(This prescription also treats Shanghan [Cold Damage], and is titled "Tianxing" [Epidemic Disease], thus it is included in the Tianxing section.)
Xu Renze said: This disease is called Shanghan [Cold Damage] by physicians, and there are two types: Yin Shanghan and Yang Shanghan. Yin Shanghan is the opposite of Yang Shanghan. The symptoms of Yang Shanghan show correspondence between exterior and interior: Heart heat causes dry mouth and bitter taste; Liver heat causes red eyes and blurred vision; Spleen heat causes slight astringency in the anus; Kidney heat causes hot and red ears; Lung heat causes dry nose and thirst; Stomach heat causes vomiting and rebellious Qi; Large Intestine heat causes constipation and astringency; Small Intestine heat causes scanty and red urine; Skin heat causes a flooding and rapid pulse and body fever. The reverse of these conditions constitutes Yin Shanghan. Shanghan refers to injury by cold pathogen. Cold generates Yin, and Yin governs killing/destruction. When a person's Yin and Yang are harmonious, they are free from disease. Once the Qi is injured by cold, this illness arises.
Furthermore, regarding Yin and Yang Shanghan, it is because toxic vapors injure the Yin and Yang Qi of the body. The human body contains Yin and Yang Qi; Yin and Yang correspond to cold and heat. If the primordial Yin is injured by toxins, it cannot circulate freely, causing Yang-heat to reign supreme alone; therefore, Tianxing diseases often manifest as heat syndromes. Among various diseases, this one is the most difficult to treat. Between the Yin and Yang diseases, Yin Shanghan is particularly worrying. It is often heard that many people die suddenly from this disease. This may be due to imprecise diagnosis and pulse-taking, prescriptions failing to hit the therapeutic target, improper diet, or loss of regulation in cold and warmth. Thus, if one cannot distinguish thoroughly oneself, other doctors will find it hard to be exquisite. Rather than treating it forcibly, it is better to let nature take its course. As long as one adapts flavors to cold and warmth, and manages temperature during care, enduring for seven days, this is the best strategy. However, among cases where medication is necessary and cannot be ignored, only those who have not had a bowel movement for many days and temporarily need purgation fall into this category. After one or two days of illness, feeling strong body fever, headache, soreness and pain in bones and muscles, stiffness in the back and spine, dry mouth and nose, slightly cold hands and feet, and yellow-red urine—these are the signs. In such cases, one should first prepare and boil the Peach and Willow Three-Ingredient Decoction [Taoliu Sanwu Tang] for bathing.
Peach Branch (finely cut, five dou) Willow Leaf (finely cut, five dou) Vinegar Fermented Water (one dou)
Take the above herbs, first boil the peach branches and willow leaves in one shi of water, extract seven dou of juice, remove the dregs, add vinegar fermented water, stir, and use while warm for bathing. After bathing, wipe the body dry, massage with powder, avoid wind exposure, then prick the sides behind the head and eyes in private areas, and under the tongue until blood flows. Apply salt powder to the pricked sites to suppress bleeding, then enter the quilt and lie down.
Subsequently, take the Muscle-Releasing Kudzu Five-Ingredient Drink [Jieji Gan Ge Wuwu Yin]. Cover slightly to induce sweating. If the root of the disease is mild, recovery may occur through this method.
Kudzu Root (cut, five he) Scallion White (cut, one sheng) Fresh Ginger (cut, one he) Fermented Soybean Core (one sheng, wrapped in silk) Japonica Rice (two he, ground)
Cut the above herbs. Boil the four ingredients above the fermented soybean core in five sheng of water until three and a half sheng of liquid remains. Remove dregs, add rice flour, and cook until the rice is soft. Drink while warm in one go, wait for sweat to appear, and cover slightly. No dietary restrictions apply.
If, following the previous bathing methods, recovery is not felt, prepare the Egg Soup [Jizi Tang] again to purge.
Five fresh eggs from hens that have just laid
Break the top of each egg, pour into a cup. Add the water from one additional egg. Stir vigorously with chopsticks until very turbid. Separately, boil one sheng of water until boiling, then drop the eggs into the soup, stirring slightly. When just cooked, pour into a bowl, add a little clear sauce until it smells slightly fishy/spoiled. Drink while warm until finished, cover to induce sweating.
If, after sweating from the previous Egg Soup, the condition does not subside, prepare the Gardenia Six-Ingredient Powder [Zhizi Liuweisan] to purge.
