In the streams of Jiangdong and Jiangnan, there are insects at the sources named Duanhu Xidu (Short Fox Stream Poison), also known as Shegong (Arrow Worker). These insects have no eyes but sensitive ears that can hear. Hearing human voices in mountain streams, they shoot poison from their mouths at people, hence the name Shegong. Those with minor or light toxicity, or those chasing each other, if they shoot a person's shadow, will not immediately cause sores. Instead, the patient first experiences chills and fever, dislikes cold, has a stiff body and tight sinews, headache, and eye pain, opens mouth for yawning and coughing, feels stifled and chaotic breathing, is slightly refreshed in the morning, and suffers chills and fever again in the late afternoon. Some cases resemble the reaction after taking Shisan Powder for Taiyang disease (Taiyang disease: a syndrome caused by pathogenic factors affecting the Taiyang channel), similar to Zhongshi (Pianic Disease) where one cannot speak. Given these symptoms, unless one is a local who frequently walks in mountains and streams, one would not know this condition and might mistake it for a reaction to Shisan Powder. If treated incorrectly, severe toxicity may cause sores, leading to suspicion of Biaoju (Acute Carbuncle), resulting in vomiting blood or bloody stools.
It is stated: Humans are born endowing heaven and earth, thus internally possessing five zang organs, six fu organs, essence, qi, bone marrow, and sinews; externally having four limbs, nine orifices, skin, hair, nails, teeth, throat, lips, tongue, anus, and bladder. This constitutes the body. Therefore, if nourished properly, all pulses remain harmonious; if overworked improperly, it leads to the afflictions of Wu Lao (Five Excesses), Qi Shang (Seven Injuries), and Liu Ji (Six Extremes). With prescriptions, even if ill, there is no other harm; without methods to rely on, one suddenly departs forever. Thus, this middle volume details the roots of blood and vessels of the five zang and six fu organs, their circulation and flow, and their correspondence with the nine orifices. It discusses the weight, size, length, width, and capacity of these organs. It lists counter-treatment methods, including pills, powders, wines, decoctions, soups, pastes, massages, moxibustion, acupuncture, and acupoints. Those attentive to medicine can study and practice them. Regarding cold, heat, deficiency, excess, wind, and qi, using them according to medicinal properties ensures no internal or external hundred diseases escape. The five zang correspond to five stars in heaven, five great mountains on earth, five elements by season, and five zang organs in humans. The zang organs house spirit, soul, hun, po, and yi. Discussing yin and yang, examining deficiency and excess, knowing disease origins, using tonification and purging, one should align with the 365 joints and ultimately connect the twelve meridians.
It is stated: The Liver governs Hun (Ethereal Soul), acting as an official, coming and going with the Shen (Spirit), called Hun. Hun is stored in the Liver. The eyes are the opening of the Liver; Liver Qi connects to the eyes, and when harmonious, one can distinguish five colors. The left eye corresponds to Jia (Wood Yang), the right to Yi (Wood Yin), circulating through the Purple Palace, flourishing in the nails. Externally governing sinews, internally governing blood. The Liver weighs four jin and four liang, with three lobes on the left and four on the right, totaling seven lobes. It is guarded by six boys and three jade girls. Its spirit is named Lanlan, governing the storage of Hun, known as the Hun Organ, responding to solar terms. Thus, it is said the Liver stores blood, blood houses Hun, manifests as speech in Qi, and tears in fluids. Liver Qi deficiency causes fear; excess causes anger. Liver Qi deficiency leads to dreaming of grass growing in gardens at the proper time, or hiding under trees afraid to rise. Liver Qi excess leads to dreaming of anger. When Jue Qi invades the Liver, one dreams of forests and trees.
When humans sleep, blood returns to the Liver. The Liver receiving blood enables vision; feet receiving blood enable walking; palms receiving blood enable grasping; fingers receiving blood enable holding.
