A person suffers from abdominal distension and fullness, frequent belching, distension and fullness in both hypochondriac regions, and obstruction in the throat and diaphragm. This is due to Liver Qi excess. The pulse is wiry and long. It is appropriate to cool the Liver.
A person suffers from abdominal enlargement, pain in the lower abdomen, aversion to cold and fever resembling malaria, red urine, and a surging and large pulse. This is due to Heart Heat. It is appropriate to cool the Heart.
A person suffers from pain in the lower abdomen, somnolence, inability to urinate, headache, eyes feeling as if they are protruding, vertex feeling as if being pulled, and waist feeling as if broken. This is due to Spleen Dampness excess. The pulse should be turbid and restless. It is appropriate to warm the Spleen.
A person suffers from distension and fullness on the right side, swelling of the fu organs, heart pain, Lung pathology, abdominal distension and fullness with bloating, panting, and coughing. This is due to Qi being overcome by Fire. The pulses at the Cun Kou (radial artery) on both hands are surging and large; the Lung pulse is also surging. The bone-force is weak, similar to the Heart pulse. This indicates a stalemate between Lung and Heart Qi. It is appropriate to resolve the heat of Metal (Lung) and Fire (Heart) to balance the Lung and Heart.
Herbs: Huang Qin (Scutellariae Radix, 1.5 liang), Maidong (Ophiopogonis Tuber, heart removed), Fuling (Poria), Ziwan Cao (Asturi Herba, prepared, 4 zhu).
Preparation: Grind the above herbs into a coarse powder. Take at any time. Decoct 3 qian of the powder with one sheng of water, one slice of Shengjiang (Zingiberis Rhizoma), and one Zao (Jujubae Fructus). Boil down, strain, and take the decoction.
A person suffers from edema of the body and abdomen, orthopnea (breathing with head elevated), dysentery with red and white discharge, and hemoptysis. This is due to Shaoyang Phase-Fire excess, presenting similarly to Fire excess syndromes, including the pulse. Treat with the aforementioned formula.
People generally discuss that all cases of panting (asthma/dyspnea) are attributed to the Lung. However, some do not recover after taking Lung-targeted medicines, leading to the conclusion that "the Lung does not accept medicine," which is a difficult-to-treat condition. How erroneous is this statement! Others attribute panting to Lung Heat and mistakenly administer cooling medicines. While this statement is somewhat plausible, it only explains Lung-related panting and fails to clarify panting arising from the Five Zang organs. Furthermore, according to the Classics, "all flaccidity syndromes, panting, and vomiting belong to the Upper Jiao." Panting has never been solely attributed to the Lung. Regarding the variations of diseases in the Five Zang and the vicissitudes of the Six Qi, the origin of panting can be clarified by pointing out its manifestations, and the administration of medicines can be effective by following the specific syndrome.
Now, a person suffers from vexation and heat in the chest, dry throat, distension and fullness on the right side, coughing, panting, and spitting blood, Lung pathology, abdominal distension and bloating with panting. This is due to Fire punishing Metal (Lung), causing Lung injury and panting. Use this formula.
Herbs: Maidong (Ophiopogonis Tuber, heart removed, 1.5 liang), Jiegeng (Platycodonis Radix), Mahuang (Ephedrae Herba, nodes removed), Ziwan Cao (Asturi Herba, prepared).
Preparation: Grind the above herbs into a coarse powder. Take 3 qian per dose. Decoct with one sheng of water and three slices of Shengjiang (Zingiberis Rhizoma) until reduced to 70%. Strain and take warm.
A person suffers from pain in the left hypochondrium, internal coldness, coughing, and panting. This is caused by damage to the Clear Qi of the Lung. Use this medicine. The six pulses are fine and slightly floating (mao); the Kidney pulse is deep and small. This differs from panting caused by Fire excess punishing Metal. Generally, when Fire excess punishes Metal causing panting, the six pulses are purely Fire pulses: rapid, large, and forceful. If the Cun Kou pulse is disproportionately large and the Guan pulse presents with a hollow (Kou) quality, hemoptysis must occur.
A person suffers from abdominal enlargement, leg edema, panting, coughing, night sweats, aversion to wind, chest fullness, indigestion, decreased appetite, and heaviness of the body. The six pulses are heavy and turbid, rushing like a bundle (hunhun ge zhi), as if restrained. This is due to excessive Kidney Cold. It is appropriate to warm the Kidney.
