Pulse
Postpartum regulation should focus on supporting deficiency and eliminating blood stasis; the pulse at this time should appear deficient. Wang Shuhe stated: A postpartum pulse that is slow and smooth is auspicious, whereas a solid, large, wiry, or rapid pulse portends death. If the Cun Kou (radial artery) pulse is extremely rapid and irregular, it indicates death; if it is deep and fine but unbroken upon deep palpation, it indicates survival.
Symptoms
Women experiencing fever after childbirth may have various causes: excessive blood loss, incomplete expulsion of lochia, dietary injury, contraction of wind-cold, cold complicated by food stagnation and qi stagnation, or milk secretion (steaming milk) occurring three days postpartum. All these conditions can cause fever, aversion to cold, body pain, and abdominal pain. The presentations differ significantly, and treatments cannot be applied indiscriminately.
Treatment
For fever caused by excessive blood loss postpartum, the pulse will likely be large and weak without force. In the absence of internal pain, this is not excess heat but deficiency heat generated by yin insufficiency. Use Siwu Tang (Four Substances Decoction) removing Shaoyao (Paeonia Lactiflora), adding Renshen (Ginseng), Baizhu (Atractylodes Macrocephala), and Fuling (Poria). This promotes diuresis to drain heat. If high fever persists, add Chao Hei Ganjiang (Charred Dried Ginger), which is highly effective. Ganjiang is acrid and hot; it guides blood-tonifying herbs into the blood level to generate new blood, and guides qi-tonifying herbs into the qi level to replenish qi. Alternatively, using Xionggui Tiaoxue Yin (Cyperi and Angelica Blood-Regulating Drink) is excellent. Any fever caused by overexertion or early morning labor follows this same treatment principle.
First, incomplete expulsion of lochia accompanied by fever and aversion to cold necessarily involves distension in the hypochondrium and painful masses in the lower abdomen, known as "Erzhen Tong" (Child's Pillow Pain). Postpartum abdominal pain due to blood stasis is treated with Siwu Tang (Four Substances Decoction) added with Wulingzhi (Trogopterus Dung), Mudanpi (Moutan Cortex), Taoren (Peach Kernel), Honghua (Safflower), Yuanhusuo (Corydalis Rhizome), Xiangfu (Cyperus Rhizome), Qingpi (Green Tangerina Peel), Ganjiang (Dried Ginger), Guigui (Cinnamon Twig), wine, and water (one qian each), plus a handful of Heidou (Black Soybean). Afterward, grind Muxiang (Aucklandia Root) and administer with Tongbian (Infant Urine) and Jiangzhi (Ginger Juice). Efficacy is confirmed by the discharge of foul matter. Using Heishen San (Black Divine Powder) is even better, followed by调理 (regulation) with Bawu Tang (Eight Substances Decoction) added with Ganjiang (Dried Ginger) and Chenpi (Tangerina Peel), lightly supplemented with Tongbian (Infant Urine) and Chao Xiangfu (Stir-fried Cyperus Rhizome).
Second, postpartum spleen and stomach weakness leads to reduced appetite and poor digestion, causing stagnation fever. Symptoms include belching with acid regurgitation, aversion to food odors, tastelessness, chest fullness, and a tight, vigorous pulse at the Kou position, accompanied by fever, aversion to cold, and headache. Zhitong Tang (Regulate the Middle Decoction) added with Shenqu (Medicated Leaven), Shanzha (Hawthorn Fruit), and Sharen (Amomum Fruit), assisted by Chao Huanglian (Stir-fried Coptis Rhizome), Chuanxiong (Ligusticum Rhizome), and Danggui (Angelica Sinensis) is required. Lizhi San (Regulate Spleen Powder) is more effective.
Third, postpartum deficiency of Ying (Nutritive Qi) and Wei (Defensive Qi) results in loose interstices. If wind-induced fever occurs, the pulse is floating and slightly weak, possibly with spontaneous sweating. Xiongzhi Xiangsu San (Cyperi, Chrysanthemum, and Perilla Decoction) added with Qianghuo (Notopterygium Rhizome) and Fangfeng (Saposhnikovia Root) is indicated. For cold invasion, characterized by a wiry and tight pulse or obstructed lochia, Wuji San (Five Accumulations Powder) is used. For simultaneous wind-cold invasion, with a floating and tight pulse, Wuji Jiajiao San (Five Accumulations Combined Powder) is used. Remove Mahuang (Ephedra) if there is sweating; remove Renshen (Ginseng) if pathogenic factors are strong.
Fourth, for postpartum internal dietary injury combined with external wind-cold, or fever exacerbated by anger, where both Renying (Carotid pulse) and Kou (Radial pulse) positions are tight and vigorous, Xingqi Xiangsu San (Qi-Moving Perilla Decoction) is prescribed.
Fifth, fever and aversion to cold due to "steaming milk" (initial lactation) involve swollen, hard, and painful breasts. Massaging the breasts to promote milk flow resolves the fever without medication.
