Lu Cangzhou treated a young girl. She suffered from lethargy (excessive sleepiness), with red cheeks but no body heat. All physicians considered it to be slow convulsion syndrome (a pediatric condition characterized by convulsions and lethargy) and repeatedly administered wind-dispelling agents, yet after twenty days there was no improvement. Lu palpated her pulse and found that only the right Guan position was slippery and rapid, while other positions were equal in size and harmonious. He informed them, "The girl is not ill. A slippery pulse at the Guan position indicates retained food stagnation. I suspect this was caused by the wet nurse. The wet nurse must have a fondness for alcohol and breastfeeds the child immediately after drinking, causing the child to become 'intoxicated.' This is not due to wind pathogen." Upon questioning the mother, she admitted, "The wet nurse recently held the key to the wine cellar and secretly drank until satisfied." An inspection revealed several empty wine jars in the sleeping quarters. The key was confiscated, and the child was given a decoction of *Armeniacae Semen Amarum* (Zhiqi Zi) and *Puerariae Radix* (Gehua). Administered two to three times daily, the child recovered as usual.
The son of Temple Attendant Li, aged three, experienced convulsions lasting from Mao hour to Si hour. Multiple physicians failed to produce results. Qian Yi examined the child and observed convulsions, eyes deviated to the right, and loud crying. Mr. Li asked, "Why are the eyes deviated to the right during convulsions?" Qian Yi replied, "This is a counter-indication." Mr. Li asked for clarification. Qian Yi explained, "Males belong to Yang and typically manifest symptoms starting on the left; females belong to Yin and typically manifest on the right. Therefore, males look to the left when ill and are silent during convulsions; looking to the right with sound indicates a different pattern. Females look to the right silently during convulsions, and look to the left with sound. This is because the Left corresponds to Liver Wood and the Right to Lung Metal. If a male looks to the right, it indicates Lung Metal overcoming Liver Wood; Metal punishing Wood causes conflict between the two organs, resulting in sound. The treatment method should be to drain the excess and tonify the deficiency. If Heart Qi is excess, it should also be drained. Lung Qi deficiency cannot be drained. Signs of Lung Qi deficiency include chest stuffiness, restlessness, sighing, and long exhalations. In this case, the male exhibits female characteristics, so treating males is easier than females. Suppose a female has convulsions and looks to the left; this indicates Lung Metal overcoming Liver Wood. If the disease occurs in Autumn, when Lung Metal is prosperous, Liver Wood cannot resist, leading to loud crying. One should strongly drain Lung Metal, then treat the Heart, and subsequently nourish the Liver. The reason both cases mention eye deviation is that the Liver governs the eyes. Generally, convulsions result from Wind-Heat fighting internally. Wind belongs to the Liver, hence it manifests in the eyes." Qian Yi used Xie Gan Tang (Drain Liver Decoction) for treatment. After two days, without chest stuffiness or restlessness, it was known that the Lung pathology had receded. Subsequently, Dihuang Wan (Rehmannia Pill) was used to tonify the Kidney. After three doses, Xie Qing Wan (Drain Green Pill) and Liang Jing Wan (Cool Convulsion Pill) were each administered twice. Using Heart and Liver draining medications led to recovery in five days, without erroneous treatment. When asked why Lung deficiency should not be drained, Qian Yi said, "Suppose a male looks to the left; this is Wood counter-overcoming Metal, where Liver excess overcomes Lung. It is appropriate to only drain the Liver. If the illness occurs in Spring or Summer, when Metal Qi is extremely deficient, one should tonify the Lung and not drain it."
Court Physician Qian Ying, during the Xuande reign, treated the grandson of Marquis Ningyang. The infant, nine months old, suffered from palpitations, crying, and sweating, which could not be cured by various methods. Qian Ying finally examined the patient and instructed placing the child on the ground to play with water. The fright and crying stopped immediately. When asked for the reason, he said, "It was late spring. The child wore heavy clothes and thick curtains, never leaving the embrace. Heat was constrained internally, unable to vent. Bringing him near water suppressed the Fire pathogen; gaining Earth energy (from the ground/soil) pacified the Zang Qi. The disease healed; why use medicine?"
