Jingxian (Convulsive Epilepsy) refers to the conditions of Jijingfeng (Acute Convulsive Disorder) and Manjingfeng (Chronic Convulsive Disorder). However, when Jingxian attacks, the patient falls to the ground making sounds, and upon waking, spits out foam. In contrast, Jijing and Manjing do not make sounds nor spit foam.
When Yangxian (Yang-type Epilepsy) first begins, the patient's body becomes hot initially, followed by spasms, convulsions, crying, and shouting before the full onset. A floating pulse indicates Yangxian, which is Jijingfeng (Acute Convulsive Disorder). The pathogen is internally located in the Six Fu-organs and externally affects the skin; it is relatively easy to treat. If the patient's body becomes cold initially, without convulsions or crying/shouting upon onset, and with a deep pulse, this is Yin-xian (Yin-type Epilepsy), which is Manjingfeng (Chronic Convulsive Disorder). The pathogen is internally located in the Five Zang-organs and externally penetrates to the bone marrow; severe cases are difficult to treat.
[Qian Yi] When treating the five types of Xian (Epilepsy), one should treat according to the affected Zang-organ. Each Zang-organ corresponds to an animal, and the primary formula is Wuse Wan (Five-Color Pill).
Gouxian (Dog-type Epilepsy): Body arches backward and upward, barking like a dog; associated with the Liver.
Yangxian (Sheep-type Epilepsy): Eyes staring wide, tongue protruding, bleating like a sheep; associated with the Heart.
Niuxian (Ox-type Epilepsy): Eyes staring straight, abdominal distension, lowing like an ox; associated with the Spleen.
Jixian (Chicken-type Epilepsy): Startled jumping, body arching backward, hands flaccid, crowing like a chicken; associated with the Lung.
Zhuxian (Pig-type Epilepsy): Lying like a corpse, spitting foam, grunting like a pig; associated with the Kidney.
Wuse Wan (Five-Color Pill)
Cinnabar (ground, 0.5 liang) Mercury (1 fen) Realgar (calcined, 1 liang) Lead (3 liang, calcined with mercury) Pearl Powder (ground, 1 liang)
Process: Refine honey to form pills the size of hemp seeds. Dosage: 3–4 pills per administration, taken with Yinjin Bohe Tang (Silver and Mint Decoction).
[Zhang Yuansu] If there is exogenous phlegm-heat causing fever with audible symptoms, administer Wuse Wan (Five-Color Pill) with Dahuang Jingjie Tang (Rhubarb and Schizonepeta Decoction). If tidal fever occurs at regular intervals, indicating accumulated heat, Taozhi Wan (Peach Twig Pill) is indicated. If high fever persists, use Digupi San (Lycium Bark Powder) to administer Wuse Wan (Five-Color Pill). For wind-heat, use Fangfeng Huangqin Tang (Saposhnikovia and Scutellaria Decoction) to administer Daqing Gao (Indigofera Paste). If the body is warm but not hot, use Baihu Tang (White Tiger Decoction) plus Cangzhu (Atractylodes Rhizome) to administer Wuse Wan (Five-Color Pill).
[Bencao] Cihuang Wan (Realgar Pill) treats pediatric epilepsy impending attack, characterized by dim vision, spasms, harsh voice, and tongue biting.
Realgar Yellow (Cihuang) Minium (Huangdan) (each 1 liang) Musk (ground, 1 qian)
Process: Grind into fine powder, mix thoroughly. Add half a sheng of cow's milk and boil into a paste. Incorporate the powder and pound 300–500 times. Form pills the size of mung beans. Dosage: 3 pills per administration, taken with warm water, three times daily. This formula, obtained from famous physicians, is extremely effective.
[Tang] Shehuang Wan (Snake Gallstone Pill) treats Jingxian (Convulsive Epilepsy).
True Snake Gallstone (quench in vinegar, 3 pieces) Curcuma Tuber (Yujin) (3 fen, ground together) Musk (separately ground, 1 qian)
Process: Grind into powder. Use glutinous rice paste to form pills the size of sophora seeds. Dosage: 2–3 pills per administration, dissolved in Yinjin Modao Shui (Silver and Knife-Mossing Water) and taken.
[Hai] Ancient mirrors have a pungent taste and are non-toxic. They treat Jingxian (Convulative Epilepsy) and evil qi, as well as various malignant diseases in children. Boil to extract juice and decoct with other herbs for administration. Older mirrors are particularly effective.
Zhi Xian Fang (Formula for Treating Epilepsy) treats diseases affecting the Taiyang (Greater Yang) and Yangming (Bright Yang) meridians.
