sting, sticky and itchy, hard to open/close. During weather changes, chest stuffiness and Qi knotting, closed and not erupting, leading to tooth itch, throat pain, mouth ulcers, and tongue sores. Upon eruption, continuous sneezing, thick sputum then clear water. Darkness before eyes, wind sound behind head, cicada noise in ears, eye muscle twitching. Treatments vary: loose pores allowing wind invasion, upper excess lower deficiency, deficiency, cold, or pathogen eruption. Only Dongxu Zi understands this suffering and can treat it. Disease onset feels like two gases twisting in chest/abdomen, choking and depression, smoke/fire rushing up, hot head/face, dizziness, tinnitus, phlegm/saliva/tears surging from lungs/stomach, hair standing, hundreds of sneezes. Then whole-body irritability, removing clothes and chilling body brings temporary relief. Adding clothes in cool spring/autumn also helps. Drinking ice water or getting drunk calms it, but root cause remains. Obtained the mysterious Chenxiang Wan formula, effective for thousands, but kept secret. Poem: [Metaphorical description of ingredients].
Dahuang (steamed, 8 liang), Huangqin (8 liang), Qingmengshi (1 liang, fired with Xiao until gold-colored), Chenxiang (0.5 liang).
Grind into powder, make water pills, walnut-sized. Take with warm water after meals on an empty stomach. For all new or old mental loss, mania, or madness, take 100 pills. For excess Qi, eating ability, and severe mania, add 20 pills, adjust as needed. For all wind-stroke paralysis, phlegm-saliva obstruction, regular or irregular bowels, take 80-90 pills, up to 100, preventing constipation. For all Yang syndrome wind-toxic Hong Kong foot, wandering pain, take 80-90 pills, up to 100 if ineffective. For healthy people with unnamed bone/joint pain, headache/toothache, shaking/itching, wind-boring, wind-cold nasal congestion, body pain/non-pain, non-Shanghan conditions, take 80-90 pills. Increase for excess phlegm and Qi. For all acid regurgitation, belching,
