宋徽宗妃苦痰，面浮如盘。李医用蚌粉，新瓦上炒令通红，放地上出火毒，每半两加青黛少许，同淡齑水
滴入麻油数点，调服而愈。
〔《本草》〕治喘嗽。（汗多者忌用。）
草乌（五钱） 麻黄（三钱）
上为末，每服三大钱，萝卜一个，同煮令熟，只吃萝卜妙。
〔《千》〕杏仁煎 润肺丸 （以上二方见燥痰条。）
〔丹〕润肺散。
贝母（一两） 栝蒌仁（半两） 青黛（五钱）
上为末，姜蜜调成膏，噙。
抑痰丸
栝蒌仁（一两） 半夏（二钱） 贝母（三钱）
上为末，炊饼丸，如麻子大。
咳逆上气，痰饮心痛。
海蛤（烧为粉，研极细，过数日，火毒散用之） 栝蒌仁（带穣同研）
上为丸，如麻子大，白汤下。
杨文治痰嗽利胸膈方
栝蒌肥大者，割开去子，洗净槌破刮皮，细切焙干。半夏四十九个，汤洗十遍，槌破焙干为末。用
洗栝蒌实热水并穣同熬成膏为丸，如桐子大，生姜汤下二十丸。
〔《千》〕温脾汤 治食饱则咳。
甘草（四两） 大枣（二十枚）
上二味， 咀，水五升，煮水二升，温分三服。若咽中痛而声鸣者，加干姜一两。
〔《圣》〕甘胆丸 治吃醋抢喉，因成咳嗽不止，诸药无效。
用甘草二两，去赤皮，作二寸段，中半掰开，用猪胆汁五枚，浸三日，取出火上炙干，为细末，炼蜜丸，
每服四十丸，茶清吞下，卧服神效。曾有人患此，诸药不效，用此方一服愈。
〔丹〕饮酒伤肺痰嗽。以竹沥煎入韭汁，就吞栝蒌仁、杏仁、黄连丸。
治酒嗽。
青黛 栝蒌仁
用蜜姜丸，含化，散肺毒也。
食停积嗽，非青黛、栝蒌实不除。有食积人，面青白黄色不常，面上如蟹爪路，一黄一白者是也。
清金丸治食积嗽。（方见虚实条。）
栝蒌丸 治食痰壅滞喘咳。
栝蒌实 半夏 山楂 神曲（各等分）
为末，栝蒌穣水为丸，竹沥姜汤送下。
又方治食积痰嗽。
杏仁 萝卜子（各二两） 为末，粥丸服。
食积痰作嗽，发热者。
半夏 南星 栝蒌实 萝卜子（为佐） 青黛 石碱（为使） 随症加减，丸服之。
肺痿，专在养肺养气养血清金。
〔仲〕生姜甘草汤 治肺痿咳吐涎沫不止，咽燥而渴。
生姜（五两） 人参（二两） 甘草（四两） 大枣（十五枚）
上四味，以水七升，煮取三升，分温三服。
炙甘草汤 治肺痿涎唾多，心中温温液液者。
用甘草一味，以水三升，煮半升，分三温服。
桂枝去芍药加皂荚汤 治肺痿吐涎沫。
桂枝 生姜（各三两） 甘草（一两） 大枣（十枚） 皂荚（十枚，去皮弦，炙焦）
上五味，以水七升，微微火煮取三升，分三温服。
肺痿吐涎沫而不咳者，其人不渴必遗尿，小便数。所以然者，以上虚不能制下故也。此为肺中冷，必眩多涎吐，
甘草干姜汤 以温之。若服汤口渴者，属消渴。
甘草（炙，四两） 干姜（二两，炮）
上 咀，以水三升，煮取一升五合，去渣，分温再服。
上治肺痿症，或咳沫，或咳血。今编咳沫者入此门，咳血者入咳吐血门。
热在上焦者，因咳为肺痿。肺痿之病从何得之？师曰∶或从汗出，或从呕吐，或从消渴，小便利数，或从
便难，又被快药下利，重亡津液，故得之。
〔《脉》〕咳而口中自有津液，舌上胎滑，此为浮寒，非肺痿也。
咳而上气烦躁者为肺胀。仲景云∶咳而上气，此为肺胀。脉浮大者，越婢加半夏汤主之。又云∶肺胀，咳
而上气，烦躁而喘，脉浮者，心下有水，小青龙汤加石膏主之。（二方并见喘门。）
〔丹〕肺胀而咳者，用诃子、青黛、杏仁。诃子能治肺气因火伤极，遂成郁遏胀满，不得眠一边，取其味
酸苦，有收敛除火之功。佐以海石、香附、栝蒌仁、青黛、半夏曲，姜，蜜调噙之。（香附用童便浸三日。）
脉郁痰嗽睡不安，宜清化丸。贝母、杏仁为末，砂糖入姜汁蒸饼为丸，噙化。（清化丸方见火热条。）肺胀
而嗽，或左或右不得眠，此痰夹瘀血碍气而病，宜养血以流动乎气，降火疏肝以清痰，四物汤加桃仁、诃子、
青皮、竹沥之类。如血碍气作嗽者，桃仁、大黄、姜汁丸服。
一法，灸但可一边眠者，可左侧者灸右足三阴交，可右侧者灸左足三阴交，立安。
〔《经》〕治气嗽，嗽久者亦主之。生诃子一枚含之，咽津，瘥。瘥后不知食味，煎槟榔汤服之，便有味。
〔丹〕定喘劫方
诃子 百药煎 荆芥（各等分）
为末，姜蜜丸，含化。
治嗽劫方。
五味子（半两） 甘草（二钱半） 五倍子（一钱） 风化硝
为末，蜜丸，干含化。
〔世〕治痰嗽并喘。白矾飞，五倍子等分为末，每二钱，猪肝蘸药临卧服。
〔《本》〕化痰涎方。
明矾（二两，枯） 白僵蚕（半两，炒，去丝）
上为细末，生研薄荷令烂，和丸如绿豆大。每服二十丸，薄荷汤下，日三服。
治远年近日喘嗽。用皂角不蛀者三大茎，锉去黑皮，切开去子，每子仓内入巴豆肉一粒，合就，麻皮缚定，
用生姜自然汁和蜜涂，令周匝慢火炙之，又涂又炙，以焦黄为度。劈开，去巴豆，以好明矾一两枯过，蓖麻七
个，姜汁和蜜涂炙，前三味为末，却以杏仁二两去皮尖，研成膏，与前药和匀。每服一钱，用柿饼炙过候冷，蘸