The Liver corresponds to Wood, combining with the Gallbladder as a Fu organ. Its meridian is Foot Jueyin, exterior-interior related to Shaoyang. Its pulse is Xian (Wiry). It associates with Winter and flourishes in Spring. In Spring, when all things begin to grow, its Qi comes soft and weak, broad and empty, so the pulse is Xian. Ruo (Soft) means sweating is contraindicated; Ruo (Weak) means purging is contraindicated. Kuan (Broad) means opening; opening means unblocking; unblocking means benefiting. Hence, it is called broad and empty.
Spring pulse like Xian; Spring pulse belongs to Liver, Eastern Wood, the reason for the beginning of all things. Thus, its Qi comes soft, weak, light, empty, slippery, straight, and long, hence called Xian. Opposite to this indicates disease. How is it opposite? If its Qi comes solid and wiry, this is Taiguo (Excess), disease in the Exterior; if its Qi comes insubstantial and slight, this is Buji (Deficiency), disease in the Interior. Liver pulse coming soft and weak, waving like the tip of a raised pole is Ping (Normal) (*Chao Yuan* writes: graceful like pressing lute strings, like raising a long pole). Spring relies on Stomach Qi. Zhen Gan Mai (True Liver Pulse) arriving, interior and exterior tense, like touching knife edges raspingly (*Chao Yuan*: mysterious), like pressing lute strings (*Chao Yuan*: like a newly strung bowstring), color cyan-white without luster, hair breaking indicates death.
Spring with Stomach Qi slightly Xian is Ping; Xian much, Stomach Qi little is Gan Bing (Liver Disease); only Xian, no Stomach Qi is Si (Death); Stomach Qi with Mao (Hair-like/Float) pulse indicates Autumn disease; Mao excessive indicates current disease.
Liver stores blood, blood houses Hun. Sorrow moving within injures Hun. Hun injured leads to madness, essence not guarding (one version: madness not precise, dare not face people directly), causing genital retraction and tendon cramping, two hypochondriac ribs lifting (one version: not lifting), hair withered, color faded, dying in Autumn.
Foot Jueyin Qi exhaustion leads to tendon contraction pulling testicles and tongue. Jueyin is Liver Meridian; Liver is the meeting of sinews; sinews gather at the genitalia and connect to the root of the tongue. Thus, if meridians do not nourish, sinews contract and tighten, pulling testicles and tongue, causing blue lips, curled tongue, and retracted testicles. Sinews die first; Geng day worsens, Xin day dies, because Metal overcomes Wood.
Dead Liver Zang pulse: Floating weak, pressing middle like a cord that does not return, or curved like a snake moving, indicates death.
Spring Liver Wood flourishes; pulse Xian, thin, and long is Ping. Receiving Chen Ru Hua (Deep, Soft, Slippery) instead is Kidney overcoming Liver, Mother returning to Son, Son is Xu Xie (Deficient Evil), though ill, easy to treat. Receiving Fu Da Hong (Floating, Large, Surging) instead is Heart overcoming Liver, Son overcoming Mother is Shi Xie (Excessive Evil), though ill, self-heals; receiving Wei Se Duan (Slight, Choppy, Short) (*Qianjin Yi* says: slight floating and short choppy) instead is Lung overcoming Liver, Metal overcoming Wood is Zei Xie (Thieving Evil), major reversal, ten deaths untreatable; receiving Da Huan (Large and Slow) instead is Spleen overcoming Liver, Earth...
Left hand Guan position Yin exhausted means no Liver pulse. If there is strangury, involuntary urination, difficulty speaking, evil qi in hypochondrium, frequent vomiting, puncture Foot Shaoyang, treat Yang.
Left hand Guan position Yin excess is Liver Excess. Suffering pain in flesh, movement causes tendon twisting, vomiting, puncture Foot Jueyin, treat Yin.
Liver pulse coming zhuozhuo like leaning pole, like lute strings. Two beats per respiration is Ping; three beats is Li Jing Bing (Disease leaving the channels); four beats is Tuo Jing (Essence depletion); five beats is death; six beats is life ending. This is Foot Jueyin pulse.