Shi Shan treated a six-year-old girl who had been unable to move her left hand for three years. Later, she developed epilepsy. Initially treated with *Ren Shen* (Ginseng) and *Ban Xia* (Pinellia), with variable efficacy. Wang diagnosed a floating and surging pulse on the left and a relatively harmonious pulse on the right, stating, "This is Phlegm-Heat." He ordered using silk to bind the abdomen, taking three qian of shelled tea seeds, crushing them, and filtering through a bowl of boiling water. After fasting overnight, the juice was taken warm in the morning. Approximately three bowls of phlegm were vomited up, the hand regained movement, and epilepsy ceased.
Fang Yingshan treated the son of the Zhu family, aged eight or nine, who was staying with his maternal grandparents. He was fed rice mixed with meat broth. While holding rice in his mouth, unswallowed, he ran and fell, resulting in locked jaw and convulsions of hands and feet. Medical treatment was ineffective. Seven days passed, and even medicines like Kai Guan were forced into his mouth, opening the jaw only slightly, while limb convulsions, fever, lethargy, and muteness persisted. The pulse was surging and slippery. Fang arrived and prescribed *Shi Gao* (Gypsum), *Qing Dai* (Indigo Naturalis), *Gan Cao* (Licorice), *Chen Pi* (Tangerine Peel), *Nan Xing* (Arisaema), *Tian Ma* (Gastrodia), *Bo He* (Mint), *Zhu Ling* (Polyporus), *Ze Xie* (Alisma), *Bai Zhu* (Atractylodes), *Fu Ling* (Poria), *Dou Ling* (Loquat Leaf), *Yuan Shen* (Scrophularia), and *Huang Qin* (Scutellaria), plus one slice of ginger. That night, the child slept soundly without moving, urinating only once. Fever subsided, body cooled, and pulse became tranquil. After one more dose, full recovery was achieved.
Qian Cun treated the nephew of Sun Zifan from Xiantan, diagnosed with Slow Spleen Syndrome (a critical pediatric condition of severe Spleen-Yang deficiency). Symptoms included abundant salivation, cough, fever, limb convulsions, spontaneous sweating, lethargy with eyes open showing whites, grasping at air, and hand tremors. Repeated administration of Spleen-tonifying combined with phlegm-resolving and wind-expelling herbs yielded no effect. Consultation with Dr. Yang ensued. Yang stated, "The wind in this syndrome arises internally and cannot be expelled. The phlegm moves due to deficiency and need not be resolved. Only by tonifying Spleen Earth will all symptoms recede. Based on the accompanying signs, his complexion must be pale and lusterless, eyes dilated, tongue fat and slippery, color tender white, neck soft, and head drooping." The father replied, "Indeed. But why did rescue via tonification fail?" Yang answered, "All symptoms indicate Cold, whereas previous formulas only addressed Deficiency. If the Tianzhu bone (neck support) had not collapsed, efficacy might have been seen, provided wind-expelling and phlegm-resolving agents were removed. Now, with a soft neck and drooping head, the Tianzhu has collapsed, indicating Deficiency compounded by Cold, with clear evidence. Without Pao Jiang (Prepared Ginger) and Rou Gui (Cinnamon Bark), how can we retrieve the departing Yang and revive the nearly extinct Qi?" Thus, Shenyang Yangying Tang (Tonify Qi and Nourish Blood Decoction containing Ginseng and Aconite) was prescribed, with the warning: "If anyone objects based on the premise that infants lack Yang and thus cannot receive Yang-tonifying methods, less than one in a hundred will survive." After three doses, complete recovery occurred. Subsequently, Wuwei Yigong San (Five-Ingredient Efficacious Powder) with baked ginger and Bai Shao (White Peony Root) was used for regulation, leading to robust health.