药细嚼，临卧服。忌鱼 鲞油面酒米醋煎爆热毒等物。
上诃子、五味、五倍、白矾例。丹溪云∶黄昏嗽者，火浮于肺，酸味敛而降之是也。
〔罗〕九仙散 治一切咳嗽。
桑白皮 人参 桔梗 阿胶 五味子 乌梅 款冬花（各一两） 贝母（半两） 粟壳（八两，去蒂，蜜
炒黄）
上为细末，每服三钱，白汤点服。嗽住，止后服。
款花清肺散 治咳嗽喘促，胸膈不利，不得安卧。
人参（去芦） 甘草（炒） 白矾（枯） 乌梅（和核杵碎） 款冬花（各一两） 甜葶苈（生用，一
钱） 粟壳（醋炒，四两）
上为细末，每服二钱，温米饮汤调服，食后。忌油腻物。多言语损气。
人参清肺汤 治男子妇人肺胃虚寒，咳嗽气急，胸膈咽塞，腹胁胀满，迫塞短气，喜欲饮冷，咽嗌隐痛，及疗
肺痿劳嗽吐血腥臭，干呕烦热，声音不出，肌肉消瘦，倦怠减食。
乌梅 地骨皮 人参 甘草（炙） 阿胶（炮） 杏仁（去皮，炒） 桑白皮 知母 粟壳（去顶蒂，
炒。各一两）
上 咀，每服三钱，水一盏半，入乌梅一个，同煎至一盏，去渣温服，食后临卧。
人参款花散 治咳嗽久不愈者。
人参 款冬花（各五钱） 知母 贝母 半夏（各三钱） 粟壳（一两，炒黄色）
上为粗末，每服五钱，乌梅同煎，临卧服。
安肺散 治痰嗽，不问新旧。
麻黄（二两，去节） 甘草（二两，炒） 御米壳（四两，去蒂，炒黄色）
上为末，每三钱，水一盏，入乌梅一个，煎七分，去渣温服，临卧下。
人参理肺散 治喘嗽不止。
麻黄（去节，炒黄色） 当归 木香（各一两） 御米壳（三两，炒） 人参（二两） 杏仁（二两，麸炒。）
上 咀，每服四钱，水煎。
上御米壳、乌梅例，收涩药也。
〔世〕治嗽。贝母去心略炒，知母去毛各一两，巴豆霜十四粒，为末，和匀，老生姜切如钱浓，两面蘸药
末卧嚼，嚼时开口勿咽。
〔《千》〕治咳嗽，胸膈满，多喘上气。
生姜（汁，一升半） 砂糖（五两）
上二味，煎姜汁减半，纳糖更煎服。
咳嗽上气，厥在胸中，过在手阳明太阴。（全文见治虚实法。）
〔海〕肺咳喘息而有音，甚则吐血，麻黄汤。太阳咳而遗矢，赤石脂禹余粮汤、桃仁汤。不止，用猪苓分
水散。心咳心痛，喉仲介介鲠状，甚则咽喉痹，用桔梗汤。小肠咳，咳而失气，气与咳俱失，芍药甘草汤。（与夏
脉不及病同，盖小肠为心之府故也。）肝咳，两胁下痛，甚不可以转，转则两 下满，小柴胡汤。（仲景云∶
咳引胁痛为悬饮，宜十枣汤。丹溪云∶咳引胁痛宜疏肝气，用青皮、枳壳、香附子等，实者白芥子之属。）
胆咳，咳呕胆汁，黄芩加半夏生姜汤。脾咳，右胁下痛，阴阴痛引肩背，甚则不可以动，动则咳剧，升
麻汤。胃咳，呕，呕甚则长虫出，乌梅丸。肾咳，腰背相引而痛，甚则咳涎，麻黄附子细辛汤。膀胱咳，
遗溺，茯苓甘草汤。三焦咳，腹满不欲饮食，此皆聚于胃关于肺，使人多涕唾而面浮肿气逆，钱氏异功散。
〔《素》〕咳嗽烦冤者，是肾气之逆也。（示从容篇。）一阳发病，少气善咳，善泄。夏脉不及，则
令人上见咳唾，下为气泄。（全文见治虚实法。）
凡咳嗽面赤，胸腹胁常热，惟于足乍有凉时，其脉洪者，热痰在胸膈也，宜小陷胸汤、礞石丸之类清
膈降痰。甚而不已者，宜吐下其痰热也。面白悲嚏，或胁急胀痛，或脉沉弦细迟而咳者，寒饮在胸腹也。
宜辛热去之。
〔世〕小陷胸汤，治咳嗽膈有热痰，胸胁腹常热者，神效。（方见伤寒。）
〔丹〕黄连化痰丸
黄连 吴茱萸（一钱半，煮，去沫） 陈皮（五钱） 半夏（一两半）
上为末，面糊丸，如绿豆大。每服百丸，生姜汤下。
上黄连、半夏治痰嗽例。
青礞石丸 去痰或痞痛。经络中有痰，皆能治之。

上炒神曲、姜糊丸。
又方礞石丸 治痰。

定嗽化痰方
黄芩（一两半，酒洗） 滑石（半两） 贝母 南星（各一两） 风化硝（二钱半） 白芥子（五钱，去壳）
上为末，汤浸蒸饼丸。（丹溪云∶胁下痰非白芥子不能除。）
上风化硝佐礞石、茯苓、半夏治痰嗽例。丹溪云∶礞石丸重在风化硝。
〔洁〕小黄丸 治咳嗽面赤，烦躁脉洪。（方见前湿痰条。）
〔罗〕人参半夏丸 化痰坠涎，止嗽定喘，或风痰茶痰食痰，一切痰逆呕吐，厥头痛。或风气偏正头风
疼，或壅头目昏眩，耳聋鼻塞，咽干胸膈不利。
人参 茯苓 天南星（各半两） 半夏 干姜 白矾 寒水石（各一两） 蛤粉（二两） 薄荷（五钱）
藿香（二钱半）
上为细末，水糊丸，如桐子大。每服三十丸，生姜汤下，食后，日三服。
金珠化痰丸 治痰热安神定志，除头痛眩晕，心怔忡恍惚，胸膈烦闷，涕唾稠粘，痰实咳嗽，咽膈不利。
生龙脑（研细，半两） 皂荚仁（炒黄，一两） 天竺黄（一两） 金箔（二十片，为衣） 白矾（枯，二两）
半夏（姜汁制，四两）
上皂角仁半为末，与诸药同拌匀，生姜自然汁为丸，如桐子大。每服三十丸，生姜汤下，食后临卧。
