During pregnancy, conception occurs when essence and spirit combine in the Kidney (Kidney), receiving congenital Qi (vital energy) to form the embryo; nourishment relies on the Spleen (Spleen), utilizing acquired Qi to grow and develop. Thus, the Kidney and Spleen are the fundamental roots of generation from beginning to end. However, in treatment, if one does not specifically emphasize these two meridians, but instead begins by clearing fire and ends by regulating Qi, thereby ensuring fetal Qi remains peaceful and maternal Qi remains robust, why is this? It is unknown that without this Fire (metabolic/physiological heat), Heaven cannot generate all things, and humans cannot sustain life. Conception between man and woman relies on the Fire of the Gate of Vitality (Mingmen), planting vitality before it becomes formless, and after the aggregation of essence and blood, it forms a physical body. Although this is primarily governed by Kidney Yin (Kidney Yin), the Chong Mai (Penetrating Vessel), Ren Mai (Conception Vessel), and Jueyin (Liver/Pericardium) meridians all nourish the uterus (Bao Gong), yet the Fire within them tends to rush upward and rebel. Therefore, upon pregnancy, women experience nausea, vomiting, aversion to tastes, and cessation of menstruation. This is because the Chong Mai, the Sea of Blood, declines, while the Fire in the uterine collaterals flares up; the Zong Qi (Gathering Qi) of the Ren Mai weakens, and the Yangming Stomach Qi cannot flourish alone. The *Suwen* states: "All rebellious upward rushing symptoms belong to Fire." It also says: "Shao Huo (Lesser Fire/Physiological Fire) generates Qi, Zhuang Huo (Excessive Fire/Pathological Fire) consumes Qi." Therefore, within the first hundred days, if urgent tonification of Earth (Spleen/Stomach) fails to allow food intake, often clearing fire and calming rebellion brings relief. By the fourth or fifth month, diet gradually improves, but maternal Qi declines daily. Once maternal Qi declines, it cannot constrain the fetus, causing fetal Qi to expand significantly. At this stage, Baizhu (Atractylodis Rhizoma) must be used to strengthen maternal Qi, Huangqin (Scutellariae Radix) to nourish fetal Yin, along with Chenpi (Citri Reticulatae Pericarpium) to promote Qi flow, so the fetus can stabilize. For example, "Zi Fan" (Fetal Agitation/Restlessness) is caused by Heart Fire; "Zi Sou" (Fetal Cough) by Lung Fire; "Zi Xuan" (Fetal Suspension/Chest Distension) by Liver Fire; "Zi Zhong" (Fetal Edema) by Spleen Fire. These should be treated with herbs such as Maidong (Ophiopogonis Radix), Huangqin, Sangbaipi (Mori Cortex), Ziwan (Asteri Radix), Shengdi (Rehmanniae Radix), and Zhimu (Anemarrhenae Rhizoma), selected according to symptoms, as they are invariably caused by internal heat. By the seventh or eighth month, maternal Qi is even weaker, and maternal Blood is deficient. One must use Bazhen Tang (Eight Treasure Decoction) to tonify Qi and Blood, along with Ejiao (Asini Corii Colla), Huangqin, Chenpi, Xiangfu (Cyperi Rhizoma), and Sharen (Amomi Fructus), both securing and unblocking, making maternal Qi robust. Although the fetus grows, it is constrained properly. Because maternal Qi and Blood can transport it, fetal Qi will not over-expand, preventing discomfort for the mother. By the ninth or tenth month, Qi and Blood must be tonified, while fetal Qi must be unblocked. If Qi and Blood are not tonified, there is no foundation for childbirth; if fetal Qi is not unblocked, there is a risk of difficult labor. Thus, Danxi stated that taking Daseng San (Deliverance Decoction) several doses within the eighth and ninth months is very beneficial. Daseng San uses Dafupi (Arecae Pericarpium) as the sovereign herb to diffuse Qi through the maternal abdomen and circulate around the fetus; assisted by Zisu (Perillae Folium) and Chenpi, and tonified by Renshen (Ginseng Radix), Baizhu, Danggui (Angelicae Sinensis Radix), and Shaoyao (Paeoniae Radix) without stagnation. If there is heat, add Huangqin; if irritability, add Shengdi to nourish Yin and Blood, ensuring throughout that Fire does not consume them. I have never heard of difficulty in delivery at birth due to this. Even from conception to full term, occasionally external pathogens invade or Seven Emotions cause injury. One must distinguish severity and urgency, prioritizing fetal security while simultaneously resolving pathogens, treating according to symptoms. The *Neijing* states: "If there is pathology, there is no injury; nor is there injury [to the fetus]." If one attacks without pathology, how could there be no injury?