Schizonepeta Head (Jingjie Sui) (4 liang) Alum (Baifan) (ground to fine powder, 2 liang)
Process: Use jujube flesh to form pills the size of sophora seeds. Dosage: Start with 20 pills, taken with Jingjie Tang (Schizonepeta Decoction). Subsequently increase to 30, 40, and then 50 pills, all taken before meals.
[Luo] Chenxiang Tianma Tang (Agarwood and Gastrodia Decoction)
The four-year-old son of Wei Jingfu received a blessing touch on the head from an elder monk while monks chanted spells. He became greatly frightened, leading to convulsions and spasms. Phlegm-saliva obstructed the airway, eyes showed white sclera, neck and back were stiff and rigid, and there was sound in the throat. He woke after about one shichen (two hours). Later, he would attack whenever he saw men in black clothes. After taking over forty days of sedative and cooling drugs such as Cinnabar, Rhinoceros Horn, Dragon Bone, and Musk, the previous symptoms persisted, and new symptoms appeared: abnormal gait and movements, and a foolish mental state.
I palpated his pulse: deep, wiry, and rapid. The *Huangdi Zhenjing* (Yellow Emperor’s Classic of Acupuncture) states: "If the Heart pulse is full and large, it leads to epileptic tendon spasms." It also states: "If the Liver pulse is small and urgent, it leads to epileptic tendon spasms." Generally, because a child's blood and qi are not yet settled and their spirit/qi is weak, fright causes the spirit to lose its foundation and agitates the Liver. Since the Liver governs tendons, this results in epileptic tendon spasms. Prolonged illness depletes qi. Children are prone to both deficiency and excess. Overuse of sedative and cold/cooling agents further damages qi, leading to the foolish movements. The *Neijing* (Inner Canon) states: "For sudden spasms, epilepsy, dizziness, and inability of the feet to support the body, select Tianzhu (BL10)." Tianzhu (BL10) is where the Foot-Taiyang meridian emerges, and the Yang Qiao vessel attaches and travels. It also states: "For epilepsy and spasms where the patient does not know their suffering, the two Qiao vessels govern it. For males, treat Yang Qiao; for females, treat Yin Qiao." Jiegu states: "For daytime attacks, treat Yang Qiao and Shenmai (BL62); for nighttime attacks, treat Yin Qiao and Zhaohai (KI6). First, moxa both Qiao vessels, 7 moxa cones each." Then prescribe:
Agarwood (Chenxiang) (2 qian) Gastrodia Rhizome (Tianma) (3 qian) Notopterygium Rhizome (Qianghuo) (5 qian) Saposhnikovia Root (Fangfeng) (3 qian) Bitter Apricot Seed (Yizhi) (2 qian) Sichuan Aconite Root (Chuanwu) (processed, skin and core removed, 2 qian) Licorice Root (Gancao) (roasted) Angelica Sinensis Root (Danggui) (each 1.5 qian) Pinellia Tuber (Banxia) (soaked in boiling water, 3 qian) Actaea Rhizome (Duhuo) (4 qian) Prepared Aconite Root (Fuzi) (processed, skin and core removed, 3 qian) Ginger Scrapings (Jiangxie) (1.5 qian)
Process: Cut into pieces. Dosage: 5 qian per administration, with 3 slices of fresh ginger, decocted in water and taken warm. The *Jutong Lun* (Treatise on Pain) states: "Fear causes qi to sink, essence becomes timid, and the Upper Jiao closes." It also states: "For pathogens rising from below, guide them downward and expel them." Qianghuo and Duhuo are bitter and warm, with thin flavors, representing yang within yin. They guide qi upward and enter the Taiyang meridian as guiding herbs, thus serving as the Sovereign (Jun) herb. Tianma and Fangfeng are acrid and warm to disperse; Danggui and Gancao are acrid, sweet, and warm to tonify deficient qi and blood, and nourish Stomach qi, thus serving as Ministers (Chen). Black Fuzi and Chuanwu are highly acrid and warm, promoting yang and dispelling yin/cold, and treating cold invading the Stomach. The Kidney governs the five fluids; when entering the Spleen, they transform into saliva/phlegm. Fresh ginger and Banxia dry dampness and resolve phlegm. The *Shi Ji* (Ten Categories) states: "Heavy substances remove timidity." Agarwood is acrid and warm, heavy in substance but clear in qi, removing timidity and calming the spirit, thus serving as an Envoy (Shi) herb. The combined flavors promote ascending yang and tonifying the Stomach; the qi of fear and timidity naturally becomes balanced.