人参清金丸 治热止嗽，消痰定喘。
柴胡 人参（各一两） 黄芩 甘草（炙） 半夏（各七钱） 麦门冬 青黛 陈皮（各二钱） 五味子（二十一
粒）
上为细末，水面糊为丸，如桐子大。每服三十丸，温白汤下，食后。
〔垣〕脾肺受寒痰嗽用药法。《难经》云∶肺太过则外症面白善嚏，悲愁不乐，欲哭；其内症喘咳上 ，
逆气烦心，胸满烦热，夜则涕出多嚏，鼻塞不通。肺金大实，以子助母也。心脾肺皆受寒邪，涎退场门甘，水反
侮土，寡于畏也。腹中大寒，病名曰寒中，痰白作泡，肺中气虚而为大寒，子助母也。当于肺中泻肾水，非辛
热之药不退也。口甘涎沫者，胃中寒而不和，以辛甘热去之。饮酒者多有此反化。
半夏温肺汤 治心腹中脘痰水冷气，心下汪洋嘈杂，常多唾，口中清水自出，胁肋急胀，痛不欲食，此胃气虚
冷所致，其脉沉弦细迟。
旋复花 人参 细辛 桂心 甘草 陈皮 桔梗 芍药 半夏（制。各半两） 赤茯苓（三分）
上 咀，每服四钱，姜三片，煎，食后服。
丁香半夏丸 治心下停饮冷痰，头目眩晕，睡卧口中多涎。
槟榔（三钱） 丁香 半夏（各一两） 细辛 干姜 人参（各五钱）
上为细末，生姜汁糊为丸，如桐子大。每服二十丸，姜汤下，日三服，食后。
〔罗〕温胃化痰丸 治膈间有寒，脾胃停饮，胸膈不快，痰涎不已。
半夏（炮，三两） 陈皮（去白） 干姜（炮） 白术（焙。各二两）
上为细末，生姜面糊为丸，如桐子大。每服二十丸，生姜汤下，不拘时。
〔洁〕姜桂丸治咳嗽面黧黑，足寒脉沉。（方见前湿痰条。）
〔《局》〕胡椒理中丸 治肺胃虚寒，咳嗽气急，逆气虚痞不能食，呕吐痰水。
款冬花（去枝梗） 荜茇 陈皮 干姜（炮） 良姜（炒） 甘草（炙） 细辛（去苗）
胡椒（各四两） 白术（五两）
上末，炼蜜丸，如桐子大。每服三十丸，温汤下。
〔仲〕吴茱萸汤（方见伤寒。）
〔洁〕夏月嗽发寒热者，谓之热嗽，用小柴胡汤三两，加石膏七钱，知母三钱治之。（柴胡汤方见伤寒。）冬月嗽而
宋徽宗的妃子深受痰证困扰，面部浮肿如盘。李医使用蚌粉，在新鲜瓦片上炒至通红，放置于地面以去除火毒，每次取半两加入少许青黛，配合淡齑水，滴入数点麻油，调匀服用后痊愈。
〔《本草》〕治疗喘咳。（汗多者忌用。）
草乌（五钱） 麻黄（三钱）
以上药物研为细末，每次服用三大钱，配一个萝卜，一同煮至熟透，只吃萝卜效果极佳。
〔《千金方》〕杏仁煎、润肺丸（以上两个方剂见“燥痰”条目）。
〔朱丹溪〕润肺散。
贝母（一两） 栝蒌仁（半两） 青黛（五钱）
以上药物研为细末，用姜汁和蜂蜜调成膏状，含服。
抑痰丸
栝蒌仁（一两） 半夏（二钱） 贝母（三钱）
以上药物研为细末，用蒸饼粉制丸，大小如麻子。
主治咳嗽气逆上冲，痰饮心痛。
海蛤（烧成粉末，研磨极细，放置数日待火毒散去后使用） 栝蒌仁（带瓤一同研磨）
以上药物制成丸剂，大小如麻子，用白开水送服。
杨文治疗痰嗽、利胸膈的方剂
选用肥大的栝蒌，割开去掉种子，洗净捣碎刮去外皮，细细切碎焙干。半夏四十九个，用热水清洗十遍，捣碎焙干研为细末。用清洗栝蒌实的热水连同其瓤一起熬制成膏，制成丸剂，大小如梧桐子，用生姜汤送服二十丸。
〔《千金方》〕温脾汤 治疗饱食后咳嗽。
甘草（四两） 大枣（二十枚）
以上两味药，切碎，加水五升，煮取二升，分三次温服。如果咽喉疼痛且声音嘶哑的，加干姜一两。
〔《圣济总录》〕甘胆丸 治疗因吃醋导致喉部紧缩感，进而引起咳嗽不止，其他药物无效的病症。
取甘草二两，去掉红皮，切成二寸长的段，从中间掰开，用五个猪胆汁浸泡三天，取出在火上烤干，研为细末，用炼蜜制丸，每次服用四十丸，用茶清送下，睡前服用效果神奇。曾有人患此病，其他药物无效，用此方一次服用即愈。
〔朱丹溪〕饮酒损伤肺脏导致的痰嗽。用竹沥煎煮后加入韭菜汁，吞服栝蒌仁、杏仁、黄连丸。
治疗酒嗽。
青黛 栝蒌仁
用蜂蜜和生姜制丸，含化，以疏散肺毒。
饮食停滞积滞引起的咳嗽，非用青黛、栝蒌实不能消除。有食积的人，面色呈现青、白、黄色交替不定，面上有如蟹爪般的纹路，一黄一白即是此症。
清金丸治疗食积咳嗽。（方剂见“虚实”条目）。
栝蒌丸 治疗食痰壅滞引起的喘咳。
栝蒌实 半夏 山楂 神曲（各等分）
研为细末，用栝蒌瓤的水制丸，用竹沥姜汤送服。
又一方治疗食积痰嗽。
杏仁 萝卜子（各二两） 研为细末，用米粥制丸服用。
食积痰作咳嗽，伴有发热的。
半夏 南星 栝蒌实 萝卜子（作为佐药） 青黛 石碱（作为使药） 随症状加减，制丸服用。
〔张仲景〕生姜甘草汤 治疗肺痿咳嗽吐涎沫不止，咽喉干燥而口渴。
生姜（五两） 人参（二两） 甘草（四两） 大枣（十五枚）
以上四味药，加水七升，煮取三升，分三次温服。
炙甘草汤 治疗肺痿唾涎多，心中感觉温温液液不适。
只用甘草一味，加水三升，煮取半升，分三次温服。