Liver pulse Ji (Tense) extreme is evil speech (one version: delirious speech); slight Ji is Feiqi (Fat Qi), under hypochondrium like an inverted cup. Huan (Slow) extreme is vomiting; slight Huan is Shui Jia Bi (Water Mass Obstruction). Da (Large) extreme is Nei Yong (Internal Abscess), prone to vomiting and epistaxis. Wei Da is Gan Bi Suo (Liver Obstruction Contraction), cough pulling lower abdomen. Xiao (Small) extreme is polydipsia; Wei Xiao is Xiao Dao (Consumption Fever). Hua (Slippery) extreme is Tui Shan (Descending Hernia); Wei Hua is enuresis. Se (Choppy) extreme is Dan Yin (Pale Fluid); Wei Se is convulsions and tendon cramping.
Liver pulse Bo Jian Er Chang (Beats firm and long), color not cyan, should be fall injury. If beating due to blood under hypochondrium, causes wheezing and reverse Qi. If Ruo and San (Scattered) with lustrous color, should be Yi Yin (Overflowing Drink) disease. Yi Yin is sudden thirst from drinking excessively, water overflowing into muscles, skin, and outside intestines/stomach (*Suwen*: Yi Ru makes Yi Ru).
Cyan pulse arriving, long and vibrating left and right, with accumulated Qi supporting under heart, named Gan Bi (Liver Obstruction). Caused by Cold-Damp, same method as Hernia, lower back pain, cold feet, headache.
Bian Que said: Liver with disease loses eye essence; deficiency causes cold; cold makes Yin Qi strong; strong causes dreaming of mountains and trees. Excess causes heat; heat makes Yang Qi strong; strong causes dreaming of anger.
Liver sound is Hu (Shouting); movement is Wo (Grasping); emotion is Nu (Anger). Anger injures Liver. Essence and Qi merging into Liver causes You (Worry). Liver deficiency causes Kong (Fear); excess causes Nu (Anger). Unending anger also generates You.
Color governs Spring. Disease manifesting in color, take Ying (Spring) points.
Disease starting in Liver: head and eyes dizzy, hypochondriac pain and fullness; one day reaches Spleen, blockage不通, body pain and heaviness; two days reaches Stomach, abdominal distension; three days reaches Kidney, lower abdomen, waist, spine pain, shin soreness; ten days without improvement leads to death, during winter sunset or early summer meal time.
Disease in Liver: Huì (Clear) at dawn, Shèn (Severe) in late afternoon, Jìng (Quiet) at midnight.
Suppose Liver disease, obtained by traveling west or eating chicken meat, should manifest in Autumn, on Geng/Xin days.
If there is bloody death at home, women seeing it consider Ming Yao (Bright Key?) as disaster; otherwise, if affected by gold/silver objects.
General signs of Liver disease: must have pain in both hypochondria pulling to lower abdomen, making one prone to anger. Deficiency causes blindness, deafness, fear, as if being chased. To treat, take Foot Jueyin and Shaoyang meridians. Qi rebellion causes headache, eye pain, deafness, cheek swelling; take blood.
Liver pulse Chen (Deep) and Ji (Tense), Fu (Floating) similarly. Suffering hypochondriac pain with Qi support and fullness pulling lower abdomen, occasional difficult urination, dizziness, headache, back pain, cold feet with strangury, women amenorrhea or irregular bleeding, obtained from past falls.
Liver disease color cyan, limb stiffness, hypochondriac fullness, occasional dizziness/fainting, pulse Xian Chang (Wiry Long), this is treatable. Suitable for Fangfeng Zhuli Tang (Saposhnikovia Bamboo Juice Decoction), Qinjiao San (Gentiana Powder). Spring puncture Dacun, Summer Xingjian, Winter Ququan, all Tonify; Late Summer Taichong, Autumn Zhongdu, all Purge.
Also moxa Qimen 100 cones, Back 9th Vertebra 50 cones.
Evil in Liver: pain in both hypochondria, cold in middle, evil blood inside, prone to..., joints occasionally swollen. Take Xingjian to guide hypochondriac Qi, supplement Zusanli to warm Stomach middle, take blood vessels to scatter evil blood, take green veins behind ear to remove...