Herbs: Xuduan (Dipsaci Radix), Niuxi (Achyranthis Bidentatae Radix), Xixin (Asari Radix et Rhizoma), Wuzi (Schisandrae Fructus) (each 1.5 liang), Muxiang (Aucklandiae Radix, 3 zhu).
Preparation: Make pills the size of wu zi (wuze tree seeds) using refined honey. Take 50-70 pills on an empty stomach with salt-rice soup, administered together with Shenhe San.
A person suffers from Liver Wind congestion, with accumulated phlegm-saliva lodging between the diaphragm. Symptoms include dry mouth with sticky saliva, nausea upon eating, complete aversion to meat odor, mental restlessness, inability to sleep properly at night, throat obstruction as if blocked by an object, and frequent panting. Diagnosis reveals all six pulses are large and deep/floating. With heavy pressure, the pulse is felt faintly near the bone. Repeated searching reveals a pulse that comes rapidly and departs slowly. It is appropriate to use medicines to treat phlegm-saliva.
Herbs: Jingjie Sui (Schizonepetae Spica), Tiannanxing (Arisaematis Rhizoma), Fangfeng (Saposhnikoviae Radix), Qianghuo (Notopterygii Rhizoma et Radix), Bai Jiangcan (Bombyx Batryticatus), Lianqiao (Forsythiae Fructus), Duhuo (Angelicae Pubescentis Radix), Mahuang (Ephedrae Herba), Gan Heye (Nelumbinis Folium), Gan Xie (Scorpio), Tianma (Gastrodiae Rhizoma, processed), Banxia (Pinelliae Rhizoma).
Preparation: Equal parts of the above herbs. Process and grind into fine powder. Take 3 qian per dose. Add a amount of Banxia powder equivalent to one bean. Decoct with one sheng of water and three slices of Shengjiang (Zingiberis Rhizoma) until reduced to 70%. Take with the dregs after meals.
Someone who contracts this phlegm-saliva syndrome but is deficient due to chronic illness, or has mistakenly taken hot medicines, or has original Qi deficiency from the start, may present with large pulses lacking bone-force, appearing floating, surging, and rapid. Heavy pressure reveals a pulse that floats off the finger and is deficient, having exterior but no interior. In such cases, the aforementioned formula is contraindicated. This disease is difficult to treat; use this formula instead.
Herbs: Renshen (Ginseng Radix, 1.5 liang), Tiannanxing (Arisaematis Rhizoma, processed), Fangfeng (Saposhnikoviae Radix), Duhuo (Angelicae Pubescentis Radix), Banxia (Pinelliae Rhizoma).
Preparation: Except for Banxia, which is ground separately into powder, grind the remaining herbs into fine powder. Take at any time. Decoct with one sheng of water and three slices of Shengjiang (Zingiberis Rhizoma) until reduced to 70%. Additionally, it is advisable to frequently use tonifying medicines to support Original Qi, and then gradually use medicines to dissolve stubborn phlegm.
A person suffers from chronic cough, physical emaciation and weakness, lack of Qi, faint Qi in the chest, inability to take deep breaths (incomplete respiration). The six pulses are deficient, minute, and rapid. This syndrome carries signs of consumption disease (Lao Ji) and is difficult to treat. If the pulse is merely deficient and minute without floating or rapid qualities, and the Cun and Chi positions are balanced, recovery will follow medication. If the pulse is floating, rapid, and deficient, it is also a sign of consumption. If the pulse is floating, large, rapid, and deficient, light pressure reveals no bone-force, and heavy pressure yields no response. This is an incurable syndrome known as Fei Wei (Lung Flaccidity). Use this formula.
Herbs: Renshen (Ginseng Radix, 1.5 liang), Wuzi (Schisandrae Fructus), Fuling (Poria) (each 1.5 liang), Biejia (Testudinis Carapax et Plastrum, fried with butter), Gancao (Glycyrrhizae Radix et Rhizoma), Pipaye (Loquat Leaf, hairs removed) (each 1 fen), Huangqi (Astragali Radix), Ziwan (Asturi Herba) (each 10 zhu), Banxia (Pinelliae Rhizoma), Ejiao (Colla Corii Asini, thoroughly fried) (each 4 zhu).
Preparation: Grind the above herbs into a coarse powder. Use three slices of Shengjiang (Zingiberis Rhizoma). Add an amount of Banxia powder equivalent to one bean per dose. Take before meals or at any time. Additionally, it is advisable to take medicines to tonify Kidney Qi to restore Original Qi. Take 3 qian per dose, decocted with one sheng of water until reduced to 70%.