桂枝去芍药加皂荚汤 治疗肺痿吐涎沫。
桂枝 生姜（各三两） 甘草（一两） 大枣（十枚） 皂荚（十枚，去皮弦，炙焦）
以上五味药，加水七升，用微火煮取三升，分三次温服。
肺痿吐涎沫但不咳嗽的，患者不渴，必定遗尿，小便频数。之所以这样，是因为上焦虚寒不能制约下焦的缘故。这是肺中寒冷，必然出现眩晕、多涎唾，用甘草干姜汤来温补。如果服药后感到口渴的，属于消渴病。
甘草（炙，四两） 干姜（二两，炮制）
以上药物切碎，加水三升，煮取一升五合，去渣，分两次温服。
上述治疗肺痿的症状，有的咳吐涎沫，有的咳血。现在将咳吐涎沫的编入此门，咳血的编入咳吐血门。
热邪在上焦的，因咳嗽发展为肺痿。肺痿这个病是怎么得的？老师说：或者因为出汗过多，或者因为呕吐，或者因为消渴病导致小便频数，或者因为大便困难，又被峻下药攻下导致腹泻，严重耗伤津液，所以得此病。
〔《脉经》〕咳嗽而口中自有津液，舌苔滑润，这是浮寒证，不是肺痿。
咳嗽兼有气逆、烦躁的是肺胀。张仲景说：咳嗽气逆，这是肺胀。脉象浮大的，用越婢加半夏汤主治。又说：肺胀，咳嗽气逆，烦躁气喘，脉浮的，心下有停水，用小青龙汤加石膏主治。（这两个方剂并见“喘”门。）
〔朱丹溪〕肺胀而咳嗽的，用诃子、青黛、杏仁。诃子能治疗肺气因火邪损伤至极，遂成郁遏胀满，无法侧卧睡眠，取其味酸苦，有收敛降火的功效。辅以海浮石、香附、栝蒌仁、青黛、半夏曲，用姜、蜜调匀含服。（香附用童子尿浸泡三天。）
脉郁痰嗽、睡不安稳的，宜用清化丸。贝母、杏仁研为细末，砂糖加入姜汁蒸饼制丸，含化。（清化丸方剂见“火热”条目）。肺胀而咳嗽，或左侧或右侧无法安睡，这是痰夹瘀血阻碍气机所致，宜养血以流动气机，降火疏肝以清痰，用四物汤加桃仁、诃子、青皮、竹沥之类。如果是血碍气引起的咳嗽，用桃仁、大黄、姜汁制丸服用。
一种方法，灸法只适用于只能向一侧侧卧的患者，可以向左睡的灸右足三阴交穴，可以向右睡的灸左足三阴交穴，立即安宁。
〔《内经》〕治疗气嗽，久咳者也适用。生诃子一枚含在口中，咽下津液，病愈。病愈后不知食物味道，煎槟榔汤服用，便知味道。
〔朱丹溪〕定喘劫方
诃子 百药煎 荆芥（各等分）
研为细末，姜蜜制丸，含化。
治嗽劫方。
五味子（半两） 甘草（二钱半） 五倍子（一钱） 风化硝
研为细末，蜜制丸，干含化。
〔世医〕治疗痰嗽并喘。白矾飞过，与五倍子等分为细末，每次二钱，蘸猪肝，临睡前服用。
〔《本草》〕化痰涎方。

治疗远年近日喘嗽。选用不蛀的皂角三大茎，锉去黑皮，切开去掉种子，每个种子仓内放入一粒巴豆肉，合拢，用麻皮绑紧，用生姜自然汁和蜜涂抹，使其周围均匀，慢火炙烤，又涂又炙，直到焦黄为止。劈开，去掉巴豆，用好明矾一两枯过，蓖麻七个，姜汁和蜜涂抹炙烤，前三味研为细末，再将杏仁二两去皮尖，研磨成膏，与前药混合均匀。每次服用一钱，用柿饼炙过候冷，蘸药细嚼，临睡前服用。忌食鱼鲞、油面、酒、米醋、煎爆热毒等物。
上述诃子、五味子、五倍子、白矾为例。丹溪说：黄昏时咳嗽的，是火浮于肺，用酸味收敛而降之。
〔罗天益〕九仙散 治疗一切咳嗽。
桑白皮 人参 桔梗 阿胶 五味子 乌梅 款冬花（各一两） 贝母（半两） 粟壳（八两，去蒂，蜜炒黄）
以上研为细末，每次服用三钱，白开水冲服。咳嗽停止，停止后续服用。
款花清肺散 治疗咳嗽喘促，胸膈不利，不得安卧。
人参（去芦） 甘草（炒） 白矾（枯） 乌梅（和核杵碎） 款冬花（各一两） 甜葶苈（生用，一钱） 粟壳（醋炒，四两）
以上研为细末，每次服用二钱，温米饮汤调服，饭后服用。忌油腻食物。多言语耗气。

乌梅 地骨皮 人参 甘草（炙） 阿胶（炮） 杏仁（去皮，炒） 桑白皮 知母 粟壳（去顶蒂，炒。各一两）
以上切碎，每次服用三钱，水一盏半，加入乌梅一个，同煎至一盏，去渣温服，饭后临睡前服用。
人参款花散 治疗咳嗽久不愈者。
人参 款冬花（各五钱） 知母 贝母 半夏（各三钱） 粟壳（一两，炒黄色）
以上为粗末，每次服用五钱，乌梅同煎，临睡前服用。
安肺散 治疗痰嗽，不论新久。
麻黄（二两，去节） 甘草（二两，炒） 御米壳（四两，去蒂，炒黄色）
以上研为细末，每次三钱，水一盏，加入乌梅一个，煎至七分，去渣温服，临睡前服用。
人参理肺散 治疗喘嗽不止。
麻黄（去节，炒黄色） 当归 木香（各一两） 御米壳（三两，炒） 人参（二两） 杏仁（二两，麸炒。）
以上切碎，每次服用四钱，水煎。
上述御米壳、乌梅为例，均为收涩药。
〔世医〕治疗咳嗽。贝母去心略炒，知母去毛各一两，巴豆霜十四粒，研为细末，和匀，老生姜切如钱厚，两面蘸药末，睡前咀嚼，嚼时开口勿咽。
〔《千金方》〕治疗咳嗽，胸膈满，多喘气上逆。
生姜（汁，一升半） 砂糖（五两）