Any fall leaves evil blood inside. If angry, Qi rises but cannot descend, accumulates under left hypochondrium, injuring Liver.
Liver struck by Wind: head and eyes..., two hypochondria pain, walking stooped, making one crave sweet food like a pregnant woman.
Liver struck by Cold: shivering aversion to cold, slight fever, face flushed red, profuse sweating, chest烦热 (vexing heat).
Liver struck by Cold: arms unable to lift, tongue root dry, frequent sighing, chest pain, inability to turn, occasional night sweats, cough..., vomiting juice after eating.
Liver governs chest. Wheezing, cursing, pulse Chen, chest stifling, desire for pushing/pressing. Heat causes nasal obstruction. Liver injury: emaciation, lying with mouth open, hands/feet occasionally blue, eyes closing, pupil pain. This is caused by Liver injury.
Liver Water: abdomen large, unable to turn, pain under hypochondria and abdomen, occasional slight saliva production, continuous urination.
Liver Distension: hypochondriac fullness and pain pulling lower abdomen.
Liver Zhuo: patients often want someone to tread on their chest. Initially, before suffering, only desire hot drinks.
Diagnosing Liver Ji (Accumulation): pulse Xian and Xi (Thin), pain in both hypochondria, evil Qi moving to heart region, cold calves, hypochondriac pain pulling lower abdomen. Men: Ji Han (Accumulated Hernia); Women: Jia Lin (Mass Strangury). Body lacks luster, prone to tendon cramping, nails withered and black. Better in Spring, worse in Autumn, color cyan.
Liver Ji named Feiqi (Fat Qi), under left hypochondrium like an inverted cup, with head and feet like turtle/tortoise. Long-term unresolved, causes cough, reverse Qi, malaria lasting years. Obtained on Wu/Ji days of Late Summer. Why? Lung disease transmits to Liver. Liver should transmit to Spleen. Spleen is flourishing in Late Summer. Flourishing organs do not accept evil. Liver wants to return to Lung, but Lung refuses. Thus, it stays and forms accumulation. Hence, Feiqi is obtained in Late Summer.
Liver disease: chest fullness, hypochondriac distension, prone to anger/shouting, body heat with aversion to cold, limbs unable to move, pale face, smooth body. Pulse should be Xian Chang Ji (Wiry, Long, Tense), but is now Duan Se (Short, Choppy). Color should be Cyan, but is White. This is Metal overcoming Wood.
Duke Xiang asked Bian Que: I wish to diagnose without pulse, observe voice, watch color, know disease survival/death. Is this possible? Answer: This is the great essence of Sage Dao, not transmitted by teachers, valued by Huangdi more than gold/jade. Entering to see disease, observing color, hearing breathing, one knows incoming/outgoing, auspicious/inauspicious signs. Jue tone people govern Liver sound. Liver sound is Hu (Shout), tone Qin (Lute), emotion Nu (Anger), meridian Foot Jueyin. Jue Ni (Reversal) in Shaoyang causes Ying-Wei blockage, Yin-Yang mixing. External Yin injury, internal Yang attack. Attack causes cold, cold causes deficiency, deficiency causes sudden aphonia. This is Li Feng (Malignant Wind) entering Liver. Xuming Tang (Continuation Life Decoction) governs it (Formula in Vol 8). But sitting upright unable to bow head, face/cyan-black, limbs slack, fecal/urinary incontinence, severe... untreatable. Delayed: within ten days/months, Guizhi Jiu (Cinnamon Twig Wine) governs it (Formula in Vol 8). Also shouting then crying, crying then groaning. This is Metal overcoming Wood, Yin attacking Yang. Yin Qi rises, Yang Qi hides. Hidden becomes Excess, Excess becomes Heat, Heat causes wheezing, wheezing causes reverse Qi, reverse Qi causes stifling, stifling causes fear, vision unclear, speech urgent, hallucinations. This is Evil Heat injuring Liver, severe... untreatable. If lip color cyan but eyes respond, treatable. Dihuang Jian (Rehmannia Paste) governs it (Formula in...).