Sixth, postpartum blood syncope has three causes: fainting from excessive exertion, fainting from excessive bleeding, and fainting from minimal bleeding. Although the symptom is the same, treatments differ. For fainting due to excessive bleeding, tonify blood using Xionggui Tang (Cyperi and Angelica Decoction) as the main formula. If lochia does not stop, double the dose of Charred Ganjiang (Dried Ginger) to stop bleeding. For fainting due to minimal bleeding, use Mingdan (Life-Snatching Pill). For unconsciousness due to blood syncope, place hot charcoal near the mother and pour vinegar on it so the vapor enters the patient's nose and mouth; mild cases recover immediately, severe cases regain consciousness.
Seventh, after fresh delivery, Wuji San (Five Accumulations Powder) should be used to eliminate stale blood, generate new blood, harmonize Ying and Wei, nourish the zang-fu organs, balance yin and yang, and prevent pathogenic interference, thereby avoiding chills and fever. It also treats postpartum qi deficiency, external cold invasion, headache, body pain, fever, and aversion to cold. Add a small amount of rice vinegar to the decoction; remove Mahuang (Ephedra); add Huangqin (Scutellaria Root) if heat is severe.
Eighth, postpartum facial paralysis with locked jaw (Jinkou) results from blood deficiency allowing wind to invade the cheeks and mouth, causing tendon tension. If opisthotonos (arched back) occurs, it is due to wind invading the Yang meridians in a weak body, causing spasm of the waist and back. Treat with Siwu Tang (Four Substances Decoction) added with Qinjiao (Gentiana Macrophylla Rhizome) and Qianghuo (Notopterygium Rhizome). Also, stir-fry Jingjie (Schizonepeta Spike) until slightly charred, powder it, and take two qian mixed with wine from black soybeans or infant urine. Another prescription uses equal parts Danggui (Angelica Sinensis) and Jingjie (Schizonepeta Spike), decocted in water and wine. If the jaw is locked, pry it open gently and administer slowly; efficacy is immediate upon swallowing.
Ninth, postpartum fever, aversion to cold, or hemifacial paralysis indicate extreme blood deficiency. Urgently tonify qi and blood. If the left hand pulse is insufficient, emphasize blood-tonifying herbs over qi-tonifying ones; if the right hand pulse is insufficient, emphasize qi-tonifying herbs.
Tenth, postpartum aphonia (loss of voice) is caused by blood invading the heart and lung orifices and wind pathogens lodging in the epiglottis. Use Fuling Buxin Tang (Poria Heart-Nourishing Decoction) removing Chenpi (Tangerina Peel), Zhike (Aurantium Fructus), and Chuanxiong (Ligusticum Rhizome), adding Shengma (Cimicifuga Rhizome), Fangfeng (Saposhnikovia Root), Bohe (Mentha Haplocalyx), Chishao (Red Peony Root), Danggui (Angelica Sinensis), Shengdi (Rehmannia Root), Honghua (Safflower), Huanglian (Coptis Rhizome), Dansheng (Arisaema Cum Bile), and Shengjiang (Fresh Ginger).
Eleventh, postpartum mutism is caused by stale blood confusing the heart orifices. Use Siwu Tang (Four Substances Decoction) added with Chencha (Cinnabar), Shichangpu (Acorus Rhizome), Honghua (Safflower), and Renshen (Ginseng).
Twelfth, postpartum abdominal pain from blood masses is often due to sitting on the ground during confinement, allowing cold-dampness and wind to invade, causing blood stasis and mass formation. Treat with Buhuanjin Zhengqi San (Golden Exchange Upright Qi Powder) added with Lagui (Cinnamon Bark), Chuanxiong (Ligusticum Rhizome), Baizhi (Angelica Dahurica Root), Ezhu (Curcuma Rhizome), and Ganjiang (Dried Ginger). Decoct and add vinegar while hot; take two doses consecutively. As cold-dampness and wind dissipate, the mass disappears, and stasis exits via the stool.
Thirteenth, fetal death in utero (Shijia) presents after six months of amenorrhea with sudden lower abdominal pain and a stone-like hardness in the vagina. This is Shi Xia (Stone Fibroid). Use Siwu Tang (Four Substances Decoction) added with Taoren (Peach Kernel), Dahuang (Rhubarb Root), Binglang (Areca Seed), Sanleng (Sparganium Rhizome), Yuanhusuo (Corydalis Rhizome), Zexie (Alisma Rhizome), and Xuexie (Dragon's Blood). Decoct in water.
Fourteenth, postpartum blood deficiency fever with cough, sputum, wheezing, palpitations, and dry mouth is treated with Fuling Buxin Tang (Poria Heart-Nourishing Decoction) added with Maidong (Ophiopogon Tuber) and Wuweizi (Schisandra Berry).
Fifteenth, postpartum uterine prolapse is treated with Buzhong Yiqi Tang (Middle-Boosting Qi-Rescuing Decoction) added with Baishao (White Peony Root), Cu Chao Xiangfu (Vinegar-stir-fried Cyperus Rhizome), Banxia (Pinellia Tuber), and Jiu Chao Huangqin (Wine-stir-fried Scutellaria Root). If fever persists, add Jiubai (Wine-processed Phellodendron Bark). Alternative methods include fumigation with Duanshi (Calcined Alum) or washing with Jingjie (Schizonepeta Spike), Huoxiang (Patchouli Herb), and Chouchunpi (Toona Leaf Bark).