以上两味药，煎姜汁减半，纳入糖再煎服。
咳嗽上气，厥气在胸中，病涉手阳明、太阴经。（全文见治虚实法。）
〔王好古〕肺咳喘息而有声音，甚则吐血，用麻黄汤。太阳经咳而遗矢（大便失禁），用赤石脂禹余粮汤、桃仁汤。不止，用猪苓分水散。心咳心痛，喉咙中有介介如鲠状异物感，甚则咽喉痹阻，用桔梗汤。小肠咳，咳而失气（放屁），气与咳俱失，用芍药甘草汤。（与夏脉不及病相同，盖小肠为心之腑的缘故。）肝咳，两胁下痛，甚则不可以转身，转身则两胁下满闷，用小柴胡汤。（张仲景说：咳引胁痛为悬饮，宜十枣汤。丹溪说：咳引胁痛宜疏肝气，用青皮、枳壳、香附子等，实证用白芥子之类。）胆咳，咳呕胆汁，用黄芩加半夏生姜汤。脾咳，右胁下痛，隐隐作痛牵引肩背，甚则不可以动，动则咳剧，用升麻汤。胃咳，呕，呕甚则长虫出，用乌梅丸。肾咳，腰背相引而痛，甚则咳涎，用麻黄附子细辛汤。膀胱咳，遗溺，用茯苓甘草汤。三焦咳，腹满不欲饮食，此皆聚于胃关于肺，使人多涕唾而面浮肿气逆，用钱氏异功散。
〔《素问》〕咳嗽烦冤的，是肾气之逆也。（示从容篇。）一阳发病，少气善咳，善泄。夏脉不及，则令人上见咳唾，下为气泄。（全文见治虚实法。）
凡咳嗽面赤，胸腹胁常热，惟于足乍有凉时，其脉洪大的，是热痰在胸膈也，宜小陷胸汤、礞石丸之类清膈降痰。甚而不已的，宜吐下其痰热也。面白悲嚏，或胁急胀痛，或脉沉弦细迟而咳的，是寒饮在胸腹也。宜辛热药去之。
〔世医〕小陷胸汤，治疗咳嗽膈有热痰，胸胁腹常热的，神效。（方剂见伤寒。）
〔朱丹溪〕黄连化痰丸
黄连 吴茱萸（一钱半，煮，去沫） 陈皮（五钱） 半夏（一两半）
以上研为细末，面糊制丸，如绿豆大。每次服用一百丸，生姜汤送下。
青礞石丸 去痰或痞痛。经络中有痰，皆能治之。
青礞石（硝煅，五钱） 半夏（二两） 风化硝（二钱） 陈皮（七钱半） 白术（一两） 茯苓（七钱半） 黄芩（半两）
以上炒神曲、姜糊制丸。
又方礞石丸 治疗痰。
礞石（半两，硝煅） 半夏（七钱半） 南星 茯苓（各五钱） 风化硝（二钱）
以上研为细末，神曲糊制丸。
定嗽化痰方
黄芩（一两半，酒洗） 滑石（半两） 贝母 南星（各一两） 风化硝（二钱半） 白芥子（五钱，去壳）
以上研为细末，汤浸蒸饼制丸。（丹溪说：胁下痰非白芥子不能除。）
上述风化硝佐礞石、茯苓、半夏治痰嗽为例。丹溪说：礞石丸重在风化硝。
〔李东垣〕小黄丸 治疗咳嗽面赤，烦躁脉洪。（方剂见前湿痰条。）
〔罗天益〕人参半夏丸 化痰坠涎，止嗽定喘，或风痰茶痰食痰，一切痰逆呕吐，厥头痛。或风气偏正头风疼，或壅头目昏眩，耳聋鼻塞，咽干胸膈不利。
人参 茯苓 天南星（各半两） 半夏 干姜 白矾 寒水石（各一两） 蛤粉（二两） 薄荷（五钱） 藿香（二钱半）
以上研为细末，水糊制丸，如桐子大。每次服用三十丸，生姜汤送下，饭后，每日三次。
金珠化痰丸 治疗痰热安神定志，除头痛眩晕，心怔忡恍惚，胸膈烦闷，涕唾稠粘，痰实咳嗽，咽膈不利。
生龙脑（研细，半两） 皂荚仁（炒黄，一两） 天竺黄（一两） 金箔（二十片，为衣） 白矾（枯，二两） 半夏（姜汁制，四两）
以上皂角仁一半为末，与诸药同拌匀，生姜自然汁制丸，如桐子大。每次服用三十丸，生姜汤送下，饭后临睡前。
人参清金丸 治疗热止嗽，消痰定喘。
柴胡 人参（各一两） 黄芩 甘草（炙） 半夏（各七钱） 麦门冬 青黛 陈皮（各二钱） 五味子（二十一粒）
以上研为细末，水面糊制丸，如桐子大。每次服用三十丸，温白汤送下，饭后。
〔李东垣〕脾肺受寒痰嗽用药法。《难经》说：肺太过则外症面白善嚏，悲愁不乐，欲哭；其内症喘咳上气，逆气烦心，胸满烦热，夜则涕出多嚏，鼻塞不通。肺金大实，以子助母也。心脾肺皆受寒邪，涎退场门甘，水反侮土，寡于畏也。腹中大寒，病名曰寒中，痰白作泡，肺中气虚而为大寒，子助母也。当于肺中泻肾水，非辛热之药不退也。口甘涎沫者，胃中寒而不和，以辛甘热去之。饮酒者多有此反化。
半夏温肺汤 治疗心腹中脘痰水冷气，心下汪洋嘈杂，常多唾，口中清水自出，胁肋急胀，痛不欲食，此胃气虚冷所致，其脉沉弦细迟。
旋复花 人参 细辛 桂心 甘草 陈皮 桔梗 芍药 半夏（制。各半两） 赤茯苓（三分）
以上切碎，每次服用四钱，姜三片，煎，食后服。
丁香半夏丸 治疗心下停饮冷痰，头目眩晕，睡卧口中多涎。
槟榔（三钱） 丁香 半夏（各一两） 细辛 干姜 人参（各五钱）
以上研为细末，生姜汁糊制丸，如桐子大。每次服用二十丸，姜汤送下，每日三次，饭后。
〔罗天益〕温胃化痰丸 治疗膈间有寒，脾胃停饮，胸膈不快，痰涎不已。
半夏（炮，三两） 陈皮（去白） 干姜（炮） 白术（焙。各二两）
以上研为细末，生姜面糊制丸，如桐子大。每次服用二十丸，生姜汤送下，不拘时。