Liver disease as Malaria: makes person look pale/grey, sighing, appearance like dead. Wumei Wan (Mume Pill) governs it (Formula in Vol 10). If person...
Originally lacking sorrow/anger, suddenly furious, abnormal speech, alternating wide/narrow urgency, gesturing to eyes while speaking, as if fearful. If not immediate disease, disaster will come. This is sign of Liver disease voice. If deficient, injured by Cold Wind; if excess, damaged by Hot Qi. Yang purge, Yin tonify.
Cyan is Liver, Liver combines with Sinews. Cyan like kingfisher feather is Auspicious. Liver governs Eyes; Eyes are surplus of Liver. Person with Wood shape: cyan upper horn, small head, long face, large shoulders, flat back, straight body, small hands/feet, capable/labor-loving, small heart/strong strength, worried about affairs, tolerates Spring/Summer, not Autumn/Winter. Autumn/Winter infection causes disease, Foot Jueyin. Tuotuo Ran (Sturdy) broad hypochondria combined firm/brittle/tilted/upright, Liver responds. Pure cyan fine texture: Liver small, small means organ safe, no hypochondriac disease. Coarse texture: Liver large, large means deficient, deficient means cold, compresses Stomach/throat, prone to diaphragm fullness and hypochondriac pain. Broad hypochondria reversed: Liver high, high means excess, excess means Liver Heat, supports up to Fen, adds hypochondriac urgency, becomes Xi Ben (Breath Accumulation), combines with hypochondriac danger (one version: Rabbit). ... means Liver low, low compresses Stomach, empty hypochondria, empty means easily accepts evil. Hypochondriac bones firm: Liver firm, firm means organ safe, hard to injure. Hypochondriac bones weak: Liver brittle, brittle means prone to Xiao Dao (Consumption Fever), easy to injure. Hypochondriac/abdomen well-matched: Liver upright, upright means harmonious/beneficial, hard to injure. Hypochondriac bones uneven: Liver tilted, tilted means unilateral hypochondriac pain.
Wherever body parts sink or rise, disease must arise. Gallbladder Shaoyang is Liver's region. Liver Qi connects interior/exterior, region responds accordingly. Chen Zhuo (Deep/Turbid) is Interior; Fu Qing (Floating/Clear) is Exterior. If color moves from outside to inside, disease arises from outside, region rises. If color moves from inside to outside, disease arises from inside, region sinks. Treat Yin before Yang for internal disease; treat Yang before Yin for external disease. Yang governs Exterior, Yin governs Interior. Human life/death/rest depends on Zang Spirit changing form externally. Before Liver disease, eyes become colorless. Before Liver death, eyes lose essence. If Heaven/Earth equal division, grave color responds, must die untreatable. Look to increase/decrease/deliberate delay/accelerate. Delay: not exceeding 400 days; Accelerate: not extending beyond 10 days/month. Liver disease slightly better then sudden death. Why know? Cyan-white like thumb size, ... dot appears on face/cheek, this must be sudden death. Liver exhaustion dies in 8 days. Why know? Face cyan, eyes red, only desires lying prone, sees but doesn't recognize people, sweat flowing like water non-stop. (One day/two days death.) Face black, eyes cyan: not dead. Cyan like grass growth: death. Auspicious/inauspicious colors lie in regions. Smoothly seen, cyan-white entering eyes must get disease, not exceeding year. If year doesn't correspond, within three years, disaster must occur.
Spring, Wood, Liver, Pulse, Color Cyan, governs Foot Shaoyang pulse. Spring take Luo Mai (Collateral Vessels) and Fen Rou (Interstices). Spring Wood begins governance, Liver Qi begins generation. Liver Qi urgent, wind fast, meridians usually deep, Qi scarce cannot enter deep, so take Collateral Vessels and Interstices. Pulse roots are in Qiaoyin interstices, should correspond to Chuanglong...