Gardenia Fruit (thirty seeds, split) Dried Kudzu Root (five liang) Artemisia Capillaris (two liang) Shu Cimicifuga (three liang) Rhubarb (five liang) Mirabilite (five liang)
Cut and pound the above herbs into a powder. Take three square-cun spoons with drink. Shortly after ingestion, one should feel abdominal movement and diarrhea. If no effect after one meal time, administer hot drink first. If still no effect, take another square-cun spoon shortly thereafter with drink. Continue until purgation occurs. Afterwards, adjust to cold/warmth and rest; no further need for this medicine.
If, after purgation with the previous Gardenia Six-Ingredient Powder, the condition still does not recede, accompanied by vomiting, inability to eat, and dryness of the mouth, nose, throat, and tongue, prepare the Raw Reed Eight-Ingredient Drink [Sheng Lugeng Bawei Yin] and take it slowly.
Raw Reed Rhizome (cut, one sheng) Raw Ophiopogon Tuber (two sheng, heart removed) Fresh Ginger (five liang) Ginseng (two liang) Anemarrhena Rhizome (two liang) Smoked Plum (ten pieces) White Honey (one he) Bamboo Sap (three he)
Cut the above herbs. Boil in eight sheng of water until three sheng remain. Remove dregs, add honey and bamboo sap, and stir well. Drink slowly, regardless of frequency, temperature, or relation to meals. Although this drink cannot instantly eliminate heat pathogens, it prevents worsening of the constitution across various symptoms, stabilizing the heart and abdomen without cold sensations. To obtain this drink, any combination is acceptable as long as taken as previously directed. If heat persists, chest/abdominal fullness increases, food intake decreases, and epigastric palpitation forms, do not take it again.
If, after drinking the previous Raw Reed Eight-Ingredient Drink, symptoms persist, appetite gradually disappears, abdomen becomes hard and knotted, painful to touch, sometimes unbearable upon contact—since this is a heat disease, the body qi should be hot, bones and muscles painful, pulse flooding and rapid, mouth bitter and crisp, food leads to vomiting, yet body qi feels cold, pulse becomes deep and fine, food passes down but pain is not felt, stool and urine are blocked, epigastrium feels like stone, intolerable pain, lips tight, nostrils flaring, hands and eyes restless, delirious speech—this is due to extreme heat. Resting in extreme cold, seeking only cold in diet and sleep, heat knots in the heart with no outlet for discharge. If such cases arise, fewer than two out of ten can be saved. One cannot rely on routine methods; one must consider it a critical situation. Only the Pinellia Ten-Ingredient Decoction [Banxia Shiwei Tang] can address this. If symptoms are similar but include yellowing of the body and sclera yellow like Phellodendron bark, this is Acute Yellow [Ji Huang]. Such cases also require abandoning routine methods and using the subsequent rescue decoction.
Pinellia Tuber (five liang, from Xiong Prefecture, boiled to remove slipperiness, juice drained, possibly referring to Xiong character) Dried Ginger (three liang) Evodia Fruit (two liang) Cinnamon Bark (one liang) Atractylodes Macrocephala Rhizome (three liang) Asarum Root (three liang) Bupleurum Root (three liang) Moutan Cortex (three liang) Rhubarb (five liang) Mirabilite (two liang)
Cut the above herbs. Boil in one dou of water until three sheng remain. Remove dregs, add Mirabilite, stir until dissolved. Divide into three warm doses. Each dose separated by the time it takes to walk ten li. If purgation occurs after one dose, monitor and rest; do not take more decoction. Instead, grind fine millet to make juice and drink slowly. If purgation is excessive, stop with sour rice or thick sour porridge. Avoid mutton, malt sugar, raw scallions, raw vegetables, peaches, plums, sparrow meat, and coriander.
Following the critical assessment, after taking the Pinellia Ten-Ingredient Decoction, although toxic heat subsides and purgation continues, physical strength gradually weakens. Prepare the Ginseng Five-Ingredient Powder [Renshen Wuweis San] and take it slowly.
Ginseng (five liang) Fresh Rhinoceros Horn (powdered, two liang) Smoked Plum Flesh (three liang, roasted) Fresh Ginger Scrapings (three liang) Coptis Rhizome (three liang, hairs removed; if unavailable, substitute with four liang of Dragon Bone)
Pound and sieve the above herbs into powder. Take one square-cun spoon with drink, three times daily, gradually increasing to two spoons. Avoid pork and cold water. (Wu Sheng agrees.)