Regarding anorexia during pregnancy, some call it "Zu E" (Obstruction Nausea). The classics state that pregnant women dislike tastes within a hundred days. In the first one or two months of pregnancy, the Foot Jueyin (Liver) and Foot Shaoyang (Gallbladder) meridians nourish the fetus. Jueyin and Shaoyang belong to Wood, containing Fire, prone to vomiting. Women are easily angered; those with excess Liver Wood tend to desire astringency, hence preferring sour foods. Danxi treated a woman with incessant vomiting who worsened after taking Renshen and Baizhu. He prescribed Yiqing Wan (Suppress Green Pill), which resolved it. This illustrates the principle. Another formula uses Huoxiang (Agastaches Herba), Houpo (Magnoliae Officinalis Cortex), Fuling (Poria), Zhigancao (Prepared Licorice Root), Guangpi (Tangerine Peel), and Zhuru (Bambusa Caulis in Taenia). Add Huangqin and Zhizi (Gardeniae Fructus) for heat; Danggui and Shaoyao for Blood deficiency; Renshen and Baizhu for Qi deficiency. Banxia (Pinelliae Rhizoma) may be added despite folk beliefs that it harms the fetus. The *Qianjin Fang* frequently uses it, and processing via decoction soaking, oil frying, or roasting mitigates risks.
In the third or fourth month of pregnancy, the Hand Shaoyin (Heart) and Hand Taiyang (Small Intestine) meridians nourish the fetus. Shaoyin and Taiyang belong to Fire. Danxi said: "Fire consumes matter; this is natural transformation." Hence, miscarriages occur at three or four months. Tonification must include clearing Fire. For "Zi Xuan" where the fetus presses on the heart, take equal parts Huangqin and Xiangfu powder (2 qian each) mixed with water for relief.
In the fifth or sixth month, the Foot Taiyin (Spleen) and Foot Yangming (Stomach) meridians nourish the fetus. Taiyin and Yangming belong to Earth. The fetus steals Spleen Qi for self-nourishment, further depleting Earth. Shutai Wan (Fetus-Binding Pill) is a sacred medicine for stabilizing the fetus. Add Danggui and Shaoyao for Blood deficiency; Renshen, Fuling, and Gancao for Qi deficiency; Ejiao for threatened abortion.
In the seventh or eighth month, the Hand Taiyin (Lung) and Hand Yangming (Large Intestine) meridians nourish the fetus. Taiyin and Yangming belong to Metal, governing Qi. Use Bazhen Tang plus Huangqin, Ejiao, and Duzhong (Eucommiae Cortex). Or add Zisu and Chenpi to promote Qi flow.
In the ninth or tenth month, the Foot Shaoyin (Kidney) and Foot Taiyang (Bladder) meridians nourish the fetus. Qi accumulation is nearly complete. Pre-administer Daseng San and Dushen Tang (Single Ginseng Decoction) to supplement True Qi, facilitating easy delivery. Renshen tonifies Lung Qi; Metal generates Water, strengthening the Shaoyin Kidney.
For "Zi Fan," use Maidong, Zhimu, Fuling, and Huangqin. For "Zi Zhong," use Wupi Yin (Five Peel Decoction) plus Baizhu, Cheqianzi (Plantaginis Semen), Huangqin, and Zhike (Aurantii Fructus). Adjust per symptoms. For "Zi Sou," use Maidong, Ziwan, Sangbaipi, Zhimu, Jiegeng (Platycodon Radix), and Zhuru. Add Ejiao if there is bleeding. For external pathogen coughs, use exterior-resolving methods.
For malaria, dysentery, vomiting, or diarrhea during pregnancy, use standard methods but avoid harsh purgatives that damage fetal Qi.
For injuries from Seven Emotions or overwork/dietary irregularities, use modified Buzhong Yiqi Tang, Guipi Tang, or Xiaoyao San.
For "Zhuan Bao" (reversed bladder causing urinary difficulty), use lifting and Qi/Blood tonification. If Liver Meridian depression-heat exists, use Xiaochaihu Tang plus Zhizi, Shengdi, and Mutong (Akebiae Caulis). If uterine prolapse occurs, also attributed to Liver Fire, use simultaneous lifting and clearing for efficacy.
Women with constitutional deficiency-cold must not use cold-cooling medicines during pregnancy; warm-tonification is required. If illness results from excessive cold medicine or raw/cold food consumption, hot medicines must be used to reverse it, disregarding standard prohibitions.