〔李东垣〕姜桂丸治疗咳嗽面黧黑，足寒脉沉。（方剂见前湿痰条。）
〔《局方》〕胡椒理中丸 治疗肺胃虚寒，咳嗽气急，逆气虚痞不能食，呕吐痰水。
款冬花（去枝梗） 荜茇 陈皮 干姜（炮） 良姜（炒） 甘草（炙） 细辛（去苗） 胡椒（各四两） 白术（五两）
以上研末，炼蜜制丸，如桐子大。每次服用三十丸，温汤送下。
The imperial consort of Emperor Huizong of Song suffered severely from phlegm disorders, with her face swollen like a plate. Physician Li used Concha Margaritifera (Concha Margaritifera, pearl shell powder), stir-fried on a new tile until red hot, then placed on the ground to eliminate fire toxicity. Each dose consisted of half tael added with a small amount of Qingdai (Indigo Naturalis), mixed with light pickled vegetable water and a few drops of Sesame Oil (Semen Sesami Album, black sesame oil), taken orally after mixing, resulting in recovery.
[Bencao] Treats wheezing and coughing. (Contraindicated for those with excessive sweating.)

Grind the above into powder, make pills using steamed wheat cake dough, size similar to hemp seeds.
Indications: Cough with rebellious Qi ascending, phlegm-fluid retention causing heart pain.
Haige (Haimenis Concha, Haishi clam shell) (Calcined into powder, ground extremely fine, left for several days until fire toxicity dissipates before use)
Gualou Ren (Trichosanthis Semen, Trichosanthes seed) (Ground together with the pulp)
Make into pills, size similar to hemp seeds, take with warm white water.
Yang Wen’s Formula for Treating Phlegm-Cough and Unblocking the Chest and Diaphragm
Select large Gualou (Trichosanthis Fructus, Trichosanthes fruit), cut open, remove seeds, wash clean, crush, scrape off skin, finely slice, and dry by baking. Banxia (Pinelliae Rhizoma, Pinellia rhizome) 49 pieces, wash ten times with hot water, crush, dry by baking, and grind into powder. Use the hot water from washing the Gualou fruit along with its pulp to boil into a paste, make pills size similar to paulownia seeds, take 20 pills with Ginger Decoction.
[Qianjin Fang] Wenspi Tang (Spleen-Warming Decoction) Treats cough induced by overeating.
