Commentary states that the Lung Qi connects upward to the throat, and the Stomach Meridian connects externally to the pharynx. When Qi is harmonious, respiration and swallowing proceed without obstruction. However, if Spleen and Lung accumulate heat, it steams and affects the Upper Jiao (upper burner), attacking the throat and causing swelling, pain, and induration that fails to dissipate. If the pathogenic heat becomes excessive, an abscess forms, hindering exhalation and inhalation. Ancient texts state that a lesion one cun in size is called a furuncle (small boil), while those two to five cun in size are called carbuncles (large abscesses). The symptoms include chills, coughing, and expectoration of thick turbid sputum. A skilled practitioner using acupuncture should assess whether puncture is indicated and promptly lance the abscess, simultaneously administering topical or internal medicaments.