Its sinews start above little toe and second toe, knot at lateral malleolus, ascend along lateral margin; knot at lateral knee margin. Branch starts from lateral fibular bone, ascends to thigh. Anterior branch knots above Futu; posterior branch knots at sacrum. Straight branch ascends along lateral costal margin, ascends to anterior axillary margin,夹 (clamps) chest/breast, knots at Quepen. Straight branch exits armpit, penetrates Quepen, exits anterior Taiyang, follows behind ear, ascends to frontal angle, crosses vertex, descends to chin, knots at Jiu (Nostril). Branch knots at outer canthus as Wai Wei (Outer维系).
Its pulse starts at outer canthus, ascends to frontal angle, descends behind ear, follows neck anterior to Hand Shaoyang, reaches shoulder, crosses behind Hand Shaoyang, enters Quepen. Branch from behind ear enters ear, exits anterior ear to posterior outer canthus. Branch separates outer canthus, descends to Daying, merges with Hand Shaoyang below. Adds Jiache, descends neck, merges Quepen, descends chest, penetrates Diaphragm, connects Liver, belongs to Gallbladder, follows inside hypochondrium, exits Qiegang, surrounds pubic hair, horizontally enters Bi Yan (Hip Pit). Straight branch from Quepen descends armpit, follows chest, passes lateral costal margin, merges Bi Yan below, descends along Bi Yang, exits lateral knee margin, descends anterior to lateral fibular bone, straight down to Duogu end, exits anterior to lateral malleolus, follows foot dorsum, exits end of little toe/second toe. Branch separates dorsum, enters between big toe, follows big toe inner bifurcation, exits end, returns penetrates nail bed, exits San Mao (Three Hairs), merges Foot Jueyin as exterior-interior. Jue... Cun, should be Back Shu, meets Hand Taiyin together.
Its Foot Shaoyang alias is Guangming, five cun above ankle, separately walks Jueyin, descends connects foot dorsum. Governs Liver disease. Excess: Gallbladder Heat. Heat causes Jue (Reversal). Jue causes Yang disease. Yang pulse reverses larger than Cunkou by one fold. Disease: heat in chest, heart/hypochondriac/head/chin pain, Quepen/armpit swelling. Deficiency: Gallbladder Cold. Cold causes Wei (Atrophy). ... causes Yin disease. Yin pulse reverses smaller than Cunkou. Disease: cold in chest, shortness of breath, bitter taste, body lacks luster, externally to ... Duogu/lateral malleolus anterior and all joints painful. If Yin/Yang both quiet or both move, like pulling rope with sudden stops, is disease (This is entirely Foot Shaoyang Gallbladder meridian sinew/pulse branch diseases, now append Foot Jueyin Liver meridian after).
Foot Jueyin pulse starts at pubic hair of big toe, ascends along foot dorsum medial margin, one cun from medial malleolus, eight cun above ankle, crosses Taiyin posterior, ascends medial thigh margin, follows股阴 (inner thigh) into pubic region, circles genitalia, reaches lower abdomen, clamps Stomach, belongs to Liver, connects Gallbladder, penetrates Diaphragm, spreads hypochondriac ribs, follows posterior throat, ascends to Hangsang (Pharynx), connects Eye System, ascends forehead, meets Du Mai at Vertex (One version: branch from lower abdomen meets Taiyin/Shaoyang at third/fourth space below lumbar sacrum); branch from Eye System descends cheek interior, circles lips interior; another branch separates from Liver, penetrates Diaphragm, ascends injects Lung. Shi Dong (Moving) disease: lower back pain unable to bend/extend, men ... hernia, women lower abdomen swelling, severe throat dry, face dusty/color loss. Shi Zhu Gan Suo Sheng Bing (Governor of Liver Diseases): chest fullness, vomiting/reverse Qi, loose diarrhea, fox hernia, enuresis/strangury. Excess: Cunkou one fold larger than Renying. Deficiency: Cunkou smaller than Renying.
Foot Jueyin branch, name Ligou, five cun above medial malleolus, separately walks Shaoyang. Its branch follows meridian up scrotum, knots at penis. Disease Qi rebellion...