Gancao (Glycyrrhizae Radix et Rhizoma, Licorice root) (4 liang)
Dazao (Ziziphi Jujubae Fructus, Jujube fruit) (20 pieces)
Chop the above two ingredients, add 5 sheng of water, boil to obtain 2 sheng, divide into three warm doses. If accompanied by sore throat and hoarseness, add Ganjiang (Zingiberis Rhizoma, dried ginger) 1 liang.
[Shengji Zonglu] Gandan Wan (Licorice-Pig Bile Pill) Treats persistent cough caused by acid sensation constricting the throat after consuming vinegar, unresponsive to other medicines.
Take Gancao (Glycyrrhizae Radix et Rhizoma, Licorice root) 2 liang, remove red skin, cut into 2-cun segments, split in half. Soak in 5 pig gallbladders for three days, remove, roast over fire until dry, grind into fine powder, make pills with refined honey. Take 40 pills per dose, swallow with clear tea water. Administer before sleep for miraculous effect. A case record notes someone suffering from this condition, unresponsive to other drugs, cured after one dose of this formula.
[Zhu Danxi] Phlegm-cough caused by alcohol damaging the lungs. Use Zhuli (Bambusae Caulis in Taenia, Bamboo sap) decocted with Jiuzhi (Allii Fistulosi Juice, leek juice), swallowing Gualou Ren (Trichosanthis Semen, Trichosanthes seed), Xingren (Armeniacae Semen Amarum, bitter apricot seed), and Huanglian Wan (Coptis Pill).
Treating wine-induced cough.

Make pills with honey and ginger, hold in mouth to dissolve, to disperse lung toxicity.
Food stagnation accumulation cough cannot be eliminated without Qingdai (Indigotidis Folium, Indigo naturalis) and Gualou Shi (Trichosanthis Fructus, Trichosanthes fruit). Patients with food stagnation present with fluctuating blue-white-yellow complexion, and crab-claw-like patterns on the face alternating yellow and white.
Qingjin Wan (Clear Metal Pill) treats food-stagnation cough. (Formula see "Deficiency-Excess" section).
Gualou Wan (Trichosanthes Fruit Pill) Treats wheezing and cough due to food-phlegm obstruction.

Zhigancao Tang (Roasted Licorice Decoction) Treats Feiwei (Lung Atrophy) with abundant saliva and sputum, and a feeling of warmth and fluidity in the heart.
Use Gancao (Glycyrrhizae Radix et Rhizoma, Licorice root) alone, add 3 sheng of water, boil to obtain 0.5 sheng, divide into three warm doses.
Guizhi Qu Shaoyao Jia Zaojiao Tang (Cinnamon Twig Decoction minus Peony plus Gleditsia Sinensis Fruit) Treats Feiwei (Lung Atrophy) with expectoration of saliva foam.
Guizhi (Cinnamomi Ramulus, Cinnamon twig)
Shengjiang (Zingiberis Rhizoma Recens, fresh ginger) (Each 3 liang)

Add 7 sheng of water to the above five ingredients, boil slowly over low heat to obtain 3 sheng, divide into three warm doses.
In Feiwei (Lung Atrophy) with expectoration of saliva foam but no cough, the patient is not thirsty and must suffer from enuresis and frequent urination. The reason is that upper deficiency fails to control the lower. This is cold in the Lung, inevitably presenting with vertigo and abundant saliva/foam; treat with Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) to warm it. If thirst occurs after taking the decoction, it belongs to Xiaoke (Wasting-Thirst syndrome).

Chop the above, add 3 sheng of water, boil to obtain 1.5 sheng, strain, divide into two warm doses.
The above treatments cover Feiwei (Lung Atrophy) symptoms: either coughing foam or coughing blood. Now, cases of coughing foam are included in this section, while coughing blood are included in the "Coughing Blood" section.
Heat in the Upper Jiao causes cough developing into Feiwei (Lung Atrophy). How is Feiwei (Lung Atrophy) acquired? The Master said: It may arise from excessive sweating, vomiting, Xiaoke (Wasting-Thirst syndrome) with frequent urination, or constipation followed by harsh purgatives causing severe loss of body fluids, thus leading to the disease.
[Mai Jing] Cough with natural saliva in the mouth and slippery tongue coating indicates floating Cold, not Feiwei (Lung Atrophy).
Cough with rebellious Qi, irritability, and restlessness indicates Feizhang (Lung Distension). Zhang Zhongjing stated: Cough with rebellious Qi is Feizhang (Lung Distension). If the pulse is floating and large, treat with Yesheng Banxia Tang (Ma Huang Decoction plus Banxia). He also stated: Feizhang (Lung Distension), cough with rebellious Qi, irritability, wheezing, and floating pulse indicate water retention below the heart; treat with Xiaolongtan Tang (Minor Blue Dragon Decoction) plus Shigao (Gypsum Fibrosum, gypsum). (Both formulas are seen in the "Wheezing" section).
[Zhu Danxi] For Feizhang (Lung Distension) with cough, use Hezi (Myrobalani Fructus, Chebula fruit), Qingdai (Indigotidis Folium, Indigo naturalis), and Xingren (Armeniacae Semen Amarum, bitter apricot seed). Hezi (Myrobalani Fructus, Chebula fruit) treats extreme damage to Lung Qi by Fire, leading to depression, distension, and inability to lie on one side for sleep; its sour and bitter taste has astringent and fire-reducing functions. Assist with Haishi (Turbinarum Concha, Turban shell), Xiangfu (Cyperi Rhizoma, Cyperus rhizome), Gualou Ren (Trichosanthis Semen, Trichosanthes seed), Qingdai (Indigotidis Folium, Indigo naturalis), Banxia Qu (Pinelliae Fermentatum, fermented Pinellia), ginger, and honey, held in the mouth. (Soak Xiangfu (Cyperi Rhizoma, Cyperus rhizome) in child's urine for three days).
For pulse depression, phlegm-cough, and restless sleep, use Qinghua Wan (Clearing Transformation Pill). Beimu (Fritillariae Bulbus, Fritillaria bulb) and Xingren (Armeniacae Semen Amarum, bitter apricot seed) ground into powder, made into pills with sugar and ginger juice steamed wheat cake, held in mouth to dissolve. (Qinghua Wan formula see "Fire Heat" section). Feizhang (Lung Distension) with cough, unable to sleep on either left or right side, is caused by phlegm obstructing Qi with blood stasis; nourish Blood to move Qi, descend Fire, soothe Liver, and clear Phlegm, using Siwu Tang (Four Substances Decoction) plus Taoren (Persicae Semen, peach seed), Hezi (Myrobalani Fructus, Chebula fruit), Qingpi (Citri Reticulatae Pericarpium Viride, green tangerine peel), Zhuli (Bambusae Caulis in Taenia, Bamboo sap), etc. If blood stasis obstructing Qi causes cough, use Taoren (Persicae Semen, peach seed), Dahuang (Rhei Radix et Rhizoma, rhubarb), and ginger juice pills.
One method: Moxibustion is suitable only for patients who can lie on one side. Those who can lie on the left should be moxa-ed at Zu Sanyinjiao (SP6, Three Yin Intersection of Foot-Taiyin), and those who can lie on the right at Zu Sanyinjiao (SP6, Three Yin Intersection of Foot-Taiyin); relief is immediate.
[Jing] Treats Qi-cough, also applicable for chronic cough. Hold one raw Hezi (Myrobalani Fructus, Chebula fruit) in the mouth, swallow saliva, and the illness will heal. If taste is lost after healing, drink Binglang Tang (Areca Seed Decoction) to restore taste.
[Zhu Danxi] Dingchan Jie Fang (Asthma-Seizing Emergency Formula)

[World Physicians] Treats phlegm-cough and wheezing. Fei Baifan (Alumen, alum) and Wubeizi (Gallae Chinensis, Chinese gall) in equal parts, ground into powder. Take 2 qian per dose, dip in pork liver, take before bed.
[Bencao] Transform Phlegm-Saliva Formula.
Mingfan (Alumen, alum, calcined) (2 liang)
Baijiangcan (Bombyx Batryticatus, dead silkworm) (0.5 liang, stir-fried, remove silk)
Grind into fine powder, grind fresh Bohe (Menthae Haplocalycis Herba, mint) until mushy, mix and make pills size similar to mung beans. Take 20 pills per dose with Bohe Tang (Mint Decoction), three times daily.
Treats long-standing and recent wheezing and cough. Use 3 large non-decayed Zaojiao (Gleditsiae Sinensis Fructus, Gleditsia sinensis fruit), chop off black skin, cut open, remove seeds. Insert one Badou (Crotonis Semen, croton seed) kernel into each seed cavity, bind tightly with hemp rope. Coat with natural ginger juice and honey, slow-roast, applying more coating and roasting until scorched yellow. Split open, remove Badou (Crotonis Semen, croton seed). Mix with 1 liang of well-quality calcined Mingfan (Alumen, alum) and 7 Biba (Ricini Semen, castor bean), coated with ginger juice and honey, roasted. Grind the first three ingredients into powder. Then, take 2 liang of Xingren (Armeniacae Semen Amarum, bitter apricot seed), remove skin and tips, grind into paste, and mix evenly with the previous powder. Take 1 qian per dose, dip in roasted persimmon cake (cooled), chew finely, take before bed. Avoid fish jerky, oily noodles, alcohol, rice vinegar, fried foods, and heat-toxic substances.
The above examples of Hezi (Myrobalani Fructus, Chebula fruit), Wuweizi (Schisandrae Chinensis Fructus, Schisandra berry), Wubeizi (Gallae Chinensis, Chinese gall), and Baifan (Alumen, alum). Danxi stated: Evening cough is due to Fire floating in the Lung; sour flavors astringe and descend it.

Spleen cough with pain in right hypochondrium, dull pain radiating to shoulder and back, severe cases inability to move, movement aggravates cough, use Shengma Tang (Cimicifuga Decoction). Stomach cough with vomiting, severe cases expulsion of roundworms, use Wumei Wan (Mume Pill). Kidney cough with pain radiating between waist and back, severe cases coughing saliva, use Mahuang Fuzi Xixin Tang (Ephedra, Aconite, and Asarum Decoction). [Suwen] Cough with vexation and resentment is due to rebellious Kidney Qi. (Shi Congrong Chapter). One Yang pathogenesis, shortness of breath, prone to cough, prone to diarrhea. Generally, cough with red face, constant heat in chest/abdomen/hypochondria, but sudden coolness in feet, with surging pulse, indicates Hot Phlegm in chest diaphragm; treat with Xiaoxianzhong Tang (Minor Trap-in-the-Chest Decoction), Mengshi Wan (Mengshi Pill) to clear diaphragm and descend phlegm. Severe and persistent cases require vomiting or purging of phlegm-heat. Pale face, sorrowful sneezing, or hypochondriac urgency/pain/distension, or deep/string-taut/thin/slow pulse with cough, indicates Cold Fluid in chest/abdomen; treat with acrid-hot herbs to dispel it.

Lung Metal is excessively excess, assisted by its son (Water). Heart, Spleen, and Lung all receive Cold pathogens; saliva retreats, sweetness appears at the anus, Water insults Earth, lacking fear. Abdominal great cold, disease named Han Zhong (Cold Middle), phlegm is white and foamy, Lung Qi deficiency leads to great cold, son assists mother. Should drain Kidney Water within the Lung; only acrid-hot herbs can dispel it. Sweet saliva and foam indicate Stomach Cold and disharmony; dispel with acrid-sweet-hot herbs. Drinkers often have this reverse transformation.
